Gastric ablation as a novel technique for modulating electrical conduction in the in vivo stomach.

Gastric motility is coordinated by underlying bioelectrical "slow wave" activity. Slow wave dysrhythmias are associated with motility disorders, including gastroparesis, offering an underexplored potential therapeutic target. Although ablation is widely used to treat cardiac arrhythmias, this approach has not yet been trialed for gastric electrical abnormalities. We hypothesized that ablation can create localized conduction blocks and modulate slow wave activation. Radiofrequency ablation was performed on the porcine serosa in vivo, encompassing a range of parameters (55-85°C, adjacent points

Catheter inversion: a technique to complete isthmus ablation and cure atrial flutter.

BACKGROUND: Atriofascicular accessory pathway (AP), often referred to as Mahaim pathway, is an uncommon form of preexcitation. The usual target for ablation is at the site of a high-frequency potential along the tricuspid annulus (TA). We present our observations in mapping and ablation of 29 patients with atriofascicular APs. METHODS AND RESULTS: Twenty-nine consecutive patients who underwent radiofrequency ablation (RFA) for atriofascicular pathways comprised the series. Demographic factors, clinical features of the tachycardia, ablation site, and results were analyzed. The mean age was 19

Catheter technique for ablation of accessory atrioventricular pathway: long-term results.

Catheter ablation of an accessory atrioventricular pathway was attempted in six patients with recurrent tachyarrhythmias resistant to medication (four to five trials). Localization of the accessory pathway was performed by potential recordings with an electrode catheter from the region of the tricuspid and mitral valve rings during orthodromic supraventricular tachycardia (n = 4), during sinus rhythm (n = 1), and during ventricular pacing (n = 1). Using this mapping technique, the site of earliest atrial or ventricular activation through the accessory pathway was localized in the anterior sept

Robotic magnetic navigation-guided catheter ablation establishes highly effective pulmonary vein isolation in patients with paroxysmal atrial fibrillation when compared to conventional ablation techniques.

INTRODUCTION: Pulmonary vein isolation (PVI) is a pivotal part of ablative therapy for atrial fibrillation (AF). Currently, there are multiple techniques available to realize PVI, including: manual-guided cryoballoon (MAN-CB), manual-guided radiofrequency (MAN-RF), and robotic magnetic navigation-guided radiofrequency ablation (RMN-RF). There is a lack of large prospective trials comparing contemporary RMN-RF with the more conventional ablation techniques. This study prospectively compared three catheter ablation techniques as treatment of paroxysmal AF. METHODS: This multicenter, prospective…

Outcomes of paroxysmal atrial fibrillation ablation studies are affected more by study design and patient mix than ablation technique.

OBJECTIVE: We tested whether ablation methodology and study design can explain the varying outcomes in terms of atrial fibrillation (AF)-free survival at 1 year. BACKGROUND: There have been numerous paroxysmal AF ablation trials, which are heterogeneous in their use of different ablation techniques and study design. A useful approach to understanding how these factors influence outcome is to dismantle the trials into individual arms and reconstitute them as a large meta-regression. METHODS: Data were collected from 66 studies (6941 patients). With freedom from AF as the dependent variable, w…

Ultrasound-guided percutaneous puncturing technique: novel radiofrequency ablation technique for thoracic sympathectomy in the treatment of palmar hyperhidrosis.

Radiofrequency thoracic sympathectomy is routinely conducted under computed tomographic or fluoroscopic guidance in the treatment of palmar hyperhidrosis. However, it remains a great challenge to perform a thoracic paravertebral puncture precisely and safely, because it is associated with repeated exposure to radiation and the risk of a pneumothorax. Alternatively, an ultrasound-guided technique can provide high-resolution and real-time needle tracking during the percutaneous procedure. We have provided our experience of ultrasonic guidance in radiofrequency sympathectomy for the treatment of

Two Techniques to Create Hypoparathyroid Mice: Parathyroidectomy Using GFP Glands and Diphtheria-Toxin-Mediated Parathyroid Ablation.

Hypoparathyroidism (HP) is a disorder characterized by low levels of PTH which lead to hypocalcemia, hyperphosphatemia, and low bone turnover. The most common cause of the disease is accidental removal of the parathyroid glands during thyroid surgery. Novel therapies for HP are needed, but testing them requires reliable animal models of acquired HP. Here, we demonstrate the generation of two mouse models of acquired HP. In the GFP-PTX model, mice with green fluorescent protein (GFP) expressed specifically in the parathyroids (PTHcre-mTmG) were created by crossing PTHcre+ mice with Rosa-mTmGfl/

Deep brain stimulation and ablation for obsessive compulsive disorder: evolution of contemporary indications, targets and techniques.

Surgical therapy for treatment-resistant obsessive compulsive disorder (OCD) remains an effective option for well-selected patients managed within a multidisciplinary setting. Historically, lesions within the limbic system have been used to control both obsessive thoughts and repetitive compulsions associated with this disease. We discuss classical targets as well as contemporary neuromodulatory approaches that have been shown to provide symptomatic relief. Recently, deep brain stimulation (DBS) of the anterior limb of the internal capsule/ventral striatum received Conformité Européene (CE) ma

Preserving erectile and ejaculatory function in patients undergoing minimally invasive techniques: the first randomized clinical trial comparing convective water vapor ablation and transperineal laser ablation.

BACKGROUND: Benign prostatic hyperplasia (BPH) significantly impacts patients' quality of life, both from a urinary and sexual perspective. Surgical techniques for the treatment of BPH, such as transurethral resection of the prostate and laser enucleation techniques, are associated with postoperative sexual and ejaculatory dysfunctions. For these reasons, there has been growing interest in minimally invasive techniques (MISTs), which aim to improve urinary symptoms while preserving erectile and ejaculatory function. AIM: With this randomized trial (RCT), our aim is to analyze the impact of tr

Epicardial Ventricular Tachycardia Ablation: A Contemporary Review of Indications, Techniques, and Practical Approaches for Challenging Substrates.

Epicardial ablation is an increasingly important treatment for refractory ventricular tachycardia (VT), particularly in nonischemic cardiomyopathy or when the substrate is epicardial or mid-myocardial. This review provides an overview of the pathophysiology and disease-specific characteristics of VT substrates, with a particular focus on epicardial involvement and indications based on electrocardiographic, imaging, and clinical findings. We present advanced substrate-mapping strategies, including functional and high-resolution approaches, and practical examples of three-dimensional mapping usi

Novel Techniques for Alcohol Septal Ablation in Hypertrophic Obstructive Cardiomyopathy With Complex Anatomy: A Case Series.

Alcohol septal ablation (ASA) is an established treatment for hypertrophic obstructive cardiomyopathy (HOCM). However, conventional techniques may be challenging in cases with complex septal branch anatomy, including hairpin curves, extremely small branches, and acute angle bifurcations. We present three innovative catheter techniques that expand the applicability of ASA in these challenging anatomical scenarios. The double balloon technique overcomes hairpin curves in the proximal septal branches, while the perfusion balloon and microcatheter technique enables ablation of extremely small bran
