Is short-stay coronary angioplasty safe?

Percutaneous transluminal coronary angioplasty (PTCA) is usually performed as an inpatient procedure and the patients are monitored for several days afterward. Over a 13-month period, in 91 of 373 PTCA procedures, the clinical condition of the patient did not necessitate inpatient status before PTCA. PTCA was done the day of admission and discharge planned the following. Overall hospital stay was planned as less than 24 hours. PTCA was done in one vessel in 62 patients, two vessels in 24, three vessels in 3, and four vessels in 2 patients. PTCA was initially successful (less than 50% residual

Laser balloon angioplasty: potential clinical applications.

Coronary laser balloon angioplasty (LBA) is a new technique which permits application of heat (generated by the laser source) and pressure (by balloon inflation) to thermally weld tissue during coronary angioplasty (PTCA). The goal of LBA is to achieve a large, smooth arterial lumen, by thermal welding of dissection flaps, elimination of elastic recoil, elimination of vasospasm, reduction in platelet activation, desiccation of thrombus, and inhibition of smooth muscle cell proliferation (Table 1). The LBA system consists of a 50 watt continuous wave Nd:YAG laser with a wavelength of 1060 nm, a

Balloon angioplasty for congenital mitral stenosis.

We attempted balloon angioplasty in 9 children (ages 0.1 to 10 years) with congenital mitral stenosis. All were symptomatic with severe congestive heart failure and failure to thrive. Effective reduction in mitral gradient was initially achieved in 7 patients. For the entire group, mean valve gradient decreased from 14.8 +/- 5.0 to 8.1 +/- 6.7 mm (p = 0.0007) and mean valve area increased from 1.1 +/- 0.5 to 1.8 +/- 0.9 cm2/m2 (p = 0.003). More than mild mitral regurgitation developed in 2 patients but none required surgery for mitral regurgitation. Poor gradient relief followed dilation of va

Balloon angioplasty combined with vascular surgery.

Seventy-nine intra-operative balloon angioplasties were performed in 72 patients undergoing a vascular reconstructive procedure over a 7-year-period (1981 through December 1987). The primary purpose of balloon angioplasty was either to increase inflow (26 procedures) or outflow (53 procedures) in association with a planned vascular operation and to avoid more extensive operative procedures in high risk patients. Peroperative balloon angioplasty involved atherosclerotic stenotic lesions in 26 iliac, 37 femoro-popliteal and 10 crural arteries. In 6 cases distal graft anastomotic stenoses were di

Peripheral vascular complications of coronary angioplasty.

Percutaneous transluminal coronary angioplasty (PTCA) is being employed with increasing frequency for the treatment of coronary artery disease. PTCA involves the use of a larger sheath and catheter assembly than does cardiac catheterization, potentially increasing the risk of injury to the peripheral vessels used for access. Over an 18-month period, 295 angioplasty procedures resulted in six major vascular injuries (2%). The peripheral vascular complications that were recognized early included thrombosis of the common or superficial femoral artery in two patients and laceration of the iliac ar

Percutaneous transluminal angioplasty in renovascular hypertension.

The results of percutaneous transluminal angioplasty of renal arteries (PTA) were evaluated in 43 patients followed up for 2-39 months (mean: 14 months). The benefit of PTA was higher (82%) in hypertensives with arterial fibrodysplasia (n = 11) than in atherosclerotic vascular lesions (53%, n = 19). Of 15 patients with reduced renal function before PTA, the glomerular filtration rate rose in seven. A major complication was in one patient arterial dissection and in another one loss of function of the transplanted kidney due to acute tubular necrosis. The results confirm that PTA is an important

Septic endarteritis after percutaneous transluminal angioplasty.

A case of septic endarteritis leading to septic arthritis and septic emboli occurred after percutaneous transluminal angioplasty. To our knowledge, this is the first reported instance of arterial wall infection potentially caused by percutaneous transluminal angioplasty. Treatment consisted of long-term intravenous antibiotics and drainage of the septic joint.

Popliteal artery branches: percutaneous transluminal angioplasty.

Percutaneous transluminal angioplasty of the branches of the popliteal artery was performed in six patients, two of whom also had femoropopliteal angioplasty. Dilatation was performed with catheters ranging from 5.5 to 7 French. None of the popliteal branch dilatations was performed with an inflated balloon. Excellent dilatation, as documented by angiography and Doppler ultrasound pressure recordings, was obtained in four cases and moderate and slight improvement in one case each. Two of the four patients with excellent results had prompt healing of toe amputations and are doing well at 19 and

Chemical angioplasty vs. balloon plus chemical angioplasty for delayed cerebral ischemia: a pilot study of PbtO2 outcomes.

BACKGROUND: Delayed cerebral ischaemia (DCI) is a major cause of morbidity and mortality after aneurysmal subarachnoid haemorrhage (aSAH). Chemical angioplasty (CA) and transluminal balloon angioplasty (TBA) are used to treat patients with refractory vasospasm causing DCI. Multi-modal monitoring including brain tissue oxygenation (PbtO2) is routinely used at this centre for early detection and management of DCI following aSAH. In this single-centre pilot study, we are comparing these two treatment modalities and their effects on PbtO2. METHODS: Retrospective case series of patients with DCI w

Restenosis after carotid angioplasty, stenting, or endarterectomy in the Carotid and Vertebral Artery Transluminal Angioplasty Study (CAVATAS).

BACKGROUND AND PURPOSE: Carotid and Vertebral Artery Transluminal Angioplasty Study (CAVATAS) patients with carotid stenosis were randomized between endovascular treatment and endarterectomy. The rates of residual severe stenosis and restenosis and their contribution to recurrent symptoms was unclear. METHODS: Endovascular patients were treated by balloon angioplasty alone (88%) or stenting (22%). Patches were used in 63% of endarterectomy patients. Carotid stenosis was categorized as mild (0% to 49%), moderate (50% to 69%), severe (70% to 99%), or occluded, using standardized Doppler ultraso

The development of a pseudo-chamber after balloon pulmonary angioplasty: long-term complications of balloon pulmonary angioplasty.

We experienced a rare complication where extravasation developed a pseudo-chamber long after the balloon pulmonary angioplasty for supravalvular pulmonary stenosis. A 3-month-old girl was diagnosed with an anomalous origin of the left coronary artery from the pulmonary artery. She underwent the Takeuchi procedure at 10 months of age. During the follow-up, the supravalvular pulmonary stenosis deteriorated, and was treated by balloon pulmonary angioplasty with the double balloon technique catheter at 6 years of age. Angiography at the main pulmonary artery showed a small amount of extravasation

Comparative safety profiles of hirulog and heparin in patients undergoing coronary angioplasty. The Hirulog Angioplasty Study Investigators.

Several new anticoagulants and antiplatelet agents have been developed for use in coronary angioplasty, but almost all have been associated with an increased incidence of bleeding complications. Hirulog, a direct thrombin inhibitor, has several theoretical advantages over heparin. It is unclear, however, whether the use of hirulog as a substitute for heparin in angioplasty would result in a higher incidence of bleeding or other side effects. The safety profile of hirulog was compared with that of heparin in a randomized, double-blind trial in 4312 patients who were scheduled to undergo angiopl

Carotid angioplasty with cerebral protection and stenting: report of 164 patients (194 carotid percutaneous transluminal angioplasties).

BACKGROUND: The introduction of cerebral protection devices with systematic stent placement has changed the nature of carotid artery stenosis treatment, reducing the immediate periprocedural complications and delayed restenosis. METHODS: We treated 164 patients with 194 carotid artery stenosis procedures; 92% of them were symptomatic patients. RESULTS: The morbidity rate of our series was 1.03% and the mortality was 1.9%. CONCLUSIONS: In the future, carotid stenosis treatment should perhaps be performed as a preventative measure and not used as a cure for full-blown symptoms. This could be

[The place of percutaneous transluminal angioplasty in the treatment of obliterative vascular diseases of the lower limbs. Angioplasty of of the iliac system].

Between 1983-91 386 patients with hemodinamically significant stenosis or short-segment occlusion of iliac arteries were treated by transluminal angioplasty. 487 dilatations and 13 recanalizations were performed. In 353 (70.2%) of these cases were carried out without medicinal preparation continuously after the diagnostic angiography. The technical success rate was 93.4%. The technical success rate did not depend on medicinal preparation. 93% of their patients became symptomless or improved. 28 complications were noted (7 thrombosis, 4 embolisms, 14 inguinal haematomas, 2 false aneurysms, 1 bl

Drug-eluting balloon angioplasty versus uncoated balloon angioplasty for the treatment of in-stent restenosis of the femoropopliteal arteries.

BACKGROUND: Stents are placed in the femoropopliteal arteries for numerous reasons, such as atherosclerotic disease, the need for dissection, and perforation of the arteries, and can become stenosed with the passage of time. When a stent develops a flow-limiting stenosis, this process is known as "in-stent stenosis." It is thought that in-stent restenosis is caused by a process known as "intimal hyperplasia" rather than by the progression of atherosclerotic disease. Management of in-stent restenosis may include performing balloon angioplasty, deploying another stent within the stenosed stent t

Angioplasty or stenting for deep venous thrombosis.

BACKGROUND: The best medical treatment (BMT) for treating deep venous thrombosis (DVT) includes anticoagulation and compression stockings. Angioplasty and stenting restore vessel patency and facilitate blood flow. In some people with DVT, angioplasty or stenting is used to minimise complications such as post-thrombotic syndrome (PTS), but their effects are under discussion. OBJECTIVES: To assess the effects of adjunctive angioplasty or stenting on a background treatment of anticoagulation and thrombolysis, compared with BMT, sham procedure, thrombolysis, or any combination of these treatments
