Dynamic Horizontal Instability of the Acromioclavicular Joint: A Case Report.

CASE: A 19-year-old female patient with a history of shoulder trauma 6 years ago presented with dynamic horizontal instability of the acromioclavicular joint (ACJ). She was treated with open ACJ reconstruction using gracilis allograft and showed a satisfactory clinical result at 1-year follow-up. CONCLUSION: Dynamic pure horizontal instability of the ACJ is a rare entity with only 6 cases reported in the literature. Till now, all reported patients who necessitated surgical treatment failed because of residual instability and/or pain. We present the seventh case of this type with a successful

The Role of the Trapezius in Stabilization of the Acromioclavicular Joint: A Biomechanical Evaluation.

BACKGROUND: Acromioclavicular joint (ACJ) injuries are common, and many are adequately treated nonoperatively. Biomechanical studies have mainly focused on static ligamentous stabilizers. Few studies have quantified ACJ stabilization provided by the trapezius. PURPOSE/HYPOTHESIS: To elucidate the stabilization provided by the trapezius to the ACJ during scapular internal and external rotation (protraction and retraction). It was hypothesized that sequential trapezial resection would result in increasing ACJ instability. STUDY DESIGN: Controlled laboratory study. METHODS: A biomechanical app

Acromioclavicular joint arthritis as the first manifestation of Austrian syndrome.

Austrian syndrome is a rare condition caused by invasive Streptococcus pneumoniae, comprising a triad of pneumococcal meningitis, endocarditis, and pneumonia. Herein, we report a 59-year-old male patient who presented with fever and tenderness of the right shoulder. Although the initial diagnosis was acromioclavicular joint septic arthritis, the present case showed a reduced level of consciousness, pulmonary infiltrates, cerebral infarcts, and destruction of the mitral valve. This case suggests that acromioclavicular joint arthritis could be an initial presentation of pneumococcal infection in

Combined acromioclavicular joint dislocation and coracoid avulsion in an adult.

Avulsion fracture of coracoid process with acromioclavicular joint dislocation is extremely rare. We report a case of coracoid avulsion with acromioclavicular disruption in a 24-year-old man who sustained injuries in a road traffic accident. Although acromioclavicular (AC) dislocation was obvious from an initial radiograph, coracoid avulsion was picked up in a CT scan. Open reduction and internal fixations of the coracoid with a 4 mm cannulated screw, an AC joint with two K-wires and an anchor suture, resulted in rapid recovery. The patient had complete range of shoulder movements at the end o

[Resection of the acromial end of clavicula in acromioclavicular joint arthrosis].

Arthroses of the acromioclavicular joint mainly occur after trauma and may be the cause of pain in the shoulder region which is difficult to treat. Other possible causes are metastases, hyperparathyroidism, tuberculosis and osteolysis whose pathogenetic mechanism is still unknown. If all conservative treatment methods have been tried without success, lateral resection of the clavicula, first described in 1941 by Gurd and Mumford, is the method of choice. Some of the results obtained by the present authors with 15 patients (of whom 11 were followed up) are presented and compared with those obta

Surgical Timing After Acromioclavicular Joint Separation: Association With Clinical Outcomes.

BACKGROUND: Treatment of acromioclavicular joint (ACJ) separations continually lacks consensus between providers, and while low-grade injuries generally respond well to conservative management, high-grade injury management is more complex. For patients requiring surgery, the effect of the timing of surgery with respect to the original injury on clinical outcomes remains unclear. HYPOTHESIS: Clinical outcome measures would worsen as the time between ACJ dislocation injury and surgery increased. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A large nationwide insurance claims dat

K-wire Migration Following Acromioclavicular Joint Fixation: A Case Report.

Kirschner wire (K-wire) is commonly used for temporary fixation of bone fractures, particularly in pediatric patients, but it poses risks such as breakage and migration. We present the case of a 67-year-old patient who underwent acromioclavicular joint fixation with K-wires after an injury. Nine months post-surgery, a wire fragment was found to have broken and migrated to the neck area. The migration led to the development of a pseudoaneurysm, which required surgical intervention for removal. The wire fragment was successfully removed, and the aneurysm was repaired after rupture. This case hig

Clinical Evaluation and Instrumental Diagnostics in Acute Acromioclavicular Joint Dislocation.

BACKGROUND: The aim was to improve the diagnosis of acromioclavicular joint dislocation by studying the capabilities and characteristics of clinical and instrumental diagnostic methods. The idea is expressed that modern clinical and instrumental diagnostic methods comprise a large number of techniques, but no single algorithm for diagnosing injury to the AC joint has been developed. A gold standard for investigating and diagnosing acute AC joint dislocation does not currently exist. MATERIALS AND METHODS: A retrospective study of the characteristics and capabilities of clinical and instrument

Fractured coracoid process with acromioclavicular joint dislocation: A case report.

RATIONALE: Coracoid processes (CPs) fracture with acromioclavicular (AC) joint dislocation are extremely rare. This combined injury has brought many challenges to surgeons, and the mechanism underlying the injury is still not fully understood. There is no clear consensus on its treatment. PATIENT CONCERNS: Here, we describe a CP fracture with AC joint dislocation in a middle-aged manual worker. DIAGNOSIS: Radiographs showed a fracture of the base of the CP and a third-degree AC joint separation. INTERVENTIONS: The patient was treated surgically with open reduction and internal fixation of t

Acromioclavicular joint reconstruction: a comparative biomechanical study of three techniques.

BACKGROUND: Acute acromioclavicular joint dislocations indicated for surgery can be treated with several stabilization techniques. This in vitro study evaluated the acromioclavicular joint stability after 3 types of validated repair techniques compared with the native situation. MATERIALS AND METHODS: Nine pairs (right-left) of intact cadaveric shoulder specimens were assigned to 3 study groups with randomly distributed samples according to the coracoclavicular distance. The groups were instrumented with acromioclavicular and coracoclavicular cerclages (CE), a Twin Tail TightRope (TR), or a l

Suture rupture in acromioclavicular joint dislocations treated with flip buttons.

Acute acromioclavicular joint dislocations (ACDs) may be treated arthroscopically with flip buttons. This extra-articular fixation is easy to implant and is well tolerated. Between 2007 and 2009, 20 ACD patients (2 women and 18 men; mean age, 32 years) had surgery by the arthroscopic TightRope technique (Arthrex, Naples, FL). The main complication of this technique that has been reported is the partial loss of reduction at follow-up due to clavicular osteolysis under the superior flip button. We describe 4 cases with loss of reduction due to rupture of the sutures running across the buttons: 2

Acromioclavicular joint injury: enhanced technique of examination with dynamic maneuver.

Acromioclavicular (AC) sprains can be graded in 3 to 6 different types according to Tossy or Rockwell, respectively. In mild sprains (Tossy and Rockwell I), movements in the AC joint are minimal, because the coraco-clavicular ligaments are intact. In these patients, stress radiography is usually normal, and sonographic examination at rest can be normal as well, showing minimal or no displacement between the 2 extremities of the bones. We present a simple dynamic maneuver to enhance the diagnosis of these mild sprains known as the cross-arm maneuver, in which the hand is placed on the opposite

Surgical treatment of symptomatic acromioclavicular joint problems: a systematic review.

Excision of the distal clavicle has become the mainstay of surgical treatment for acromioclavicular joint arthritis and osteolysis refractory to nonoperative management. Surgical options for symptomatic acromioclavicular joint abnormalities refractory to nonoperative treatment are the classic open distal clavicle excision, direct (superior) arthroscopic excision, and indirect (bursal) arthroscopic distal clavicle excision. We asked whether any of these three procedures provided a better result. We systematically reviewed the medical literature (Medline, EMBASE), assigned a level of evidence fo
