A Window to Life: A Rare Association of a Small, Proximal Aortopulmonary Window With Pulmonary Atresia/Ventricular Septal Defect.

• PA/VSD has always been associated with either a PDA or MAPCAS. • PA/VSD can exist in the absence of a PDA or MAPCAS via a small APW. • PA/VSD with a small APW can be managed successfully with a Blalock-Taussig shunt.

Pulmonary Atresia and Ventricular Septal Defect Without Major Aortopulmonary Collateral Arteries: Diagnostic Evaluation and the Role of Ductal Stenting.

Patients with pulmonary atresia (PA) and a ventricular septal defect (VSD), similar to those with tetralogy of Fallot and PA without major aortopulmonary collateral arteries, lack antegrade pulmonary blood flow, and thus require a neonatal intervention for stabilization or augmentation of pulmonary blood flow. The role of ductal stenting in the management of these patients, and the current literature supporting it, will be reviewed.

An unusual case of pulmonary atresia with ventricular septal defect and multiple major aortopulmonary collateral arteries: undiagnosed until adulthood.

Pulmonary atresia and ventricular septal defect with major aortopulmonary collateral arteries is an extremely complex, heterogeneous, and rare anomaly. This group of patients may not be able to survive until adulthood without any interventions or treatment. Although surgical management of patients diagnosed in newborn, infant, or early childhood is clear, treatment of patients diagnosed in adulthood still remains a significant problem. The pre-operative clinical status, imaging methods, and operative findings might be helpful for planning the most appropriate management. Herein, we report a un

Stenting of central aortopulmonary shunt in a child with pulmonary atresia, ventricular septal defect, and severely hypoplastic pulmonary arteries.

A male infant was born at 28 weeks gestation weighing 850 g. He was diagnosed with pulmonary atresia, ventricular septal defect and major aorto-pulmonary collaterals with severely hypoplastic true pulmonary arteries. A 3.5 mm Gore-Tex central shunt was anastomosed from the ascending aorta to the main pulmonary artery at 4 months of age and a weight of 3.5 kg. The left pulmonary artery measured 1-2 mm and the right pulmonary artery measured 1 mm. His saturations increased from 70% to 85% following placement of the central shunt. At 16 months of age, his oxygen saturation decreased to 68%. Cardi

A novel two-stage complete repair method for pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries.

BACKGROUND: Pulmonary atresia with ventricular septal defect (PA-VSD) and major aortopulmonary collateral arteries (MAPCAs) remains a challenging complex congenital heart disease nowadays. In the present study, we aimed to develop a two-stage surgical method and to evaluate outcomes of this method in managing PA-VSD and MAPCAs. METHODS: Between December 2003 and December 2008, 7 female and 4 male patients between the age of 5 and 10 years who were suffering from PA-VSD and MAPCAs were selected and recruited. The native pulmonary artery trunks were absent in all patients; the lungs were solely

Early and late results after repair of aortopulmonary septal defect and associated anomalies in infants <6 months of age.

The Richardson classification system for aortopulmonary septal defect (APSD) includes simple defects between the ascending aorta and pulmonary trunk (type I), defects extending distally to include the origin of the right main pulmonary artery (type II), and anomalous origin of the right main pulmonary artery from the ascending aorta with no other aortopulmonary communication (type III). These are rare lesions that must be repaired in early infancy to avoid development of pulmonary vascular disease. Few reports have focused on patients with complex, associated lesions who underwent repair in ea

[Transaortic repair of aortopulmonary septal defect in neonate (20 days of age, 2175 gram) using profoundly hypothermic circulatory arrest].

In a 20-days old girl, aortopulmonary septal defect (total defect type) was closed by transaortic patch repair with cardiopulmonary bypass (CPB). The postoperative course was uneventful, and she was discharged 43 days later. This is the youngest and lowest-weight case ever reported in Japan. Various procedures for aortopulmonary septal defect have been proposed with or without CPB. However, transaortic repair with the aid of profoundly hypothermic circulatory arrest seems to be the most preferable corrective surgery for total defect type of neonate and low-weight infant.

Systematic Review of the Application of Pulmonary Hypertension Treatments in Ventricular Septal Defect, Pulmonary Atresia, and Major Aortopulmonary Collateral Arteries.

Background: Pulmonary atresia (PA) with ventricular septal defect (VSD) and major aortopulmonary collateral arteries (MAPCAs), a life-threatening congenital heart defect (CHD), is frequently associated with abnormal pulmonary blood flow and vascular remodeling, causing hypoxia and heart failure. Segmental pulmonary hypertension (PH), a distinct PH type, may exist in some patients. Pulmonary vasodilators have been considered for treatment; however, evidence of their efficacy and safety remains lacking. Methods: A systematic review was conducted using PubMed, MEDLINE, The Cochrane Library, and I

Long-term management of a child with aortopulmonary septal defect and Eisenmenger syndrome: a 6-year follow-up case study.

We report the case of a boy diagnosed at age 9 with a large aortopulmonary septal defect and Eisenmenger syndrome, highlighting the challenges of late diagnosis and management of fixed pulmonary hypertension. Over the course of six years (2018-2024), the patient underwent several follow-ups, including invasive hemodynamic assessments and echocardiography, revealing progressive pulmonary vascular disease. Despite the administration of medical therapy with sildenafil, bosentan, and antiplatelets, the patient's condition remained unfavorable due to fixed pulmonary vascular obstructive disease. Th

Pulmonary atresia and ventricular septal defect: How accurate is the fetal echocardiography, and do the major aortopulmonary collateral arteries matter?

OBJECTIVE: To assess the accuracy of prenatal echocardiography in defining pulmonary vasculature in pulmonary atresia with VSD (PAVSD). The second aim is to compare the perinatal and postnatal outcomes of different pulmonary blood supply types. STUDY DESIGN: The cases prenatally diagnosed with PAVSD between 2017 and 2022 in a single tertiary fetal medicine center were identified on the electronic database. Fetal echocardiography reports and images were reviewed retrospectively. Postnatal outcomes were acquired from the hospital records of relevant pediatric cardiology and cardiovascular surge

Rare cause of dynamic aortic obstruction in pulmonary atresia, ventricular septal defect, and major aortopulmonary collateral arteries: a case report.

A 2-month-old infant with pulmonary atresia, ventricular septal defect, and major aortopulmonary collateral arteries developed an aneurysmatic elongation of the tricuspid valve tissue that partially closed and dynamically protruded through the ventricular septal defect, beneath the aortic valve. This rare finding caused dynamic left ventricle outflow tract obstruction and recurrent cardiac arrests and ultimately required surgical intervention.

Anesthetic management for cesarean delivery in a patient with uncorrected pulmonary atresia, ventricular septal defect and major aortopulmonary collateral arteries.

Pulmonary atresia witha ventricular septal defect and major aortopulmonary collateral arteries is an extremely rare congenital disorder characterized by a high risk of maternal mortality. We present the case of a 24-year-old primigravid woman with uncorrected pulmonary atresia, ventricular septal defect and major aortopulmonary collateral arterieswho presented at 35+5 weeks' gestation. Based on the pathophysiology of the congenital cardiac lesion, cesarean delivery was performed under epidural anesthesia under management by a multidisciplinary team. This report highlights the anesthesia manage

Banding of Nonrestrictive Large Hypertensive Aortopulmonary Collaterals in Patients With Ventricular Septal Defect and Pulmonary Atresia Presenting in Suboptimal Condition.

Infants with pulmonary atresia and nonrestrictive ventricular septal defect with large hypertensive aortopulmonary collaterals demand early surgical intervention. This presentation in the extremely low-weight child or in the moribund septic child may preclude single-stage repair even if anatomically suited. We propose that such infants may be temporized by means of banding of individual aortopulmonary collaterals as a means of bridging to a second-stage complete repair. Two such cases are presented.

Clinical results of staged repair with complete unifocalization for pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries.

OBJECTIVE: Our treatment strategy for pulmonary atresia with ventricular septal defect (VSD) and major aortopulmonary collateral arteries is a staged repair that comprises the first complete unifocalization (UF) with 'unification' of intrapulmonary arteries and then the definitive repair. The purpose of this study is to evaluate the outcome of our staged repair strategy with complete UF and to determine the results of our current management strategy. METHODS: From 1982 to 2004, 113 consecutive patients were treated with staged repair at our institute. We evaluated the risk of definitive repai

Construction of a pulmonary artery pump for unifocalization and repair of pulmonary atresia with ventricular septal defect and major aortopulmonary collaterals.

Single-stage repair has been presented as the treatment of choice for pulmonary atresia with ventricular septal defect and major aortopulmonary collaterals. This surgical approach may result in the difficult decision of whether to close the ventricular septal defect. This decision may significantly affect the postoperative course of the patient. There are several diagnostic techniques clinicians may use to help them decide if closure is indicated. One technique is to modify an extracorporeal circuit to deliver precise flow rates of blood into the newly created pulmonary arterial system, at the

Pulmonary atresia with ventricular septal defects and major aortopulmonary collateral arteries: unifocalization brings no long-term benefits.

OBJECTIVES: We sought to evaluate the contribution of unifocalization procedures in the management of patients with pulmonary atresia, ventricular septal defect, and major aortopulmonary collateral arteries. METHODS: From 1975 through 1995, 82 consecutive patients were entered in a multistage approach and had 189 sternotomies and thoracotomies to perform 119 shunts, 130 major aortopulmonary collateral artery transplantations, and 76 major aortopulmonary collateral artery ligations. The serial angiographies and the follow-up of these patients were reviewed. RESULTS: The concurrent follow-up r

Rehabilitation Strategy Should Not Be a Pretext for Suboptimal Repair for Pulmonary Atresia, Ventricular Septal Defect, and Major Aortopulmonary Collateral Arteries.

Pulmonary atresia, ventricular septal defect, and major aortopulmonary collaterals are a challenging congenital anomaly to manage surgically with different centers and strategies producing wide-ranging outcomes. Over the past few decades despite diverging treatment pathways, there is an emerging consensus of how these patients "should" be treated. Quite often a combination of rehabilitation strategy and a unifocalization approach has to be tailored to each patient to address the anatomic and physiological variations that characterize this congenital heart defect. Irrespective of the surgical a
