Intravenous Lipid Emulsion for Managing Local Anesthetic Toxicity in a Three-Month-Old Patient.

Local anesthetic systemic toxicity (LAST) is a well-known life-threatening local anesthetics complication, especially if given in inappropriate doses or routes. Therefore, physicians should be aware of LAST symptoms, such as neurological and cardiac symptoms. In addition, they should always consider it in the differential diagnosis when they encounter similar symptoms. The early recognition of LAST and initiation of intravenous lipid emulsion (ILE) can decrease mortality and morbidity rates. Multiple organizations and guidelines use ILE as standard LAST management. We present a three-month-old

Intravenous Lidocaine Alleviates the Pain of Propofol Injection by Local Anesthetic and Central Analgesic Effects.

OBJECTIVE: Lidocaine alleviates propofol injection pain. However, whether lidocaine works through a local anesthetic effect at the site of intravenous injection or through a systemic effect on the central nervous system remains unknown. This study aimed to determine the pain-alleviating mechanism of lidocaine. DESIGN: A randomized controlled study. SETTING: A gastroscopy facility. METHODS: The study was divided into two parts. Part 1 involved 717 patients who were randomly assigned into five groups. Groups PR, RL20, and RL40 received normal saline or saline containing 20 or 40 mg of lidocai

Vasopressin intravenous infusion causes dose dependent adverse cardiovascular effects in anesthetized dogs.

BACKGROUND: Arginine vasopressin (AVP) has been broadly used in the management of vasodilatory shock. However, there are many concerns regarding its clinical use, especially in high doses, as it can be associated with adverse cardiovascular events. OBJECTIVE: To investigate the cardiovascular effects of AVP in continuous IV infusion on hemodynamic parameters in dogs. METHODS: Sixteen healthy mongrel dogs, anesthetized with pentobarbital were intravascularly catheterized, and randomly assigned to: control (saline-placebo; n=8) and AVP (n=8) groups. The study group was infused with AVP for thr

Effects of an intravenous infusion of Ringer's solution on eruption rates of incisor teeth in anesthetized rats.

OBJECTIVE: The vasculature within the socket is reportedly involved in determining the position of continuously erupting teeth. Thus, loss of body fluid in anesthetized rats, which would affect the vascular physiology, should influence tooth movement. We investigated the effects of an infusion of Ringer's solution on the systemic arterial blood pressure, regional blood flow at the base of the incisor, and axial tooth movement in anesthetized rats to determine the cause of tooth displacement. MATERIAL AND METHODS: In the experimental group, the animals received intravenous infusions of Ringer'

Effects of intravenous anesthetic agents on glutamate release: a role for GABAA receptor-mediated inhibition.

BACKGROUND: Many anesthetic agents are known to enhance the alpha1beta2gamma2S gamma-aminobutyric acid type A (GABAA) chloride current; however, they also depress excitatory neurotransmission. The authors evaluated two hypotheses: intravenous anesthetic agents inhibit glutamate release and any observed inhibition may be secondary to GABAA receptor activation. METHODS: Cerebrocortical slices were prepared from Wistar rats. After perfusion in oxygenated Krebs buffer for 60 min at 37 degrees C, samples for glutamate assay were obtained at 2-nmin intervals. After 6 min, a 2-min pulse of 46 mM K+

Effects of an intravenous infusion of noradrenaline on the plasma concentration of free and sulfoconjugated catecholamines in anesthetized dogs.

Exogenous noradrenaline (NA) was infused intravenously at increasing rate from zero (control) to 10, 25, 50, 100, 200 and 600 ng/kg/min during 20 min in anesthetized and ventilated dogs; the mean (+/- SEM) plasma concentration of free NA was increased from 130 +/- 23 pg/ml (basal) to 7,826 +/- 787 pg/ml. This had no measurable effect on the plasma concentration of dopamine and adrenaline in either free or sulfoconjugated form; a lack of change was also observed in dogs given a 600-ng/kg/min infusion during more than 2 h. The increase of free NA concentration was highly correlated both with the

Effects of intravenous anesthetics on the activity of glutamate transporter EAAT3 expressed in Xenopus oocytes: evidence for protein kinase C involvement.

We investigated the effects of the intravenous anesthetics, thiopental, etomidate and ketamine, on the activity of one type of glutamate transporters, EAAT3 (excitatory amino acid transporter type 3). Rat EAAT3 was expressed in Xenopus oocytes by injection of its mRNA. Using two-electrode voltage clamp, membrane currents were recorded after the application of L-glutamate (30 microM) in the presence or absence of various concentrations of the anesthetics. Thiopental (0.3-30 microM) and ketamine (3-1000 microM) did not affect EAAT3 activity. Etomidate decreased EAAT3 activity in a concentration-

The Binding Mechanisms and Inhibitory Effect of Intravenous Anesthetics on AChE In Vitro and In Vivo: Kinetic Analysis and Molecular Docking.

Inhibitors of acetylcholinesterase (AChE), which have an important role in the prevention of excessive AChE activity and β-amyloid (Aβ) formation are widely used in the symptomatic treatment of Alzheimer's disease (AD). The inhibitory effect of anesthetic agents on AChE was determined by several approaches, including binding mechanisms, molecular docking and kinetic analysis. Inhibitory effect of intravenous anesthetics on AChE as in vitro and in vivo have been discovered. The midazolam, propofol and thiopental have shown competitive inhibition type (midazolam > propofol > thiopental) and Ki v

Transesophageal Echocardiography-Guided Anesthetic Management of Intravenous Leiomyomatosis With Right Atrial Extension: A Case Report.

Intravenous leiomyomatosis (IVL) is a rare benign smooth muscle tumor that can extend into the heart, posing major anesthetic challenges. We report a 57-year-old woman with IVL extending from the uterus through the inferior vena cava into the right atrium. A multidisciplinary team performed single-stage intracardiac tumor resection under cardiopulmonary bypass with hysterectomy and bilateral salpingo-oophorectomy. Continuous transesophageal echocardiography (TEE) enabled real-time assessment of tumor mobility and safe cannulation. The patient recovered uneventfully. This case highlights the es

Anesthetic Management of Intravenous Leiomyomatosis With Intracardiac Extension Using Preemptive Cardiopulmonary Bypass: A Case Report.

Intravenous leiomyomatosis (IVL) with intracardiac extension can cause circulatory collapse during anesthetic induction due to right heart obstruction. We report the case of a 63-year-old woman with IVL extending into the right ventricle, presenting with right heart failure and shock. To maintain hemodynamic stability and facilitate tumor resection, we established cardiopulmonary bypass (CPB) under local anesthesia before inducing general anesthesia. Preoperative imaging revealed a tumor extending from the right ovarian vein to the right ventricle, causing circulatory failure. In the operating

The effect of obstructive jaundice on the sensitivity of intravenous anesthetic of remimazolam: study protocol for a controlled multicenter trial.

BACKGROUND: It is well known that obstructive jaundice could affect the pharmacodynamics of some anesthetics, and the sensitivity of some anesthetics would increase among icteric patients. Remimazolam is a new ultra-short-acting intravenous benzodiazepine sedative/anesthetic, which is a high-selective and affinity ligand for the benzodiazepine site on the GABAA receptor. However, no study has reported the pharmacodynamics of remimazolam in patients with obstructive jaundice. We hypothesize that obstructive jaundice affects the pharmacodynamics of remimazolam, and the sensitivity of remimazolam

[Sufentanil during anesthetic induction of remifentanil-based total intravenous anesthesia: a randomized controlled trial].

BACKGROUND: Postoperative pain represents an important concern when remifentanil is used for total intravenous anesthesia because of its ultrashort half-life. Longer acting opioids, such as sufentanil, have been used during induction of remifentanil-based total intravenous anesthesia as a means to overcome this shortcoming. However, the effectiveness and safety of such strategy still lacks evidence from randomized clinical trials. Hence, we aimed to assess the postoperative analgesic efficacy and safety of a single dose of sufentanil administered during the induction of remifentanil-based tota

Comparison of outcomes for pediatric paraphimosis reduction using topical anesthetic versus intravenous procedural sedation.

BACKGROUND: Paraphimosis is an acute urologic emergency requiring urgent manual reduction, frequently necessitating procedural sedation (PS) in the pediatric population. The present study sought to compare outcomes among pediatric patients undergoing paraphimosis reduction using a novel topical anesthetic (TA) technique versus PS. METHODS: We performed a retrospective analysis of all patients <18years old, presenting to a tertiary pediatric ED requiring analgesia for paraphimosis reduction between October 2013 and September 2016. The primary outcome was reduction first attempt success; second

Adverse events associated with a large dose of intravenous lipid emulsion for suspected local anesthetic toxicity.

BACKGROUND: Intravenous lipid emulsion (ILE) has gained favor as a rescue treatment for cardiovascular collapse due to intravenous local anesthetic overdose, however, goals of ILE therapy are still being defined. We describe a case of a girl given 66 mL/kg of 20% lipid emulsion (ILE) in the treatment of presumed mepivacaine toxicity. CASE REPORT: An 11-year-old girl weighing 55.6 kg developed pallor, rolling back of the eyes, and rhythmic muscle twitching after receiving a mandibular nerve block injection with a 1.8 mL ampule of 3% mepivacaine. With concern for persistent seizures she was giv

Bidirectional regulation of intravenous general anesthetic actions by α3-containing γ-aminobutyric acid A receptors.

BACKGROUND: γ-aminobutyric acid A (GABAA) receptors mediate the actions of several intravenous general anesthetics. However, the contribution of α3-containing GABAA receptors to the action of these drugs is unknown. METHODS: The authors compared anesthetic endpoints (hypnosis, immobility, hypothermia) in response to various intravenous anesthetics in mice lacking the α3 subunit of the GABAA receptor (α3 knockout) and in wild-type mice. Furthermore, the authors generated and analyzed conditional mutant mice expressing the GABAA receptor α3 subunit exclusively in noradrenergic neurons. RESULTS
