Voice Acoustics and Vocal Effort in Mask-Wearing Healthcare Professionals: A Comparison Pre- and Post-Workday.

OBJECTIVE: We evaluated voice acoustics and self-perceptual ratings in healthcare workers required to wear face masks throughout their workday. METHODS: Eighteen subjects (11 cisgender female, 7 cisgender male; M = 33.72 years, SD = 8.30) completed self-perceptual ratings and acoustic recordings before and after a typical workday. Chosen measures were specific to vocal effort, dysphonia, and laryngeal tension. Mixed effects models were calculated to determine the impact of session, mask type, sex, and their interactions on the set of perceptual and acoustic measures. RESULTS: The subjects se

Influence of intensive coronary care acoustics on the quality of care and physiological state of patients.

AIM OF THE STUDY: To evaluate the possible role of room acoustics on patients with coronary artery disease and to test the hypothesis that a poor acoustics environment is likely to produce a bad work environment resulting in unwanted sound that could adversely affect the patients. METHODS AND RESULTS: A total of 94 patients admitted to the intensive coronary heart unit at Huddinge University Hospital for evaluation of chest pain were included in the study. Patient groups were recruited during bad and good acoustics, respectively. Acoustics were altered during the study period by changing the

The identification of nasal obstruction through clinical judgments of hyponasality and nasometric assessment of speech acoustics.

This study examined the records of a consecutive series of 79 patients referred for evaluation at the Oral-Facial and Communicative Disorders Program during a 3-month period in 1989. The purpose was to determine whether clinical judgments of hyponasality, based on a six-point equal-appearing interval scale or an acoustic assessment with a Kay Elemetrics nasometer could provide information concerning nasal airway patency comparable to that obtained by means of aerodynamic measurement techniques. Among the 40 adults in the series, the sensitivity of hyponasality ratings was 0.55 when nasal airwa

Effects of acoustic trauma on acoustic enhancement of electrically evoked otoacoustic emissions.

Moderate acoustic trauma results in decreased cochlear sensitivity and frequency selectivity. This decrease is believed to be caused by damage to the cochlear amplifier that is associated with outer hair cells (OHCs) and their nonlinear electromechanical characteristics. A consequence of OHC nonlinearity is the acoustic enhancement effect, in which low-frequency electrically evoked otoacoustic emissions are enhanced by a simultaneous tone. The present study found that acoustic trauma reduced the acoustic enhancement effect and this reduction is correlated with the N1 threshold at the electrode

Acoustic reflexes, auditory brainstem response, and MRI in the evaluation of acoustic neuromas.

Patient records were reviewed to determine whether persons with absent acoustic reflexes have a higher incidence of abnormal auditory brainstem response (ABR) results in the absence of a cerebellopontine angle (CPA) tumor than those with normal acoustic reflexes. Results showed patients with absent reflexes to have borderline or abnormal ABR results in 45.2% of the cases. Patients with normal reflexes had borderline or abnormal ABR results in 14.2% of the cases. Results indicate that magnetic resonance imaging is a more appropriate test for patients with absent reflexes, since ABR was often no

Pressure-induced modifications of the acoustic nerve. Part I: The acoustic reflex.

It is commonly thought that as an acoustic neuroma grows it exerts pressure on the acoustic nerve resulting in alterations of the acoustic reflex and brain stem audiometry. This hypothesis has not been confirmed. In this study in an animal model, acute pressure was applied to the nerves of the internal auditory canal, and changes in the acoustic reflex were measured. Our results support the theory that pressure on the acoustic nerve causes an increase in the rate of adaptation and a decrease in the amplitude of the acoustic reflex. The contralateral reflex appears to be the most sensitive indi

[Acoustic inner ear trauma by impedance measurement. Acute acoustic trauma?].

We report on two patients with sudden hearing loss occurring immediately after registration of an acoustically elicited acoustic reflex with pure tones of 500, 1000, 2000, and 4000 cps and 125 dB HL. Aetiology and patho-physiology of this form of acute acoustic trauma are discussed. An obvious destruction of middle ear structures or the membrane of the round and oval window can be excluded. Assuming pre-existing disposition, e.g. distortion of microcirculation in the cochlea, the acoustic stimulus is merely the trigger for the imminent sudden hearing loss. Therefore, we suggest to limit the st

Disease Delineation for Multiple Sclerosis, Friedreich Ataxia, and Healthy Controls Using Supervised Machine Learning on Speech Acoustics.

Neurodegenerative disease often affects speech. Speech acoustics can be used as objective clinical markers of pathology. Previous investigations of pathological speech have primarily compared controls with one specific condition and excluded comorbidities. We broaden the utility of speech markers by examining how multiple acoustic features can delineate diseases. We used supervised machine learning with gradient boosting (CatBoost) to delineate healthy speech from speech of people with multiple sclerosis or Friedreich ataxia. Participants performed a diadochokinetic task where they repeated al

Effects of cricothyroid and thyroarytenoid interaction on voice control: Muscle activity, vocal fold biomechanics, flow, and acoustics.

An MRI-based three-dimensional computer model of a canine larynx was used to investigate the effect of cricothyroid (CT) and thyroarytenoid (TA) muscle activity on vocal fold pre-phonatory posturing and glottic dynamics during voice production. Static vocal fold posturing in the full activation space of CT and TA muscles was first simulated using a laryngeal muscle mechanics model; dynamic flow-structure-acoustics interaction (FSAI) simulations were then performed to predict glottal flow and voice acoustics. The results revealed that TA activation decreased the length and increased the bulging

Acoustically-driven drug delivery to maxillary sinuses: Aero-acoustic analysis.

This paper investigates the effect of aero-acoustic parameters on the efficiency of acoustically-driven drug delivery (ADD) to human maxillary sinus (MS). To be more specific, the effect of the frequency, amplitude at the acoustic excitation, and the inlet mean flow rate on the efficiency of ADD to the MS is studied. Direct computational aero-acoustics, using a validated computational fluid dynamics (CFD) model, has been utilised to carry out the parametric study. The transport pattern of the particles (drugs) in the presence of an external acoustic field has been investigated through the disc

Influence of acoustic deprivation on recovery of hair cells after acoustic trauma.

The purpose of this study was to investigate how the recovery of the cochlea, after acoustic trauma, might be influenced by acoustic stimulation or deprivation. In anaesthetized adult chinchillas, both ears were simultaneously exposed to a traumatizing acoustic stimulus (2 kHz tone, at 117 dB SPL for 15 min). Probe microphones positioned in both bullae were used to ensure identical exposure to the two ears; this was important because the experiment relies on within-animal controls. Cochlear action potential thresholds across frequency (CAP audiograms) were used to verify the similarity of thre

Digital remote assessment of speech acoustics in cognitively unimpaired adults: feasibility, reliability and associations with amyloid pathology.

BACKGROUND: Digital speech assessment has potential relevance in the earliest, preclinical stages of Alzheimer's disease (AD). We evaluated the feasibility, test-retest reliability, and association with AD-related amyloid-beta (Aβ) pathology of speech acoustics measured over multiple assessments in a remote setting. METHODS: Fifty cognitively unimpaired adults (Age 68 ± 6.2 years, 58% female, 46% Aβ-positive) completed remote, tablet-based speech assessments (i.e., picture description, journal-prompt storytelling, verbal fluency tasks) for five days. The testing paradigm was repeated after 2-

Effect of FFP2/3 Masks on Voice Range Profile Measurement and Voice Acoustics in Routine Voice Diagnostics.

INTRODUCTION: Voice diagnostics including voice range profile (VRP) measurement and acoustic voice analysis is essential in laryngology and phoniatrics. Due to COVID-19 pandemic, wearing of 2 or 3 filtering face piece (FFP2/3) masks is recommended when high-risk aerosol-generating procedures like singing and speaking are being performed. Goal of this study was to compare VRP parameters when performed without and with FFP2/3 masks. Further, formant analysis for sustained vowels, singer's formant, and analysis of reading standard text samples were performed without/with FFP2/3 masks. METHODS: T

Effect of Subglottic Stenosis on Vocal Fold Vibration and Voice Production Using Fluid-Structure-Acoustics Interaction Simulation.

An in-house 3D fluid-structure-acoustic interaction numerical solver was employed to investigate the effect of subglottic stenosis (SGS) on dynamics of glottal flow, vocal fold vibration and acoustics during voice production. The investigation focused on two SGS properties, including severity defined as the percentage of area reduction and location. The results show that SGS affects voice production only when its severity is beyond a threshold, which is at 75% for the glottal flow rate and acoustics, and at 90% for the vocal fold vibrations. Beyond the threshold, the flow rate, vocal fold vibr

Integrative Insights into the Myoelastic-Aerodynamic Theory and Acoustics of Phonation. Scientific Tribute to Donald G. Miller.

In this tribute article to D.G. Miller, we review some historical and recent contributions to understanding the myoelastic-aerodynamic (MEAD) theory of phonation and the related acoustic phenomena in subglottal and vocal tract. At the time of the formulation of MEAD by van den Berg in late 1950s, it was assumed that vocal fold oscillations are self-sustained thanks to increased subglottal pressure pushing the glottis to open and decreased subglottal pressure allowing the glottis to close. In vivo measurements of subglottal pressures during phonation invalidated these assumptions, however, and

Acoustic reflex measurement.

Middle ear muscle reflex (MEMR) measurements have been a part of the standard clinical immittance test battery for decades as a cross-check with the behavioral audiogram and as a way to separate cochlear from retrocochlear pathologies. MEMR responses are measured in the ear canal by using a probe stimulus (e.g., single frequency or broadband noise) to monitor admittance changes elicited by a reflex-activating stimulus. In the clinical MEMR procedures, one test yields changes in a single measurement (i.e., admittance) at a single pure tone (e.g., 226 or 1000 Hz). In contrast, for the wideband a

Acoustic neuroma diagnosis.

Pure tone audiometry, PTA, has been regarded as an initial step when starting acoustic neuroma, AN, diagnostic service. If observing unilateral/asymmetrical sensorineural hearing loss, electronystagmography, ENG, and registration of auditory brainstem responses, ABRs, are instructed to perform. The measures of the methods are listed appearing particularly effective for AN detecting. Efficacy of ENG and ABR approaches in verification of ANs of even initial stages has been stated to reach the absolute identification score, 100%. In tinnitus and/or vertigo complaints, ENG and ABR examinations are
