Impact of chronic alcohol consumption on inflammatory response and periapical bone resorption in induced apical periodontitis: a systematic review of animal studies.

Alcohol consumption is linked to reduced bone mass and strength due to an imbalance in bone remodeling, characterized by decreased bone formation and an increased inflammatory response. Apical periodontitis affects the tissues surrounding the teeth and may also influence the systemic immune response by altering inflammatory marker levels. This systematic review examined how alcohol consumption affects the severity of experimental apical periodontitis in animal models. Following PRISMA guidelines, studies were sourced from three databases (PubMed, Scopus, and Web of Science) up to January 26, 2

Unveiling dental diagnostic dilemmas: a national survey of US dentists.

BACKGROUND: In dentistry, very little is known about diagnostic errors (DEs) despite their critical role in assessing patient safety. Many DE cases significantly impact the patient’s quality of life and daily function without necessarily causing medical harm. The primary goal of this study was to describe dentists past experiences or observations of DEs and their associated characteristics. METHODS: We conducted a national cross-sectional study of US dentists (May-June, 2021). The primary outcomes were self-reported personal experiences of DEs and observations of DEs made by other dentists. Se

Dens evagination and complete invagination in the same tooth with extraoral fistula: A rare case report.

KEY CLINICAL MESSAGE: Successful management of a rare case involving both dens evaginatus and dens invaginatus in the same tooth, monitored over a 24-month follow-up. ABSTRACT: Dens invaginatus (DI) is a congenital dental anomaly characterized by the presence of a tooth that resembles a "tooth within a tooth." Conversely, dens evaginatus (DE) is a developmental anomaly distinguished by an additional tubercle or cusp on the tooth's crown. Both DI and DE can result in pulpal and periapical diseases in the affected tooth. This article presents a case of a healthy 14-year-old male with recurrent

Sodium hypochlorite accident diagnosis and management: Analysis from the literature and the French pharmacovigilance database.

PURPOSE: Sodium hypochlorite (NaOCl) is considered as the reference irrigation solution in endodontics. However, NaOCl-related accidents may occur, and non-dentist health professionals might under-recognize this rare adverse effect although it is potentially severe, with possible medical and aesthetic sequelae. We performed a literature review to provide to non-dentist healthcare professionals a large picture of symptoms, management and potential consequences of NaOCl accidents. METHODS: We queried PubMed and the French Pharmacovigilance database and retrieved 76 cases for analysis (70 from 5

Long term outcomes of en-block autotransplantation of a tooth.

Repositioning of impacted teeth in the dental arch through en-bloc autotransplantation showed short-term efficacy of 89%. The objective of this study was to evaluate the technique for long-term efficacy. The analysis involved 23 teeth transplanted to the alveolar ridge through harvesting the tooth with the adjacent bone tissue. Development of the root was completed and the apical foramen was closed in all the teeth. The teeth were examined clinically and radiologically (CBCT) and compared to contralateral ones. The obtained data were analysed statistically. The efficacy of the procedure amount

Changes in the craniofacial complex and alveolar bone height of young adults: Applications to dental medicine.

The few studies on craniofacial complex changes in adults have reported contradictory findings. The aim of this study was to radiographically evaluate changes in the craniofacial complex and alveolar bone height of young adults over a 4-year period. This prospective study included 82 young adults (62 men; mean age, 19.0 ± 0.8 years; 20 women; mean age, 18.8 ± 0.9 years). Three radiographic examinations were performed longitudinally: the first (T0) was conducted at the start of the study, the second (T1) was conducted 2 years later, and the third (T2) was conducted at the end of the study perio

Accuracy of conventional and digital radiography in detecting external root resorption.

INTRODUCTION: External root resorption (ERR) is associated with physiological and pathological dissolution of mineralized tissues by clastic cells and radiography is one of the most important methods in its diagnosis. The aim of this experimental study was to evaluate the accuracy of conventional intraoral radiography (CR) in comparison with digital radiographic techniques, i.e. charge-coupled device (CCD) and photo-stimulable phosphor (PSP) sensors, in detection of ERR. METHODS AND MATERIALS: This study was performed on 80 extracted human mandibular premolars. After taking separate initial p

Protective role of osteopontin in endodontic infection.

Endodontic infections are polymicrobial infections resulting in bone destruction and tooth loss. The host response to these infections is complex, including both innate and adaptive mechanisms. Osteopontin (OPN), a secreted, integrin-binding protein, functions in the regulation of immune responses and enhancement of leucocyte migration. We have assessed the role of OPN in the host response to endodontic infection using a well-characterized mouse model. Periapical bone loss associated with endodontic infection was significantly more severe in OPN-deficient mice compared with wild-type 3 weeks a

An in vitro study of coronal leakage after intraradicular preparation of cast-dowel space.

STATEMENT OF PROBLEM: Coronal leakage can produce contamination of periapical tissues, resulting in endodontic failure. PURPOSE: The purpose of this in vitro study was to evaluate the ability of 2 sealers to prevent coronal leakage in canals filled with gutta-percha and prepared for cast dowels but without coronal sealing. MATERIAL AND METHODS: The crowns of 60 extracted single-rooted teeth were amputated. The root canals were prepared corono-apically and filled with gutta-percha cones and 1 of 2 different endodontic sealers: a resin-based sealer (AH Plus) and a calcium hydroxide-based seale

Recent development in endodontic research.

In essence, endodontics as a clinical discipline is concerned with the prevention and treatment of pulpal and periapical infection. In recent research the infective process has been investigated as well as the mechanisms by which the pulp and periodontium deal with microbial insults. With regard to the pulp, findings on the hemodynamics of pulpitis suggest that the inflammatory response in this tissue is much less influenced by the special anatomic environment of the tooth than was previously believed. Pulpal diseases are being underdiagnosed, mostly because of inadequate examination methods.

Clinical considerations in the diagnosis and treatment of intra-alveolar root fractures.

Clinical and radiographic data collected from the 14 cases of intra-alveolar root-fractures, with follow-ups of one to 25 years, complement the findings of other investigators in that the prognosis of the endodontium is extremely favorable. A survey of reports indicate that 75% to 80% of the pulps in intra-alveolar root fractures recover with no need of any endodontic therapy. Furthermore, pulpal pain is seldom encountered. Although the electric pulp tester is considered an important diagnostic tool, it does have limitations. The significance of the negative response is questionable, especiall

Comparison of Crestal Bone Loss and Peri-Implant Clinical Outcomes Between Platform-Switched and Platform-Matched Dental Implants: A Prospective Observational Clinical Study.

Introduction Preservation of the crestal bone is essential for the long-term success of dental implants. Platform switching has been proposed to minimize marginal bone loss by shifting the implant-abutment interface away from the crestal bone and improving stress distribution. This study aimed to compare the crestal bone loss and peri-implant clinical outcomes between platform-switched and platform-matched dental implants over a 12-month follow-up period. Materials and methods This prospective longitudinal observational study included 90 patients undergoing implant-supported rehabilitation, wi

Differences in Inflammation and Bone Resorption between Apical Granulomas, Radicular Cysts, and Dentigerous Cysts.

INTRODUCTION: Dental cysts can be of inflammatory (radicular cysts) or noninflammatory (dentigerous cysts) origin. Apical periodontitis is a necrosis of the pulp and infection of the root canal causing the development of apical granulomas or radicular cysts. The immunology of granuloma and cyst formation is important because modern root filling materials are immunologically active and can contribute to the resolution of apical granulomas. In contrast, radicular cysts often require apicectomy. A better understanding of the pathophysiology of inflammation and bone resorption in apical periodonti

The management of thermomechanically compacted gutta percha extrusion in the inferior dental canal.

Endodontic material inadvertently forced into the inferior dental canal during root-canal therapy can cause damage to the underlying nerve. The effect of toxic filling materials on nervous tissues has been well publicised, however, the thermal and pressure changes produced by chemically bland materials has not been adequately highlighted. In the case reported, thermoplasticised gutta percha was inadvertently introduced into the canal during endodontic treatment of a lower molar. Factors affecting the outcome are discussed and guidelines are presented for the management of such a case with refe
