Rufloxacin once daily in acute exacerbations of chronic bronchitis.

In this open study the efficacy and tolerability of rufloxacin in a single dose of 400 mg the first day and 200 mg the nine consecutive days was studied in 26 patients with an acute exacerbation of chronic bronchitis. Twenty-two patients were evaluable for efficacy. Four patients stopped treatment prematurely after five days because of clinical cure. At the enrollment visit a pathogen was isolated in the sputum sample in 19 of 22 evaluable patients. The predominant pathogens were Streptococcus pneumoniae and Moraxella catarrhalis. In 17 of these 19 bacteriologically evaluable patients the init

Rapid Synthesis of Covalent Organic Framework at Room Temperature Incorporated Electrospun Nanofiber for Thin Film Microextraction of Quinolone Antibiotics in Honey and Chicken Samples.

In this work, the covalent organic frameworks-incorporated electrospun nanofiber membranes were used as a highly efficient adsorbent to enrich quinolone antibiotics in food samples. Covalent organic frameworks composed of 1,3,5-tris(4-aminophenyl) benzene and 2,5-dihydroxyterephthalaldehyde were rapid prepared only 20 min at room temperature, then were further synthesized into electrospun polyacrylonitrile nanofiber membranes by electrospinning the binary precursors solution directly. Coupled with high-performance liquid chromatography with ultraviolet detector, the method exhibited good linea

Newer concepts in antimicrobial therapy.

Antimicrobial agents continue to play a significant role in clinical practice not only due to their active role in the treatment of bacterially induced infections. The accompanying anti-inflammatory characteristics and their antagonism against superantigens add to their importance. The practitioner must also be aware of both overt and covert unwanted effects. During the past decade, the new quinolones, advanced macrolides, and better cephalosporins have been introduced. The staid penicillins have been up-graded with the addition of a beta-lactamase inhibitor. Many antibiotics have been availab

The effects of quinolones on xanthine pharmacokinetics.

Caffeine, theobromine, and theophylline are among the most widely consumed compounds in beverages and in pharmaceutical preparations. These methylxanthine alkaloids are metabolized by similar pathways involving demethylation and hydroxylation that are predominantly cytochrome P-450 mediated. In vivo and in vitro evidence suggests that the cytochrome P-450 isozymes involved in the demethylation pathways are distinct from the cytochrome P-450 isozymes involved in the hydroxylation pathways. Although distinctions can be made between demethylation and hydroxylation pathways, the evidence suggests

[Advances in the medical treatment of bacterial endophthalmitis].

Currently, the systemic antibiotics intraocular penetration's rate is low and we have to perform intraocular antibiotics injections to reach levels above the minimal inhibitory concentration of the bacterial agents found commonly in endophthalmitis. Authors try to specify a rational strategy based on pharmacokinetics and bacteriological aspects of a new class of antibiotics, the fluoroquinolones, using their own studies and those from other teams about the intraocular distribution of these antibiotics. Further investigation is mandatory to assess the safety and efficacy of our first clinical r

In vitro Antibacterial Activity of 7-Substituted-6-Fluoroquinolone and 7-Substituted-6,8-Difluoroquinolone Derivatives.

BACKGROUND: Fluoroquinolones are widely prescribed synthetic antimicrobial agents. Quinolones act by converting their targets, gyrase and topoisomerase IV, into toxic enzymes that fragment the bacterial chromosome; the irreversible DNA damage eventually causes the killing of bacteria. Thorough knowledge of the structure-activity relationship of quinolones is essential for the development of new drugs with improved activity against resistant strains. METHODS: The compounds were screened for their antibacterial activity against 4 representing strains using the Kirby-Bauer disk diffusion method.

Analytical profiling of mutations in quinolone resistance determining region of gyrA gene among UPEC.

Mutations in gyrA are the primary cause of quinolone resistance encountered in gram-negative clinical isolates. The prospect of this work was to analyze the role of gyrA mutations in eliciting high quinolone resistance in uropathogenic E.coli (UPEC) through molecular docking studies. Quinolone susceptibility testing of 18 E.coli strains isolated from UTI patients revealed unusually high resistance level to all the quinolones used; especially norfloxacin and ciprofloxacin. The QRDR of gyrA was amplified and sequenced. Mutations identified in gyrA of E.coli included Ser83Leu, Asp87Asn and Ala93G

Screening Hepatotoxic Components in Euodia rutaecarpa by UHPLC-QTOF/MS Based on the Spectrum-Toxicity Relationship.

Euodia rutaecarpa is a common traditional Chinese medicine (TCM) in clinical practice, having the ability to suppress pain and cease coughing; however, with the increasing reports showing that it is toxic, particularly hepatotoxic, the concerns raised by what cause its toxicity is growing. In the current study, an analysis method based on the spectrum effect has been employed to screen the major hepatotoxic components in Euodia rutaecarpa so that the toxic material's basis would be elucidated. A fingerprinting method of the Euodia rutaecarpa extracts (which were petroleum ether, chloroform, et

Immediate allergic hypersensitivity to quinolones associates with neuromuscular blocking agent sensitization.

BACKGROUND: We identified a case of quinolone allergic hypersensitivity associated with quaternary ammonium (QA) sensitization, the allergic determinant of neuromuscular blocking agents (NMBAs). Concomitant sensitization to several chemically different drugs is rarely reported and raises the question of a nonfortuitous association. OBJECTIVE: We evaluated a potential association between quinolone immediate allergic hypersensitivity and NMBA sensitization. METHODS: QA-specific IgE detection was prospectively performed in 26 patients who presented an immediate hypersensitivity reaction to quin

Mutational analysis of quinolone resistance in the plasmid-encoded pentapeptide repeat proteins QnrA, QnrB and QnrS.

OBJECTIVES: Pentapeptide repeat proteins (PRPs) QnrA, QnrB and QnrS confer reduced susceptibility to quinolones. This study presents an in vitro analysis of the genetic evolution of quinolone resistance mediated by changes in the coding sequences and promoter regions of qnrA1, qnrS1 and qnrB1 genes. METHODS: A random mutagenesis technique was used to predict the evolutionary potential of these PRPs against nalidixic acid and fluoroquinolones. After comparing the amino acid sequences of these and other PRPs protecting bacteria from quinolone activity, several conserved positions were found. Th

Quinolone resistance mechanisms among extended-spectrum beta-lactamase (ESBL) producing Escherichia coli isolated from rivers and lakes in Switzerland.

Sixty extended-spectrum β-lactamase (ESBL)-producing Escherichia coli isolated from rivers and lakes in Switzerland were screened for individual strains additionally exhibiting a reduced quinolone susceptibility phenotype. Totally, 42 such isolates were found and further characterized for their molecular (fluoro)quinolone resistance mechanisms. PCR and sequence analysis were performed to identify chromosomal mutations in the quinolone resistance-determining regions (QRDR) of gyrA, gyrB, parC and parE and to describe the occurrence of the following plasmid-mediated quinolone resistance genes: q

The effects of quinolones on the adherence of type-1 fimbriated Escherichia coli to mannosylated agarose beads.

Bacterial adherence is reported to be antagonized by several classes of antibiotics including quinolones, beta-lactams and tetracyclines, based primarily on in-vitro studies in which bacterial cells are exposed to antimicrobials, incubated in the presence of uroepithelial cells (UECs) and assessed for adherence by light microscopy. Some problems associated with the use of this approach, include low sensitivity, high variability and, in the case of adherence of mannose-sensitive Escherichia coli interference by mannose-containing uromucoid. To avoid these problems, mannosylated agarose beads (M

[Studies on susceptibility of isolated organisms from pediatric infections against various antimicrobial agents].

Twelve oral antimicrobial agents were tested for their antimicrobial activities against causative organisms isolated from pediatric infections. Activities of these antimicrobial agents against Streptococcus pyogenes were also examined in relation to T-antigen typing of the species. The results of the investigation are summarized as follows. 1. Against Staphylococcus aureus, rokitamycin (RKM), josamycin, ofloxacin, minocycline exhibited strong antimicrobial activities, and few strains of S. aureus showed resistance to these antimicrobial agents. More strains exhibited resistance to erythromycin

Topoisomerase Inhibitors Addressing Fluoroquinolone Resistance in Gram-Negative Bacteria.

Since their discovery over 5 decades ago, quinolone antibiotics have found enormous success as broad spectrum agents that exert their activity through dual inhibition of bacterial DNA gyrase and topoisomerase IV. Increasing rates of resistance, driven largely by target-based mutations in the GyrA/ParC quinolone resistance determining region, have eroded the utility and threaten the future use of this vital class of antibiotics. Herein we describe the discovery and optimization of a series of 4-(aminomethyl)quinolin-2(1H)-ones, exemplified by 34, that inhibit bacterial DNA gyrase and topoisomer

Total Synthesis and Antimalarial Studies of Caelestines A-C.

Caelestines A-D are 2-carboxy-4-quinolones (2C4Qs) first isolated from the Australian ascidian Aplidium caelestis. The caelestines are brominated analogues of kynurenic acid and xanthurenic acid, and the only brominated 2C4Qs isolated from nature. A convenient methodology exists for the Conrad synthesis of 2C4Qs from substituted anilines and acetylene dicarboxylates cyclised in Eaton's reagent. This approach yields regioisomers from asymmetric anilines and is hampered by the absence of chromatographic methods for isolation of pure isomers. Here, we report de novo synthesis of caelestines A-C u

[Effectiveness of new quinolones in experimental aerogenic plague infection of albino mice].

It was shown that unlike nalidixic acid the 3rd generation quinolones i.e. the nitrogen-containing quinolones (LIB-71 and LIB-80) and the fluorine-containing quinolones (pefloxacin and ciprofloxacin) were highly efficient in the prophylaxis and treatment of experimental plague in albino mice infected via the plague microbe inhalation. By the ED50 the efficacy of pefloxacin was 25-30 times higher than that of nalidixic acid and 60-90 times higher than that of ciprofloxacin. The fluorine-containing quinolones proved to be more active than the nitrogen-containing ones.

Use of pefloxacin after failure of initial antibiotic treatment in children with severe salmonellosis.

Sixteen children (7 males, 9 females) aged 0.1 to 9.5 years (mean 3.6 years) who were hospitalized for severe salmonella infection were treated with conventional antibiotics. In nine children disappearance of fever and bloody diarrhea was obtained 2 to 4 days after the beginning of antibiotic therapy. Although the antibiotic administered was active against the pathogen, after 4 to 8 days the seven other children experienced clinical failure of the initial therapy, with a temperature of over 39 degrees C, bloody diarrhea and positive stool culture in all cases. The seven Salmonella strains isol

Activity of quinolones against Chlamydia pneumoniae.

Quinolones are currently being used as empirical therapy for the treatment of community-acquired pneumonia and other respiratory infections as they cover a broad range of conventional bacterial and 'atypical' pathogens, including Chlamydia pneumoniae. C. pneumoniae has been associated with 10 to 20% of community-acquired pneumonia in adults and recently has been implicated as being associated with several nonrespiratory conditions, including atherosclerosis. However, data on the treatment of even respiratory infection due to C. pneumoniae are limited. Although currently available quinolones ha
