Human immunodeficiency virus (HIV) proteins in neuropathogenesis of HIV dementia.

Human immunodeficiency virus (HIV) infection of the nervous system is unique when compared with other viral encephalitides. Neuronal cell loss occurs in the absence of neuronal infection. Viral proteins, termed "virotoxins," are released from the infected glial cells that initiate a cascade of positive feedback loops by activating uninfected microglial cells and astrocytes. These activated cells release a variety of toxic substances that result in neuronal dysfunction and cell loss. The virotoxins act by a hit and run phenomenon. Thus, a transient exposure to the proteins initiates the neuroto

An update on selected organic mental syndromes.

The authors review recent literature on four medical conditions that constitute important examples of organic mental disorders: AIDS dementia complex, cocaine abuse, cerebrovascular accident, and traumatic brain injury. These disorders are of particular interest because they are frequently the result of high-risk behaviors and the patients are often young adults. Progress in classification and diagnosis, along with greater understanding of pathophysiological mechanisms, has facilitated therapies to ameliorate some symptoms. More specific classification of organic mental disorders should facili

Neurologic disorders associated with HIV infections.

At least 60% of patients infected with the human immunodeficiency virus (HIV) develop neurologic disorders. These may be the direct result of human immunodeficiency virus (HIV) infection, opportunistic infections, neoplastic disorders, or cerebrovascular complications. Neurologic diseases associated with HIV infection include encephalopathy, aseptic meningitis, vacuolar myelopathy, peripheral neuropathy, and myopathy. The pathogenesis of these diseases is not known, but it is likely that they will differ. There is evidence that HIV is the etiologic agent of HIV-associated meningitis and subacu

AIDS dementia.

The most frequently documented neurologic manifestation in human immunodeficiency virus (HIV) seropositive patients is known as acquired immunodeficiency syndrome (AIDS) dementia complex. Three stages of dementia progression have been identified with impairment noted in cognitive, behavioral and motor function. Specific diagnostic findings include diffuse cortical atrophy on computed tomography (CT) scan and magnetic resonance imaging (MRI) and alterations in cerebrospinal fluid (CSF) findings. Current research hypothesizes that AIDS dementia is the result of direct HIV infiltration of the cen

The Changing Spectrum of Cognitive Impairment in People with HIV; Establishment and Updates of the International HIV-Cognition Working Group.

With the widespread availability of effective antiretroviral therapy, the spectrum of cognitive impairment in people with HIV has changed from a subcortical dementia to milder impairment. The 2007 criteria for HIV-associated neurocognitive disorders (HAND) provided a methodological approach to classify milder and asymptomatic forms. Research using these criteria found that around 45% of people with HIV had HAND. This does not align with clinical observations that people with HIV supressed on modern treatment generally have good clinical outcomes, including cognitively. HAND criteria have sever

Update on neurological complications of HIV.

PURPOSE OF REVIEW: The prevalence of neurological complications among people with HIV (PWH) is expected to increase as PWH live longer due to increased access to antiretroviral treatment (ART). This review provides updates to the understanding of the neurological sequelae, including neurocognitive impairment, neuropathy, neurological opportunistic infections, and others, which are crucial for improving care and outcomes of PWH. RECENT FINDINGS: Recent literature highlights several key themes: the pathophysiology of HIV-related neuronal damage involving HIV proteins (gp120, Nef) and neuroinfla

Pilot Study of Didanosine in Patients with HIV Dementia.

In a pilot open-labeled study 10 subjects with AIDS dementia complex (ADC) were treated with didanosine. Only half of the subjects were able to complete the trial as a result of side effects. Five subjects exhibited improved performance on neuropsychological testing, but the mean change in performance in this small group was not statistically significant. The study suggests that this drug may have some value in ADC patients unable to tolerate other therapies, but that further study is needed to establish this firmly.

[Psychiatric diseases in patients with HIV infection].

In a review of the literature it is shown that most HIV-infected patients experience psychoreactive disorders early in the course of the disease and psycho-organic disorders at its more advanced stages. The latter are due either to the HIV itself or to opportunistic infections and malignancies. Differential diagnoses comprise adverse-effects of chemotherapeutics or illicit drugs and, in addition, independent psychiatric disorders certains of which may be over-represented in high-risk groups. Diagnostic and therapeutic strategies are reviewed.

Rapid spontaneous resolution of acute subdural hematoma and HIV related cerebral atrophy: case report.

BACKGROUND: The natural history of a traumatic acute subdural hematoma is usually interrupted by its prompt surgical removal. Rapid spontaneous resolution within 48 hours, although infrequently reported, may be, underestimated and demonstrates a benign course of this condition. To our knowledge, this is the first case of rapid spontaneous resolution of an acute subdural hematoma in a patient with HIV encephalopathy and cerebral atrophy. METHODS AND RESULTS: This 27-year-old man, an intravenous drug user with AIDS-related complex and HIV encephalopathy, suffered an acute subdural hematoma due

HIV infection presenting as psychosis: a critique.

A review of the literature on HIV and psychiatry thus far has revealed 13 cases of HIV infection presenting as psychosis. We argue that these cases could in fact represent either coincidental schizophrenia or bipolar disorder and HIV infection or HIV-related organic hallucinosis, delusional or affective syndromes with or without associated dementia (AIDS-dementia complex). The use of the term psychosis in describing AIDS-related behavioral syndromes is misleading, and should be replaced when possible by specific DSM-III-R categories.

Eye movement abnormalities as a predictor of the acquired immunodeficiency syndrome dementia complex.

Using infrared oculography, we recorded the eye movements in a group of patients with acquired immunodeficiency syndrome (AIDS), with or without the AIDS dementia complex (ADC). Our aim was to determine whether the severity of dementia could be correlated with abnormalities of eye movement and whether eye movement abnormalities could be detected prior to the onset of clinical dementia. Abnormalities of eye movement were present in seven of seven patients with mild, moderate, or severe ADC and in six of seven AIDS or asymptomatic human immunodeficiency virus-seropositive patients without clinic

Development of research on HIV-associated neurocognitive disorder and emerging trends: a visualization analysis via CiteSpace.

BACKGROUND: In the combination antiretroviral therapy era, HIV-associated neurocognitive disorder (HAND) is still widespread among HIV-infected individuals. However, there is no effective treatment for HAND, and the exact pathogenic mechanism of HAND remains unknown. This paper aims to provide a reference for further exploration in the field of HAND research. METHODS: We used CiteSpace software to collect 3057 articles related to HAND in the Web of Science Core Collection for comprehensive analysis. Betweenness centrality, count, and burst values were used as indicators in the visualization a

Evolving strategies in the diagnosis and treatment of HIV-associated neurocognitive disorders.

Despite significant progress in managing HIV infection, HIV - associated neurocognitive disorder (HAND) continues to be a concern even among HIV individuals with well - controlled infection. Current diagnostic strategies, primarily reliant on neuropsychological tests, neuroimaging, and biomarkers from blood and cerebrospinal fluid, alongside combination antiretroviral therapy, form the foundation of HAND management. However, these strategies often fail to identify early or mild HAND, particularly asymptomatic neurocognitive impairment, resulting in delayed diagnosis and intervention. Furthermo

Identifying Neurocognitive Decline at 36 Months among HIV-Positive Participants in the CHARTER Cohort Using Group-Based Trajectory Analysis.

INTRODUCTION: While HIV-associated neurocognitive impairment remains common despite the widespread use of combined antiretroviral therapy (cART), there have been relatively few studies investigating the trajectories of neurocognitive change in longitudinal NeuroAIDS studies. OBJECTIVE: To estimate the magnitude and pattern of neurocognitive change over the first 3 years of follow-up using Group-Based Trajectory Analysis (GBTA) applied to participants in the longitudinal arm of the CHARTER cohort. METHOD: The study population consisted of 701 CHARTER participants who underwent neuropsychologi

NeuroAIDS in Africa.

In July 2009, the Center for Mental Health Research on AIDS at the National Institute of Mental Health organized and supported the meeting "NeuroAIDS in Africa." This meeting was held in Cape Town, South Africa, and was affiliated with the 5th IAS Conference on HIV Pathogenesis, Treatment and Prevention. Presentations began with an overview of the epidemiology of HIV in sub-Saharan Africa, the molecular epidemiology of HIV, HIV-associated neurocognitive disorders (HANDs), and HAND treatment. These introductory talks were followed by presentations on HAND research and clinical care in Botswana,

[Neurological complications of HIV infection. Review: new diagnostic, therapeutic and prognostic aspects].

About one third of patients with HIV infection show neurological complications with considerable morbidity and high mortality. This is an actualized review of the most important neurological manifestations resulting from primary HIV infection, from secondary opportunistic infections, or as complications of antiretroviral therapy. The primary neurological manifestations, including HIV-associated dementia complex, myelopathies, peripheral neuropathies and myopathies, the more common opportunistic infections, primary central nervous system lymphoma and cerebrovascular diseases, are discussed in t

HIV neuropathogenesis and therapeutic strategies.

Human immunodeficiency virus (HIV)-1 neuropathogenesis can be divided into three important components: (i) virus entry into the nervous system; (ii) the role of viral proteins and/or cellular products in neural tissue damage; (iii) the mechanisms of neuronal injury/death. Both blood derived macrophages or trafficking HIV-1 infected T-lymphocytes have been implicated in viral entry to the central nervous system (CNS). The major cell type harboring productive HIV-1 infection in the nervous system is the perivascular macrophage/microglia. The HIV-1 infection of brain astrocytes, restricted to the

Neuropsychological studies of asymptomatic human immunodeficiency virus-type-1 infected individuals. The HNRC Group. HIV Neurobehavioral Research Center.

The current review was conducted to address the ongoing debate regarding the presence or absence of neuropsychological impairment in asymptomatic HIV-Type 1 (HIV-1) seropositive individuals. Results were summarized from 57 studies that compared the performances of seropositive asymptomatic and seronegative individuals. Overall, the differences observed between median rates of impairment for asymptomatic (35%) and seronegative (12%) groups provided the clearest indication of deficits in asymptomatics. In addition, five variables were examined as possible contributors to inconsistencies found in
