Evaluation of anticonvulsants for possible use in neuropathic pain.

Neuropathic pain is a kind of pain related with functional abnormality of neurons. Despite large progress in pharmacotherapy, neuropathic pain is still considered an unmet need. Nowadays, there are few drugs registered for this condition, such as pregabalin, gabapentin, duloxetine, carbamazepine, and lidocaine. Among them, pregabalin, gabapentin and carbamazepine are well known antiepileptic drugs. Among the group of new antiepileptic drugs, which are addressed to 1% of human world population suffering from seizures, it turned out that 30% of the seizures resistant to pharmacotherapy has not e

The use of anticonvulsants in the aftermath of mania.

This paper reviews the evidence for the efficacy of anticonvulsants in the continuation and maintenance phases of treatment of bipolar disorder, when there has been an episode of mania (Bipolar-I disorder). The evidence to be considered is primarily that arising from randomized controlled trials. A literature search was performed to identify all randomized controlled trials of anticonvulsants used as maintenance treatment for bipolar-I disorder. Placebo-controlled studies were preferred, however there have been relatively few of these and clinical practice must therefore be guided by evidence

Benzodiazepines and anticonvulsants for social phobia (social anxiety disorder).

Both benzodiazepines and conventional anticonvulsants have been evaluated as treatments for social phobia (social anxiety disorder). Among the benzodiazepines, clonazepam is the best studied, although there is reason to expect that all benzodiazepine anxiolytics would be effective for this condition. Among the anticonvulsants, gabapentin and pregabalin, an analogue of gamma-aminobutyric acid (GABA), have been shown to be more effective than placebo in double-blind studies. Other than a small negative open study of valproic acid for social phobia, there is a paucity of information on whether ot

Hypersensitivity vasculitis and systemic lupus erythematosus induced by anticonvulsants.

Vasculitis can be a systemic manifestation of hypersensitivity to many drugs, among them anticonvulsants. The clinical manifestations include rash and renal, hepatic, and pulmonary involvement. Diagnosis is based upon clinical findings and a characteristic biopsy showing granulocytic and sometimes eosinophilic infiltrates around small blood vessels, especially venules. A severe form of hypersensitivity vasculitis, with extensive visceral involvement and poor prognosis, has been encountered very rarely following phenytoin and in isolated cases following carbamazepine and trimethadione administr

[Disorders of calcium metabolism and ossification caused by anticonvulsants].

Disturbances of Calcium-metabolism and of ossification are well known and well documented side-effects of anticonvulsant drugs since the first description in 1967. Elevated alkaline phosphatase and hypocalcemia till generalised osteoporosis and vitamin-D-sensitive rickets can be seen. The primary cause of these dose-related disturbances is an accelerated elimination of vitamin D because of induction of microsomal enzymes in the liver by anticonvulsant drugs; a suboptimal vitamin-D-intake and reduced sunlight exposure in institutionalised patients are often combined causative factors. In all pa

Continuous phrophylcatic anticonvulsants in selected childrens with febrile convulsions.

The recurrence rate of febrile convulsions is assessed in 108 children who had had either prolonged or repeated initial fits or had both a positive family history and persisting neurological disorder. Only 7 of 40 (17%) who received continuous prophylactic phenobarbitone or primidone for at minimum of 18 months had another fit compared with 40 of 68 (59%) children who were untreated (p less than 0.0005). Indications for continuous therapy are discussed.

[The frequency of adult anticonvulsant osteomalacia in relation to duration of therapy and dosage of anticonvulsants (author's transl)].

Of 837 epileptics over 16 years of age treated with mono- or combined hydantoin therapy 20.3% showed radiographic signs of anticonvulsant osteomalacia. With the exception of the patients with severe disturbances of the skeletal system no positive correlation was found with duration of therapy. The percentage of moderate bone changes was the highest in the patients treated for 1 to 2 years; the percentage of severe bone changes in the group treated over 10 years. The rate of osteomalacia correlated with the total dose of hydantoin, phenobarbital, or primidone; the correlation with the dose per

Anticonvulsant drugs and hematological disease.

Many antiepileptic drugs (AEDs) are associated with hematological disorders that range from mild thrombocytopenia or neutropenia to anemia, red cell aplasia, until bone marrow failure. Fortunately, potentially fatal hematological disorders such as aplastic anemia are very rare. This review investigates hematological effects associated with classic and newer AEDs: a PubMed search indexed for MEDLINE was undertaken to identify studies in adults, children and animals using the name of all anticonvulsant drugs combined with the terms "hematological disease" and "hematological abnormalities" as key

Phenytoin: an anti-bipolar anticonvulsant?

Phenytoin, a classical anticonvulsant has been little studied in bipolar disorder. We completed a trial of phenytoin in mania and schizoaffective disorder, manic type. Thirty-nine patients entered a 5-wk double-blind controlled trial of haloperidol+phenytoin vs. haloperidol+placebo; 30 patients completed at least 3 wk; 25 completed 5 wk. Significantly more improvement was observed in those patients receiving phenytoin. Phenytoin has not previously been studied prophylactically in bipolar patients. Bipolar patients were studied who had at least one episode per year in the previous 2 yr despite

Anticonvulsant activity of omeprazole in rats.

Omeprazole has long been used as an effective agent to treat peptic ulcer. Recent studies have shown that in addition to inhibiting the H(+)-K(+)ATPase, it also inhibits carbonic anhydrase (CA) types I, II and IV. This led us to investigate its anticonvulsant effect in a rat model of electroconvulsion. Since other carbonic anhydrase inhibitors like acetazolamide induce tolerance upon repeated use, we tested the tolerance potential of omeprazole upon repeated administration of up to 1 week. The animals were divided into four groups receiving normal saline, omeprazole 0.5, 1 or 2 mg/kg intraperi

[Hyperparathyroidism secondary to anticonvulsant treatment].

Treatment with some anticonvulsants is known to lead to liver enzyme induction. Two cases suggest that this interaction is also the mechanism behind anticonvulsant osteomalacia, which is characterised by hypocalcaemia, elevated alkaline phosphatase, and low 25(OH) cholecalciferol.

Parenteral antiepileptic/anticonvulsant drugs.

A large number of drugs can be given parenterally for the control of acute seizures, although many of these compounds are associated with serious adverse effects. Phenobarbital, the first antiepileptic drug (AED), has long been available in an injectable formulation. The sodium salt of phenobarbital is water soluble, and its parenteral formulation can be given for maintenance therapy or treatment of acute seizures. The introduction of phenytoin in 1938, and its subsequent parenteral formulation, represented a significant advance in AED therapy owing to its relative absence of sedation. However

Anticonvulsant drugs: mechanisms of action.

A variety of the anticonvulsant drugs, including carbamazepine, phenytoin, primidone, phenobarbital, clonazepam, valproic acid, and ethosuximide, are available for use in the treatment of patients with seizure disorders. These anticonvulsants vary in their efficacy against experimental seizures in animals and against seizures in humans. The mechanistic basis for this variability in anticonvulsant drug action remains uncertain, but numerous mechanisms of action have been proposed. We have used mouse neurons in primary dissociated cell culture to study the action of these anticonvulsant drugs on

Acetazolamide-accelerated anticonvulsant osteomalacia.

Severe osteomalacia was present in two epileptic patients who were under long-term treatment with congeners of phenytoin, phenobarbital, and acetazolamide. These patients showed slightly low serum calcium, normal or low serum phosphate, and normal parathyroid hormone concentrations. Discontinuation of acetazolamide produced an immediate threefold drop in the level of urinary calcium excretion and a slight rise in tubular reabsorption of phosphate, with no dectectable change in serum calcium or phosphate concentrations. Acetazolamide may have accelerated the development of osteomalacia by sever

[The mechanism of the anticonvulsive action of diazepam].

Diazepam was shown to be highly efficient in preventing the convulsive siezures induced by thiosemicarbazide and connected with GABA insufficiency. In recording the recovery cycles of the intracortical response of the cat motor cortex it was found that diazepam induced a decrease of the testing respone. These data indicate the enhancement of the inhibitory processes. Using this test an antagonistic relationship was observed between diazepam on the one hand, and biculline and thiosemicarbazide--on the other Diazepam was capable of increasing GABA content in the brain by suppressing GABA-transam
