Endovascular Aortic Repair Is a Viable Strategy for Treatment of Primary Infected Abdominal Aortic Aneurysm.

BACKGROUND: Infected abdominal aortic aneurysm (IAAA) is rare, and information is limited whether endovascular aortic repair (EVAR) can be considered a permanent treatment or is a temporary fix preceding open surgery. This retrospective study reviewed the short- and long-term outcomes of open surgery, emergent EVAR, and elective EVAR in the treatment of primary IAAA. METHODS: Between January 2008 and January 2017, 16 men and 3 women (aged 60.7 y; range 30-76 y) with IAAA were treated with emergent open repair, emergent EVAR, or elective EVAR, after adjunctive antibiotic therapy. Demographics,

Total resection of an infected aortic arch aneurysm caused by Mycobacterium avium: a case report.

BACKGROUND: Infected aortic arch aneurysms caused by Mycobacterium avium are rare in immunocompetent individuals. Promptly recognizing these aneurysms is important because delays in treatment result in aneurysm rupture and a high fatality rate. Although Salmonella species, Streptococcus species, Staphylococcus aureus, and S. epidermis are commonly found in immunocompetent individuals, to our knowledge, infected aortic arch aneurysms caused by M. avium have not yet been reported. CASE PRESENTATION: We report the case of a 63-year old immunocompetent man who underwent total arch replacement fol

Outcomes of Surgical Explantation of Infected Aortic Grafts After Endovascular and Open Abdominal Aneurysm Repair.

OBJECTIVES: Infection of the vascular graft represents one of the most threatening complications after aortic repair. It is rare and associated with high morbidity and mortality rates. The aim of this study was to present short-term outcomes after surgical treatment of infected aortic grafts after endovascular and open repair of abdominal aortic aneurysms (AAAs). METHODS: Data of all patients affected by aortic graft infection after aneurysm repair who underwent an explantation of a conventional or endovascular aortic graft between January 2008 and December 2016 were retrospectively reviewed.

Infected False Aneurysm of the Aortic Arch After Endoscopic Transurethral Instillation of Bacillus Calmette-Guérin.

The case of an 81-year-old male operated on for an infected false aneurysm of the aortic arch by Mycobacterium bovis (M. bovis) is described. Arch reconstruction with cryopreserved aortic patch was successfully performed under hypothermic circulatory arrest. Antituberculous chemotherapy was given for 12 months and presently the patient is leading a normal life. Vascular infection after bacillus Calmette-Guérin bladder therapy is uncommon and aortic arch involvement near exceptional. This diagnosis has to be considered in patients with such previous urologic interventions.

Bridge use of endovascular repair and delayed open operation for infected aneurysm of aortic arch.

We present the first clinical report of the successful treatment of an infected thoracic aortic arch aneurysm with the use of endovascular repair as a bridge to second-stage open operation. A 70-year-old patient underwent urgent endovascular repair through the right femoral approach because of a diagnosis of sepsis and impending rupture of an infected thoracic aortic arch aneurysm. After 2 weeks of medical treatment, we successfully performed explantation of the stent graft, wide debridement of the surrounding tissue, and in-situ replacement using a rifampicin-bonded four-branched prosthetic g

Infected Abdominal Aortic Aneurysm Successfully Treated With Endovascular Aortic Repair and Antibiotics: A Case Report.

Surgical treatment of infected aneurysms is problematic due to their high complication and mortality rates. Infected aortic aneurysms are at high risk of rupture and should be operated on as soon as possible after diagnosis. A 72-year-old female patient with a medical history of diabetes mellitus, hyperlipidemia, and hypertension presented with a fever of 38°C and back pain, without any apparent cause, in 2021. Her C-reactive protein (CRP) level increased to 20 mg/dL. Further evaluation with contrast-enhanced computed tomography (CT) revealed a low-density area with air pockets surrounding the

Lawsonella clevelandensis is a rare cause of infected chronic contained rupture of abdominal aortic aneurysm.

Lawsonella clevelandensis is an anaerobic, partially acid-fast, Gram-positive bacillus associated with abscess formation. We present the case of a 70-year-old male with chronic contained rupture of abdominal aortic aneurysm (CCR-AAA) complicated by intra-abdominal abscess formation. An abdominal computed tomography scan revealed a rim-enhancing retroperitoneal collection tracking into the subcutaneous layer of the left flank and buttock, suggestive of CCR-AAA with infected haematoma. He underwent ultrasound-guided needle aspiration of the intra-abdominal collection. Conventional culture techni

Sandwich EVAR occludes Celiac and Superior Mesenteric Artery for Infected Suprarenal Abdominal Aortic Aneurysm Treatment.

Introduction. Infected aortoiliac aneurysms are rare, representing only 1% to 2% of all aortic aneurysms; we present a case of infected suprarenal aortic aneurysm with a nearly occluded celiac artery and superior mesenteric artery treated using an endovascular technique to preserve collateral in the retroperitoneal space from the inferior mesenteric artery for supplying visceral organs.

In situ repair of a primary Brucella-infected abdominal aortic aneurysm: long-term follow-up.

Infected aortic aneurysms represent 0.85 to 1.3% of aortic aneurysms. Most often, the implicated bacteria species are Salmonella sp., Staphylococcus sp. and Streptococcus sp. Brucella-related infected aortic aneurysms are very rare. Most often, they result from endocarditis or from a local septic focus. Combined treatment by antibiotics and surgery is the standard for infectious aneurysms. In the absence of formal factual data, the surgical treatment is still discussed in the literature, especially since endovascular treatments have been in full expansion. We are reporting the case of a female

[Successful management of a ruptured infected aneurysm of the thoracic aorta: report of a case].

A successful removal of the infected thoracic aortic aneurysm ruptured to the lung is reported. A 59-year-old man who had been suffering from high fever showed sudden hemoptysis and shock during antibiotic therapy. Immediately a so-called temporary bypass grafting from ascending aorta to descending aorta was placed with a woven Dacron prosthesis, and then aneurysmectomy with upper lobectomy of the left lung was carried out. Finally the graft was left as a permanent bypass graft. In addition, the left subclavian artery was reconstructed with a EPTFE graft. The patient is now well one year after

Operation for an infected thoracoabdominal aneurysm in a patient previously treated with an axillobifemoral bypass for an infected abdominal aortic prosthesis: a case report.

High operative mortality of infected thoracoabdominal aortic aneurysms (ITAA) is partly attributable to ischemic injury during aortic clamping. A case is presented of an 88-year old man who was admitted with imminent rupture of an ITAA. Axillobifemoral bypass grafting had been performed after removal of an infected abdominal aortic prosthesis six years earlier. In situ graft replacement was performed during 70 minutes of aortic clamping just below the pulmonary hilum without causing any but transient renal ischemic injury. Since the infrarenal aorta was absent after previous removal of an infe

Infected Superior Mesenteric Artery Aneurysm following Streptococcus gallolyticus Bacteremia Associated with Cecal Cancer: A Case Report.

Streptococcus gallolyticus bacteremia is associated with colorectal malignancy and infective endocarditis, whereas infected superior mesenteric artery aneurysm (SMAA) is rare. A 94-year-old woman with cecal cancer developed S. gallolyticus sepsis that improved with intravenous antibiotics. Two months later, contrast-enhanced computed tomography revealed a newly developed 17-mm distal SMAA, suggesting an infected aneurysm secondary to the prior bacteremia. Because of the high risk of rupture, urgent aneurysmectomy and ileocecal resection were performed. This rare case highlights an infected SMA

An infected aneurysm in the internal carotid artery caused by Salmonella enterica subsp. enterica serovar Enteritidis.

An infected aneurysm in the carotid artery is a rare but life-threatening condition. The most common cause of such infections is Staphylococcus aureus; however, Salmonella is a possible but uncommon cause. We report the case of an elderly man with an infected aneurysm in the left internal carotid artery caused by Salmonella enterica subsp. Enterica serovar Enteritidis despite the absence of gastrointestinal symptoms. Following confirming the absence of neurological symptoms after blood flow disruption through balloon test occlusion (BTO), the patient underwent coil embolization of the aneurysm

Successful Endovascular Repair of a Thoracic Infected Aortic Aneurysm with Concomitant Liver Abscess: A Case Report.

BACKGROUND Concomitant infected aortic aneurysm and liver abscess is extremely rare and potentially fatal. Their simultaneous occurrence suggests an aggressive disease course and presents substantial diagnostic and therapeutic challenges. Because symptoms are often nonspecific, diagnosis is frequently delayed, which can lead to worse outcomes. Early recognition through timely imaging and a coordinated multidisciplinary approach are essential for optimizing clinical results. CASE REPORT A 65-year-old woman with a history of hypertension and diabetes mellitus presented with a 1-week history of e

Homemade pericardial bifurcated graft for Q fever-infected abdominal aortic aneurysm open repair: a case report.

Q fever is a zoonotic infection caused by Coxiella burnetii. In rare cases, it can lead to vascular complications, including infected aneurysms. Successful treatment involves surgery and antibiotics, but there is no established consensus or clear recommendation for the choice of material graft. We report a case of abdominal aortic aneurysm infected by C. burnetii treated by open surgery with complete resection of the aneurysm and homemade bovine pericardial bifurcated graft reconstruction and long-term antibiotherapy using doxycycline. One year postoperatively, the patient had no sign of persi
