[Venous angiodysplasias with endoral manifestation. The problems of therapy].

Five oral venous angiodysplasia cases are presented by the authors. Besides subjective aesthetic and psychological factors, the treatment of these lesions depends on objective chewing problems and on haemorrhagic risk. This can be either spontaneous or in relation with dental surgery during teeth extraction or third molar pathology therapy. In two patients the lesions affected the tongue and cheek mucosa including the submandibular and masseter region. In one patient there was just one localization at the tongue apex. In another patient the lesion affected only the cheek mucosa and in another

[Angiodysplasias in the digestive tract and associated cardiovascular diseases].

To clarify the possible relationship between angiodysplasia of the gastrointestinal tract and cardiovascular disease, we performed a retrospective study of 58 consecutive patients. In all patients angiodysplasias were identified endoscopically. The median age was 72 years. Fifty-seven per cent of the patients had some clinically obvious cardiovascular disorder. Mitral valve insufficiency was found more often (21%) than aortic valve stenosis (10%). A high incidence of coronary artery disease (45%) and peripheral artery disease (10%) was noted. These findings do not support the concept that angi

Intraoperative enteroscopy for bleeding angiodysplasias of small intestine.

Six patients with angiodysplasias of the small intestine were diagnosed preoperatively with selective angiography during the past eight years. Enteroscopy was used to identify these vascular lesions intraoperatively. Lesions were ulcerated in four patients, had a bleeding vessel tip in one patient and appeared as a nonulcerated reddish patch in the other. Exact localization of these lesions allowed limited segmental resection of the small intestine to be performed. With complete enteroscopy, we were confident that no bleeding lesion was left undetected. The feeding artery and vein to the resec

Angiodysplasia of the colon: an expression of occlusive vascular disease.

We performed a prospective case control study and found among 306 patients, in whom a complete colonoscopy was done, 19 patients (6%) with angiodysplasia of the colon. These patients were significantly older than control subjects (p less than 0.05). Nine patients (47%) with angiodysplasia had no bleeding and were identified incidentally by colonoscopy performed for other indications. To clarify the possible role of underlying occlusive arterial diseases we compared the 19 patients with angiodysplasia with a control group, matched for sex and age, in whom a complete colonoscopy did not reveal a

Transcatheter embolization of angiodysplasia of the rectum. Report of a case.

A 51-year-old man with repeated and massive rectal bleeding was found to have a large angiodysplasia of the rectum. A superselective embolization of the distal part of the superior rectal artery with Ivalon controlled the rectal bleeding for 4 months. Lower gastrointestinal bleeding recurred and a recurrent lesion was re-embolized with Ivalon with excellent control of bleeding in a follow-up of 8 months. Embolization is an alternative to surgery for the treatment of rectal angiodysplasia and hopefully may result in a long-term cure.

Management of bleeding angiodysplasia of the upper small intestine.

Five patients thought to have bleeding angiodysplasia in the distal duodenum and proximal jejunum were treated surgically after conservative measures failed to control hemorrhage. Aspiration through a long oral tube preoperatively and intraoperative endoscopy were most helpful in locating the site of bleeding.

Diffuse Mesenteric and Bowel Angiodysplasia: A Case Report.

Angiodysplasia is one of the causes of recurrent episodes of lower gastrointestinal (GI) bleeding. Angiodysplasia could be associated with few lesions or multiple diffuse lesions, causing diversity in the clinical presentation of such patients. We report a case of a 19-year-old male presenting with life-threatening gastrointestinal bleeding due to diffuse angiodysplasia of the bowel extending from the jejunum to the sigmoid colon and requiring multiple investigations and management.

Gastrointestinal angiodysplasia and bleeding in von Willebrand disease.

Von Willebrand disease (VWD), the most common genetic bleeding disorder, is characterised by a quantitative or qualitative defect of von Willebrand factor (VWF). Patients with VWD suffer from mucocutaneous bleeding, of severity usually proportional to the degree of VWF defect. In particular, gastrointestinal bleeding associated with angiodysplasia is often a severe symptom of difficult management. This review focuses on the pathophysiology, diagnosis and treatment of VWD-associated gastrointestinal angiodysplasia and related bleeding.

Review article: gastrointestinal angiodysplasia - pathogenesis, diagnosis and management.

BACKGROUND: Angiodysplasia (AD) of the gastrointestinal (GI) tract is an important condition that can cause significant morbidity and -rarely - mortality. AIM: To provide an up-to-date comprehensive summary of the literature evaluating this disease entity with a particular focus on pathogenesis as well as current and emerging diagnostic and therapeutic modalities. Recommendations for treatment will be made on the basis of the current available evidence and consensus opinion of the authors. METHODS: A systematic literature search was performed. The search strategy used the keywords 'angiodysp

Von Willebrand's disease and angiodysplasia treated with thalidomide.

A 54-year-old man with type IIB VWD and severe angiodysplasia had such a large blood loss from the bowel that it was difficult to keep up with transfusion requirements. Treatment with factor eight concentrate barely slowed the loss. D.D.A.V.P., Octreotide, and recombinant activated Factor VII, tried separately, were ineffective. The use of Thalidomide at a dose of 150 mg daily has rendered him free from blood loss for the last six months and we suggest would be worth a trial in similar cases.

Multiple jejunal angiodysplasia detected by enema-helical CT.

The small bowel angiodysplasia is a rare entity that causes lower intestinal bleeding; the diagnosis is difficult and based on selective angiogram. In our case, an 85-year-old woman was hospitalized after frequent episodes of melena. We performed an enema-helical CT abdominal examination before and after contrast medium administration per venam, detecting some increased intensity areas that surgery confirmed to be a vascular dysplasia on the jejunal first loop. We found the source of hemorrhage without performing an angiographic examination.

[Colonic angiodysplasia: an important cause of digestive hemorrhage].

Angiodysplasia of the colon is one of the most common causes of lower gastrointestinal bleeding in elderly patients. The diagnosis can be made by colonscopy or angiography. The best treatment, endoscopic coagulation or surgical resection, is still being debated. Over a period of nearly seven years, 5 patients with angiodysplasia of the colon were treated in our Department. The treatment was right hemicolectomy for all patients with localization at the caecum or ascending colon; in the only patients with segmoid localization a segmentary resection was performed. During a mean follow-up of 22.8

Importance of small bowel involvement in bleeding angiodysplasia.

Lower intestinal bleeding related to enterocolic angiodysplasia is now accepted as a common clinical situation in the elderly. A planned approach is mandatory to allow early localization and appropriate therapy. Colonoscopy, scintigraphy and angiography used judiciously have almost entirely replaced exploratory laparotomy as a diagnostic tool. Nonoperative treatment comprising arteriographic selective vasopressin infusion and endoscopic coagulation has been followed in some cases by hemorrhage control. Such techniques, if easily obtainable, have their place; however, surgery remains the ultima

Endothelial dysfunction in von Willebrand disease: angiogenesis and angiodysplasia.

In recent years, new functions for the haemostatic protein von Willebrand Factor (VWF) have emerged. Amongst these is the ability to modulate the development of new blood vessels, a process called angiogenesis. The subtle effects that VWF exerts on blood vessel formation and stability may be relevant for the small but significant fraction of patients with von Willebrand disease (VWD) who also present with vascular malformations (angiodysplasia) in the gastrointestinal tract, often responsible for intractable bleeding. This review will briefly summarise the evidence and discuss the molecular pa
