BACKGROUND: Adenosarcoma is classified as a mixed epithelial and mesenchymal tumor composed of a benign epithelial component and a malignant stromal component. The stromal component in adenosarcoma is usually low grade, and consequently the prognosis is relatively favorable. While, adenosarcoma with sarcomatous overgrowth (SO) is defined as an adenosarcoma in which the sarcomatous component constitutes more than 25% of the tumor. The stromal component is also high-grade sarcoma showing greater nuclear pleomorphism and mitotic activity, thus, it is associated with worse prognosis. MRI findings

Mullerian adenosarcoma (MA) is a rare biphasic tumor with a malignant stromal component and benign glandular epithelium with periglandular stromal condensation occurring mainly in the uterus. The diagnosis is essentially morphologic and may be difficult particularly in distinguishing MA from other biphasic tumors at the lower end of the spectrum with histologic features similar to, but falling short-off adenosarcoma; i.e., uterine atypical polyp/low-grade or well-differentiated adenosarcoma with low mitotic count and lacking significant nuclear atypia. We investigated the transcriptomic profil

Mullerian adenosarcoma is a biphasic neoplasm composed of benign or atypical Müllerian epithelium and a malignant mesenchymal component that is usually, but not always, of low grade. Focal architectural or cytologic atypia of the epithelial component resembling atypical hyperplasia may uncommonly be present and foci of adenocarcinoma have been rarely reported. Whether the coexistence of these 2 tumor components is a result of independent primaries (collision tumor), adenocarcinoma arising from the epithelial component of the adenosarcoma, an unusual form of carcinosarcoma or some other mechani

"Mixed endometrioid adenocarcinoma and adenosarcoma" is a recently described malignancy of the uterus and ovary characterized by discrete components of Mullerian adenosarcoma and typically low-grade endometrioid adenocarcinoma. Its molecular biology, natural history, and proper classification remain uncertain. We analyzed clinicopathologic data and performed comparative molecular analysis on the adenocarcinomatous and adenosarcomatous components of 14 tumors using a 168-gene next-generation sequencing panel (n=11) and the MSK-IMPACT assay (n=3). Thirteen tumors were stage I, and 1 stage II. Th

BACKGROUND: Müllerian adenosarcoma (MA) is a rare low-grade malignant tumor. It is characterized by benign glandular epithelium and malignant sarcomatous stromal components. This tumor commonly arises in the uterine corpus. Primary ovarian MA is uncommon and typically presents as a unilateral lesion (FIGO stages I-III). However, bilateral ovarian involvement coupled with extensive metastasis in ovarian adenosarcoma has not been previously reported.
CASE PRESENTATION: This article reports a rare case of bilateral primary ovarian adenosarcoma with extensive metastasis and lymphovascular invasio

Uterine Müllerian adenosarcoma rarely presents as a cervical "cancer-like" mass; we report the first detailed case in a 79-year-old woman whose 5-cm exophytic lesion mimicked advanced cervical carcinoma on imaging. She presented with 2 weeks of post-menopausal bleeding and a friable polyp protruding through the cervical os; MRI described a bulky cervical tumour extending into the lower uterine segment, while tumour markers remained normal. Initial biopsy was inconclusive; subsequent excision and hysterectomy revealed FIGO stage Ib1 low-grade adenosarcoma originating from the lower uterine segm

Müllerian adenosarcoma harbors low malignant potential, except in cases with myometrial invasion or sarcomatous overgrowth. The presence of a high-grade stromal component has been proposed as an important pathologic predictor of outcome. We hypothesized that high-grade adenosarcoma has distinct clinical and molecular features, distinct from low-grade adenosarcoma. We analyzed the clinicopathologic features and follow-up of 9 high-grade adenosarcomas and a control group of 9 low-grade adenosarcomas. Comprehensive genomic analysis of the high-grade group was performed targeting exons of 409 onco

Müllerian adenosarcoma (MA) is a mixed Müllerian neoplasm composed of malignant stroma and benign epithelium. Endometrial and endocervical polyps are common entities in surgical pathology practice and most can be readily distinguished from MA; however, some have overlapping features, causing diagnostic confusion. In this study, we examined uterine polyps falling short of the diagnosis of MA quantitatively, qualitatively, or both. Our aims were to (1) characterize formally the morphologic features of atypical uterine polyps and (2) determine clinical outcome. Cases were evaluated for morphologi

Background. Mullerian adenosarcoma with sarcomatous overgrowth (MASO) is a very rare variant of uterine sarcomas first described by Clement et al. as early as 1974. The presence of heterologous sarcomatous components is associated with aggressive biological behavior. Case Presentation. This is a case report of a 62-year female (P(2 + 0)) presenting with postmenopausal vaginal bleeding. Her preoperative USG revealed subserosal fibroid with adherent omentum. She underwent abdominal hysterectomy with bilateral oophorectomy. Histopathological diagnosis of resected specimen was Mullerian adenosarco

In this report, we describe a case of adenosarcoma of the uterine corpus with overgrown and poorly differentiated sarcoma. The patient was an 80-year-old Japanese woman with abnormal genital bleeding. The endocervical biopsy showed undifferentiated carcinoma, and a cytologic examination at that time revealed the presence of cell types ranging from dispersed spindle to round atypical cells, suggestive of mesenchymal origin. The resected uterus had a polypoid mass in the corpus of the uterus and a solid white mass with the appearance of frank malignancy in the endocervix. The phyllode pattern wa

Six consultation cases of mullerian adenosarcoma of the uterus were encountered in women who were receiving adjuvant tamoxifen therapy for carcinoma of the breast. To our knowledge, only one previous similar case has been reported. The women, who were 48-76 years of age, had received tamoxifen for periods of 6 months to 4 years (mean 2.7 years) in five of the cases; the duration of tamoxifen therapy in the sixth case is unknown. All of the tumors were polypoid endometrial masses that superficially invaded the myometrium in two cases. The microscopic appearance of the tumors was similar to that

SMARCA4 gene encodes BRG1 , a member of the SWItch/sucrose non-fermentable protein family involved in epigenetic transcriptional regulation of important cellular processes. In the uterine corpus, SMARCA4 / BRG1 deficiency is associated with a novel class of undifferentiated uterine sarcomas, characterized by younger age onset, rhabdoid histology, focal phyllodiform architecture, high-risk pathologic findings, and dismal prognosis. Herein, we report a case of a 34-year-old Asian woman with a SMARCA4 / BRG1 -deficient uterine tumor fulfilling the clinicopathologic features of an undifferentiated

BackgroundSMARCA4 genetic alterations have been associated with aggressive neoplasms in various organs. In the uterus, SMARCA4-deficiency has been reported rarely, typically presenting as undifferentiated uterine sarcoma with a poor prognosis and younger age onset. The spectrum of SMARCA4-driven uterine tumors has recently expanded reporting hybrid features to include adenosarcoma components.Clinical PresentationWe report a 27-year-old woman who presented with abnormal uterine bleeding. The endometrial curettage revealed a high-grade adenosarcoma. One month later, the patient underwent tumor d

The molecular pathogenesis of uterine adenosarcoma (uAS; synonym: Müllerian adenosarcoma) is confounded by its pathologic heterogeneity, with DICER1 mutations and TP53 pathway alterations reported in most tumors with high-grade morphology, stromal overgrowth and/ or rhabdomyoblastic differentiation. However, a small subset of tumors harbor gene fusions, but their molecular spectrum and clinicopathological correlations have not been defined. We identified 12 uAS-like neoplasms in our files carrying ESR1 gene fusions and reviewed 4 previously reported cases (total: 16). Patient's age range was 3

BACKGROUND: In patients with uterine adenosarcoma, a total abdominal hysterectomy (TAH) with bilateral salpingo-oophorectomy (BSO) is typically recommended as an initial treatment. There is no consensus on adjuvant therapies.
CASE: We report the case of a patient with uterine adenosarcoma with postoperative residual disease. We performed four courses of adjuvant chemotherapy, including Ifosfamide, Mesna, and Adriamycin, and whole pelvic radiation with a dose of 50.4 Gy/28 Fr.
CONCLUSION: A combination of chemotherapy and radiotherapy may be a promising treatment option for uterine adenosarco

A 28-y-old woman was found to have a large subserosal uterine mass that was excised and interpreted as a "clear cell leiomyoma." Five years later, the tumor recurred as serosal-based ileal and uterine masses; they were treated by partial ileal resection and hysterectomy. All 3 masses were predominantly characterized by conspicuous edema separating bland cells growing in cords and clusters, with scant to moderately conspicuous clear cytoplasm. The edema was indistinguishable from the hydropic change commonly seen in benign smooth muscle tumors and the cords similar to those often present in the

OBJECTIVE: To present the case of a patient diagnosed with high grade adenosarcoma of the endocervix and the endometrial cavity, with a heterologous component, and to conduct a review of the literature focusing on the diagnosis and therapeutic management of this disease condition.
MATERIALS AND METHODS: We present the case of a 31-year-old female patient who came to Virgen Macarena University Hospital of Seville, a Level III regional institution, complaining of genital bleeding arising from an endocervical polypoid mass. The biopsy of the mass revealed a high grade, poorly differentiated leio
