Amputation of a lower extremity after severe trauma.

OBJECTIVE: Serious lower extremity injuries sometimes warrant emergency amputation. The goal of amputation in polytrauma patients is to increase chances of survival, while the goal of amputation in a single limb injury is to prevent further complications, e.g., infection, septic shock. INDICATIONS: Rescue from life-threatening lower extremity bleeding in a critically injured patient. Severe injury of a lower extremity: crushed, burned, frozen, advanced infection. CONTRAINDICATIONS: Patient refusal. SURGICAL TECHNIQUE: Supine position, determination of resection border, skin incision, identi

Using a programmable pneumatic device with truncal therapy to facilitate wound healing: a case series.

Chronic, recurring limb ulcers require a comprehensive approach that addresses the wound pathology, vascular status, and swelling. Although studies have shown that compression, especially high compression, is more effective than dressings alone, evidence to support one particular method of compression is limited. A sequential intermittent pneumatic compression (IPC) device was evaluated in four patients (two men, two women, age range 47 to 59 years) with nonhealing (history of 1 to 6 months) venous insufficiency (n = 2), postsurgical complication (n = 1), and radiation burn (n = 1) wounds and

Factors associated with response to compression-based physical therapy for secondary lower limb lymphedema after gynecologic cancer treatment: a multicenter retrospective study.

BACKGROUND: Lower limb lymphedema (LLL) is one of the most refractory and debilitating complications related to gynecological cancer treatment. We investigated factors associated with response to compression-based physical therapy (CPT) for secondary LLL after gynecologic cancer treatment. METHODS: We performed a multicenter retrospective study using the records of seven medical institutions from 2002 and 2014. Patients who developed LLL after gynecological cancer treatment were included. Limb volumes were calculated from the lengths of the limb circumferences at four points. All participants

Venous leg ulcer management in clinical practice in the UK: costs and outcomes.

The aim of this study was to estimate the patterns of care and annual levels of health care resource use attributable to managing venous leg ulcers (VLUs) in clinical practice by the UK's National Health Service (NHS) and the associated costs of patient management. This was a retrospective cohort analysis of the records of 505 patients in The Health Improvement Network (THIN) Database. Patients' characteristics, wound-related health outcomes and health care resource use were quantified, and the total NHS cost of patient management was estimated at 2015/2016 prices. Overall, 53% of all VLUs hea

Prevalence and clinical features of lymphedema in patients with lymphangioleiomyomatosis.

BACKGROUND: Lymphangioleiomyomatosis (LAM) is a rare cystic lung disease predominantly affecting young women. Some of these patients develop lymphedema of the lower extremities and buttocks; however, neither the exact frequency of LAM-associated lymphedema nor the clinical features of such patients is well delineated. OBJECTIVES: To document the frequency, features, and treatment of LAM-associated lymphedema. METHODS: We reviewed all medical records of patients listed in the Juntendo University LAM registry for the 30 years preceding August 2010. RESULTS: Of 228 patients registered with a d

Tumescentless endovenous radiofrequency ablation with local hypothermia and compression technique.

INTRODUCTION: Modern surgical management of chronic venous insufficiency is possible since the development of catheter-based minimally invasive techniques, including radiofrequency ablation (RFA) and the application of colour Doppler sonography. RFA technology requires the use of tumescent anaesthesia, which prolongs the operating time. Instilling tumescent anaesthesia percutaneously below the saphenous fascia is the steepest part of the learning curve. In our study, we compared operative and postoperative results of tumescentless RFA and RFA with tumescent anaesthesia, to investigate the nece

Splint versus no splint after ankle fracture fixation; Results from the multi-centre post-operative ankle splint trial (PAST).

BACKGROUND: There is considerable variation in the rehabilitation of ankle fractures. Ankle fractures treated surgically are often immobilized or splinted in the early post-operative period, despite the lack of robust evidence supporting this intervention. Thus, this randomized controlled trial aims to investigate the anecdote that splinting reduces pain and oedema. METHODS: A prospective multi-centre randomized controlled trial was performed in three trauma centres. Eligible patients were over 18 years of age that have sustained an isolated unilateral ankle fracture requiring surgical interv

Incidence of and predisposing factors for pseudoaneurysm formation in a high-volume cardiovascular center.

PURPOSE: To evaluate factors associated with pseudoaneurysm (PSA) development. METHODS: Between January 2016 and May 2020, 30,196 patients had invasive vascular radiological or cardiac endovascular procedures that required arterial puncture. All patients with PSA were identified. A matched (age, gender, and type of the procedure) control group of 134 patients was created to reveal predictors of PSA formation. RESULTS: Single PSAs were found in 134 patients. Fifty-three PSAs developed after radiological procedures (53/6555 [0.8%]), 31 after coronary artery procedures (31/18038 [0.2%]), 25 aft

Labia majora hypertrophy.

A case involving hypertrophy of the labia majora is described. A married 34-year-old mother of a 5-year-old daughter reported that from adolescence, her large labia majora and its protuberance have resembled male genitalia even when she is wearing a slip. For that reason, in the summer, she is ashamed of her appearance and thus has avoided swimming pools and beach areas. With the patient in supine position, the crural creases were marked. The marks were drawn 1 cm apart medially in to the medial part of the labia majora parallel to the natural vulvar crease. The estimated amount of skin and fat

Lymphedema: a comprehensive review.

BACKGROUND: Lymphedema is a chronic, debilitating condition that has traditionally been seen as refractory or incurable. Recent years have brought new advances in the study of lymphedema pathophysiology, as well as diagnostic and therapeutic tools that are changing this perspective. OBJECTIVE: To provide a systematic approach to evaluating and managing patients with lymphedema. METHODS: We performed MEDLINE searches of the English-language literature (1966 to March 2006) using the terms lymphedema, breast cancer-associated lymphedema, lymphatic complications, lymphatic imaging, decongestive

The role of surgery in the treatment of acute phlebitis in the lower members.

UNLABELLED: Most phlebitis responds favorably to medical treatment. Iliofemoral and iliocaval phlebitis, however, involve considerable risk as they may endanger life (pulmonary embolism), limb (ischemic forms) or the functioning of the limb (severe post-phlebitic syndromes). Because of this triple risk it is important to be able to judge when to opt for surgery. Surgical treatment is called for in cases of certain iliofemoral phlebitis developing in spite of medical treatment. Threat of "blue phlebitis" (acute arterial ischemia); threat of severe post-phlebitis syndrome (the thrombosed iliac v
