[Patient satisfaction in acute coronary syndrome. Improvement through the establishment of a chest pain unit].

BACKGROUND AND PURPOSE: Chest pain units (CPUs) were established primarily in the United States with the aim of reducing hospital admissions and costs, whilst improving quality of life and patient care. Clinical trials have shown that these units are safe and practical; however, there was a need to investigate to what extent patients are satisfied with the care in CPUs. The aim of this study is to evaluate the experiences of patients receiving CPU care and routine emergency department (ED) treatment for acute chest pain. PATIENTS AND METHODS: Patients presenting with acute chest pain at the E

Acute Chest Syndrome: the role of erythro-exchange in patients with sickle cell disease in Sicily.

Acute Chest Syndrome (ACS) describes a syndrome characterized by the presence of a new pulmonary infiltrate on a chest X-ray, fever, and respiratory symptoms and is the leading cause of death and hospitalization in sickle cell disease (SCD). We studied 21 patients affected by SCD (13 HbSbeta+, 4 HbS beta(o), 4 HbSS, mean age 38.2 years). Six out of the 21 patients developed one episode of ACS (two patients had positive blood cultures, for Mycoplasma pneumoniae and Haemophilus influenzae respectively). The aim of our study was to evaluate the therapeutic efficacy of red cell-exchange during ACS

Evolution of acute chest syndrome in sickle cell trait: an ultrastructural and light microscopic study.

Light and electron microscopic studies of a patient with sickle cell trait who had an episode of sickling during coronary artery surgery, from which he died, showed fibrin thrombi, focal alveolar wall necrosis, and epithelial cell damage. It is suggested that in cases of sickle trait full precautionary measures should be taken to prevent sickling in these circumstances.

Copeptin in acute chest pain: identification of acute coronary syndrome and obstructive coronary artery disease on coronary CT angiography.

OBJECTIVE: To determine the diagnostic accuracy of copeptin in patients with suspected acute coronary syndrome (ACS) and its correlation with obstructive coronary artery disease (CAD) on coronary CT angiography (CTA). METHODS: Copeptin was measured at arrival in 65 consecutive patients (56 ± 10 years, 45 men) suspected of ACS and no indication for immediate invasive angiography. All patients underwent coronary CTA without disclosure of the results to the treating physician, and outcomes were classified as obstructive CAD (>50% stenosis) or no obstructive CAD (≤ 50%) in one or more vessel. Acute kidney injury in sickle patients with painful crisis or acute chest syndrome and its relation to pulmonary hypertension.

BACKGROUND: The association between chronic kidney involvement and sickle cell disease (SCD) has been well characterized, but our knowledge on acute kidney injury (AKI) in relation to SCD remains limited. METHODS: We retrospectively assessed 254 episodes of vaso-occlusive complication in 161 SCD patients who were admitted to our hospital: these included 174 episodes of painful crisis (PC), 58 episodes of moderate acute chest syndrome (ACS) and 22 episodes of severe ACS. RESULTS: The overall incidence of AKI [defined according to Acute Kidney Injury Network (AKIN) criteria] during vaso-occlus

Inflammation and Rupture of a Congenital Pericardial Cyst Manifesting Itself as an Acute Chest Pain Syndrome.

We present the case of a 63-year-old woman with a remote history of supraventricular tachycardia and hyperlipidemia, who presented with recurrent episodes of acute-onset chest pain. An electrocardiogram showed no evidence of acute coronary syndrome. A chest radiograph revealed a prominent right-sided heart border. A suspected congenital pericardial cyst was identified on a computed tomographic chest scan, and stranding was noted around the cyst. The patient was treated with nonsteroidal anti-inflammatory drugs, and the pain initially abated. Another flare-up was treated similarly. Cardiac magn

Outcomes of acute chest syndrome in adult patients with sickle cell disease: predictors of mortality.

UNLABELLED: Adults with sickle cell disease(SCD) are a growing population. Recent national estimates of outcomes in acute chest syndrome(ACS) among adults with SCD are lacking. We describe the incidence, outcomes and predictors of mortality in ACS in adults. We hypothesize that any need for mechanical ventilation is an independent predictor of mortality. METHODS: We performed a retrospective analysis of the Nationwide Inpatient Sample(2004-2010),the largest all payer inpatient database in United States, to estimate the incidence and outcomes of ACS needing mechanical ventilation(MV) and excha

The impact of parenteral narcotic choice in the development of acute chest syndrome in sickle cell disease.

BACKGROUND: Acute chest syndrome (ACS) represents a serious morbidity and often fatal complication in patients with sickle cell disease. Painful episodes which require hospitalization are most often treated with opioids, which may then influence the development of ACS. Nalbuphine is a parenteral opioid which effectively treats pain and may cause less ACS. PROCEDURE: This retrospective chart review documented 988 admissions for painful episodes at 2 institutions and recorded the incidence of ACS and opioid used. RESULTS: At the Children's Hospital in St Louis, Missouri, the incidence of ACS i

Healthcare Workers' Conceptions of Acute Coronary Syndrome-Related Chest Pain: A Cross-Sectional, Descriptive Study.

BACKGROUND: Rapid identification of acute coronary syndrome (ACS) is critical to facilitate timely treatment. Chest pain is the most common ACS symptom; however, little research has focused on healthcare workers' conceptions of this symptom. OBJECTIVE: In this study, we aimed to describe how healthcare workers conceive of chest pain related to ACS, including symptom dimensions of timing, distress, intensity, and quality. METHODS: We used a cross-sectional, descriptive-comparative design, leveraging an electronic survey that included the Chest Pain Conception Questionnaire. Data were collecte

Silent Lung Injury: Acute Respiratory Distress Syndrome without Chest Trauma Following a High-Level Fall.

Acute respiratory distress syndrome (ARDS) is a life-threatening form of non-cardiogenic respiratory failure that can occur after severe systemic or traumatic insults. We report a rare case of ARDS in a 17-year-old female who sustained a burst fracture at the second lumbar vertebra and bilateral calcaneal fractures after jumping from a height of six meters. She underwent uneventful spinal decompression and instrumentation six hours post-injury, with stable hemodynamics and balanced fluid administration. Initial chest CT showed no lung abnormalities. On postoperative day 2, she developed respir

Chest pain management in acute coronary syndrome and chronic stable angina with diluted xylocaine injections.

Pain following myocardial ischemia is a significant clinical concern, contributing to increased sympathetic tone, mortality and morbidity. Standard management includes beta blockers, nitrates, aspirin-clopidogrel and opioids. We propose a robust pain portal block technique using 0.5% lignocaine at designated sites on the chest wall and upper limb. This method offers rapid and effective analgesia for both cardiac and non-cardiac chest pain, extending beyond lignocaine's typical duration of action. Early implementation of this technique may positively impact the management and outcomes of acute

Score Predicting Acute Chest Syndrome During Vaso-occlusive Crises in Adult Sickle-cell Disease Patients.

BACKGROUND: Vaso-occlusive crisis (VOC), hallmark of sickle-cell disease (SCD), is the first cause of patients' Emergency-Room admissions and hospitalizations. Acute chest syndrome (ACS), a life-threatening complication, can occur during VOC, be fatal and prolong hospitalization. No predictive factor identifies VOC patients who will develop secondary ACS. METHODS: This prospective, monocenter, observational study on SS/S-β0thalassemia SCD adults aimed to identify parameters predicting ACS at Emergency-Department arrival. The primary endpoint was ACS onset within 15days of admission. Secondary

Risk factors for acute chest syndrome in children with sickle cell disease undergoing abdominal surgery.

BACKGROUND/PURPOSE: The reported incidence of acute chest syndrome (ACS) in children with sickle cell disease (SCD) is 15% to 20%. Our current objective was to assess risk factors and morbidity associated with ACS. METHODS: The authors reviewed the outcome of children with SCD undergoing abdominal surgery over a 10-year period. RESULTS: From 1991 to 2003, 60 children underwent laparoscopic cholecystectomy (LC; n = 29), laparoscopic splenectomy (LS; n = 28), or both (LB; n = 3). Mean age was 8.6 (0.7 to 20) years, and 35 (58%) were boys. Fifty-four (90%) had a preoperative hemoglobin greater

[Spontaneous pneumomediastinum (Hamman's syndrome) with pneumorrhachis as a rare cause of acute chest pain in a young patient with acute asthma exacerbation].

Pneumomediastinum, defined as abnormal presence of air in the mediastinum, is a rare cause of acute chest pain. The condition may occur spontaneously as well as a secondary consequence of trauma or medical interventions. The spontaneous pneumomediastinum (Hamman's syndrome) is associated with a good prognosis, even without intervention. However, undelying severe conditions such as gastrointestinal perforations should be excluded. Diagnosis might be made using conventionell chest x-ray; a CT scan may give additional useful information. A subcutanous emphysema is a common finding in patients wit

Chest CT findings in severe acute respiratory distress syndrome requiring V-V ECMO: J-CARVE registry.

BACKGROUND: Chest computed tomography findings are helpful for understanding the pathophysiology of severe acute respiratory distress syndrome (ARDS). However, there is no large, multicenter, chest computed tomography registry for patients requiring veno-venous extracorporeal membrane oxygenation (V-V ECMO). The aim of this study was to describe chest computed tomography findings at V-V ECMO initiation and to evaluate the association between the findings and outcomes in severe ARDS. METHODS: This multicenter, retrospective cohort study enrolled patients with severe ARDS on V-V ECMO, who were
