Functional outcome of rehabilitated bilateral lower limb amputees.

The functional outcome of rehabilitated bilateral lower limb amputees was studied. The study included 31 amputees who were admitted during 1980-1990 to a rehabilitation centre in the north of the Netherlands. The clinical notes made during the patients' admission were studied to obtain information about their characteristics, while mobility and prosthetic use were studied at discharge. The patients who were alive and willing to participate in the study were interviewed by a physician at their residence in November 1992, using among other things, the Sickness Impact Profile (SIP) and the Life S

Running gait impulse asymmetries in below-knee amputees.

In running, large gait asymmetry is expected due to the inability of the foot prosthesis to comply with the kinematic demands and produce a powerful plantarflexion moment. In this work, interlimb asymmetry in below-knee (BK) amputee running gait was assessed for one rigid and three flexible keel prostheses, using vertical and anteroposterior ground reaction forces and respective impulses. Nine BK amputees and 6 controls participated in this study. The running speed was monitored by two light sensitive detectors while the ground reaction forces were measured with a Kistler force plate. Improved ankle-foot system for above-knee amputees.

Above-knee amputees were studied while wearing a prosthesis containing either a standard or a modified single axis foot. The modified foot allowed 12-15 degrees of dorsiflexion and reduced stiffness in the area of the metatarsal-phalangeal (MTP) joint. The amputees were allowed to vary the stiffness at the two joints to obtain what "felt best." The amputees agreed that in comparison to the standard single-axis foot, the modified foot: a) offered a more comfortable gait while walking on level ground; b) made walking up inclines easier; c) affected knee stability while walking on declines; and d

Distal blood pressure measurement in lower-limb amputees.

Reamputation above the knee after failure of primary below-knee amputation was related to the distal blood pressure. With pressures of 20, 30 and 40 mmHg, two-thirds, one-third and one-seventh, respectively, of the below-knee amputations required reamputation above the knee.

Musculoskeletal model of osseointegrated transfemoral amputees in OpenSim.

This study presents a generic OpenSim musculoskeletal model of people with an osseointegrated unilateral transfemoral amputation wearing a generic prosthesis. The model, which consists of seventy-six musculotendon units and two ideal actuators at the knee and ankle joints of the prosthesis, is tested by designing an optimal control strategy that guarantees the tracking of experimental amputee data during level-ground walking while finding the actuators' torques and minimizing the muscle forces. The model can be made subject-specific and, as such, is able to reproduce the kinematics and dynamic

A Pneumatically Controlled Prosthetic Socket for Transfemoral Amputees.

Amputees typically experience changes in residual limb volume in their daily lives. It causes an uncomfortable fit of the socket by applying high pressure on the sensitive area of the residual limb or by loosening the socket. In this study, we developed a transfemoral prosthetic socket for above-the-knee amputees that ensures a good socket fit by maintaining uniform and constant contact pressure despite volume changes in the residual limb. The socket has two air bladders in the posterior femoral region, and the pneumatic controller is located on the tibia of the prosthesis. The pneumatic syste

Alternative muscle synergy patterns of upper limb amputees.

BACKGROUND: The ketone bodies beta-hydroxybutyrate (BHB) and acetone are endogenous products of fatty acid metabolism. Although ketone levels can be monitored by measuring either blood BHB or breath acetone, determining the precise correlation between these two measurement methods has been challenging. The purpose of this study is to characterize the performance of a novel portable breath acetone meter (PBAM) developed by Readout, Inc., to compare single versus multiple daily ketone measurements, and to compare breath acetone (BrAce) and blood BHB measurements. METHODS: We conducted a 14-day

[Walking activity in prosthesis-bearing lower-limb amputees].

PURPOSE OF THE STUDY: A large body of literature has been devoted to gait analysis in amputees. Most studies have been conducted in the laboratory setting where numerous variables are analyzed: gait efficiency, energy cost, walking velocity. At the present time however, data are lacking on the real-life use of walking prostheses. Little is known about how long patients wear their prosthesis or how difficult it is for them to walk during different periods of the day. Currently, such information depends on the quality of the interview and the patient-physician relationship. A precise assessment

The relationship between diabetic amputees and vascular reviews.

This paper reviews the data in the literature pertaining to risk factors for amputation in the diabetic population, and describes a retrospective study to assess the vascular and other assessments carried out on a sample of 50 diabetic patients undergoing minor (toe and foot) and major (below-knee and above-knee) amputations. The study found that 50% of the patients did not have Doppler studies, 52% did not have angiography, and 78% did not have angioplasty. Major amputations were performed in 76% of the sample. This is an unacceptably high figure. Thorough vascular assessments need to be perf

Analyses of prosthetic episodes in trans-tibial amputees.

The prosthetic episodes, i.e. actual processes of provision, identifying number of prostheses, their maintenance, repairs and replacements were analysed for 104 trans-tibial amputees aged 16 and over, over a 10 year period. The purpose of this investigation was to identify how frequently a new prosthesis is actually required for this group of amputees and what are their maintenance requirements. Variations of requirement between the amputee groups of aged 16-60 and over 60 years are also addressed. In the 10 years period of the study the amputees needed an average of 5.04 new prostheses, 6.25

Ten-year survival of Finnish lower limb amputees.

Data on mortality for the ten years following lower limb amputation were obtained from all the 16 surgical units in Southern Finland and the National Social Insurance Institution. In Southern Finland during the period 1984-1985, amputations of the lower limb were performed on 705 patients, of whom 382 (54%) were women and 323 (46%) men. The majority of the amputations, 47% were performed for vascular diseases and 41% were performed for diabetes mellitus. The overall survival was 62% at one year after amputation, 49% at two years, 27% at five years and 15% at ten years. The median survival afte

Factors influencing rehabilitation of arteriosclerotic lower limb amputees.

This survey considered 598 arteriosclerotic amputees over a period of 9 years: 267 below-knee; 81 Gritti-Stokes; 195 above-knee; and 55 double amputees. A walking ability index (WAI) ranging from 1 for a normal gait to 6 for inability to walk was determined for these amputees by clinical grading at 3, 6, 9, and 12 months after prosthesis fitting. Amputees with the below-knee operation had better WAI at 3 and more months than those with either Gritti-Stokes or above-knee operations. There was no statistical evidence for a difference between Gritti-Stokes and above-knee operations at any time of

Use of lower limb prosthesis among elderly amputees.

Forty-seven elderly patients (21 males and 26 females) with lower limb amputation, and with prostheses fitted were studied. Their ages ranged from 65 years to 91 years old, with an average of 75.4 years. The three most common contributory causes for amputation were diabetes mellitus, vascular occlusion and carcinoma of the skin. Among the lower limb amputations, below knee amputation was the most common (93.6%). About 30% of the patients developed complications from prostheses, the 3 most common in order of frequency were stump pain, pressure ulcers and stump infection. Male patients tended to
