BACKGROUND: Diagnosis of vestibular disorders relies heavily on clinical history, yet how patients adapt their behaviour in response to dizziness remains relatively unexplored. We investigated whether adopted avoidance behaviours and coping strategies provide additional diagnostic information and insights into disease mechanisms.
METHODS: Consecutive patients attending a specialist dizziness clinic underwent a structured interview before clinical assessment. Participants reported dizziness triggers, avoidance behaviours, and coping responses. Final diagnoses were established independently by

INTRODUCTION: Dizziness is a common symptom that can occur in an unpredictable and episodic manner leading to the imprecise reporting of symptoms. Patients will often see many specialists before receiving a diagnosis and treatments can vary in terms of risk and invasiveness which places a significant burden on health services. Achieving an early precise diagnosis could be key in reducing the impact of symptoms on patients and health services.
METHODS AND ANALYSIS: The continuous ambulatory vestibular assessment (CAVA) trial is a single-arm, non-randomised, multicentre diagnostic accuracy devi

OBJECTIVE: The caloric test (C-test) and video head impulse test (vHIT) are known to occasionally show contradictory results in patients with Meniere's disease (MD). The reasons underlying this discrepancy between the two tests are currently unclear. We aimed to reveal the mechanisms responsible for this discrepancy by performing volumetric evaluation of the endolymphatic space (ELS) by using endoluminal contrast-enhanced inner ear MRI (ieMRI).
METHODS: We enrolled 136 patients (174 ears) who visited the vertigo/dizziness center of our university and underwent the C-test and vHIT between Febr

During pregnancy, women experience substantial anthropometric, cardiovascular, hormonal and psychological changes that affect several organs involving the circulatory, respiratory, gastrointestinal, musculoskeletal, dermatological and sensory systems. The main aim of this review was to analyse the available literature on postural strategies throughout pregnancy in both static and dynamic conditions. The secondary aim was to assess and discuss the current knowledge regarding vestibular disorders during pregnancy. Pregnant women with vestibular disorders need appropriate and safe treatments to r

OBJECTIVE: The aim of this work was to assess through a questionnaire the features of vertiginous episodes, accompanying symptoms, familial history, and migraine precursors in a sample of 252 subjects with a diagnosis of definite vestibular migraine.
BACKGROUND: Migraine is a common neurological disorder characterized by episodic headaches with specific features. About two-thirds of cases run in families, and patients may refer symptoms occurring in infancy and childhood, defined as episodic syndromes that may be associated with migraine. Migraine is associated with episodic vertigo, called v

ObjectiveTo explore the clinical characteristics of patients with dizziness/vertigo who showed a dissociation between the results of the caloric test and video head impulse test (vHIT).MethodsA total of 327 patients who complained of dizziness/vertigo were continuously included. All patients underwent both the horizontal vHIT (h-vHIT) and caloric tests. Of the 327 patients, 69 patients showed a dissociation between the results of the two tests, 4 patients were excluded because the interval between the two tests exceeded 7 days. Finally, 65 patients were included in the analysis.ResultsAmong th

BACKGROUND: Exposure to environmental chemicals has been suggested to alter the physiologic state of the inner and middle ear. However, it is unknown if particulate matter exposure is associated with acute vestibular dysfunction.
OBJECTIVES: To estimate the effects of particulate matter exposure on the number of hospital visits related to three major diseases of vestibular dysfunction, Meniere's disease (MD), benign paroxysmal positional vertigo (BPPV), and vestibular neuronitis (VN).
METHODS: Our study subject is from Korean National Health Insurance Service-National Sample Cohort, which is

The aims of this study were to identify the most common vestibular syndromes in a dizziness unit, and to observe their clinical aspects and response to treatment. Five hundred and fifteen patients were studied retrospectively in two institutions. Aspects of anamnesis, physical examination and the response to treatment were evaluated. The most frequent syndromes were: benign paroxysmal positioning vertigo (VPPB) (28.5%), phobic postural vertigo (11.5%), central vertigo (10.1%), vestibular neuritis (9.7%), Meniere disease (8.5%), and migraine (6.4%). A good response to treatment was observed in

BACKGROUND: Caloric testing and the video head impulse test (vHIT) assess vestibular function at different frequency ranges and provide complementary information in patients with acute vertigo. Although a canal paresis (CP) threshold of 25% is commonly used to define caloric abnormality, interpreting CP values in relation to vHIT-defined unilateral high-frequency vestibular dysfunction remains clinically challenging.
OBJECTIVE: We evaluated how conventional and alternative caloric CP thresholds align with vHIT-defined acute unilateral vestibular dysfunction and sought an interpretive CP thres

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is a common cause of vertigo encountered in general practice. The definitive diagnosis relies on the Dix-Hallpike test (DHT), which remains underperformed in primary care due to limited training and unfamiliarity among general practitioners.
AIM: To develop the color-bar bedsheet as a simple visual positioning guide for DHT and demonstrate its usefulness.
DESIGN AND SETTING: After the color-bar bedsheet was developed, a prospective within-subject comparative design in healthy participants was conducted.
METHODS: Healthy adult subjects

BACKGROUND: Dizziness is a highly prevalent and often disabling complaint that cannot always be explained by peripheral vestibular pathology alone. Psychological factors, particularly psychological flexibility and resilience, may play an important role in shaping symptom severity and disability, yet their combined contribution remains insufficiently explored. This study aimed to examine how psychological flexibility and resilience contribute to the clinical expression of dizziness-related disability.
METHODS: Ninety-eight patients with dizziness (benign paroxysmal positional vertigo, persiste

BACKGROUND: There is limited evidence that anti-vertigo drugs (AVDs) are effective in patients with vestibular symptoms. Still, betahistine is one of the most frequently prescribed off-label drugs. GPs are likely to contribute substantially to these potentially inappropriate prescriptions.
AIM: To evaluate the frequency of (long-term) AVD prescriptions in primary care and characteristics associated with long-term prescriptions.
DESIGN & SETTING: We conducted a retrospective observational cohort study using anonymised routine primary care data from ≥1.2 million patients registered at 269 gene

OBJECTIVE: To characterize migraine pathophysiology, presentation, and current treatment strategies, specifically in regard to vestibulocochlear manifestations of migraine.
METHODS: Narrative review of available literature.
RESULTS: Migraine disorder can be described as a spectrum of otologic manifestations, with vestibular migraine now recognized with fully-fledged diagnostic criteria. Otologic manifestations are theorized to be due, in part, to trigeminal innervation of the inner ear structures and calcitonin gene-related peptide (CGRP) expression within the labyrinth. Patients can experie

Despite the huge progress in the definition and classification of vestibular disorders within the last decade, there are still patients whose recurrent vestibular symptoms cannot be attributed to any of the recognized episodic vestibular syndromes, such as Menière's disease (MD), vestibular migraine (VM), benign paroxysmal positional vertigo (BPPV), vestibular paroxysmia, orthostatic vertigo or transient ischemic attack (TIA). The aim of the present international, multi-center, cross-sectional study was to systematically characterize the clinical picture of recurrent vestibular symptoms not ot

Acute vestibulopathy is characterized by the acute or subacute onset of vertigo, dizziness or imbalance with or without ocular motor, sensory, postural or autonomic symptoms and signs, and can last for seconds to up to several days. Acute vestibular lesions may result from a hypofunction or from pathological excitation of various peripheral or central vestibular structures (labyrinth, vestibular nerve, vestibular nuclei, cerebellum or ascending pathways to the thalamus and the cortex). This update focuses on new aspects of the aetiology, pathophysiology, epidemiology, and treatment of (i) acut

Dizziness is not an uncommon complaint in the Otolaryngological clinics among other symptoms. To a large number of practitioners, the treatment of dizziness remains the same irrespective of the etiology, i.e., anti-vertiginous drugs. The objective of this study was to document the evaluation, causes and treatment of vestibular disorders among our patients. The design includes descriptive prospective study conducted in the Oto-rhino-laryngology and Orthopedic Clinics of Lagos University Teaching Hospital, Lagos and Nigerian Army Audiological Centre, 68 Nigerian Army Reference Hospital, Yaba, La
