Trauma stabilization points in armed conflict: operational evolution and strategic reflections from mosul to Gaza.

BACKGROUND: Forward trauma care remains a critical gap in emergency responses to armed conflict, particularly where conventional medical evacuation is disrupted. Trauma Stabilization Points (TSPs) were introduced in previous conflicts to bring life-saving care closer to the point of injury. To improve standards and adaptability, the World Health Organization convened a Technical Working Group to develop operational guidance for TSPs, drawing on diverse field experiences, including the ongoing Gaza deployments. CASE PRESENTATION: Between February and July 2024, three TSPs were established in G

Mangled extremity: limb salvage for reconstruction versus primary amputation.

PURPOSE OF REVIEW: While MESS has historically influenced limb salvage versus amputation decisions, its universal applicability remains uncertain. With trauma systems expanding and advancements in trauma care, the need for a nuanced understanding of limb salvage has become paramount. RECENT FINDINGS: Recent literature reflects a shift in the management of mangled extremities. Vascular surgery, plastic surgery, and technological advancements have garnered attention. The MESS's efficacy in predicting amputation postvascular reconstruction has been questioned. Machine learning techniques have em

Major haemorrhage: past, present and future.

Major haemorrhage is a leading cause of morbidity and mortality worldwide. Successful treatment requires early recognition, planned responses, readily available resources (such as blood products) and rapid access to surgery or interventional radiology. Major haemorrhage is often accompanied by volume loss, haemodilution, acidaemia, hypothermia and coagulopathy (factor consumption and fibrinolysis). Management of major haemorrhage over the past decade has evolved to now deliver a 'package' of haemostatic resuscitation including: surgical or radiological control of bleeding; regular monitoring o

Care of the dying patient in austere environments: a systematic review.

INTRODUCTION: Deployed end-of-life care represents an area of increasing importance amidst near-peer conflicts where air superiority is no longer guaranteed. This complex intersection of medical ethics and battlefield realities is an area where current doctrine is lacking. This leaves soldiers and military clinicians vulnerable to moral injury, without a clear decision-making framework. This systematic review explores the existing literature surrounding end-of-life care, euthanasia, reverse triage and end-of-life decision-making in austere environments. METHODS: A systematic search of MEDLINE

The Management of Pregnant Trauma Patients: A Narrative Review.

Trauma is the leading nonobstetric cause of maternal death and affects 1 in 12 pregnancies in the United States. Adhering to the fundamentals of the advanced trauma life support (ATLS) framework is the most important component of care in this patient population. Understanding the significant physiologic changes of pregnancy, especially with regard to the respiratory, cardiovascular, and hematologic systems, will aid in airway, breathing, and circulation components of resuscitation. In addition to trauma resuscitation, pregnant patients should undergo left uterine displacement, insertion of 2 l

Pediatric Trauma Readiness: A Trauma-Specific Assessment to Complement the National Pediatric Readiness Project.

OBJECTIVES: The National Pediatric Readiness Project of the Emergency Medical Services for Children surveyed emergency departments in the United States in 2013 for readiness to provide emergency care to children. However, that survey did not query for many elements considered essential to Advanced Trauma Life Support (ATLS). METHODS: Our pediatric trauma center and state department of health collaborated to develop a survey reflecting ATLS principles regarding pediatric-specific trauma stabilization, clinical/administrative resources, and interfacility transfer to complement the 2017 PedsRead

Adverse Childhood Experiences and Pediatric Dermatology: Implications for Care, Equity, and Research.

Adverse childhood experiences (ACEs) are increasingly recognized as important determinants of lifelong physical and mental health through mechanisms involving chronic stress, immune dysregulation, and inflammation. Although ACEs have been associated with numerous chronic medical conditions, their relevance to pediatric dermatology has received comparatively little attention. Existing evidence directly linking ACEs to dermatologic disease is limited and focuses primarily on atopic dermatitis and psoriasis, while research involving other pediatric skin disorders has largely examined psychosocial

End-of-Life Care in the Austere Military Environment.

INTRODUCTION: Throughout the Global War on Terror (GWOT), the U.S. military built a revolutionary capacity to deliver life-saving care-even in austere environments-through rapid evacuation, far-forward providers and resources, and advances in prehospital and trauma-critical care. However, the operational reality of future Large Scale Combat Operations (LSCO), as evidenced by wars in Ukraine, Sudan, and Gaza, will be marked by high casualty rates and limited medical resources. The number of patients with non-survivable injuries because of operational constraints such as inability to evacuate, m

[Psycho-oncological and psychosexual consequences of penile cancer: what can we offer our patients?].

Penile cancer is a rare but highly distressing form of cancer in men, associated with both physical and psychosocial challenges. With an incidence of 1-10 cases per 100,000 men, its prevalence varies worldwide, with higher rates observed in developing countries. The disease often leads to physical deficits, including loss of the penis and impairments in sexual life, which can result in significant psychological burdens such as anxiety, depression, and a diminished self-esteem. Stigmatization and social isolation play a central role, especially in cultures where masculinity and sexuality are cl

The immediate lifesaving management of maxillofacial, life-threatening haemorrhages due to IED and/or shrapnel injuries: "when hazard is in hesitation, not in the action".

Maxillofacial/neck vascular injuries caused by improvised explosive devices IEDs or ballistics injuries are life threatening when they cause severe haemorrhage resulting in airway compromise. One should always keep in mind that the best technique used is that which saves the patient's life and not the most expensive and/or technologically advanced. Medical professionals on the scene should have the necessary experience to handle the emergency situations of airway compromise and haemorrhage control. In this instance there is only, "one to a few minutes" to clear airway obstruction and arrest ha

High-ACE Low Wage Workers: Occupational Health Nursing Research and Praxis Through a Trauma-Informed Lens.

Individuals with a history of adverse childhood experiences (ACEs) disproportionately have poor mental and physical health outcomes. These experiences affect individuals across the life span extending beyond health with deleterious impact on work-related outcomes. Low-wage workers are particularly at risk. Social service and health organizations are becoming aware of the extent to which the populations they serve have been affected by these experiences. Employment support programs may serve high-ACE individuals but likely are unaware of their histories and the developmental or health deficits

Artificial Intelligence-Assisted Ultrasound in Austere Trauma Environments: Implications for Role 1 Military Medicine.

Pararescue jumpers are United States Air Force medical tactical operators who provide advanced trauma and prolonged casualty care in austere, high-risk environments. Trauma represents the predominant clinical scenario within their operational mission set, and care is frequently delivered at or near the point of injury when evacuation to definitive treatment is delayed or unavailable. Despite extensive medical training, care at the most forward level, referred to as Role 1, remains fundamentally limited by the absence of diagnostic imaging. Clinical assessment relies primarily on physical exami

The lethal triad: Past, present, and future.

The post-traumatic "lethal triad" is a cornerstone theory in modern trauma care, and its evolution mirrors the paradigm shift in trauma medicine from empirical treatment to evidence-based precision medicine. This review traces the development of this theory, from the "bloody vicious cycle" first described in 1982, to the reconceptualization of acute traumatic coagulopathy in 2003, the establishment of damage control resuscitation strategies in 2007, and the revision of diagnostic criteria in 2016 to focus on fibrinolysis. Current clinical controversies primarily center on 3 aspects: (1) confus

Diagnostic Modalities in Critical Care: Point-of-Care Approach.

The concept of intensive care units (ICU) has existed for almost 70 years, with outstanding development progress in the last decades. Multidisciplinary care of critically ill patients has become an integral part of every modern health care system, ensuing improved care and reduced mortality. Early recognition of severe medical and surgical illnesses, advanced prehospital care and organized immediate care in trauma centres led to a rise of ICU patients. Due to the underlying disease and its need for complex mechanical support for monitoring and treatment, it is often necessary to facilitate bed

Severe Necrotizing Fasciitis Following Minor Neglected Left-Hand Wound Complicated With Toxic Shock Syndrome and Multiorgan Failure: A Case Report and Literature Review.

Necrotizing fasciitis (NF) is a life-threatening soft tissue infection requiring urgent intervention. Despite advances in care, delays in diagnosis contribute to high morbidity and mortality. Here, we present a 17-year-old male patient previously healthy, who developed NF and toxic shock syndrome (TSS) following minor hand trauma. He developed multiorgan failure in the form of acute respiratory distress (ARDS), circulatory failure, high hepatic enzymes, and coagulopathy. The patient was managed successfully with early surgical debridement, antibiotics, immunoglobulins, and critical care suppor

Blunt Traumatic Isolated Left External Iliac Vein Injury Without Pelvic Fracture.

Isolated iliac vein injuries from blunt trauma are uncommon but potentially life‑threatening. We report a case of isolated left external iliac vein injury in a 39-year-old female patient following blunt trauma sustained in an electric scooter-related motor vehicle collision. Despite the absence of pelvic fractures, the patient presented with hypotension requiring massive hemorrhage protocol activation and vasopressor support. Computed tomography demonstrated a pelvic hematoma with active venous contrast extravasation. Diagnostic angiography was performed, but endovascular repair was not feasib
