Complications of implant removal in ankle fractures.

BACKGROUND: Although the complications of internal fixation in ankle fractures are well-known in a number of reports, there have been few reports revealing the complications of implant removal in ankle fractures. The aim of this study was to investigate the perioperative complications of implant removal in ankle fractures and analyze the associated factors of such complications. METHODS: Patients who underwent open reduction and internal fixation using metal implants for ankle fractures and had their implants removed between 2010 and 2015 were enrolled in the study. We investigated the rate a

Interventions for treating ankle fractures in children.

BACKGROUND: Ankle fractures, which usually occur after a twisting incident, are a diverse collection of injuries with different levels of complexity and severity. They have an incidence of 1 in 1000 a year in children. Treatment generally involves splints and casts for minor fractures and surgical fixation with screws, plates and pins followed by immobilisation for more serious fractures. OBJECTIVES: To assess the effects (benefits and harms) of different interventions for treating ankle fractures in children. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Speci

Complications of Pediatric Foot and Ankle Fractures.

Ankle fractures account for 5% and foot fractures account for approximately 8% of fractures in children. Some complications are evident early in the treatment or natural history of foot and ankle fractures. Other complications do not become apparent until weeks, months, or years after the original fracture. The incidence of long-term sequelae like posttraumatic arthritis from childhood foot and ankle fractures is poorly studied because decades or lifelong follow-up has frequently not been accomplished. This article discusses a variety of complications associated with foot and ankle fractures i

Radiographic assessment of posterior malleolar ankle fractures.

The anatomy of the posterior tibial malleolus plays an important role in the structure and function of the ankle mortise. With specific respect to ankle fractures, the presence, size, and displacement of posterior malleolar fractures (Volkmann's fracture) helps determine which will be amendable to operative fixation. The objective of the present study was to increase the body of knowledge with respect to the ability of foot and ankle reconstructive surgeons to assess posterior malleolar ankle fractures using plain film radiography. Three different variables were investigated on Sawbones(®) mod

[Causes of failed osteosynthesis of ankle fractures].

Ankle fractures are the most common osseous injuries of the lower extremity. In most cases, open reduction and internal fixation is indicated due to fracture dislocation. Operations of the ankle are generally considered classic, standardized, training procedures. An exact reconstruction with correct length and rotation of the joint as well as stabilization of the tibiofibular ligamentous complex is essential. Beside age and gender of the patient, outcome depends on fracture morphology and comorbidities, e. g., osteoporosis, vascular status, neuropathic disorders, and diabetes mellitus. Additio

Missed talar neck fractures in ankle distortions.

OBJECTIVES: Thirty-nine percent of the ankle and midfoot fractures in ankle distortions could be missed during initial evaluation in emergency department because of inadequate clinical and radiological evaluation in a limited time. We aimed to evaluate the follow-up and treatment outcomes of subjects with missed fractures, which were not diagnosed with plain radiographs obtained for ankle distortion, but with advanced imaging studies. METHODS: Eight patients (4 females, 4 males) who were initially treated with a diagnosis of ankle distortion due to trauma between 2004 and 2008 were included i

Immediate internal fixation of open ankle fractures.

BACKGROUND: Open ankle fractures are one of the most frequently treated open fractures. Immediate internal fixation in open ankle fractures is widely reported to produce good results, with a very low infection rate. However, there are few reports in the literature discussing distinct features of open ankle fractures. Thus, the aim of this study was to explore the injury features of such fractures and the operative technique; pilon fractures were excluded. MATERIALS AND METHODS: Ninety-six patients with open ankle fractures underwent surgery between August 2001 and March 2008. Ninety-two patie

Infection following operative treatment of ankle fractures.

UNLABELLED: Information on the microbiology of infections after operative ankle fractures, on the details of a treatment protocol used when the ankle joint is preserved, and on the outcome of this protocol will be helpful for the physicians managing patients with this complex problem. We therefore determined the most common pathogen of these infections, the infection recurrence rate, and the amputation rate. We retrospectively reviewed 26 patients of a mean age of 43 years with infections following operative treatment of ankle fractures. Twenty-one of 26 patients (81%) were compromised hosts a

Ankle fractures. The Lauge-Hansen classification revisited.

Rational treatment of ankle fractures requires knowledge of the extent of bone and soft tissue injury. Although the Lauge-Hansen classification attempts to do this by relating specific fracture patterns to injury mechanism, the experimental underpinning for this classification has not been reexamined rigorously using modern experimental methods. This study examines the hypothesis that the clinically occurring supination and external rotation injury pattern does not result from the mechanism described by Lauge-Hansen. Thirty-two anatomic specimen ankles were mounted on an MTS machine for combin

Infection in diabetic patients with ankle fractures.

Between January 1992 to June 1993, 93 ankle fractures underwent surgical treatment, of which, 10 patients were diabetic and 83 were non-diabetic. Infection occurred in 5 patients and all belonged to the diabetic group. The average follow-up period of the infected cases was 16.2 months. Wound infection occurred in 4 patients treated with open reduction and internal fixation. An infected pressure sore developed in the other patient treated initially in a below-knee cast for which arthrodesis of the ankle was performed. Infection was resistant to treatment in 2 patients and they ended up with bel

Immediate internal fixation of open ankle fractures.

Seventy-six consecutive displaced open ankle fractures were treated by immediate internal fixation between 1983 and 1986 at Los Angeles County University of Southern California Medical Center. Sixty-two patients were available for clinical and radiological examination at an average of 16.4 months post-fixation. Seventy-two percent of the patients had a satisfactory result. Poor results (20%) were most commonly due to non-anatomic reductions, articular surface damage, or deep infection. Deep infection occurred in 5% of the patients while 8% required late ankle arthrodesis. We believe immediate

Late results in 65 physeal ankle fractures.

Fifty-nine Salter-Harris III and IV lesions of the medial malleolus, Tillaux fractures, and triplane fractures were examined after 9 (3-32) years to assess the frequency of late symptoms, deformity, joint incongruity, and secondary arthrosis. Six out of 18 Salter-Harris III and IV lesions of the medial malleolus were reduced open, none of which had any signs of growth disturbance, whereas 1 case, left with a 3-mm residual dislocation, had 5 degrees of anterior angulation. Of the 17 Tillaux fractures, ten were not reduced and two of these gave slight symptoms, whereas five openly reduced fractu

Tillaux Fracture Associated with Adult Ankle Fracture Injury.

Tillaux fractures in adults are rare and, if unrecognized, can lead to ankle fracture healing complications, early progression of arthritis, and limited ankle movement caused by pain and degenerative changes. The Tillaux fracture was first described by Paul Tillaux as an external rotation injury of the ankle, involving an avulsion fracture of the distal anterolateral tibia. This fracture can be easily overlooked on plain radiographs in the adult. A high index of suspicion for this type of fracture pattern along with the use of computed tomographic scanning can help confirm the suspected diagno

[Progress in diagnosis and treatment of fibular fracture in ankle fracture].

OBJECTIVE: To summarize the diagnosis and treatment of fibular fracture in ankle fracture. METHODS: Relevant literature at home and abroad in recent years was widely consulted, and various types of fractures were discussed and summarized according to Danis-Weber classification. RESULTS: Conservative treatment is not an ideal treatment for type A avulsion fracture. The operation method should be determined according to the position and size of bone mass. For simple type B fracture, both conservative treatment and surgical treatment can achieve good results. When the position of type C fractur

Rotational Ankle Fracture Dislocation With Associated Lisfranc Fracture.

While ankle fractures most often result from a rotational injury, Lisfranc injuries are more commonly associated with an axial load on a plantarflexed foot. Due to differing mechanisms of injury, rotational ankle fractures with Lisfranc injuries are uncommon and rarely discussed in the literature. Here we present a case of a rotational ankle fracture-dislocation with a concomitant Lisfranc injury. The Lisfranc injury, which was ultimately treated nonoperatively, was discovered seven weeks after operative fixation of the ankle fracture. At the last follow-up nine months after the initial injury

Effectiveness of ankle arthroscopic debridement in acute, subacute ankle- bimalleolar, and trimalleolar fractures.

BACKGROUND: The purpose of this retrospective study was to evaluate the usefulness of ankle arthroscopy at the time of ORIF or after six months in patients with residual symptoms. The hypothesis was that in patients treated arthroscopically at the time of ORIF, there were faster and better clinical results than those treated after or untreated with arthroscopy. METHODS: In this retrospective study, we compared three homogeneous groups of selected patients with specific inclusion criteria (144 in total, mean age 38.2 years). They have been surgically treated for an ankle fracture (bimalleolar

Ankle fusion following failed initial treatment of complex ankle fractures in neuropathic diabetics.

BACKGROUND: Unstable ankle fractures in diabetics with peripheral neuropathy have an increased risk of postoperative complications, often leading to amputation. Primary ankle arthrodesis has been suggested as an alternative when acceptable reduction and mechanical stabilization cannot be obtained. METHODS: Over a fourteen year period, thirteen diabetic patients with peripheral neuropathy underwent an attempt at primary ankle arthrodesis following the early post-fracture development of acute neuropathic (Charcot) deformity of the ankle after sustaining a low energy unstable ankle fracture. Ankle performance after ankle fracture: a randomized study of early mobilization.

In a prospective, randomized study, 30 patients were evaluated after ankle fracture treated by means of open reduction and internal fixation. The patients were randomized to either postoperative immobilization in a plaster cast for 6 weeks or early mobilization (1-2 weeks after surgery) in an ankle brace. Both regimens allowed weightbearing. Evaluation after 10 weeks and after 12 months included clinical assessment and isokinetic muscle strength measurements. Patients with impaired ankle function, as shown by means of an ankle score at 12 months, were followed for 3 years. At 10 weeks, impaire

The Diagnosis, Treatment, and Aftercare of Ankle Fractures.

BACKGROUND: Ankle fractures are among the most common types of fracture among adults in Germany, with an incidence of 74 ± 32 cases per 100 000 persons per year. Treatment strategies have changed considerably over the past decade. In this review, we present the current treatment strategies for ankle fractures, aftercare, and long-term results. METHODS: Current treatment strategies for ankle fractures were summarized in the framework of a narrative review and expert consensus of the German Orthopaedic Foot and Ankle Society (Deutsche Assoziation für Fuß und Sprunggelenk, D.A.F.). Complications of Foot and Ankle Fractures in Diabetics.

Patients with diabetes have an increased risk of both foot and ankle fractures. Patients with mild disease may be managed similarly to the population with no diabetes, although patients with advanced diabetes are likely to require more robust fixation with prolonged periods of non-weight-bearing. All patients with diabetes should be screened for peripheral neuropathy, as this is a marker for poor outcomes following both surgical and nonsurgical management. The overall goal of treatment is to achieve a stable, plantigrade foot that fits into normal footwear, supports functional ambulation, and
