A new 3-phase therapy protocol in temporomandibular joint ankylosis: our experience.

Limitations in oral opening may be due either to intra-articular or extra-articular ankylosis (pseudoankylosis). The principal means of therapy usually consist of 2 steps: surgical removal of the ankylotic block and immediate functional rehabilitation. In the postoperative period, however, immediate and adequate functional rehabilitation is not always possible because of pain and swelling of the temporomandibular joint, resulting in a very high risk of relapse. To prevent this, the authors introduce a third phase between the 2 already mentioned (surgery and functional therapy) in which the ora

A 10-year report from a multicentre randomised controlled trial: Immediate non-occlusal versus early loading of dental implants in partially edentulous patients.

PURPOSE: To compare peri-implant bone and soft-tissue levels of immediate non-occlusally loaded versus non-submerged early loaded implants in partially edentulous patients 10 years after loading. MATERIALS AND METHODS: Fifty-two patients were randomised in five Italian private practices: 25 in the immediately loaded group and 27 in the early loaded group. To be immediately loaded, single implants had to be inserted with a torque of at least 30 Ncm, and splinted implants with a torque of at least 20 Ncm. Immediately loaded implants were provided with non-occluding temporary restorations within

Nonunion of the external maxillary distraction in cleft lip and palate: analysis of possible reasons.

PURPOSE: Distraction osteogenesis (DO) is an effective way to treat severe maxillary deficiency in patients with cleft lip and palate. It has been reported to have long-term stability and low relapse rate compared with conventional maxillary osteotomy in large maxillary movement. However, complications such as nonunion are seldom reported. While developing our external distraction techniques, we had 4 nonunion cleft lip and palate (CLP) patients. The aim of this study is to analyze the possible reasons for these conditions by comparing them to patients successfully treated at 1 craniofacial in

Self-perceived effects of occlusal appliance therapy on TMD patients: an eight-year follow-up.

There are few long-term follow-up studies of treatment of temporomandibular disorders (TMD).The aim of this questionnaire study was to evaluate eight-year outcomes of appliance therapy in patients suffering from arthralgia/osteoarthritis and/or myofascial pain. The subjects comprised 120 patients, originally randomly assigned to treatment with an occlusal or a control (palatal) appliance. Eight years later, a questionnaire was sent to 118 eligible patients: 90 (76%) responded. The outcome measures were intensity and frequency of pain, physical and emotional functioning, and overall improvement

Surgical-orthodontic treatment of patients suffering from severe periodontal disorders - a clinical case study.

INTRODUCTION: Orthodontic or, more precisely, surgico-orthodontic treatment of patients suffering from periodontal disorders generally requires a multidisciplinary approach by a qualified dental team. Periodontal bone healing is an essential factor for successful orthodontic treatment in a compromised periodontal situation. CLINICAL CASE: We report on the case of an adult patient suffering from severe chronic periodontitis; he was a hyperdivergent skeletal Class III with dento-alveolar compensation, esthetic problems and a significant lack of dental material. A multidisciplinary approach was

Diagnosis and treatment of temporomandibular disorders.

Current concepts and recommended treatment for temporomandibular disorders (TMDs) and temporomandibular joint pain and dysfunction have evolved over time. This article attempts to distill the current information for this often confusing topic into relevant clinical issues that will allow the general dental practitioner to be better able to diagnose and interpret clinical findings, and institute a therapeutic regimen that will provide needed relief to patients suffering from TMD dysfunction. Current management methods, both surgical and nonsurgical, are reviewed and discussed.

Surgical treatment modality for facial esthetics in an obstructive sleep apnea patient with protrusive upper lip and acute nasolabial angle.

For patients with severe obstructive sleep apnea syndrome (OSAS), maxillomandibular advancement (MMA) offers a good treatment option by physically expanding the skeletal framework. However, facial esthetics can be aggravated by MMA in patients with OSAS who have a normally positioned maxilla, a protrusive upper lip, and an acute nasolabial angle. Therefore, surgical treatment planning should be customized according to diverse skeletodental and soft-tissue patterns to produce a favorable change in facial esthetics and sleep function in patients with OSAS. In this case report, good treatment res

Reliability of Digital Artery Palpation and Pulse Oximetry Waveform As Alternatives to Radial Pulse in the Assessment of Upper Limb Injuries.

INTRODUCTION: Radial pulse palpation is widely accepted as a gold standard clinical method to assess distal vascular perfusion of the upper limb. In some instances, the radial pulse may not be accessible due to splints, casts, or swelling, or the injury may be at the level or distal to the radial artery. Here, the authors assess two alternative methods of assessing perfusion of the hand more distally: palpation of the digital pulse and pulse oximetry (PO) waveform. METHODS: Twenty-four healthy adult volunteers (48 hands) were assessed by two assessors. Digital artery pulses were palpated, and
