Adherence to a 2-layer compression system for chronic venous ulceration.

Adherence to compression therapy is fundamental for healing venous leg ulcers (Moffatt, 2004). The aim of this study was to investigate adherence and comfort associated with using a 2-layer compression system over a 6-week period for the management of chronic venous leg ulceration in patients with a history of non-adherence to other compression systems. Self-report rating scales were completed each week by the patients to record adherence and comfort of the compression system, while condition and ease of use of the system were recorded each week by the nurses involved in their care. The majori

Current practices in the prophylaxis of venous thromboembolism in bariatric surgery.

BACKGROUND: Morbidly obese patients undergoing bariatric surgery have commonly been concluded to be at high risk for the development of perioperative venous thromboembolism. Due to its clinically silent nature, primary prevention is the key to reduce morbidity and mortality. There is no clear consensus in the literature regarding the optimum approach to minimize this preventable phenomenon. METHODS: Members of the American Society for Bariatric Surgery were surveyed regarding their current practices in the prophylaxis of venous thromboembolism in their bariatric patients. RESULTS: 31% of the

Treatment of Interprosthetic Femoral Fracture Nonunion: A Systematic Review.

BACKGROUND: Interprosthetic fractures (IFs) of the femur present complex injuries associated with increased morbidity and mortality. Limited bone stock between the proximal and distal prostheses makes fracture reduction and stabilization challenging. Nonunion is a common and severe complication of IFs, often requiring multiple interventions. This systematic review aimed to identify the most successful treatment for femoral IFs and determine the overall incidence and best management strategies for IF nonunion. METHODS: There were five electronic databases (Medline/PubMed, Scopus, Web of Scienc

Evidence of venous stasis after abdominal insufflation for laparoscopic cholecystectomy.

Intraoperative venous stasis may increase the risk for perioperative deep vein thrombosis and pulmonary embolism. To determine if abdominal insufflation during laparoscopic cholecystectomy causes venous stasis, eight patients undergoing this procedure had their left common femoral veins examined by a duplex scanner before and after abdominal insufflation; the veins then were examined again before and after deflation. The right femoral veins were catheterized to measure femoral venous pressures. Abdominal insufflation to 14 millimeters of mercury pressure increased femoral venous pressures (10.

The effect of percutaneous intervention on wound healing in patients with mixed arterial venous disease.

BACKGROUND: Open venous ulcers in patients with combined arterial and venous insufficiency are notoriously hard to treat. Patients with an ankle-brachial index (ABI) of 0.5-0.8 have been shown to heal poorly. Because adequate compression therapy is contraindicated in patients with an ABI of <0.7, we decided to undertake an aggressive approach of percutaneous revascularization for these patients. METHODS: A total of 27 patients with clinical and duplex scan evidence of chronic venous insufficiency, active leg ulcers, and impaired arterial perfusion (ABI: <0.7) were treated using a protocol tha

Pulse oximetry index: a simple arterial assessment for patients with venous disease.

OBJECTIVE: To provide additional safety data comparing ankle brachial pressure index (ABPI) and pulse oximetry (Lanarkshire Oximetry Index, LOI) as measures of arterial circulation in patients with venous disease of the leg. METHOD: A total of 107 (195 legs) attending hospital leg ulcer clinics participated in this prospective open study. We attempted to measure brachial and foot arterial pressures in all patients using both the handheld Doppler method (ABPI) and pulse oximeter method (LOI). Features of patients with limbs in which either the ABPI or LOI could not be assessed were documented.

Influence of a metaphyseal sleeve on the stress-strain state of a bone-tumor implant system in the distal femur: an experimental and finite element analysis.

BACKGROUND: Aseptic loosening of distal femoral tumor implants significantly correlates with the resection length. We designed a new "sleeve" that is specially engaged in the metaphysis at least 5 cm proximal to the knee joint line to preserve as much bone stock as possible. This study investigates the influence of a metaphyseal sleeve on the stress-strain state of a bone tumor implant system in the distal femur. METHODS: Cortex strains in intact and implanted femurs were predicted with finite element (FE) models. Moreover strains were experimentally measured in a cadaveric femur with and wit

Reverse distal femoral locking compression plate a salvage option in nonunion of proximal femoral fractures.

BACKGROUND: When primary fixation of proximal femoral fractures with implants fails, revision osteosynthesis may be challenging. Tracts of previous implants and remaining insufficient bone stock in the proximal femur pose unique problems for the treatment. Intramedullary implants like proximal femoral nail (PFN) or surface implants like Dynamic Condylar Screw (DCS) are few of the described implants for revision surgery. There is no evidence in the literature to choose one implant over the other. We used the reverse distal femur locking compression plate (LCP) of the contralateral side in such

Incidence and management of pulmonary embolism following spinal surgery occurring while under chemical thromboprophylaxis.

Patients undergoing spinal surgery are at risk of developing thromboembolic complications even though lower incidences have been reported as compared to joint arthroplasty surgery. Deep vein thrombosis (DVT) has been studied extensively in the context of spinal surgery but symptomatic pulmonary embolism (PE) has engaged less attention. We prospectively followed a consecutive cohort of 270 patients undergoing spinal surgery at a single institution. From these patients, only 26 were simple discectomies, while the largest proportion (226) was fusions. All patients received both low molecular weig

Approach to leg edema of unclear etiology.

A common challenge for primary care physicians is to determine the cause and find an effective treatment for leg edema of unclear etiology. We were unable to find existing practice guidelines that address this problem in a comprehensive manner. This article provides clinically oriented recommendations for the management of leg edema in adults. We searched on-line resources, textbooks, and MEDLINE (using the MeSH term, "edema") to find clinically relevant articles on leg edema. We then expanded the search by reviewing articles cited in the initial sources. Our goal was to write a brief, focused

Large leg of superficial venous stasis, excluding disorders of the deep venous circulation.

Any abnormal increase in the volume of a lower member may be defined as "swollen leg", whether it is general and segmentary, or partial and local. This work concentrates on swollen legs caused by primary varicose disorders and does not deal with obstructions in the deep venous trunks. Swollen leg due to varicose disorder is marked by huge varices in clusters with total avalvulation. This constitutes a "phantom" swollen leg, for the evidence disappears when the patient lies down. It usually involves the long saphenous vein. It can be treated efficiently by surgery or sclerotherapy. Swollen leg

Relationships between preventive activities, psychosocial factors and recurrence of venous leg ulcers: a prospective study.

AIM: The aim of this study was to identify relationships between preventive activities, psychosocial factors and leg ulcer recurrence in patients with chronic venous leg ulcers. BACKGROUND: Chronic venous leg ulcers are slow to heal and frequently recur, resulting in years of suffering and intensive use of healthcare resources. METHODS: A prospective longitudinal study was undertaken with a sample of 80 patients with a venous leg ulcer recruited when their ulcer healed. Data were collected from 2006 to 2009 from medical records on demographics, medical history and ulcer history; and from sel

Indications for catheter-directed thrombolysis in the management of acute proximal deep venous thrombosis.

Deep vein thromboses (DVTs) cause significant morbidity and mortality in the general population. Oral anticoagulation therapy may reduce thrombus propagation but does not cause clot lysis and therefore does not prevent postthrombotic syndrome (PTS). Catheter-directed thrombolysis (CDT) can be used to treat DVTs as an adjunct to medical therapy, but there is no consensus defining exact indications. Current evidence suggests that CDT can reduce clot burden and DVT recurrence and consequently prevents the formation of PTS compared with systemic anticoagulation. Appropriate indications include you

Postoperative periprosthetic fractures associated with compressive osseointegration: a systematic review of the Zimmer Biomet Compress® device.

The Zimmer Biomet Compress® Compliant Pre-Stress (CPS) device uses a compressive osseointegration technique for endoprosthetic fixation and provides an alternative to stemed implants. Despite having benefits, such as bone stock retention, periprosthetic fractures continue to remain a concern. Therefore, this systematic review presents the prevalence, risk factors, and outcomes of the CPS device and periprosthetic fractures after treatment. Medical databases (PubMed, Embase, and Google Scholar) were searched to identify studies reporting complications related to the CPS device. Peer-reviewed st

Alcohol embolization versus non-invasive treatment for pain relief in peripheral venous malformations: a comparative study.

BACKGROUND: Pain is one of the most important symptoms affecting quality of life in patients with venous malformations. Alcohol embolization is a common approach among other treatment modalities. However, the benefits and complications of this therapy require rigorous evaluation in the face of emerging alternative treatments. The aim of this study was to evaluate the efficacy of alcohol embolization in pain management compared with non-invasive treatment, and to explore associated complications. METHODS: We performed a retrospective, comparative study at the Vascular Malformation Center, Univ
