Reconstruction of total maxillectomy defects with preservation of the orbital contents.

Reconstruction after total maxillectomy with preservation of the orbital contents is technically more challenging than when the maxillectomy is combined with orbital exenteration. Reconstruction of such defects should (1) provide support to the orbital contents, (2) obliterate any communication between the orbit and nasopharynx, (3) reconstruct the palatal surface, and (4) achieve facial symmetry and a good aesthetic result. We report our experience in performing reconstructive surgery on 14 patients who had a total maxillectomy and preservation of the orbital contents using nonvascularized bo

[Prevention and management of locoregional complications of botulinum A toxin injections in cosmetic treatment].

Botulinum toxin serotype A injections used in treating dynamic wrinkles is one of the least invasive cosmetic procedures. High patient satisfaction and low onset of always moderate side effects contribute to the growing popularity of botulinum toxin injections in cosmetic treatment over the past few years. Years of experience and use, in therapeutics [1,2] and esthetics (20 years) have proven the efficacy and the safety of this wrinkle treatment. Today, no severe or long-term side effects have been reported in esthetics. This article discusses only the most frequent locoregional effects. They

V3 midface lifting through a subciliary approach: a three-vector physiologic repositioning technique for midface rejuvenation, lower eyelid support, and lower lid-malar contour improvement.

BACKGROUND: Conventional midface rejuvenation through temporal or preauricular incisions often fails to adequately restore the central midface and tear trough vector. We describe the "V3 midface lift"-a transcutaneous subciliary technique utilizing a three-vector physiologic repositioning system to address midface ptosis, lower eyelid bags, and lower lid-malar contour disharmony. METHODS: A consecutive cohort of 42 patients (38 females, 4 males; mean age: 46.2 ± 6.8 years) who underwent the V3 midface lift between January 2022 and December 2024 was retrospectively analyzed. The technique inco

Complex Surgical Management of PDLLA-Induced Lower Eyelid Granulomas.

BACKGROUND: Poly-D, L-lactic acid (PDLLA) is a biostimulatory injectable increasingly used for infraorbital rejuvenation. Although considered safe when properly handled and injected, evidence of severe complications in the thin, anatomically complex lower eyelid region remains limited. CASE: A 67-year-old woman underwent 3 periorbital PDLLA injection sessions performed with a sharp needle. ∼1 month after the final treatment, she developed delayed-onset, multiple bilateral granulomatous nodules in the subcutaneous plane, the orbicularis oculi muscle, and the sub-orbicularis oculi fat. The lesi

Treatment of orbital fractures - a critical analysis of ophthalmic outcomes and scenarios for re-intervention.

BACKGROUND: Malplaced implants in orbital reconstruction may lead to serious complications and necessitate re-intervention. The aim of this study was to describe outcomes, complications and scenarios of re-intervention in a historical case series of orbital fractures treated with free-hand orbital wall reconstruction. The main hypothesis was that early re-interventions are mainly because of malplaced implants in the posterior orbit. METHODS: Retrospective review of 90 patients with facial fractures involving the orbit, reconstructed with radiopaque orbital wall implants, from 2011 to 2016. Da

Mutations in CPAMD8 Cause a Unique Form of Autosomal-Recessive Anterior Segment Dysgenesis.

Anterior segment dysgeneses (ASDs) comprise a spectrum of developmental disorders affecting the anterior segment of the eye. Here, we describe three unrelated families affected by a previously unclassified form of ASD. Shared ocular manifestations include bilateral iris hypoplasia, ectopia lentis, corectopia, ectropion uveae, and cataracts. Whole-exome sequencing and targeted Sanger sequencing identified mutations in CPAMD8 (C3 and PZP-like alpha-2-macroglobulin domain-containing protein 8) as the cause of recessive ASD in all three families. A homozygous missense mutation in the evolutionaril

A 15-year review of midface reconstruction after total and subtotal maxillectomy: part II. Technical modifications to maximize aesthetic and functional outcomes.

BACKGROUND: Midface reconstruction of oncologic defects is one of the most challenging problems in plastic surgery. Over time, the senior author (P.G.C.) has developed modifications in his technique for oncologic midface reconstruction to minimize complications and maximize outcomes. METHODS: Over a 15-year period (1992 to 2006), 100 flaps for midface oncologic reconstruction were performed at Memorial Sloan-Kettering Cancer Center. The first 50 flaps were performed from 1992 to 2000, and the second 50 flaps were performed from 2001 to 2006. Over time, technical improvements were made in the

Reducing eyelid retraction following subperiosteal face lift.

The most common complication of lower blepharoplasty is malposition of the lower eyelid. This deformity may present as scleral show, ectropion, entropion, or canthal dystopia, with dry eye or epiphora. These problems occurred frequently in our initial series of patients undergoing blepharoplasty with subperiosteal face lift. The key to prevention lay in avoiding the potential causes of these problems: overresection of eyelid skin, retraction of the orbital septum, orbicularis oculi dysfunction, postoperative swelling, and downward pull from the facial suspension. This article reviews the cases
