Usefulness of computed tomography aortography and dye infusion in confirming the feeding arteries for effective treatment of a large squamous cell carcinoma of the lower lip with a single intra-arterial infusion chemotherapy combined with radiotherapy.

A 73-year-old male presented with a lower lip squamous cell carcinoma (SCC) (23 mm in diameter). Systemic imaging, including computed tomography (CT) and positron emission tomography, showed no evidence of distant metastasis. Due to the large size of the tumor, reconstruction after surgical tumor removal was considered to be difficult. Therefore, we decided to initially perform intra-arterial chemotherapy. Under local anesthesia, a catheter was inserted via the femoral artery. CT aortography from the ascending aorta was performed to visualize the precise vascular anatomy. The tumor's feeding a

Magnetic resonance angiography for detection of occlusive neck and proximal vascular diseases.

We reviewed magnetic resonance (MR) aortographies of the thoraco-cervical region of 422 patients. Two hundred sixty patients had ischemic cerebrovascular disease, 86 vertigo or dizziness, 22 syncope, 4 ischemic disease of the retina, 3 cervical bruit, and 47 had other intracranial diseases. MR aortography was performed on a 1.5 Tesla MRI using a special head-and-neck coil. Images were obtained using an overlapping multi-slab and three-dimensional time-of-flight technique. Of the 422 patients, 59 underwent conventional angiography, and the findings from their conventional arteriogram were compa

Doppler echocardiographic evaluation of pseudoaneurysms complicating composite grafts of the ascending aorta.

BACKGROUND: Pseudoaneurysms of the ascending aorta is a rare and serious complication after composite graft surgery for combined disorders of the aortic valve and ascending aorta. METHODS AND RESULTS: Echocardiographic and Doppler findings are described in eight patients (seven men, one woman; mean age, 45 +/- 12 years) with documented pseudoaneurysm of the ascending aorta and are compared with those by aortography and at surgery. The diameter of the ascending aorta ranged from 6 to 14 cm. Pseudoaneurysm was diagnosed by echocardiography in seven cases (six transthoracic, one transesophageal)

Usefulness of transesophageal echocardiography in the assessment of aortic graft dehiscence.

The diagnosis and site of graft dehiscence determined by transesophageal echocardiography (TEE), transthoracic echocardiography (TTE), and aortography in 6 patients, and by computed tomography (CT) scan in 3 patients were compared with surgery. Sensitivity of TEE was 100%, aortography 83%, CT scan 67%, and TTE 33%; TEE accurately identified severe aortic regurgitation (3), graft narrowing (2), distal aortic dissection (1), and a fistulous communication between the aortic root and the right ventricle (1) when compared with surgery or aortography.

[Traumatic rupture of thoracic aorta: review of a 10-year experience].

We retrospectively reviewed the diagnostic imaging examinations of 22 patients affected with traumatic rupture of the thoracic aorta acquired in a 10-year period. Our study was aimed at investigating if the diagnostic approach to these patients has changed in the last 10 years, especially relative to the extensive use of Computed Tomography (CT). All the patients in our series were submitted to chest radiography and aortography; only 15 of them were submitted also to CT. Plain radiography showed enlarged mediastinum and altered aortic profiles in 22/22 patients, rightward deviation of the trac

Conventional CT of the aorta.

The advent of CT has changed the way thoracic aortic aneurysms and dissections are evaluated. In many cases, CT is the only roentgenographic examination needed beyond plain radiograph. In evaluating aneurysm. CT's advantage over aortography is that it shows the wall and the mural thrombus, not just the contrast column. In some cases of aneurysm, however, particularly those arising near the aortic arch, aortography may be necessary to establish the precise location of the lesion with respect to the brachiocephalic arteries. In evaluating dissection, CT is less invasive and at least as accurate

The Role of Non-Contrast Cone Beam CT in Identifying Incomplete Treatment during Uterine Artery Embolization.

PURPOSE: To describe the utility of post-procedure noncontrast cone-beam computed tomography (CT) in identifying cases of incomplete treatment and the need to search for additional vascular supply during uterine artery embolization (UAE). MATERIALS AND METHODS: From June 2013 to June 2018, 427 patients (age, 45 ± 5 years) underwent 430 consecutive UAEs with post-embolization noncontrast cone-beam CT. If noncontrast cone-beam CT showed an area of the uterus lacking contrast retention, aortography was performed to search for collateral supply. Procedures were characterized as suspected complete

Traumatic aortic rupture: roentgenographic indications for angiography.

The principal radiological indication for thoracic aortography following blunt chest trauma has been a widened mediastinum on chest roentgenogram. The presence or absence of sixteen findings on 100 cm anteroposterior supine chest roentgenograms were noted and compared in 47 consecutive patients who underwent aortography following blunt thoracic trauma and 100 patients without trauma. On the basis of our data, we propose six radiological indications for thoracic aortography following chest trauma: mediastinum greater than 8 cm on 100 cm AP supine chest film; tracheal shift to the right; blurrin

Crossed, fused renal ectopia with severe hydronephrosis: angiographic diagnosis.

The authors describe two children with crossed, fused renal ectopia in whom severely damaged hydronephrotic ectopic kidneys were not shown by intravenous urography. Aortography confirmed the diagnosis in both patients. Aortography is of great value if a diseased ectopic kidney must be resected; demonstration of the vascular supply to both kidneys enables the surgeon to preserve all of the normal renal tissue.

Traumatic thoracic aortic rupture: investigation determines outcome.

Blunt thoracic aortic rupture (TAR) initially presents with subtle signs but is usually fatal if not diagnosed and treated early. Does the diagnostic process affect outcome? The definitive test most widely promoted is thoracic (arch) aortography but is usually only available in major teaching hospitals. Thoracic computerized tomography (CT) scanning is more readily available but its role in diagnosis of TAR is unproven. A retrospective review of trauma databases and medical record indexes over a 7 year period identified 38 patients presenting with TAR at Westmead and Royal North Shore Hospital

Usefulness of transesophageal echocardiographic monitoring in transluminal endovascular stent-graft repair for thoracic aortic aneurysm.

The endovascular stent-graft has been devised for the treatment of thoracic aortic aneurysm (TAA) to reduce complications associated with conventional surgical repair. The present study assessed the usefulness of transesophageal echocardiography (TEE) for intra- and post-operative examinations in patients treated with transluminal endovascular stent-graft repair for TAA. Nine patients with TAA and 2 with chronic type B aortic dissection were studied. Immediately after stent-graft deployment, perigraft leakage was evaluated with both intraoperative TEE and aortography. In 9 of 11 patients, TEE

Transesophageal echocardiography in the diagnosis of traumatic rupture of the aorta.

BACKGROUND: Rupture of the aorta is a major cause of death after motor vehicle accidents. Survival depends on early diagnosis, and emergency aortography is the standard imaging method. Although transesophageal echocardiography is noninvasive and can provide high-resolution images of the aorta, information about its value in patients with trauma is limited. We conducted this study to assess prospectively the value of transesophageal echocardiography in the emergency evaluation of patients at risk for aortic injury. METHODS: Transesophageal echocardiography of the aorta was attempted in 101 pat

Trauma to the aorta and major vessels.

Techniques for the diagnosis of blunt thoracic vascular injuries continue to evolve, and both helical CT scanning and transesophageal echocardiography are being compared with thoracic aortography. Repair of a tear in the descending thoracic aorta often is performed with the antriofemoral centrifugal pump. Patients with penetrating thoracic vascular injuries have a variety of presentations, with aortography restricted to those with proximity of wound to the superior mediastinum or the presence of a hematoma on admission chest radiograph.

[The value of the chest radiograph in thoracic aortic dissection and traumatic rupture (author's transl)].

The plain chest radiographs of 40 patients with angiographically proven thoracic aortic dissection or traumatic rupture are analyzed retrospectively with regard to their value. Only in two cases completely normal findings were observed. In all other cases characteristic changes could be observed leading to an immediate aortography. The plain chest radiograph is very important in the decision whether and when an aortography has to be performed on clinical suspected patients.

[Emergency operation for traumatic thoracic aortic rupture diagnosed by enhanced chest computed tomography; report of a case].

Thoracic aortic injury caused by blunt chest trauma is often fatal. Although aortography had been inevitable for thoracic surgery until recently, image of computed tomography (CT) is often superior to aortogram nowadays. We present a case of 64-yaer-old man with blunt chest trauma by traffic accident, who was successfully diagnosed and operated without invasive aortography. Thoracic aortic rupture was suspected by plain chest X-ray. His enhanced CT showed the localized leakage of contrast media near the arterial ligament of aortic arch. Because his condition was critically ill, operation was p

Role of computed tomography scan in evaluating the widened mediastinum.

Thirty-eight blunt trauma patients with a suspicion of thoracic aortic injury were assessed by computed tomography (CT) scan, aortography, and chest x-ray. Twenty-eight of the 38 patients had widened mediastinum by chest x-ray; four of 13 CT scans were abnormal. No patient with a negative CT scan had aortic injury by aortogram. A normal, non-dynamic CT scan of the chest should influence one's decision-making in the need for aortography.

[Chronic intestinal ischemia. Apropos of a case successfully treated with surgery].

A case is presented of chronic intestinal ischemia due to arteriosclerotic obstruction of the celiac trunk and superior mesenteric artery. The diagnosis was reached by right transfemoral aortography. Surgical treatment was direct reimplantation of the superior mesenteric artery on the anterior aspect of the healthy aorta and installation of a retrograde Dacron graft between the aorta and common hepatic artery. The patient's postoperative course was satisfactory, with total remission of the symptomatology; revascularization was confirmed by a new aortography at six months. The diagnostic and th
