Description of the combined evidence-based, theory-based and person-based approaches used to develop a behavioural intervention package to support non-allergist healthcare workers to remove incorrect penicillin allergy records from medical and surgical adult inpatients in a UK hospital.

OBJECTIVES: To develop a behavioural intervention package to support non-allergist healthcare workers (HCWs) to remove incorrect penA records from medical and surgical adult inpatients. This paper describes the development of the penicillin allergy de-labelling (PADL) intervention and the implementation intervention that will support non-allergist-delivered PADL. DESIGN: We combined evidence-based, theory-based and person-based approaches. Qualitative research with healthcare professionals and patients explored barriers and enablers to implementation of the proposed PADL pathway. Key interven

Baked Milk and Baked Egg Survey: A Work Group Report of the AAAAI Adverse Reactions to Foods Committee.

Most milk- and egg-allergic children can tolerate milk and egg in baked forms. Some allergists have extended the use of baked milk (BM) and baked egg (BE) to advocating for the stepwise introduction of small amounts of BM and BE to children who are reactive to larger amounts of BM and BE. Little is known about the practice of introducing BM and BE and existing barriers to this approach. The purpose of this study was to gather a current assessment of the implementation of BM and BE oral food challenges and diets for milk- and egg-allergic children. We conducted an electronic survey of North Ame

How to Recognize and Manage the Psychosocial Implications of Having a Food Allergy.

Children with food allergy (FA) and their families are vulnerable to maladaptive coping patterns to managing their FA, often characterized by excessive avoidance and distress. Allergists have an important role in promoting adaptive coping with FA, identifying when anxiety has become excessive, and implementing steps to support reductions in anxiety and distress as a part of patient care in FA management. Allergists can prevent and reduce maladaptive coping with FA by providing anticipatory guidance tailored to the individual to ensure accurate perception of risk associated with their FA and by

[Management of allergic conjunctivitis. An observational study among ophthalmologists].

PURPOSE: To describe the management of allergic conjunctivitis in private ophthalmology practice and to evaluate the collaboration between ophthalmologists and allergists. PATIENTS AND METHODS: Multicenter retrospective study. Each ophthalmologist was required to collect demographic and clinical data from the records of 10 patients with a 2- to 3-year history of allergic conjunctivitis. RESULTS: A total of 353 records were interpretable, corresponding to 885 visits. Slightly more than 60% of patients had a history of allergy (rhinitis, asthma or cutaneous allergy), and 40% of patients had ha

Using oral food challenges to provide clarity and confidence when diagnosing food allergies.

A verified food allergy can be an impactful life event that leads to increased anxiety and measurable effects on quality of life. Allergists play a key role in framing this discussion and can help alleviate underlying fears by promoting confidence and clarifying safety concerns. Correctly diagnosing a patient with an immunoglobulin E (IgE) mediated food allergy remains a nuanced process fraught with the potential for error and confusion. This is especially true in situations in which the clinical history is not classic, and allergists rely too heavily on food allergy testing to provide a confi

Prescriptions for self-injectable epinephrine and follow-up referral in emergency department patients presenting with anaphylaxis.

BACKGROUND: Anaphylaxis guidelines recommend that patients with a history of anaphylactic reaction should carry self-injectable epinephrine and should be referred to an allergist. OBJECTIVE: To evaluate how frequently patients dismissed from the emergency department after treatment for anaphylaxis received a prescription for self-injectable epinephrine or allergist referral. METHODS: A retrospective medical record review identified patients with anaphylaxis in a community-based study from 1990 through 2000. Records of patients with Hospital Adaptation of the International Classification of D

Treatment update: allergic rhinitis.

In addition to the introduction of several new pharmacologic agents, two of the most significant recent developments in the management of allergic rhinitis have been the renewed emphasis on preventive measures, such as allergen avoidance and immunotherapy, and the importance of performing an accurate differential diagnosis of the disease. Recently, these evolving management trends were delineated in an algorithm proposed by the Joint Task Force on Practice Parameters in Allergy, Asthma and Immunology, which suggests that an initial evaluation be performed by a primary care physician. Based on

Immunology of the eye.

The eye is one of the major targets of allergic disorders, accounting for over 20% of the conditions presenting to the allergist. It is also a major target organ of many systemic immunologic reactions (e.g., cytotoxic antibody mediated-cicatricial pemphigoid, immune complex or cell mediated-vasculitis, uveitis). The allergist/immunologist is uniquely trained to assist the ophthalmologist in the systemic immunologic evaluation and treatment of suspected hypersensitivity allergic and immunologic disorders.

Differences between allergy and dermatology in referral, evaluation, and management patterns for pediatric patients with atopic dermatitis.

INTRODUCTION: Allergists and dermatologists often take different approaches to caring for pediatric patients with atopic dermatitis (AD). METHODS: A retrospective chart review was performed on patients <18 years old treated for AD within the University of Virginia health system from 2015 to 2020. Data were collected on patient and referring provider demographics as well as initial visit evaluation and management. RESULTS: A total of 269 patients presented to allergy, 685 patients presented to dermatology, and 14 patients presented to a combined allergy-dermatology clinic as an initial visit

Tourette's syndrome in patients referred for allergy evaluation.

BACKGROUND: Tourette's syndrome is a relatively common disorder that may present as a spectrum that can include both motor and behavioral features. METHODS: We report four patients referred to allergists to rule out atopy as a reason fro their presumed respiratory symptoms who were subsequently found to have Tourette's syndrome. A review of the literature was also performed using Medline. RESULTS: The symptoms of Tourette's syndrome may overlap those associated with allergy involving the respiratory tract. There is little published information on the association of Tourette's syndrome with a

Drug allergy in hospitalized perioperative patients: the contribution of a structured questionnaire for accurate diagnosis.

AIMS: History of drug allergy is a major cause for deviation from standard of care, as well as prescribing expensive medications. We aimed to evaluate drug allergy-related history taking by surgery interns and compare it with history taking by an allergist and an anesthetist using a simple, structured questionnaire. METHODS: Patients with a declared drug allergy were prospectively recruited from surgical wards. The interns' drug allergy diagnosis was compared with that of the allergy specialist and anesthetist that used a structured questionnaire. RESULTS: A total of 195 patients with 305 re

Anesthetic procedure outcomes after initial perioperative anaphylaxis: A retrospective multicenter US review.

BACKGROUND: Limited information exists regarding outcomes during repeat surgical procedures in patients with perioperative anaphylaxis (POA). Although current guidelines recommend that future procedures be guided by allergy evaluations, little is known about adherence to these recommendations or risk of recurrence. OBJECTIVE: Our aim was to assess the incidence of recurrent POA and evaluate adherence to allergist recommendations during subsequent anesthetic procedures. METHODS: We performed a retrospective review of 100 patients with POA across the Mayo Clinic Health System (2010-2021) to as

Management of a surgical patient with a label of penicillin allergy: narrative review and consensus recommendations.

Unsubstantiated penicillin-allergy labels are common in surgical patients, and can lead to significant harm through avoidance of best first-line prophylaxis of surgical site infections and increased infection with resistant bacterial strains. Up to 98% of penicillin-allergy labels are incorrect when tested. Because of the scarcity of trained allergists in all healthcare systems, only a minority of surgical patients have the opportunity to undergo testing and de-labelling before surgery. Testing pathways can be modified and shortened in selected patients. A variety of healthcare professionals c

Unproven Diagnostic Tests for Adverse Reactions to Foods.

Patients often seek opinions from allergists regarding unconventional testing for adverse reactions to foods. These tests include flow cytometry to measure the change in white blood cell volumes after incubation with foods, measurement of serum IgG or IgG4 antibodies directed against foods, intradermal provocation-neutralization with food allergens, hair analysis, electrodermal testing, and applied kinesiology. In some cases, although the laboratory methods may be valid, there are no studies showing correlation with disease. In other cases, blinded, controlled studies have shown a lack of repr

Skin testing and drug challenge in the evaluation of drug hypersensitivity reactions.

Immediate hypersensitivity to drugs is characterized by symptoms such as hives, swelling, and wheezing. To prevent a negative impact on care, assessment by an allergist is important. Evaluation requires a clear clinical history, but it is often lacking or vague, which makes a diagnosis difficult. Allergists instead can use skin testing and drug challenge to evaluate drug hypersensitivity reactions, which help the patient and provider understand the causative drug(s) and, more importantly, enables the use of the exonerated drug(s). Although penicillin skin testing is standardized, well describe

Atrial fibrillation associated with anaphylaxis during venom and pollen immunotherapy.

BACKGROUND: Electrographic changes after insect stings have been reported both with and without systemic symptoms. To our knowledge, however, there have been no reported cases of arrhythmias in the maintenance phase of venom or pollen immunotherapy. OBJECTIVE: We report a case of a patient who developed paroxysmal atrial fibrillation after receiving venom and pollen immunotherapy. This patient was initially evaluated in an outpatient setting, and the events described occurred at an allergist's private office. We believe this is an atypical presentation and would be very pertinent to the pract

Multiple Chemical Sensitivity/Idiopathic Environmental Intolerance: A Practical Approach to Diagnosis and Management.

Patients with multiple chemical sensitivity, now called idiopathic environmental intolerance, frequently present to clinical immunologists and allergists for diagnosis and treatment. Patients report a plethora of respiratory and multisystem problems attributed to a wide variety of unrelated, otherwise non-noxious, triggers. They may go to extreme, often seemingly bizarre lengths to avoid contact with everyday exposures and may become housebound, unable to work or function socially. Often beginning with exposure to odors, triggers can multiply to involve foods, clothing, medications, and even e

Management of the Pregnant Patient with Beta-Lactam Allergy.

PURPOSE OF REVIEW: We review the literature and discuss the logistics of testing pregnant patients for penicillin allergy. RECENT FINDINGS: As in the general population, pregnant patients commonly report a penicillin allergy, but most patients are able to tolerate penicillin. Avoidance of beta-lactams in pregnancy is associated with increased morbidity: longer hospitalizations, more frequent infections, and more complications. Penicillin allergy testing is safe in pregnant patients, and obstetricians are eager for allergists to offer this procedure to their patients. As allergists, we can imp
