Body mass index and quality of bowel preparation: Real life vs. clinical trials.

BACKGROUND AND STUDY AIMS: Obesity is a recognised risk factor for poor bowel preparation in retrospective studies whilst corresponding data in prospective trials are marginally reported. Aims are to evaluate the relation between body mass index (BMI) and preparation quality in retrospective and interventional prospective settings and within a single centre. PATIENTS AND METHODS: Data from a recent colorectal cancer screening registry were retrospectively analysed for the relation between BMI and adequacy of preparation. Patients were categorised as underweight (BMI<20kg/m(2)), normal (20-25k

Diabetics on Narcotics Are Less Likely to Achieve Excellent Bowel Preparation Than Are Patients with Either Condition.

BACKGROUND: Diabetes and chronic narcotic use negatively affect the quality of bowel preparation before colonoscopy. AIM: To investigate whether narcotic use and diabetes have an additive negative impact on bowel preparation. PATIENTS AND METHODS: We performed a retrospective study of 2841 patients (mean age 61 ± 10.2; 94% male) who received outpatient colonoscopies at our Veterans Affairs Medical Center between June 2012 and December 2014. We collected information related to demographics, body mass index, indications, and medical/surgical history (diabetes mellitus, stroke, cirrhosis, demen

The mechanism underlying the laxative properties of parsley extract.

Parsley has been claimed in folk medicine to possess laxative properties attributed to the presence therein of some volatile oils that are more concentrated in seeds than in stems or leaves. The advocated physiological effect of parsley, does not have, however, any proven scientific background and relies mainly on simple observations and empirical information. This work aims at providing the scientific evidence that would confirm or reject the claimed laxative role of parsley, and at determining its mechanism of action if present. A perfusion technique was used to measure the net fluid absorpt

Efficacy of rifaximin on symptoms of uncomplicated diverticular disease of the colon. A pilot multicentre open trial. Diverticular Disease Study Group.

Diverticular disease of the colon is a common health problem in western societies. Most patients with colonic diverticula are asymptomatic; it has been estimated that only 20% of individuals harboring diverticula will develop symptoms and signs of illness and a minority will develop major complications. Although the efficacy of a high fiber diet in the management of symptomatic uncomplicated diverticular disease is still controversial, bran and bulking agents are commonly used. Antibiotics are used to treat major inflammatory complications of diverticular disease but apparently there is no rat

Efficacy of 1 L PEG Ascorbate vs Sodium Picosulfate With Magnesium Citrate Bowel Preparations for Capsule Colonoscopy and Pan-Intestinal Capsule Endoscopy: A Prospective Study.

INTRODUCTION: Capsule colonoscopy (CC) and pan-intestinal capsule endoscopy ("panendoscopy") require effective bowel cleansing; however, evidence on ultra-low-volume regimens is lacking. This study compared the efficacy of 1 L polyethylene glycol with ascorbic acid (PEG-Asc) vs sodium picosulfate with magnesium citrate (SPMC) in CC and panendoscopy procedures. METHODS: In this prospective, single-center, researcher-blinded, randomized trial, 220 consecutive adult outpatients referred for CC or panendoscopy were assigned 1:1 to receive split-dose PEG-Asc or SPMC. Primary outcomes were adequate

Unintended consequences of antibiotic practices: the relationship between bowel preparation, prolonged prophylaxis, and ileus after radical cystectomy.

OBJECTIVES: Postoperative ileus (POI) after radical cystectomy (RC) is a common complication that carries significant morbidity. Prolonged antibiotic usage has been associated with an elevated risk of POI. The American Urological Association guidelines recommend that only a single dose of peri-procedural antibiotic prophylaxis is sufficient. This study aims to evaluate the association between oral antibiotic bowel preparation (ABP) and prolonged surgical antibiotic prophylaxis (SAP) (>24 h) with postoperative ileus (POI) following radical cystectomy (RC). MATERIALS AND METHODS: A retrospectiv

Transitioning barium enema preparation protocols to CT colonography in the modern imaging era.

PURPOSE: To evaluate the feasibility of applying a barium enema-style bowel preparation protocol to CT colonography (CTC) in clinical practice. MATERIALS AND METHODS: 11 patients underwent CTC using a simplified bowel preparation protocol based on magnesium citrate, similar to that used for barium enema. Two radiologists assessed the homogeneity of fluid tagging, volume of residual fluid, and degree of colonic distension in six colonic segments (cecum, ascending, transverse, descending, sigmoid, rectum) in both supine and prone positions. Each parameter was rated on a 4-point Likert scale (0

Multimodal patient education improves bowel preparation for colonoscopy: a randomized controlled trial.

BACKGROUND: Inadequate bowel preparation (BP) compromises colonoscopy quality, efficiency, and patient outcomes. We evaluated whether a resource-light multimodal educational package improves BP adequacy compared with standard instructions. METHODS: We conducted a prospective, single-blinded, parallel-group, single-center randomized clinical trial in which adults scheduled for elective colonoscopy were randomized 1:1 to standard education (leaflet) or enhanced education (face-to-face counseling, 6-minute video, illustrated booklet, smartphone-app reminders). The primary outcome was adequate pre

Adenoma detection rate and tolerability of 2 ultra-low-volume bowel preparations in screening: a noninferiority randomized controlled trial.

BACKGROUND AND AIMS: The adenoma detection rate (ADR), recognized as a surrogate marker for colorectal cancer (CRC) incidence and mortality reduction, is closely linked to the efficacy of bowel cleansing. However, there is a dearth of evidence examining the impact on ADR when using 2 distinct very-low-dose bowel cleansing products. This study sought to compare ADR in an immunochemical fecal occult blood test (iFOBT)-based organized screening program by using 1 L of polyethylene glycol plus ascorbate (1L-PEGA) versus sodium picosulfate with magnesium citrate (SPMC), both administered in a split

Ischaemic colitis associated with oral contraceptive and bisacodyl use.

A woman with a history of hip replacement presented 3 days prior to revision of the hip with abdominal pain, diarrhoea and haematochezia. These symptoms began 6 h after she began taking bisacodyl in preparation for her upcoming surgery. She was on low-dose estradiol for hormone replacement therapy (HRT). Subsequent colonoscopy and biopsies were consistent with acute colonic ischaemia (CI). She was treated with intravenous fluids and antibiotics and discharged and told to stop HRT and bisacodyl. Follow-up colonoscopy 1 month after discharge was normal. This case adds to the three other previous

Bowel preparation for colonoscopy: enema versus sodium phosphate.

BACKGROUND: Colonoscopy is considered to be the gold standard investigation for assessing the colonic mucosa. Good bowel preparation is essential in order to achieve optimal visualisation of the mucosa. Traditionally water enema is used for bowel preparation in most centres in Nigeria. This prospective study was performed at the Gastrointestinal Endoscopy Unit of Obafemi Awolowo University Teaching Hospital Complex, Ile-Ife, Nigeria between July 2008 and June 2009. AIMS AND OBJECTIVES: This study compared patients' tolerability, adverse effects, efficacy and mean duration of colonoscopy of wa

Comparison of oral polyethylene glycol plus a large volume glycerine enema with a large volume glycerine enema alone in patients undergoing colorectal surgery for malignancy: a randomized clinical trial.

AIM: Recent meta-analyses and randomized clinical trials have concluded that mechanical bowel preparation (MBP) before elective colorectal surgery is not associated with a reduction of surgical site infection (SSI). The aim of this randomized clinical trial was to evaluate the impact of preoperative MBP for colon and rectal cancer surgery in comparison with a single glycerine enema. METHOD: Patients scheduled for radical colorectal resection for malignancy with primary anastomosis were randomized to preoperative MBP (4 l of polyethylene glycol) (group 1, 114 patients) plus a glycerine 5% enem

Bowel preparation: comparing metabolic and electrolyte changes when using sodium phosphate/polyethylene glycol.

BACKGROUND: Many patients with various types of colonic pathology undergo invasive procedures that require mechanical bowel preparation. The most commonly used medications for bowel preparation include phosphate-containing drugs which are low cost and enable this procedure to be performed in an outpatient setting, as opposed to other medications, such as polyethylene glycol. Recent studies have suggested that freely using phosphate-containing drugs might lead to renal function impairment in a small group of patients. Despite this, many surgeons still use these drugs to prepare their patients.
