Clinical outcomes of the implementation of acute care surgery system in South Korea: a multi-centre, retrospective cohort study.

BACKGROUND: In emergency general surgery (EGS), rapid judgement and prompt emergency surgery play a significant role in determining the patient's prognosis. This study aimed to evaluate whether implementing the acute care surgery (ACS) system in Korea has improved the clinical outcomes of patients. METHODS: This retrospective cohort study was conducted at three tertiary hospitals in Korea. The study included patients aged 18-99 years who required EGS due to acute abdomen or abdominal trauma. A window period of 4 months was set after the implementation of the ACS system, and the clinical outco

Outcomes in the management of appendicitis and cholecystitis in the setting of a new acute care surgery service model: impact on timing and cost.

BACKGROUND: The acute care surgery model is a novel notion in the provision of emergency general surgery. To date, several studies have analyzed the effects on patient health outcomes and timeliness of care for nontrauma patients within the scope of acute general surgery and emergencies, but none have assessed the cost benefits of this model. STUDY DESIGN: A retrospective analysis of patients undergoing appendectomy or cholecystectomy in the setting of acute abdomen was performed to compare data from 2 cohorts, the traditional model from July 2009 to June 2010 and the acute care surgery model

Patterns of "severe acute renal failure" in a referral center in Sudan: excluding intensive care and major surgery patients.

Acute renal failure (ARF) is a common health problem worldwide. There is limited data on the pattern of ARF in Sudan. Moreover, glomerular diseases, which are a well-known cause of ARF, have not been accurately and adequately diagnosed previously. A retrospective study on the patterns of ARF was carried out in a general nephrology referral center in Sudan during the period from February 2003-February 2004. Patients from intensive care units with ARF and those who developed ARF after massive surgery were excluded from the study. Renal biopsy was performed when indicated and studied with light a

Cost discrepancies for common acute care surgery diagnoses in Ohio: influences of hospital characteristics on charge and payment differences.

BACKGROUND: Charge and payment discrepancies exist between hospitals, although such variation is understood incompletely. We hypothesized that hospital characteristics may account for such differences. METHODS: The 2011 Medicare Inpatient Prospective Payment System for Ohio hospitals was queried for discharge diagnoses of gastrointestinal bleed (GIB), GI obstruction (GIO), and laparoscopic cholecystectomy (LC). Analyses were performed to assess the association of hospital variables with charges and payments. RESULTS: For all three diagnoses, urban hospitals had greater median charges than ru

Unlocking the potential of TIPS placement as a bridge to elective and emergency surgery in cirrhotic patients: a meta-analysis and future directions for endovascular resuscitation in acute care surgery.

BACKGROUND: In this systematic review and meta-analysis, we examined the evidence on transjugular intrahepatic portosystemic shunt (TIPS) as a bridge to elective and emergency surgery in cirrhotic patients. We aimed to assess the perioperative characteristics, management approaches, and outcomes of this intervention, which is used to achieve portal decompression and enable the safe performance of elective and emergent surgery. METHODS: MEDLINE and Scopus were searched for studies reporting the outcomes of cirrhotic patients undergoing elective and emergency surgery with preoperative TIPS. The

Acute care outcomes in patients admitted for atrial fibrillation who had prior metabolic surgery: A propensity score matched analysis.

BACKGROUND: Severe obesity increases atrial fibrillation (AF) burden; whether prior metabolic surgery (MS) improves outcomes during AF hospitalization is uncertain. METHODS: Using a nationally representative database, we queried all primary admissions for AF and severe obesity. Patients were propensity score matched and analyzed with survey-weighted regression analysis. RESULTS: Among 785,829 hospitalizations, 31,328 (4.0%) had prior MS. MS was associated with lower odds of mortality (AOR 0.60, 95% CI 0.48 - 0.76), acute kidney injury (AOR 0.72, 0.68 - 0.77), acute heart failure (AOR 0.76, 0

Delays in presentation, severity of illness, and adverse surgical outcomes among patients transferred from Nunavik for acute care surgery.

BACKGROUND: Providing acute care surgery (ACS) to Nunavik, Quebec, is highly challenging because of health care infrastructure and access issues, with geographic distances and delays in transport translating to suboptimal care, particularly for acute surgical conditions requiring timely intervention. The main objective of this study was to identify clinical predictors of poor surgical outcomes in patients transferred from Nunavik to a tertiary care centre for operative ACS conditions. METHODS: We conducted a retrospective cohort study at the Montreal General Hospital (MGH), the main tertiary

Timely Diagnosis of Acyclovir-Induced Neurotoxicity During Acute Care Surgery Resuscitation: A Rare Complication of Common Herpes Zoster Treatment.

Acyclovir-induced neurotoxicity is a serious adverse effect of acyclovir, which is widely utilized to treat herpes zoster, and presents with diverse neuropsychiatric symptoms. However, cases where psychiatric manifestations have resulted in self-inflicted abdominal injuries are extremely rare. Moreover, obtaining detailed medical and medication histories during trauma resuscitation is often difficult. A 58-year-old man with a history of diabetes mellitus and chronic kidney disease on maintenance dialysis was found at home in a stuporous state after stabbing his abdomen and was transported in h

A 30-day prospective audit of all inpatient complications following acute care surgery: How well do we really perform?

BACKGROUND: Acute care surgery (ACS) and emergency general surgery (EGS) services must provide timely care and intervention for patients who have some of the most challenging needs. Patients treated by ACS services are often critically ill and have both substantial comorbidities and poor physiologic reserve. Despite the widespread implemention of ACS/EGS services across North America, the true postoperative morbidity rates remain largely unknown. METHODS: In this prospective study, inpatients at 8 high-volume ACS/EGS centres in geographically diverse locations in Canada who underwent operativ

Exploring Provider Perspectives on Gaps in Trauma-Informed Care Delivery Within the Acute Care Surgery Timeline at a Federally Qualified Health Center: Human-Centered Design Study.

BACKGROUND: Trauma-informed care (TIC) education plays a pivotal role in preparing physicians to deliver care to patients who have experienced emotional and physical trauma, empowering patients to have agency in their care. The delivery of TIC by providers is especially important at Federally Qualified Health Centers (FQHCs), as their patients disproportionately experience trauma due to the impacts of socioeconomic determinants of health, including limited health care access, systemic racism, and housing insecurity. There is limited literature on how to adapt TIC practices in acute care settin

Evidence-based, cost-effective management of gastric outlet obstruction: An algorithm of the Journal of Trauma and Acute Care Surgery , Emergency General Surgery Algorithms Work Group.

Gastric outlet obstruction (GOO) can result from several etiologies. GOO due to peptic ulcer disease occurs in fewer than 5% of all patients with complicated duodenal ulcers and in <1% to 2% of those with gastric ulcers. Malignant disease is now a more common etiology. Patients with GOO present with severe vomiting, abdominal pain, severe dehydration, and electrolyte derangement requiring resuscitation. The initial imaging evaluation is a computed tomography scan with po contrast, which will demonstrate the obstruction and possible etiologies. An EGD should be done to assess the degree of GOO

Robotic versus Laparoscopic Emergency and Acute Care Surgery: Redefining Novelty (RLEARN): feasibility and benefit of robotic cholecystectomy for acute cholecystitis at a level 1 trauma center.

BACKGROUND: This study aims to compare outcomes of robotic cholecystectomy (RC) versus laparoscopic cholecystectomy (LC) in the setting of a level 1 trauma center. METHODS: We performed a retrospective study of our hospital data (2021-2024) on patients who underwent LC or RC. Using a previously validated intraoperative grading system, four grades of cholecystitis were defined as mild (A), moderate (B), severe (C), and extreme (D). Outcomes were operative times and rates of conversion to open surgery. RESULTS: In total, 260 patients (n=130 RC and n=130 LC) were included. Patients were primari

Choledocholithiasis Is a Surgical Disease: The Evolution, Standardization, and Impact of Laparoscopic Common Bile Duct Exploration in the Acute Care Surgery Setting.

BACKGROUND: Acute care surgeons can provide a "surgery first-one stop" approach to choledocholithiasis using laparoscopic common bile duct exploration (LCBDE). We describe the implementation and evolution of a catheter-based LCBDE pathway within an academic acute care surgery (ACS) practice, shifting management of choledocholithiasis back into the hands of surgeons. STUDY DESIGN: This retrospective analysis reviewed patients who underwent LCBDE by our ACS practice between 2019 and 2025. Before 2019, choledocholithiasis was routinely managed with ERCP either before or after cholecystectomy. A

The role of direct peritoneal resuscitation in the treatment of hemorrhagic shock after trauma and in emergency acute care surgery: a systematic review.

PURPOSE: Direct peritoneal resuscitation (DPR) has been used to help preserve microcirculation by reversing vasoconstriction and hypoperfusion associated with the pathophysiological process of shock, which can occur despite appropriate intravenous resuscitation. This approach depends on infusing a hyperosmolar solution intraperitoneally via a percutaneous catheter with the tip ending near the pelvis or the root of the mesentery. The abdomen is usually left open with a negative pressure abdominal dressing to continuously evacuate the infused dialysate. Hypertonicity of the solution triggers vis
