Deep extubation versus no-touch awake extubation in adults undergoing elective laparotomy: A randomised controlled trial.

BACKGROUND AND AIMS: Tracheal extubation is a vulnerable phase of general anaesthesia. Smooth extubation strategies aim to reduce cough, bucking, and the sympathetic surge. This trial compared deep extubation with no-touch awake extubation in adults undergoing elective laparotomy. METHODS: This prospective, randomised, partially outcome-assessor-blinded trial enroled American Society of Anesthesiologists (ASA) physical status I-II adults scheduled for elective laparotomy, allocated to deep extubation (Group D) or no-touch awake extubation (Group N). The primary outcome was cough (grade ≥1) wi

A comparison of deep versus awake tracheal extubation in adults: a randomized controlled trial.

BACKGROUND: Awake extubation is deemed a generally safe option for tracheal extubation in low-risk patients, while anesthetized, or"deep" extubation is often considered best suited to seasoned providers due to its perceived hazards. However, inadequate data exists about the relative safety of deep and awake extubations in the adult population. METHODS: Adult patients (n=220) with favorable airways undergoing elective surgeries under general tracheal anesthesia were enrolled prospectively. Following a standardized volatile anesthetic regimen, patients were randomized to undergo deep or awake t

A rare case of difficult airway management in a Klippel-Feil syndrome pediatric patient with osseous torticollis undergone orthopedic surgery : Difficult airway in pediatric patient with torticollis.

BACKGROUND: Orthopedic surgery for cervical torticollis poses potential threat to airway management both in tracheal intubation and extubation. Klippel-Feil syndrome (KFS) is a complex syndrome of osseous and visceral anomalies. The anatomical characteristics of KFS might have significant implications for airway management. CASE PRESENTATION: This is a rare case of an 8-year-old boy presenting with osseous torticollis, congenital occipito-atlantal deformity, congenital basilar invagination and KFS undergone elective torticollis correction surgery. Though with difficulty, tracheal intubation w

A New Distal Pharyngeal Airway Device Associated with a Reduced Need for Chin-Lift and Jaw-Thrust Maneuvers in Sedated Patients.

Oropharyngeal airways (OPA) or nasopharyngeal airways (NPA) sometimes require chin-lift or jaw-thrust (CLJT) maneuvers to relieve airway obstruction which creates the burden of continuous hands-on care by the anesthesia provider. A new distal pharyngeal airway device (DPA) was used on 63 successive ambulatory surgery patients to assess the frequency of patients requiring manual CLJT maneuvers to prevent airway obstruction. Results were then compared with a contemporaneous group of patients who had used OPA or NPA devices for similar procedures. Patients using the DPA had a 38.5% lower rate of

Endoscopic survey of post-extubation stridor in children.

Endotracheal intubation is an important airway procedure in the management of neonates and infants. Prolonged intubation may result in severe laryngeal injury which may lead to chronic laryngeal disability. The nature and causes of post-extubation airway obstruction were evaluated with videoendoscopy. From June 1998 to September 2000, detailed videoendoscopic examination of pediatric airway was performed in 30 children who had persistent stridor after endotracheal extubation or failure of elective extubation. Common locations of post-extubation lesion are the posterior glottis and subglottis.

Oropharyngeal ultrafast ultrasound measurements in mechanically ventilated critically ill patients do not identify post-extubation stridor.

BACKGROUND: Post-extubation stridor is a common complication of endotracheal intubation in ICU. This study aimed to assess whether a series of pre-extubation upper airway ultrasound measurements using shear wave elastography (SWE) could help in detecting post-extubation stridor. METHODS: A prospective observational study (NCT05611437) was conducted between 2022 and 2024, which consecutively included 150 adults ICU patients intubated for more than 24 h, without prior surgical or neurological upper airway disease nor swallowing disorders. SWE measurements were performed in the 24 h before extub

Incidence of airway complications in patients using endotracheal tubes with continuous aspiration of subglottic secretions.

BACKGROUND: Continuous aspiration of subglottic secretions is effective in preventing ventilator-associated pneumonia, but it involves a risk of mucosal damage. The main objective of our study was to determine the incidence of airway complications related to continuous aspiration of subglottic secretions. METHODS: In consecutive adult patients with continuous aspiration of subglottic secretions, we prospectively recorded clinical airway complications during the period after extubation. A multidetector computed tomography of the neck was performed during the period of 5 days following extubati

Retrograde Lidocaine Spray for Awake Extubation in Pediatric ENT Surgery: Optimizing Tonsillectomy and Adenoidectomy Outcomes.

Laryngeal edema, a frequent manifestation of acute inflammation, is particularly significant due to the potential obstruction of the laryngeal orifice caused by swelling of the epiglottis and vocal cords. This presents as a risk factor that can lead to severe airway obstruction. Traditionally, deep extubation is the preferred form of extubation because it is more comfortable for the patient, eliminates the airway reflexes, and minimizes the risk of laryngeal edema. Difficult mask ventilation (DMV), characterized by an unassisted anesthesiologist's inability to maintain oxygen saturation levels

The Radiographic "Air Sign" in Neonatal Mandibular Distraction Osteogenesis.

Timing of extubation on post-mandibular distraction osteogenesis (MDO) surgery is critical, given that at baseline these infants have difficult airways and failed extubation requires either re-intubation of an already complex airway with a fragile, recently osteotomized mandible, or adjunctive airway measures such as CPAP that may apply unwanted pressure to the surgical site. Thus, the goal is to plan extubation when the risk of failure is minimal. Currently, there is a void in the literature addressing the timing of extubation post-MDO and no objective sign of extubation readiness has been el

Recurrent negative pressure pulmonary edema.

An African-American man, aged 34 years, underwent an elective uncomplicated right wrist laceration repair while under general anesthesia. Following extubation, the patient developed hypoxemia, tachypnea, shortness of breath, pulmonary rales, frothy sputum, decreased oxygen saturation, and evidence of upper airway obstruction. Chest radiograph showed pulmonary edema. The patient was diagnosed with post-extubation pulmonary edema (aka. negative pressure pulmonary edema [NPPE]) and was treated with intravenous furosemide and oxygen therapy; he improved remarkably within a few hours. Once stabiliz

Evaluation of Flexible Laryngeal Mask Airway® in Tongue Trauma Repair: A Randomized Trial.

BACKGROUND: Flexible laryngeal mask airway may be used instead of the endotracheal tube in children presented for elective repair of tongue trauma, as it may shorten the time for extubation and recovery with the amelioration of stress response to airway management. OBJECTIVES: This study aimed to evaluate the extubation time and hemodynamic response to the endotracheal tube or flexible laryngeal mask airway in children presented for tongue trauma repair. METHODS: The study recruited 90 children presented for elective repair of tongue trauma that were randomly assigned into the following grou

Perioperative airway management techniques and complications in patients with temporomandibular joint ankylosis: Experience from a tertiary care teaching institute.

BACKGROUND AND AIMS: Temporomandibular Joint (TMJ) ankylosis patients pose serious anesthetic challenges due to difficult airway and obstructive sleep apnoea (OSA). However, data are sparse on anesthetic management and perioperative outcomes of such patients. This study aimed to identify the anesthetic and airway management techniques in children and adolescents with TMJ ankylosis and whether the presence of retrognathia and OSA increases the risk of airway-related complications. MATERIALS AND METHODS: A retrospective anesthetic chart review of TMJ ankylosis patients undergoing maxillo-facial

Effectiveness of Nasal Continuous Positive Airway Pressure vs Nasal Intermittent Positive Pressure Ventilation vs Noninvasive High-Frequency Oscillatory Ventilation as Support After Extubation of Neonates Born Extremely Preterm or With More Severe Respiratory Failure: A Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: The NASONE (Nasal Oscillation Post-Extubation) trial showed that noninvasive high-frequency oscillatory ventilation (NHFOV) slightly reduces the duration of invasive mechanical ventilation (IMV) in preterm infants, whereas NHFOV and noninvasive intermittent positive pressure ventilation (NIPPV) result in fewer reintubations than nasal continuous positive airway pressure (NCPAP). It is unknown whether NHFOV is similarly effective in extremely preterm neonates or in those with more severe respiratory failure (based on the duration of previous ventilation and CO2 levels). OBJECTIVE:

A randomised controlled trial to compare tracheal extubation quality in lateral and supine positions after general anaesthesia in children.

BACKGROUND AND AIMS: Tracheal extubation after general anaesthesia in pediatrics is a critical event, with sparse research on positioning of patient during extubation. Clinical data reveal lesser airway obstruction in the lateral position with enhanced airway patency. We studied the effects of supine versus lateral positioning on the quality of tracheal extubation as judged by the modified Minogue cough score in children undergoing elective surgeries under general anaesthesia. Secondary objectives included frequency of incidences of oxygen desaturation (SPO2 <92%), laryngospasm, bronchospasm,

Surfactant status in preterm neonates recovering from respiratory distress syndrome.

OBJECTIVE: The goal was to establish whether reduced amounts of pulmonary surfactant contribute to postextubation respiratory failure in preterm infants recovering from respiratory distress syndrome. METHODS: We prospectively recruited preterm infants who needed mechanical ventilation and exogenous surfactant for treatment of moderate/severe respiratory distress syndrome and could not be extubated before day 3 of life. (13)C-labeled dipalmitoyl-phosphatidylcholine was administered endotracheally as tracer before extubation, for estimation of surfactant disaturated phosphatidylcholine pool siz

Utility of fiberoptic bronchoscopy for difficult airway in neonates.

BACKGROUND: Newborns diagnosed with craniofacial malformations or laryngeal and tracheal alterations may often need advanced airway-management for airway stabilization. Although fiberoptic bronchoscopy (FB) is currently the gold standard for difficult airway management, there is a scarcity of published data on the application of FB in newborns for intubation and controlled extubation (CE). OBJECTIVES: This study describes a case series where FB is used for intubation and/or extubation to manage newborns with difficult airway in either urgent procedures or scheduled ones. METHODS: All FB were

Ethosuximide induced macroglossia and oropharyngeal edema.

Acute macroglossia and laryngeal edema are rare adverse side effects that can cause life-threatening airway obstruction. We report a case of acute macroglossia that began after initiation of ethosuximide in a 15-year-old female with severe medically refractory epilepsy. Macroglossia worsened over the next two weeks of ethosuximide administration, preventing extubation. Macroglossia and laryngeal edema improved upon ethosuximide wean, and completely resolved after discontinuation. The patient was extubated successfully, with precautionary nasal trumpet placement and dexamethasone administration
