Survey of the POSNA Membership Reveals Variability in Use of Tibialis Anterior Tendon Transfer for Recurrent Clubfoot.

PURPOSE: The aim of this project was to survey members of the Pediatric Orthopaedic Society of North America (POSNA) regarding the use of tibialis anterior tendon transfer (TATT) in the management of recurrent clubfoot with dynamic supination and no deformity. We aimed to assess which techniques for TATT are most utilized by pediatric orthopedists. As there has been no general consensus in the literature regarding best methods, we hypothesized that treatment methods would vary widely among POSNA members. METHODS: The online survey, designed using Research Electronic Data Capture (REDCap) surv

Alteration of ankle proprioceptive threshold during gait in the presence of acute experimental pain.

OBJECTIVE: Human gait requires complex somatosensory processing of various inputs such as proprioception. Proprioception can be altered in the presence of pain. This has been shown mostly during controlled tasks, thereby limiting the influence of external perturbations. While controlling the environment is sometimes warranted, it limits the ecological validity of the data. Using robotic orthoses to apply perturbations during movements seems a promising tool to functionally assess proprioception, where the complex somatosensory processing required in real-life situations is at play. The main ob

Effect of exoskeletal joint constraint and passive resistance on metabolic energy expenditure: Implications for walking in paraplegia.

An important consideration in the design of a practical system to restore walking in individuals with spinal cord injury is to minimize metabolic energy demand on the user. In this study, the effects of exoskeletal constraints on metabolic energy expenditure were evaluated in able-bodied volunteers to gain insight into the demands of walking with a hybrid neuroprosthesis after paralysis. The exoskeleton had a hydraulic mechanism to reciprocally couple hip flexion and extension, unlocked hydraulic stance controlled knee mechanisms, and ankles fixed at neutral by ankle-foot orthoses. These mecha

A systematic review of physical interventions for patellofemoral pain syndrome.

OBJECTIVE: Physical interventions (nonpharmacological and nonsurgical) are the mainstay of treatment for patellofemoral pain syndrome (PFPS). Physiotherapy is the most common of all physical interventions and includes specific vastus medialis obliquus or general quadriceps strengthening and/or realignment procedures (tape, brace, stretching). These treatments appear to be based on sound theoretical rationale and have attained widespread acceptance, but evidence for the efficacy of these interventions is not well established. This review will present the available evidence for physical interven

3D-printed shoe soles with structural properties towards reducing knee adduction moments during walking.

PURPOSE: Lateral wedged insole (LWI) is a representative foot orthosis for the conservative management of knee osteoarthritis. However, recent research indicates the heterogeneity of LWIs' biomechanical effectiveness, indicating limitations in conventional LWI design and the need to develop new orthoses with improved effects. This study evaluated the efficacy of custom-designed 3D-printed shoe soles in reducing knee adduction moment (KAM), compared with conventional 5° LWIs. METHODS: Shoe soles were 3D-printed with a gyroid infill structure to allow adjustable stiffness, generating a variable

Assessment of the Chignon dynamic ankle-foot orthosis using instrumented gait analysis in hemiparetic adults.

OBJECTIVE: In the hemiplegic adult, gait is frequently perturbed by lack of ankle dorsiflexion at toe-off and may prompt prescription of an ankle-foot orthosis (AFO). Our objective was to evaluate the effect on gait of a dynamic AFO (the Chignon orthosis) in comparison with a prefabricated AFO (PAFO). METHOD: Ten chronic hemiplegic patients performed a 10 m gait test and then underwent an instrumented treadmill gait test under three different sets of conditions (without an orthosis, with a PAFO and with a Chignon orthosis). The energy cost was calculated by measuring the oxygen consumption du

Return to Play in National Football League Players After Operative Jones Fracture Treatment.

BACKGROUND: Jones fractures commonly occur in professional athletes and operative treatment remains the standard of care in this patient population. In our clinical experience, an aggressive postoperative rehabilitation protocol for National Football League (NFL) players with an average return to play (RTP) between 8 and 10 weeks can have successful outcomes with few complications. The purpose of this study was to quantify RTP and rate of complications, including nonunion, refracture, and reoperation among a cohort of NFL players with operatively treated Jones fractures. METHODS: Between 2004

The effects of prolonged wear of textured shoe insoles on gait, foot sensation and proprioception in people with multiple sclerosis: study protocol for a randomised controlled trial.

BACKGROUND: Many people with multiple sclerosis experience problems with walking, which can make daily activities difficult and often leads to falls. Foot sensation plays an important role in keeping the body balanced whilst walking; however, people with multiple sclerosis often have poor sensation on the soles of their feet. Wearing a specially designed shoe insole, which enhances plantar sensory information, could help people with multiple sclerosis to walk better. This study will explore whether long-term wear of a textured insole can improve walking in people with multiple sclerosis. METH

Influences of Gait Training with Knee-Ankle-Foot Orthosis on Gait Ability and Independence in Severe Hemiplegia and Pusher Behavior with Unilateral Spatial Neglect Following Stroke: A Retrospective Historical Controlled Study.

BACKGROUND: Orthotic therapy is crucial for post-stroke patients with severe motor paralysis and higher brain dysfunction, including pushers and unilateral spatial neglect (USN). These patients exhibit impaired postural control and compromised independence in activities of daily living (ADLs), often necessitating mid- to long-term intervention using knee-ankle-foot orthoses (KAFOs). However, the influences of extended orthotic intervention in severe and pusher cases remain unclear. The purpose of this study was to examine the influences of KAFO on walking ability and independence in severe hem
