Alexia without agraphia and associated disorders: importance of recognition in the rehabilitation setting.

Alexia without agraphia is readily recognized in its pure (without other neurologic findings) but rare form. However, this deficit is more common when associated with other behavioral disturbances that result from trauma or cerebral infarction to the posterior cerebral hemispheres. Two cases presented with alexia without agraphia following infarction within the posterior cerebral artery territory. Because of initial confusion and disorientation the patients were diagnosed as demented and unsuitable for rehabilitation. Both subjects had a visual field deficit and color agnosia. Multiple component agraphia in a patient with atypical cerebral dominance: an error analysis.

A 52-year-old man with atypical cerebral dominance (left-handed for writing but mixed handedness for other tasks) suffered an extensive right hemisphere stroke, resulting in a combination of deficits that has not been previously reported. There were profound visual constructive and visual perceptual disturbances and a spatial agraphia, which were consistent with a nondominant hemisphere lesion. There was also a severe apraxic agraphia, which is typically associated with a dominant hemisphere lesion, but no other signs of dominant hemisphere dysfunction such as linguistic disturbance or limb-mo

[Alexia without agraphia - not being able to read what one has written].

An elderly, right-handed, male patient presented to the emergency department at a middle sized hospital with a few hours' history of mild confusion and a difficulty expressing himself verbally. Past medical history consisted of hypertension and cured hairy cell leukemia. CT scan of the brain showed a 5 cm infarction in the left occipital lobe, and a corresponding P2 occlusion was seen. The patient was admitted to the stroke unit. While at the stroke unit, a detailed neurologic exam by a neurologist was performed. It was revealed that the patient could not read at all, but he could write perfec

Agraphia for kanji resulting from a left posterior middle temporal gyrus lesion.

OBJECTIVE: To clarify whether agraphia or alexia occurs in lesions of the left posterior middle temporal gyrus. METHODS: We assessed the reading and writing abilities of two patients with this lesion using kanji (Japanese morphograms) and kana (Japanese syllabograms). RESULTS: Patient 1 first presented with pure alexia more impaired for kana after an infarction in the left middle and inferior occipital gyri and right basal occipital cortex, and after a second infarction in the left posterior middle temporal gyrus adjoining the first lesion he showed alexia with agraphia for kanji and worsene

[Alexia-agraphia of kanji (Japanese morphogram) after left posterior-inferior temporal lesion].

Several cases of selective alexia with agraphia of kanji have been reported in Japan in this decade. It is well known that the lesion in the posterior inferior temporal lobe of the dominant hemisphere is responsible for this cognitive syndrome. Neuropsychological data in our patient suggest that the postero-inferior region of the temporal lobe of the dominant hemisphere may be the visuo-verbal association area for the analysis of the complex visuo-verbal information. The symptoms caused by the same lesion in western patients might be subangular alexia (alexia without agraphia). Alexia with agr

[Clinical picture of left posterior cerebral artery occlusion. Alexia syndrome without agraphia].

The authors present a clinical description of three cases of occlusion of the left posterior cerebral artery. The pathogenesis of the syndrome of alexia without agraphia is discussed, together with other coexistent symptoms and signs such as hemianopsia, colour anomia, disturbances of recent memory. The discussion is based on own observations and a review of the pertinent literature.

Posterior cortical atrophy with prominent alexia without agraphia in a Tourette syndrome.

We report for the first time a patient with childhood-onset Tourette's syndrome (TS) who developed alexia without agraphia, acalculia, visual agnosia for objects and faces, and preserved mnesic functions in older age. Neuroimaging studies demonstrated temporo-parieto-occipital cortical atrophy and fronto-temporo-parieto-occipital hypometabolism, both more prominent on the left side. This case fulfils the diagnostic criteria of posterior cortical atrophy (PCA). The possible link between TS and PCA is discussed.

Compensational strategies for a merchant after stroke with anosognosia for alexia without agraphia.

We report the first case, to our knowledge, of successful return to work of a patient with alexia without agraphia. This case is also interesting as it is the first report of which we are aware of anosognosia for alexia without agraphia: the patient confabulated when asked to read English text, but immediately stated that he could not read Chinese text because he did not know that language. The selective nature of this confabulation would not be inconsistent with anosognosia being one of the brain's responses to absence of afferent information.

The Gogi (word-meaning) syndrome with impaired kanji processing: alexia with agraphia.

Two cases, one with probable Pick's disease and one with herpes simplex encephalitis, are presented, focusing on Gogi (word-meaning) aphasia-like syndrome as their salient clinical feature. Their aphasic symptoms were characterized by impaired kanji processing and preserved kana processing in writing and oral reading known as a defining feature of Gogi aphasia, but little evidence of so-called "phonetic use of kanji in writing" and "confusion between on- and kun-readings of kanji in oral reading" often observed in Gogi aphasia. Systematic neuropsychological test batteries administered to them

Pure apraxic agraphia with recovery after drainage of a left frontal cyst.

A case is reported of isolated apraxia of writing in which general limb praxis was spared. Spontaneous writing, writing to dictation and copying were equally affected. Letter selection and spelling were preserved and other language functions intact. A CT scan revealed a left frontal arachnoid cyst. After surgical drainage the agraphia resolved. The anatomical basis of the disorder is discussed in the light of modern information processing models of writing.

Alexia and Agraphia Intervention Following Traumatic Brain Injury: A Single Case Study.

Purpose This case study documents the effectiveness of a multicomponent intervention for an adolescent with acquired alexia and agraphia following severe traumatic brain injury. Method Initial testing revealed severe central alexia and surface agraphia with concomitant anomic aphasia. Intervention components included sight word drills, modified Multiple Oral Reading (MOR) procedures, functional reading tasks, and modified Copy and Recall Treatment. Intervention spanned 2 months with sessions 5 days per week. Data collection and analysis involved monitoring sight word decoding, reading speed an

Alexia Without Agraphia in a Right-Handed Individual Following Right Occipital Stroke.

Alexia without agraphia is a disconnection syndrome that typically involves damage to the occipital lobe, with splenium involvement, in the dominant left hemisphere. We describe an exceptionally rare case of a right-handed individual displaying this deficit following a right-sided occipital stroke. A report of a single case of a 65-year-old man is presented with data from appointments with the neurology and neuropsychology departments that occurred approximately 10 and 12 months following the patient's stroke. During the evaluation, he exhibited a marked deficit in his ability to read, with vi

Selective Kanji agraphia in crossed aphasia after right-hemisphere infarction: a case report.

Crossed aphasia is a rare form of aphasia in right-handed individuals with right-hemisphere lesions. We report a man aged 67 years who developed crossed aphasia after a right posterior cerebral artery infarction involving the occipital lobe, thalamus, and medial parietal lobe. Neurological and neuropsychological examinations revealed mild anomic aphasia with preserved repetition and comprehension, left homonymous hemianopia, unilateral spatial neglect, prosopagnosia, and no apraxia. Writing showed a marked script-specific dissociation: kana writing was intact, whereas kanji writing was severel

Proactive interference apraxic agraphia: a writing and drawing disorder associated with corticobasal syndrome.

Proactive interference is when a previously performed task impairs performance on a current task. It is often associated with memory tasks and has not been reported to interfere with writing or drawing. We evaluated a left-handed man diagnosed with corticobasal syndrome who had a two-year history of progressive agraphia. On the sentence writing and clock drawing tasks, he initially wrote letters and numbers correctly but revealed an increase of movement errors as the tasks progressed. We propose the term "proactive interference apraxic agraphia" for this novel disorder. Prefrontal dysfunction
