Effects of Ankle Joint Angles and Surrounding Muscles on Hip Joint Musculature.

Background/Objectives: Hip abductor weakness is a common issue in patients with lower back pain, knee osteoarthritis, and hip disorders, and compromises pelvic stability, gait control, and function. Side-lying hip abduction exercises are widely used as safe and effective interventions for patients unable to perform high-load or weight-bearing activities. However, the influence of ankle joint angles and distal muscle activity on the hip abductor muscles remains unclear. This study aimed to investigate the effects of ankle joint angles and activation states on unilateral right hip abductor stren

Changes of knee joint and ankle joint orientations after high tibial osteotomy.

OBJECTIVE: We sought to determine (1) whether change in the tibial plateau inclination (TPI) after high tibial osteotomy (HTO) is different from change in the knee joint line orientation (KJLO) relative to the ground; (2) whether, in varus knee OA patients before and after HTO, these radiographic measures are different from those in normal control; and (3) whether the postoperative values of the TPI and KJLO relative to the ground are associated with short term clinical outcome scores after HTO. DESIGN: Fifty patients who underwent HTO and 75 normal controls were assessed with four radiograph

The Ankle Joint: Revision Ankle Fusion Options, Nonunion, Malunion, Protocol for Best Outcome.

Ankle arthrodesis has been a time-tested procedure for osteoarthritis, avascular necrosis of the talus, deformity correction, and significant trauma of the ankle. Technique guides have created dissection pearls, ease of fixation, and arthroscopic techniques to mitigate complications of the procedure. Major complications, such as nonunion, malunion, or implant infection are the most worrisome and cumbersome complications to handle. The aim of this article is to provide the practicing surgeon evidence to provide innovative management techniques for nonunion, malunion, and infection following pri

Clinical and Radiographic Results of Ankle Joint Preservation Surgery in Posterior Ankle Arthritis.

BACKGROUND: Little information is available about how to manage ankles with eccentric arthritis in the sagittal plane. This study aimed to report clinical and radiographic outcomes following joint preservation surgery for ankles with eccentric arthritis at the posterior tibiotalar joint and a plantarflexed talus in the sagittal plane, which we named posterior ankle arthritis. METHODS: Ten ankles with posterior ankle arthritis were treated with realignment surgery between 2017 and 2018. Posterior ankle arthritis was defined as having both (1) eccentric narrowing of the joint space at the poste

Does the subtalar joint compensate for ankle malalignment in end-stage ankle arthritis?

BACKGROUND: Patients with ankle arthritis often present with concomitant hindfoot deformity, which may involve the tibiotalar and subtalar joints. However, the possible compensatory mechanisms of these two mechanically linked joints are not well known. QUESTIONS/PURPOSES: In this study we sought to (1) compare ankle and hindfoot alignment of our study cohort with end-stage ankle arthritis with that of a control group; (2) explore the frequency of compensated malalignment between the tibiotalar and subtalar joints in our study cohort; and (3) assess the intraobserver and interobserver reliabil

Effectiveness of ankle taping on ankle joint kinematics during walking on level ground.

UNLABELLED: Prescriptions for functional ankle instability are generally oriented to taping, which need to be validated as an efficient treatment. Therefore, the aim of this study was to investigate the effectiveness of ankle taping on the subject's foot pressure and gait pattern, including mediolateral analysis, according to tape application side. A sample of 20 right-handed healthy subjects volunteered to participate in this study, which was composed of 3 taping conditions: "with taping of the right ankle," "with taping of both ankles," and "with taping of the left ankle." Participants were

Effect of simulated joint instability and bracing on ankle and subtalar joint flexibility.

It is clinically challenging to distinguish between ankle and subtalar joints instability in vivo. Understanding the changes in load-displacement at the ankle and subtalar joints after ligament injuries may detect specific changes in joint characteristics that cannot be detected by investigating changes in range of motion alone. The effect of restricting joints end range of motion with ankle braces was already established, but little is known about the effect of an ankle brace on the flexibility of the injured ankle and subtalar joints. Therefore, the purposes of this study were to (1) underst

A multidimensional analysis of temporomandibular joint and ankle joint erosion in inflammatory arthritis.

Rheumatoid arthritis (RA) and other inflammatory arthritis are systemic diseases that primarily affect the joints, characterized by synovial inflammation and progressive cartilage and bone degradation. The temporomandibular joint (TMJ) is reported to be involved in over 50% of RA cases, often leading to severe jaw pain and compromised oral function. Despite its prevalence, TMJ involvement is often underestimated, and its cellular and molecular mechanisms remain poorly understood. Due to the unique biological and functional properties of the TMJ, inflammatory pathways observed in other joints s

Differences in kinematic control of ankle joint motions in people with chronic ankle instability.

BACKGROUND: People with chronic ankle instability display different ankle joint motions compared to healthy people. The purpose of this study was to investigate the strategies used to control ankle joint motions between a group of people with chronic ankle instability and a group of healthy, matched controls. METHODS: Kinematic data were collected from 11 people with chronic ankle instability and 11 matched control subjects as they performed a single-leg land-and-cut maneuver. Three-dimensional ankle joint angles were calculated from 100 ms before, to 200 ms after landing. Kinematic control o

Joint Preservation Strategies for Managing Varus Ankle Deformities.

Joint preserving strategies have evolved to a successful treatment option in early and midstage medial ankle OA caused by varus deformity. Though talar tilt can often not be fully corrected, it provides substantial postoperative pain relief, functional improvement, and slowing of the degenerative process. Osseous balancing with osteotomies is the main step for restoration of ankle mechanics and normalization of joint load. Overall, the key for success is to understand the underlying causes that have contributed to the varus OA in each case, and to use all treatment modalities necessary to rest

Ankle Joint Injuries Accompanying Fifth Metatarsal Base Fractures.

BACKGROUND: Fifth metatarsal base fractures are the most commonly seen fractures of the foot. Ankle sprains occur with inversion and plantarflexion mechanisms, similar to most fifth metatarsal base fractures. We sought to investigate the possible ankle injuries that accompany fifth metatarsal base fractures. METHODS: A hospital's digital database was searched for the International Classification of Diseases, Tenth Revision codes for metatarsal bone fractures (codes S92.30 and S92.35) between January 2015 and January 2018. Thirty-nine patients with fifth metatarsal base fracture who underwent

A stereophotographic study of ankle joint contact area.

The purpose of this study was to measure the ankle joint contact area under physiological load magnitudes using a stereophotography technique that allows accurate analysis of the entire joint surface without disrupting the joint during loading. Ten cadaveric foot and ankle specimens were loaded to 1000 N in neutral, and 20 degrees dorsiflexion, supination, pronation, and plantarflexion. Photo targets rigidly fixed to each of the bones were imaged in the loaded joint position using a high-resolution stereophotography system. After testing, each ankle was disarticulated and the joint surfaces im

Arthroscopic assessment of the chronic unstable ankle joint.

The purpose of this study was to evaluate prospectively the findings during arthroscopy in patients with chronic instability of the ankle joint. One hundred and ten consecutive patients who had suffered at least two ankle sprains and were symptomatic for at least 6 months were included in this study. A complete rupture of the anterior talofibular ligament was found in 64%, of the calcaneofibular ligament in 41% and of the deltoid ligament in 6%. Cartilage lesions of the talus were seen in 54% of the joints, more of them medial (56%) than lateral (15%) or ventral (20%). Other frequently observe

[The lower ankle joint. Anatomic and pathophysiologic observations].

The lower ankle joint is highly variable in ligamental guidance. It has to be considered in unity with the upper ankle joint. Biomechanical investigations have shown the posterior lower ankle joint to be relatively unstable primarily in supination position. Radiological techniques are available to procedure radiographic evidence to that clinical instability of the lower ankle joint. A surgical approach to instability of both upper and lower ankle joints is proposed in this paper and is discussed in some detail.

Ankle joint evaluation--an overview of arthroscopic technique.

Arthroscopy is useful in diagnosing and treating ankle pathology. It is comparatively atraumatic, which makes early postoperative ambulation possible and eliminates the problems resulting from casting and immobility. Familiarity with arthroscopic instruments and technique is essential and close attention to details is mandatory to avoid damage to the joint and other complications.
