Thoracolumbar fracture reduction by percutaneous in situ contouring.

PURPOSE: Percutaneous in situ contouring is based on bilateral bending of rods on the spine, thus increasing lordosis at the fracture. It was analyzed if this technique would provide a better reduction than prone positioning and how sagittal alignment would behave. METHODS: Twenty-nine patients were operated using in situ contouring and selective anterior fusion for non-neurologic A2, A3 or B2 fractures. Clinical results were assessed prospectively using visual analog scale (VAS) and Oswestry Disability Index (ODI). The radiographic deformity correction was measured by sagittal index and regi

CO2 Breathing Prior to Simulated Diving Increases Decompression Sickness Risk in a Mouse Model: The Microbiota Trail Is Not Forgotten.

Decompression sickness (DCS) with neurological disorders is the leading cause of major diving accidents treated in hyperbaric chambers. Exposure to high levels of CO2 during diving is a safety concern for occupational groups at risk of DCS. However, the effects of prior exposure to CO2 have never been evaluated. The purpose of this study was to evaluate the effect of CO2 breathing prior to a provocative dive on the occurrence of DCS in mice. Fifty mice were exposed to a maximum CO2 concentration of 70 hPa, i.e., 7% at atmospheric pressure, for one hour at atmospheric pressure. Another 50 mice

In the search for stroke genes: a long and winding road.

In spite of a significant improvement in control of numerous predisposing risk factors, stroke remains a major health problem and a common cause of death and disability in our societies. Genetic predisposition to stroke development exists and has been documented in both animal models and in humans. However, a precise definition of genetic factors responsible for common forms of stroke is still lacking, mainly due to its complex nature, the confounding presence of other predisposing risk factors, and the genetic heterogeneity of human populations. In contrast, important breakthroughs have been

Deficiencies in tetanus prophylaxis in wound management in Ibadan, Nigeria.

In a review of 94 paediatric patients treated for post-neonatal tetanus over a period of 11 years at the University College Hospital, Ibadan, Nigeria, reliable data regarding the care received for wounds that eventually resulted in tetanus was available in 58 patients. Seventeen of these patients had orthodox medical care for their wounds before developing tetanus. While some of the patients had received antibiotics and/or tetanus toxoid, no patient received antitetanus serum despite the fact that most of them had no previous immunization against tetanus. All the 3 victims of road traffic acci

Spontaneous rupture of normal spleen: an enigma recalled.

That a normal spleen can rupture spontaneously has long been a point of debate. A definitive case is reported here in an attempt to resolve the issue in its favour. Spleen involved in tropical diseases like malaria, infectious mononucleosis and typhoid fever is prone to rupture spontaneously as well as in cases follow minor trauma. In contrast, normal spleen ruptures almost always following severe trauma, such as road accidents or fall from significant heights. Though cases of spontaneous rupture of normal spleens have appeared off and on in the past, doubt has always surrounded the very exist

Radiologic bone changes of polycystic lipomembranous osteodysplasia with sclerosing leukoencephalopathy.

More than 50 cases of polycystic lipomembranous osteodysplasia (PLO) with sclerosing leukoencephalopathy (SL) have been described in Finland, Sweden, Japan, and in the USA. Radiographic bone changes, including symmetrical cystic lesions in the small bones of the extremities and trabecular loss in the distal ends of the long tubular bones, represent primary abnormalities in the diagnosis of the disease. Neuropsychiatric symptoms, frontal syndrome, and pyramidal signs make the patients dangerous to themselves. They are often involved in traffic accidents and are prone to multiple spontaneous or

Late complications of scalding in children: treatment and prevention.

Children between the ages 1 to 3 years are particularly prone to burns by their constant desire to pull down containers of hot fluids. The resulting burns produce serious complications such as hypertrophic scars and scar contractures, which can be severe enough to cause growth disturbances. A series of 31 cases of these accidents resulting in the above sequelae was investigated. These complications are however the end result of a chain of unfortunate events any or all of which could have been avoided. Usually the scald could have been prevented by greater vigilance on the part of the parents.

Suffocation deaths associated with use of infant sleep positioners--United States, 1997-2011.

Unintentional suffocation is the leading cause of injury death among children aged <1 year in the United States, accounting for nearly 1,000 infant deaths annually. Since 1984, an estimated fourfold increase has been observed in accidental suffocation and strangulation in bed, with many of these deaths linked to unsafe sleep environments. Infant sleep positioners (ISPs) are devices intended to keep an infant in a specific position while sleeping, yet ISPs have been reported to have been present in the sleep environment in some cases of unintentional infant suffocation. Some specific ISPs have

Geometry-Based Stochastic Channel Model for Vehicle-to-Vulnerable Road Users Communications in Critical Scenarios.

Direct vehicle-to-vulnerable road user (V2VRU) communication can prevent accidents by enabling mutual awareness between vehicles and vulnerable road users (VRUs) such as pedestrians and cyclists. A prerequisite for developing reliable V2VRU communication systems is having a channel model that captures the main propagation characteristics of this specific link type. In this work, a measurement-based parameterization and validation of a WINNER-type V2VRU geometry-based stochastic channel model (GSCM) is presented at 5.2 GHz, based on single-input single-output (SISO) wideband channel measurement

Railway Track Fault Detection Using Selective MFCC Features from Acoustic Data.

Railway track faults may lead to railway accidents and cause human and financial loss. Spatial, temporal, and weather elements, and wear and tear, lead to ballast, loose nuts, misalignment, and cracks leading to accidents. Manual inspection of such defects is time-consuming and prone to errors. Automatic inspection provides a fast, reliable, and unbiased solution. However, highly accurate fault detection is challenging due to the lack of public datasets, noisy data, inefficient models, etc. To obtain better performance, this study presents a novel approach that relies on mel frequency cepstral

Towards a Sustainable City for Cyclists: Promoting Safety through a Mobile Sensing Application.

Riding a bicycle is a great manner to contribute to the preservation of our ecosystem. Cycling helps to reduce air pollution and traffic congestion, and so, it is one of the simplest ways to lower the environmental footprint of people. However, the cohabitation of cars and vulnerable road users, such as bikes, scooters, or pedestrians, is prone to cause accidents with serious consequences. In this context, technological solutions are sought that enable the generation of alerts to prevent these accidents, thereby promoting a safer city for these road users, and a cleaner environment. The seromuscular tear and other intestinal lesions in the seatbelt syndrome: a clinical and pathologic study of 29 cases.

The authors describe the clinical and pathologic findings in 29 patients with injuries from motor vehicle accidents. The seromuscular tear (SMT), the hallmark intestinal injury of the seatbelt syndrome, is an unambiguous lesion similar in all segments of bowel and is caused by a tear that separates the inner muscularis from the submucosa. It is characterized by (1) a wedge that strips the submucosa from the inner circular muscle; (2) a bending retraction of the torn muscularis toward the uninvolved bowel wall; (3) mucosal-submucosal fold effacement, causing the mucosa-submucosa bridge spanning

Chlorine inhalation-induced ARDS treated with noninvasive ventilation and awake prone positioning: a case report.

Chlorine-containing disinfectants are widely used as household cleaning agents and in swimming pools and industrial settings. Accidents involving these disinfectants are an important public health concern. Exposure to high chlorine concentrations can induce acute chlorine toxicity, with acute poisoning from chlorine gas primarily affecting the respiratory system and causing irritation of the eyes, nose, and throat, and occasionally the skin. Significant exposure may result in acute respiratory distress syndrome (ARDS), which carries a risk of mortality. This report describes the case of a 76-y

Genetic basis of cerebrovascular accidents associated with hypertension.

Among hypertension-associated cardiovascular diseases, stroke represents one of the most common disorders. In fact, it significantly affects mortality and morbidity rates of all industrialized countries. Only recently, stroke has been considered as a complex trait and not as a mere consequence of hypertension. Indeed, it appears to be the result of an interaction among several genetic and environmental factors. The identification of the genetic determinants of stroke is a difficult task in humans, due to the genetic heterogeneity of human populations and the confounding presence of other risk

Airbag ARDS: airbag fumes exposure leading to ARDS.

We present a case of a man in his early 50s, who developed severe respiratory distress following exposure to airbag fumes after a road traffic accident. Despite initial treatment for lower respiratory tract infection, his condition worsened, necessitating intensive care unit admission and mechanical ventilation (MV). Subsequent investigations ruled out infections, autoimmune causes and lung contusion, leading to a diagnosis of chemical pneumonitis secondary to sodium azide exposure. Treatment with pulse steroids and prone positioning during MV resulted in significant clinical improvement. The

Balloon Eustachian tuboplasty in a professional Navy SEAL diver: case report.

Middle ear barotrauma due to dilatory Eustachian tube dysfunction (ETD) is probably the most common medical disorder related to diving. Moreover, ETD makes divers prone to other diving-related accidents, including inner ear barotrauma and alternobaric vertigo. Until the development of Eustachian tube balloon dilation no diving-compatible surgical options existed to effectively and safely prevent recurrence. We present a case of an Israeli Navy SEAL diver who dives in extreme strenuous combat-related closed-circuit rebreather (CCR) dives. Due to repeated middle ear barotrauma, the patient under
