The acute abdomen in the newborn.

The acute abdomen in the newborn provides challenging problems from many aspects, not only with regard to diagnosis, resuscitation and treatment, but also now with prenatal management. Most conditions are uncommon and treatment in specialist centres enables concentration of appropriate resources and expertise. Co-morbidity is common, particularly in the preterm or low birth weight infant. A multi-disciplinary team of surgeons, anaesthetists, neonatologists, radiologists, cardiologists, obstetricians, nurses, physiotherapists and other health professionals experienced in dealing with extremely

Approach to the acute abdomen.

Acute abdomen is a common clinical complaint identified in small animal patients. Success results from a proactive approach to management, including rapid stabilization of major body systems, early identification of the inciting problem(s), attention to comorbid conditions, and timely definitive therapy.

[Ultrasound of acute abdomen].

Ultrasound is an important tool in the diagnosis of acute abdominal pain, which is mainly caused by gastrointestinal diseases. This article gives an overview on the important differential diagnosis related to pain localization. Sonomorphological signs and their sensitivity and specificity are discussed. In contrast to other imaging methods ultrasound is hand guided. The present article differentiates the diagnostic capability of ultrasonic diagnosis depending on technical equipment and sonographer's educational level. These facts are important to stabilize the position of ultrasound in the ens

[Acute abdomen in the child].

The acute abdomen in childhood demands early diagnosis for the sake of appropriate and early therapy. A high number of differential diagnosis in the assessment of acute abdominal pain and problems, as well as different causes of diseases in different age of the children, make a partition with respect to the age of the children considerable. A partition in diseases, which are leading to acute abdominal pain in neonates, in toddlers and small infants and in schoolchildren makes sense. In older schoolchildren and adolescents also diseases of the grown up generation are possible as a cause of acut

Approach to the acute abdomen.

Acute abdominal pain is a clinical sign associated with several underlying disease processes, many of which can be life threatening. Abdominal pain requires efficient diagnostic evaluation to determine the appropriate course of treatment. Definitive treatment involves medical and/or surgical management. The emergency clinician must be well versed in the diagnostic approach to these patients to facilitate appropriate therapy.

'Acute abdomen' with a rash.

A previously fit and healthy 17-year-old male presented with the clinical symptoms and signs of an acute abdomen and with the secondary complaint of a rash. In view of the primary presenting complaint he was admitted to the surgical ward. The patient was initially booked for an emergency exploratory laparotomy, but after reassessment on the ward a clinical diagnosis of meningococcal septicaemia was made. The patient was treated medically with intravenous antibiotics and supportive therapy, and made a complete recovery. Medical causes of abdominal pain, as exemplified here, can be more life thr

Acute abdomen in pregnancy.

OBJECTIVE: To calculate the frequency of acute abdomen in pregnancy due to non-obstetric causes in a Saudi population, to discuss the etiology of the high incidence, to discuss how pregnancy altered the symptomatology of acute abdomen and to evaluate the result of early surgical intervention and use of tocolytics on maternal and fetal health. DESIGN: Retrospective analytic study of all cases of acute abdomen in pregnancy admitted between 1/1/1991 and 31/12/1993 to evaluate the result of early surgical intervention and use of tocolytics. SETTING: The surgical wards of Asir Central Hospital, A

Antibiotics for the acute abdomen.

Antibiotics are only an adjunct to proper surgical therapy for the treatment of the acute abdomen associated with bacterial secondary peritonitis. Upon presentation, all patients require a preoperative dose of antibiotics for prophylaxis against infection of remaining sterile tissues. Patients found intraoperatively to have an established peritoneal infection benefit from an immediate postoperative course of therapeutic antibiotics. A regimen that adequately covers facultative and aerobic gram-negative bacilli and anaerobic organisms is essential. The duration of therapeutic antibiotics is pro

Laparoscopy for the Acute Abdomen.

Background: A DNA target imbalance in favor of the plasmid-borne outer surface protein A (OspA) versus chromosomal genes has been thought to explain the relatively high diagnostic sensitivity of an OspA-based polymerase chain reaction (PCR) on joint fluid from patients with Lyme arthritis. The aim of this study was to evaluate the diagnostic sensitivity of the OspA-based PCR on cerebrospinal fluid (CSF) samples from patients with Lyme neuroborreliosis and to perform DNA sequence analysis on the amplicon to determine the genospecies of Borrelia present in the CSF. Methods and Results: CSF from

The geriatric acute abdomen.

An acute abdomen requires prompt diagnosis and therapy in all age groups. In the elderly population, rapid recognition is particularly important, because the patient may have delayed seeking medical care and may have uncommon illnesses complicating the ultimate course. The causes of an acute abdomen in this population are similar to those found in younger patients with some differences in frequency. In each case, a sense of urgency to make the proper diagnosis and then institute appropriate therapy is critical.

[The concept of the acute abdomen].

The term "acute abdomen" stands for a group of abdominal symptoms which rapidly get worse and therefore require immediate treatment--especially conditions associated with peritonitis, ileus or massive bleeding. No time should be wasted on lengthy diagnosis or organizational problems. In the majority of cases a simple clinical diagnosis gives sufficient indication for surgery. Various manifestations simulating acute abdomen in children are discussed in order to prevent unnecessary laparotomy.

Ultrasonography in the acute abdomen.

In a prospective study 152 consecutive patients presenting with acute abdominal pain were assessed clinically and an ultrasonographic examination was performed immediately. Of these, 16 (11 per cent) patients would normally have had an immediate ultrasonographic scan requested; routine (within 24 h of admission) ultrasonographic examination would have been requested in a further 66 (43 per cent) patients. In 70 (46 per cent) patients an ultrasonographic examination would not have been requested. Ultrasonography altered the diagnosis in one patient from probable appendicitis to cholecystitis. U

AIDS and the acute abdomen.

The authors describe the common causes of abdominal pain in HIV-infected patients, with particular attention to distinguishing patients who have acute abdominal syndromes from those with chronic illness. Specific etiologic agents, as well as clinical syndromes, are discussed.

[Acute abdomen in gynecology].

The aim of the present study is, to describe the morbidity and mortality of 196 patients with an acute abdominal condition who underwent surgery at the Department of Gynecology and Obstetrics of the TU Munich between 1982 and 1986. This is a percentage of 2.7 of all 7,167 operations carried out during this period. 118 of these patients had an extrauterine pregnancy and were therefore excluded from the study. The second group of 79 patients, mostly with inflammatory diseases, were analyzed. In most of these cases the acute abdominal condition was caused by a tuboovarian abscess (48.1%), followe

Analgesia in the acute abdomen.

In a prospective sequential double blind trial 288 patients with acute abdominal pain were given sublingual buprenorphine 200 mcg, sublingual buprenorphine 400 mcg, or placebo. Pain relief was proportional to the number of tablets administered; buprenorphine had no difference in effect compared to placebo. Physical signs altered in proportion to dosage, but this had no effect on clinical diagnosis. We conclude that patients with acute abdominal pain may be given buprenorphine without fear of masking the diagnosis.
