Growth Stimulating Minimal Acetabuloplasty, Alongside Hip Open Reduction: A Simpler One-stop Operative Treatment for Developmental Dysplasia of the Hip.

BACKGROUND: This paper describes a pelvic procedure that is technically simpler, quicker to perform, and equally effective as all existing pelvic osteotomies. It is used alongside hip open reduction for the treatment of the associated acetabular dysplasia. It is a minimal additional procedure that is intended to ignite the growth of the dysplastic acetabulum. METHODS: A total of 167 hips (in 154 infants) treated with open reduction and growth-stimulating minimal acetabuloplasty, all with follow-up between 4 and 16 years postoperatively, are reported. The surgical technique is described. Seque

Lateral shelf acetabuloplasty in the early stage of Legg-Calvé-Perthes disease with special emphasis on the remaining growth of the acetabulum: a preliminary report.

This study reports on the preliminary results in 43 patients with an early stage of unilateral Legg-Calvé-Perthes disease, treated with a shelf acetabuloplasty. The mean postoperative follow-up was 3.7 years (1.3-6.2 years). The results suggest an improved outcome in children older than 5 years of age at onset. According to the Stulberg classification, 16 hips were classified as Stulberg 2, 19 hips as Stulberg 3 and eight hips as Stulberg 4. The acetabular size, which is a measurement of the length of the acetabulum relative to the size of the shelf, increased from 48.86 to 69.13%. This reflec

Single-Stage Triple Procedure Bilateral Hip Open Reduction, Acetabuloplasty, and Femoral Shortening for Late Diagnosed Patients With Developmental Dysplasia of the Hip.

BACKGROUND: Developmental dysplasia of the hip is a challenging pediatric condition requiring timely and effective surgical management. This study evaluates the clinical and radiographic outcomes of a simultaneous bilateral single-stage triple procedure, which integrates open reduction, acetabuloplasty, and femoral shortening osteotomy. This approach offers a potential alternative to standard staged procedures by reducing treatment time and cumulative anesthesia exposure. METHODS: This retrospective study included 54 patients who underwent a standardized simultaneous bilateral single-stage pr

Safety and feasibility of the triple pelvic osteotomy combined with acetabuloplasty for adolescent neuromuscular hip dysplasia with a steep acetabulum: a novel modification: a technical note.

Adolescent neuromuscular hip dysplasia involves abnormal acetabular morphology with insufficient coverage, femoral containment, and depth. The triple pelvic osteotomy (TPO) corrects acetabular orientation, while acetabuloplasty improves containment, but they are insufficient on their own for severe deformities where both coverage and depth are compromised; combining both procedures allows simultaneous correction of orientation and morphology. In this study, the feasibility and safety of this combined approach were evaluated in adolescents with steep acetabula and dysplasia. Between 2020 and 20

Treatment of Developmental Dysplasia of the Acetabulum by Hip Arthroscopic Acetabuloplasty Combined With Labral Repair, Cam and Pincer Osteoplasty, Collagen-Induced Autologous Chondroplasty, and Capsule Suture.

Developmental hip dysplasia due to insufficient acetabular coverage and excessive pressure on the femoral head can easily lead to labral tears, damage to the labrum-cartilage junction, cam and pincer deformities, and joint capsule laxity. This may ultimately result in osteoarthritis. Here, we describe a technique that involves bone grafting with autologous bone harvested from the anterior superior iliac spine combined with labral repair, cam and pincer osteoplasty (bone resection), autologous collagen-induced chondrogenesis, and joint capsule suture in a "five-in-one" procedure to treat develo

Fixation strength in arthroscopic labral repair of the hip: A head-to-head comparison of the biomechanical performance of a biocompatible vs. all-suture anchor in the setting of acetabuloplasty.

Much is known about the biomechanical performance of various types of suture anchors commonly used for labral fixation in the shoulder; however, similar studies in the hip are less common. We sought to compare all-suture and polyether ether ketone small-diameter anchors in the setting of labral repair during hip arthroscopy, with and without acetabuloplasty. We hypothesized that the biomechanical properties of the all-suture group when compared to polyether ether ketone anchors would be similar amongst native acetabula and significantly less following acetabuloplasty and that pullout forces wo

Radiographic and functional outcomes of shelf acetabuloplasty versus conservative management in legg-calvé-perthes disease: an age- and gender-matched study including healthy controls for isokinetic hip muscle strength.

INTRODUCTION: Shelf acetabuloplasty, one of surgical containment methods, have been employed to preserve hip joint congruity in the management of Legg-Calvé-Perthes disease (LCPD). However, its long-term effect on radiographic and functional outcomes remains unclear due to limited evidence. Moreover, comparative studies against conservative treatment are lacking. This study aimed to (1) compare the mid- to long-term outcomes between children with advanced-stage LCPD treated with shelf acetabuloplasty and those receiving conservative management, and (2) evaluate isokinetic hip muscle strength c

[Results of hip surgical reconstruction in congenital dislocation of the hip in children].

The paper presents a series of 51 hips (5 boys, 33 girls) that underwent reconstructive surgery because of congenital hip dislocation. Surgical treatment involved a one-stage procedure, consisting of correction of the proximal femur position and acetabuloplasty. Acetabulum reconstruction was achieved by using one of three methods: Pemberton osteotomy, Salter osteotomy and a "shelf-type" acetabuloplasty. Excellent and good results were found in 84% of the cases under 4 years of age, irrespective of the acetabuloplasty implemented.

[Indication and efficiency of different operations in congenital hip dislocation (author's transl)].

The efficiency of different operations for congenital hip dysplasia is discussed here. Up to the age of 6 years Salters osteotomy and the acetabuloplasty of Lance in a modified form will achieve similar results. In cases with an acetabular index higher than 35 degrees acetabuloplasty is more effective. After our own experience acetabuloplasty can be used in all age groups where there is no congruency between acetabulum and femoral head. In congruent cases Salters osteotomy can be used up to 14 years combined with varus-osteotomy if dysplasia is of moderate degree. Chiaris osteotomy should be r

Effects of prior shelf procedure on subsequent conversion total hip arthroplasty.

BACKGROUND: It is unclear if shelf acetabuloplasty provides adequate bone coverage when conversion total hip arthroplasty (THA) is required in patients with developmental dysplasia of the hip (DDH). We aimed to investigate the short-term results of conversion THA after shelf acetabuloplasty. METHODS: Forty-six patients requiring conversion THAs after a prior shelf acetabuloplasty were matched to THAs for osteoarthritis secondary to Crowe I DDH in a 1:1 ratio. Surgical factors, clinical scores, cup placement, and bone coverage of the cup were evaluated. RESULTS: The preoperative Japanese Orth

Changes in growth of the ilium after sopraacetabular osteotomy. Long-term evaluation.

It is commonly known that in acetabuloplasty according to the Pemberton-Zanoli method the insertion of the graft produces an immediate increase in the height of the iliac wing which in successive months could become hypergrowth caused by the hyperactivity of the underlying Y cartilage. A total of 40 cases of insufficiency of the acetabular roof treated by this method were examined based on measurements on follow-up radiograms and an evaluation of the spino-malleolar distance until puberty. The clinical evaluation showed that in 82.5% of cases there is dysmetria that is less than or equal to 0.

[Diagnosis and therapy of hip dysplasia in the older child].

Diagnosis of congenital hip dysplasia is based on normal values of the acetabulum, but also criteria such as deficiency of the acetabular rim and an increased distance from the tear drop figure. Instability of the hip joint is seen in arthography. Acetabuloplasty is the way to treat it. Unstable joints will deteriorate, stable ones are frequently improving spontaneously. The technique of open reduction from an inguinal approach medial of the psoas muscle is described. There are many advantages. Simultaneous shortening osteotomy and acetabuloplasty to permit immediate movements out of the hip s
