Functional neurological disorders in hospitalized children and adolescents: a retrospective cohort study.

Functional Neurological Disorders (FNDs) are a frequent condition in childhood and adolescence, but data on hospitalized pediatric populations remain limited. In this study, we conducted a retrospective analysis of 136 pediatric patients (< 18 years) admitted to a tertiary neurological center over 11 years with a confirmed diagnosis of FND. Demographic, clinical history, and neurophysiological data were collected, and patients were classified according to their clinical phenotype. Mean age at symptom onset was 12.5 years, with prepubertal onset observed in 22% of cases. Multiple FNDs were pres

Hospital-at-home programmes targeting adolescents with eating disorders: a scoping review.

Hospital-at-home (HaH) programmes are emerging as alternatives to inpatient care for adolescents with eating disorders (EDs). The objective of this scoping review was to identify and map the existing evidence on HaH programmes targeting adolescents with EDs in vulnerable positions. Systematic searches were conducted on the MEDLINE, Embase, and CINAHL for studies describing HaH programmes targeting adolescents (aged between 10 and 19) suffering from EDs. Eight studies met the inclusion criteria. Data are reported descriptively and narratively. Despite an overall focus on EDs, all studies includ

Household income and first psychiatric hospital admissions among children and adolescents.

BACKGROUND: Parental socioeconomic factors are associated with mental health outcomes already during childhood, but gender differences in these connections have rarely been studied. AIMS: We explored the associations between household income and severe mental health disorders requiring psychiatric inpatient hospital care, with particular focus on gender differences. METHODS: In this national register-based cohort study, we followed over 1.6 million children and adolescents born in Finland between 1991 and 2020 until first psychiatric hospital admission, moving away from parents, death, emigr

Quality Improvement: A Template-Based Approach to HEADSS Assessments for Hospitalized Adolescents.

BACKGROUND: Hospitalizations are opportunities to screen adolescents for risk behaviors, but screening occurs infrequently. At our institution, adolescent inpatients on pediatric services have an array of medical acuity and complexity, and only 11% had complete home; education; activities; drug, alcohol, and tobacco use; sexual history; and self-harm, suicidality, and mood (HEADSS) histories. The aim of this quality improvement project was to increase the HEADSS completion rate to 31% within 8 months of the initial Plan-Do-Study-Act cycle. METHODS: A working group identified key drivers of in

Adverse Childhood Experiences and Mental Health in an Adolescent Partial Hospital Program.

Childhood adversity has been associated with myriad physical, emotional, and mental health symptoms across the lifespan, including higher risk for substance abuse, depression, suicidal ideation, and premature mortality. The current study evaluates the association between cumulative adverse childhood experiences and mental health distress at admission and discharge in an adolescent partial hospital program. Data were collected from 157 adolescents through clinical assessments administered during admission and discharge procedures (Youth Outcomes Questionnaire Self-Report (YOQ-SR), Treatment Sup

The Lake Alice Hospital Child and Adolescent Unit: Accountability - The Response to Date.

In the 1970s Dr Selwyn Leeks, a registered psychiatrist set up a child and adolescent unit in the grounds of Lake Alice Hospital, a New Zealand psychiatric facility. The adolescents were not diagnosed as having mental disorders but were placed there for behavioural issues. They were controlled by "aversion therapy" and subjected to electroconvulsive therapy treatment, paraldehyde injections and other forms of abuse. A number of complaints followed, which led to Government inquiries and reports. In 1999 a class action was commenced which was settled by the Government making "ex gratia" payments

Sexually Transmitted Infection Testing in Adolescents: Current Practices in the Hospital Setting.

PURPOSE: Adolescents are disproportionately affected by sexually transmitted infections (STIs), and evidence supports expanding sexual health services to the hospital setting. Because STI testing practices in the hospital setting are poorly understood, we sought to describe current STI testing practices among adolescents seen in children's hospitals. METHODS: We performed a retrospective analysis of adolescents (14-18 years old) from 45 children's hospitals in 2015-2016, excluding visits with a billing code for sexual abuse/assault. We calculated rates of STI testing and investigated differen

[School refusal: Psychopathological profiles of adolescents followed in a day hospital setting].

INTRODUCTION: School refusal appeared when formal education became compulsory in 1882, with the Jules Ferry laws. In international psychiatric classifications, school refusal does not appear as a diagnosis by itself, but it co-exists and is inherent to the symptoms of other psychiatric conditions, in particular of anxiety and depressive disorders. The main goal of this study was to describe psychopathological disorders in teenagers presenting school refusal followed in a day hospital setting. The second goal was to measure the level of anxiety, depressive emotion, self-esteem, loneliness, atta

Rotator cuff tears in children and adolescents: experience at a large pediatric hospital.

BACKGROUND: Prior literature, limited to small case series and case reports, suggests that rotator cuff tears are rare in adolescents. However, we have identified rotator cuff tears in numerous children and adolescents who have undergone shoulder MRI evaluation. OBJECTIVE: The purpose of this study is to describe the prevalence and characteristics of rotator cuff tears in children and adolescents referred for MRI evaluation of the shoulder at a large pediatric hospital and to correlate the presence of rotator cuff tears with concurrent labral pathology, skeletal maturity and patient activity

Venous thrombotic events in hospitalized children and adolescents with inflammatory bowel disease.

BACKGROUND: Adults with inflammatory bowel disease (IBD) have an increased risk of venous thrombotic events (TEs). We sought to evaluate the risk for TE in children and adolescents with IBD using a large population database. METHODS: The triennial Healthcare Cost and Utilization Project Kids' Inpatient Database was used in a retrospective cohort study of hospitalized children in the United States across 1997, 2000, 2003, 2006, and 2009. Billing codes were used to identify discharges with Crohn disease, ulcerative colitis, pulmonary embolism, deep vein thrombosis, thrombophlebitis, thrombosis

Sexual abuse is associated with obese children and adolescents admitted for psychiatric hospitalization.

OBJECTIVE: To determine the relationship between a history of child abuse and obesity among children admitted for psychiatric hospitalization. STUDY DESIGN: The charts of 1434 youth consecutively admitted to an inpatient psychiatric facility during a 10-month period were retrospectively analyzed. Rates of physical and sexual abuse, as well as other factors believed to increase the risk of obesity, were compared between children whose body mass index (BMI) percentiles were between 5 and 80 and whose BMI percentiles were >85. RESULTS: After correcting for age, race, gender, and antipsychotic u

Prevalence of pervasive developmental disorder in a sample of psychiatrically hospitalized children and adolescents.

Pervasive developmental disorder (PDD) is underrecognized because of lack of awareness of the wide range of severity and differing manifestations of the disorder. A survey of psychiatrically hospitalized children and adolescents was conducted. 3.2% of the sample of boys was found to have PDD. Underappreciation of the presence of PDD characterized the patients. Family members experienced a wide array of psychiatric symptoms and showed evidence for the presence of familial Tourette syndrome.
