Clinical pharmacokinetics of the angiotensin converting enzyme inhibitors. A review.

The angiotensin converting enzyme inhibitors are an important therapeutic advance in the treatment of patients with hypertension and congestive heart failure. In addition, they are useful pharmacological probes to assess the contribution of the renin-angiotensin system to circulatory homeostasis. Captopril was the first angiotensin converting enzyme inhibitor approved for use in patients with hypertension and congestive heart failure. It is rapidly absorbed from the gastrointestinal tract, with detectable plasma concentrations apparent as early as 15 minutes. The extent of absorption is betwee

Development and design of specific inhibitors of angiotensin-converting enzyme.

Captopril is a remarkably effective new antihypertensive drug designed and developed as a potent and specific inhibitor of angiotensin-converting enzyme, a zinc metallopeptidase that participates in the synthesis of a hypertensive peptide, angiotensin II, and in the degradation of a hypotensive peptide, bradykinin. Earlier studies with a snake venom peptide (teprotride or SQ 20881) that could be administered only by injection demonstrated that specific inhibitors of angiotensin-converting enzyme could be highly effective as antihypertensive drugs, and helped to clarify the specificity and mech

Angiotensin-Converting Enzyme Inhibitors and Metabolic Aging: A Drosophila Perspective.

Aging is characterized by a progressive decline in physiological function that impairs performance and increases vulnerability to disease and mortality. Delaying this deterioration is key to promoting healthy aging. Age-associated functional decline is closely linked to alterations in intermediary metabolism, including disrupted lipid metabolism and impaired mitochondrial function. Counteracting these metabolic changes, particularly those affecting basal metabolic rate and energy utilization, may be a feasible strategy to extend healthspan. The Renin-Angiotensin System (RAS), which controls bl

Small molecule angiotensin converting enzyme inhibitors: A medicinal chemistry perspective.

Angiotensin-converting enzyme (ACE), a zinc metalloprotein, is a central component of the renin-angiotensin system (RAS). It degrades bradykinin and other vasoactive peptides. Angiotensin-converting-enzyme inhibitors (ACE inhibitors, ACEIs) decrease the formation of angiotensin II and increase the level of bradykinin, thus relaxing blood vessels as well as reducing blood volume, lowering blood pressure and reducing oxygen consumption by the heart, which can be used to prevent and treat cardiovascular diseases and kidney diseases. Nevertheless, ACEIs are associated with a range of adverse effec

Surgery and discontinuation of angiotensin converting enzyme inhibitors: current perspectives.

PURPOSE OF REVIEW: The current article reviews the current evidence for continuing or discontinuing angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) during the perioperative period. RECENT FINDINGS: In patients undergoing treatment of hypertension with ACEIs or ARBs, there are both benefits associated with continuing these medications during the perioperative period and higher risk for perioperative hypotension and its complications. SUMMARY: Since the introduction of ACEIs and ARBs into clinical practice, their use during the perioperative period has

Angiotensin-Converting Enzyme Inhibitors and the Risk of Congenital Malformations.

OBJECTIVE: To examine the association between first-trimester angiotensin-converting enzyme (ACE) inhibitor exposure and the risk of overall major congenital, cardiac, and central nervous system malformations. METHODS: We used a cohort of completed pregnancies linked to liveborn neonates derived from Medicaid claims from 2000 to 2010. We examined the risk of malformations associated with first-trimester exposure to an ACE inhibitor. Propensity score-based methods were used to control for potential confounders including maternal demographics, medical conditions, exposure to other medications,

[Interaction of angiotensin converting enzyme inhibitors with nonsteroidal antiinflammatory drugs].

Analysis of results of randomized controlled studies revealed pronounced attenuation of vasodilating, natriuretic, and cardioprotective effects of angiotensin converting enzyme inhibitors (ACEI) by concomitant use with of indomethacin and aspirin in patients with hypertension, chronic heart failure, and acute myocardial infarction. In some studies beneficial effects of ACEI were shown to be weakened by concomitant use with of other nonsteroidal antiinflammatory drugs, including selective inhibitors of cyclooxygenase-2.

Angiotensin-converting enzyme inhibitors as inducers of adverse cutaneous reactions.

Since adverse effects due to angiotensin-converting enzyme (ACE) inhibitors frequently occur in cutaneous locations, this review summarizes the spectrum of expected and unexpected adverse effects of these drugs, possible associated mechanisms, and their basic functions for dermatologists. ACE inhibitors block the activity of the metalloproteinase ACE by binding to its active site, thus displacing angiotensin I and preventing its conversion to vasopressive angiotensin II. Furthermore, ACE degrades bradykinin, substance P, enkephalins and some of the reproductive peptide hormones. The overall in

Angioneurotic edema as a reaction to angiotensin-converting enzyme inhibitors.

While the incidence of ACEI-induced angioedema is low, ACEI use will continue to increase its frequency. Emergency physicians and otolaryngologists should play a key role in identifying patients with angioedema associated with ACEIs. Every patient with acute angioedema, especially in the region of the mouth, the tongue, and the upper airways, must be asked for medication with ACEIs. The physician must be aware of the fact that a moderate swelling can rapidly develop to a massive swelling, which requires an intubation or even a coniotomy. Thus, in any of these cases, the patient should be obser

[Angiotensin-converting enzyme inhibitors and pregnancy. Apropos of 2 cases].

Angiotensin converting enzyme inhibitors do not have any teratogenic effect in man but their use during pregnancy has led to cases of hypotension and subsequent severe renal failure in the newborn. The authors report two cases, one involving a twin pregnancy which illustrate the variability of the foetal involvement. It is emphasized that the side effects of angiotensin converting enzyme inhibitors are not only reversible but also are not constantly found.

Adverse effects of angiotensin converting enzyme (ACE) inhibitors. An update.

The angiotensin converting enzyme (ACE) inhibitors are a group of effective drugs with a unique mechanism of action. These drugs have proven to be useful for hypertension and congestive heart failure. Early clinical trials of captopril used doses that are now known to be inappropriately high, and dose-related adverse effects were observed frequently. The recognition that lower doses are effective has reduced the incidence of adverse reactions and resulted in improved patient tolerance. When patients are properly selected and correctable risk factors are removed, serious side effects are uncomm

Progressive renal insufficiency: the role of angiotensin converting enzyme inhibitors.

Experimental animal studies have demonstrated a renal protective effect of ACE inhibition therapy in diabetes mellitus and the remnant kidney model of chronic renal failure. The mechanism of this effect is secondary, at least in part, to the drugs' effects on glomerular hemodynamics. In addition, there is further evidence to suggest that ACE inhibitors may influence other pathogenic mechanisms of progressive renal insufficiency. Preliminary data in clinical studies suggest that ACE inhibition therapy decreases proteinuria and may ameliorate the decline of the glomerular filtration rate in diab

Diagnostic usefulness of renal scanning after angiotensin converting enzyme inhibitors.

Radioisotopic renal scanning after angiotensin converting enzyme inhibition (ACEI) has proven to be an exciting area for research. The biologic activity of markers such as DTPA and hippuran, when combined with the physiological effects of ACEI, may provide noninvasive methods of diagnosing both renal artery stenosis and renovascular hypertension. Recent investigators have demonstrated that the sensitivities and specificities of these tests may vary widely; these differences are probably due to variations in study design, patient population, diagnostic criteria, and outcome measurements. Diagnostic uses of angiotensin-converting enzyme inhibitors in renovascular hypertension.

The response to angiotensin-converting enzyme inhibitors (ACEIs) can be of considerable help in the diagnosis of human renovascular hypertension (RVH) in three settings. First, a particularly dramatic antihypertensive response or a decline in glomerular filtration rate (GFR), as indexed by a rise in serum creatinine or blood urea nitrogen concentrations, are useful clues to the presence of renovascular hypertension. Second, an exaggerated rise in plasma renin activity (PRA) after short-term captopril administration is a very promising screening test for this condition. Third, ACEI-induced chan

Angiotensin converting enzyme inhibitors in the clinic: first-dose hypotension.

A fall in blood pressure occurs, in most patients, within a few hours of a single dose of an angiotensin converting enzyme (ACE) inhibitor. While a serious fall in pressure is unusual in previously untreated essential hypertension, some patients are at risk of severe first-dose hypotension. These include those with treated heart failure, severe hypertension on polypharmacy, 'renin-dependent' renovascular hypertension and the occasional elderly patient. In such groups of patients the incidence of severe, symptomatic first-dose hypotension approaches 10%. This effect is not specific for ACE inhi
