Role of general anesthetic agents in postoperative nausea and vomiting: A review of literature.

Postoperative nausea and vomiting (PONV) is considered to be the most unpleasant experience associated with surgery and is believed to be one of the most common reasons for poor patient satisfaction in the postoperative period. It also results in prolonged hospitalization and increased use of resources such as intravenous fluids and drug supplements along with prolonged medical attention, all of which have psychological and financial implications. In addition to this, PONV can result in aspiration, laryngospasm, dehydration, electrolyte disturbances, gastric bleeding, increased intracranial pr

General anesthetic techniques for enhanced recovery after surgery: Current controversies.

General anesthesia technique can influence not only immediate postoperative outcomes, but also long-term outcomes beyond hospital stay (e.g., readmission after discharge from hospital). There is lack of evidence regarding superiority of total intravenous anesthesia over inhalation anesthesia with regards to postoperative outcomes even in high-risk population including cancer patients. Optimal balanced general anesthetic technique for enhance recovery after elective surgery in adults includes avoidance of routine use preoperative midazolam, avoidance of deep anesthesia, use of opioid-sparing ap

[Influence of general anesthetic exposure in developing brain on cognition and the underlying mechanisms].

With the evolution of medical techniques and technology, an increasing number of infants, neonates, and fetuses are exposed to general anesthesia for clinical diagnostic and therapeutic process. The neurotoxic effects of general anesthetics on developing brain have been a subject of concern and considerable research interest. Population-based study confirmed that single short-term general anesthetic exposure does not affect nervous system function, but multiple exposures to general anesthesia could damage cognitive function. Animal studies further discovered the underlying mechanisms. Nervous

Anesthetic and airway management of general anesthesia in a patient with Meckel-Gruber syndrome.

Meckel-Gruber syndrome, characterized by occipital encephalocele, microcephaly, polydactyly, cleft lip or palate, mandibular micrognathism, and anatomical abnormality of the larynx and tongue, along with other associated malformations, is in the list of diseases associated with difficult airway. However, there has been no report on the management of general anesthesia and airway management for such patients. A 2-year-old girl with Meckel-Gruber syndrome was scheduled for cardioplasty and gastrostomy for gastroesophageal reflux under general anesthesia. Preoperative examination revealed obesity

[Freeman-Sheldon syndrome: generalized muscular rigidity after anesthetic induction].

We describe a case of generalized muscle rigidity in a 2-year-old patient with Freeman-Sheldon syndrome undergoing surgery for eventration of the diaphragm after anesthetic induction with halothane and succinylcholine. Anesthetic induction was by mask with oxygen, nitrous oxide and halothane, with succinylcholine as a muscle relaxant. Approximately 10 minutes after start of induction, muscular rigidity appeared and developed rapidly, becoming severe and compromising ventilation. Tracheal intubation was attempted without success, owing to stiffness of the masseter muscles. Sodium dantrolene 2.5

Effects of topical and general anesthetic agents on tracheal mucous velocity of sheep.

Tracheal mucous velocity was estimated in eleven sheep by means of cine-bronchofiberscopic technique in which Teflon discs placed on the tracheal mucosa were filmed as markers. The procedure was done using a transnasal approach in conscious nonanesthetized sheep, and in the same sheep using topical and/or general anesthetic agents. The average mucous velocity in conscious sheep was 17.3 plus or minus 6.2 (SD) mm/min; in sheep anesthetized with intravenous pentobarbital or thioamylal, the average velocity was significantly depressed to 11.1 plus or minus 3.6 mm/min. No Significant differences i

The choice of general anesthetics may not affect neuroinflammation and impairment of learning and memory after surgery in elderly rats.

Postoperative cognitive dysfunction (POCD) often occurs in elderly patients and may involve neuroinflammation. This study was to determine whether anesthetic choice (intravenous vs. volatile anesthetics) affects cognitive impairment and neuroinflammation in elderly rat. Total 54 twenty-month old male Fischer 344 rats were assigned randomly to control, right carotid exposure under propofol-buprenorphine or isoflurane-buprenorphine anesthesia groups. They were tested by Barnes maze and fear conditioning from 6 days after the surgery. Their brains were harvested 24 h after the surgery for quantif

General anesthetic management in two patients with an anaphylaxis history cholinergic urticaria.

BACKGROUND: Cholinergic urticaria is triggered by an increased body temperature after exercise, passive warming, or emotional stresses. The anesthetic management used for two patients with an anaphylaxis history after cholinergic urticaria is described. CASE PRESENTATION: Patient 1: A 34-year-old female was scheduled for a right-side thyroidectomy. At 27 years old, she experienced post-exercise anaphylaxis and repeated episodes of hives following exercise, sunbathing, mental stresses, and the consumption of spicy foods. Patient 2: A 35-year-old female was scheduled for a laparoscopic ovarian

Anesthetic management of acute nasal epistaxis occurring during general anesthesia.

Spontaneous epistaxis under general anesthesia is a rare untoward event. Typically, mild epistaxis occurs from a nasal airway manipulation and usually resolves spontaneously or with external pressure to the affected nares for a short period of time. In case of epistaxis that is difficult to control with pressure or oxymetazoline, thorough evaluation should be considered. We present a case of a 12-year-old experiencing spontaneous nasal bleeding upon emergence difficult to control with noninvasive methods and required intervention by otolaryngology.

[General anesthetic management of a patient with spinal muscular atrophy type III].

We report a 61-year-old woman (weight 49 kg, height 156 cm) with Kugelberg-Welander disease who was scheduled for bilateral mastectomy under general anesthesia. We administered rocuronium 10 mg (0.20 mg x kg(-1)) for tracheal intubation. After 80 min, train-of-four ratio (TOFR) was 46%. During the operation, we did not administer rocuronium. After surgery, TOFR was 62%. Therefore, we administered sugammadex 100 mg (2 mg x kg(-1)). After 4 min, TOFR became above 90%, and the patient was extubated. There was no respiratory distress, muscle weakness, or neurologic untoward event after the use of

Regional anesthetic techniques are an alternative to general anesthesia for infants with Pompe's disease.

Pompe's disease or glycogen storage disease type II is a genetic disorder affecting skeletal and cardiac muscle. The infantile form is associated with gross hypertrophic cardiomegaly and death in the early years. General anesthesia is associated with potential major morbidity in these patients. We present our experience of regional anesthetic blocks used in five patients with the infantile form of glycogen storage disease type II with and without sedation for 11 surgical procedures during a clinical trial of replacement therapy for this condition. Both femoral nerve blockade and caudal epidura

Anesthetic care for the child with Morquio syndrome: general versus regional anesthesia.

Morquio syndrome is a subtype of the mucopolysaccharidoses (MPS IV) characterized by the intracellular accumulation of keratin sulfate. Anesthetic issues related to the primary disease process include infiltration of tissues with keratin sulfate leading to distortion of upper airway anatomy with difficult or impossible endotracheal intubation, infiltration of the cervical spine with odontoid hypoplasia placing these patients at risk for atlanto-axial subluxation and quadriparesis, as well as alterations in cardiorespiratory function. Two children with Morquio syndrome who required anesthetic c

[In vitro effects of anesthetics on hydrogen peroxide generating system of the thyroid gland].

Effects of anesthetics on thyroidal hydrogen peroxide generating system was studied using porcine thyroid follicles. This system is indispensable in thyroid hormone synthesis and seems also to play an important role in regulation of thyroid hormone release. The results indicated that H2O2 generation was controlled by Ca2+ ion mobilization and protein kinase-C linked signal transduction system. It was also demonstrated that thiopental, halothane, and enflurane had inhibitory effects on H2O2 generation system in thyroid follicle cells, and 50% inhibitory concentrations (IC50) of each anesthetic

Use-dependent conduction block produced by volatile general anesthetic agents.

The ability of tertiary amine local anesthetic, barbiturates, and several volatile general anesthetics to block conduction of the compound action potential was measured in the frog sciatic nerve. Action potentials from large myelinated fibers were monitored using a desheathed nerve preparation in a sucrose-gap chamber. Supramaximal stimuli were applied over a range of stimulus frequencies from 0.1 Hz to 500 Hz. All three anesthetic agents produced a phasic, frequency-dependent anesthetic block, which developed with repetitive stimulation and returned to a tonic level when the nerve remained a

Bioengineered human brain organoids and organ-on-chip platforms for exposure-aware assessment of developmental neurotoxicity induced by general anesthetics and sedatives.

General anesthetic agents and sedative-hypnotic drugs are essential for fetal, neonatal, pediatric, and intensive care procedures, but their administration can overlap with vulnerable stages of human brain development. Available evidence indicates that developmental responses vary with exposure duration, repetition, developmental timing, and host susceptibility. The objective of this review is to critically evaluate bioengineered human brain organoids, brain microphysiological systems, and organ-on-chip platforms as human-relevant tools for exposure-aware assessment of anesthetic developmental

Nonallergic hypersensitivity to nonsteroidal antiinflammatory drugs, angiotensin-converting enzyme inhibitors, radiocontrast media, local anesthetics, volume substitutes and medications used in general anesthesia.

Urticaria and angioedema are common allergic manifestations and medications are one of common triggering factors. The most severe immediate drug reaction is anaphylaxis. Apart from the well established IgE-mediated immediate type hypersensitivity reactions, the pathogenesis of drug-induced urticaria, angioedema and anaphylaxis often remains obscure. In this article, emphasis is put on nonallergic reactions to the most commonly used drug groups of nonsteroidal antiinflammatory drugs, angiotensin-converting enzyme inhibitors, radiocontrast media, volume expanders and drugs used in general anesth
