Treatment outside the dental office: Replant immediately after gentle washing if practical. If replantation is not practical, store the tooth in the best medium available. Storage media in order of preference are Hank's Balanced Salt Solution (HBSS), milk, saline, and saliva (buccal vestibule). Water is the least desirable storage medium. Treatment in the office: Emergency visit; Place tooth in HBSS while exam is conducted and history is taken. Prepare socket for gentle repositioning of the tooth. Prepare the root. Extraoral dry time < 20 minutes: Closed apex--replant immediately after gentle

BACKGROUND: Avulsion is among the most severe types of dental trauma, which often occurs at young ages and can compromise the long-term prognosis of the traumatized tooth. Case Report. Herein, we report replantation of two avulsed teeth. Our patient was an 11-year-old boy with two avulsed maxillary central incisors due to a bicycle fall 2 months earlier. The patient was referred to us after rigid splinting of his teeth by a surgeon. Long-term calcium hydroxide (CH) therapy was performed for the patient, and after healing of periodontal ligament (PDL), apexification was performed for both teeth

This report presents a successful long-term outcome of an immediate replantation of an avulsed immature permanent maxillary central incisor in an eight-year-old girl. The avulsion occurred at home during recreational activity, and the tooth was replanted within 10 min by the patient's mother, a trained allied health professional. Clinical and radiographic assessments confirmed an open apex (Cvek stage 4), and the patient was managed in accordance with the International Association of Dental Traumatology guidelines, including flexible splinting and close follow-up. Over a five-year period, the

Necrotic immature permanent teeth with open apices pose a difficult management problem in clinical endodontics. Conventional apexification techniques resolve infection but do not support continued root development, leaving the tooth structurally vulnerable. Regenerative endodontic procedures (REPs) have emerged as a biologically oriented alternative that harnesses stem cells, scaffolds, and growth factors to form new tissue within the root canal space and encourage root maturation. This narrative review discusses the biological principles underlying REPs, the clinical protocols currently in us

Replantation is the treatment of choice for avulsed permanent teeth; ankylosis and cervical root resorption are among survival complications. A 9.5-year-old boy presented with an avulsed maxillary permanent central incisor with an open root apex following a school accident. The tooth was kept in milk, after a dry time of 15-20 min. Its replantation was performed 60 min after the accident. At 8 months, apexification with apical plug was attempted upon radiographic evidence of apical periodontitis. Ankylosis sound with infraocclusion and radiographic evidence of external cervical root resorption

AIM: To examine the response of immature sheep teeth with infected root canal systems to a commonly used pulp regeneration/revitalization protocol.
METHODOLOGY: Immature mandibular right first incisors in four sheep were mechanically exposed and the pulps infected. The mandibular left first incisors remained intact as controls. Five weeks later, the experimental root canals were chemo-mechanically cleaned and dressed with a triple-antibiotic paste for 4 weeks, before bleeding was induced inside the canal by mechanically irritating the periapical tissues. A collagen dressing was packed coronal

AIM: To present the successful management of an immature necrotic tooth through intentional replantation (IR) following the failure of a regenerative endodontic procedure (REP), highlighting IR as a viable alternative in complex cases.
CASE PRESENTATION: A 9-year-old patient presented with necrotic tooth 45 complicated by right mandibular cellulitis. After initial infection management, REP was attempted following European Society of Endodontology (ESE) guidelines. Despite adherence to protocol, REP failed, as evidenced by persistent symptoms and a recurring fistula. IR was chosen given the un

Pulp involvement of immature permanent teeth with dentinogenesis imperfecta is challenging and could lead to extraction. A case of dentinogenesis imperfecta-induced periapical periodontitis of an immature permanent tooth was treated with regenerative endodontic treatment (RET), and root maturation was observed in 12-month follow-up. An 8-year-old girl presented acute pain and swelling in central mandibular region. Clinical and radiographic examination revealed "shell teeth" appearance of teeth 31, 41, and 42. Periapical lesion of tooth 31 was observed. Tooth 41 was previously treated with apex

This case report describes the importance of continual examination for dental trauma and the efficacy of intentional replantation with retrograde root canal filling for a transverse root fracture in an immature incisor accompanied by subsequent periodontal-endodontic disease. In the treatment of traumatically fractured roots in immature incisors, continual examination is indispensable for the final diagnosis because roots in immature teeth are less calcified, resulting in less detailed radiological examinations. Thus, common dental trauma complications such as pulp necrosis may appear months a

OBJECTIVES: The treatment of immature necrotic teeth with apical periodontitis presents challenges in endodontic and pediatric dentistry. Revascularization is a recent treatment for such cases as an alternative to conventional apexification. The purpose is to examine the effect of a pulpal revascularization procedure on immature necrotic teeth with apical periodontitis.
STUDY DESIGN: Twenty patients were enrolled for pulp revascularization procedure by root canal disinfection using a triple antibiotic mixture for 1-2 weeks, followed by creating a blood clot, sealing the root canal orifice usi

The aim of this study was to explore the feasibility and efficacy of regenerative endodontic procedures (REPs) for treating avulsion injuries through a comparative retrospective analysis of avulsed immature permanent teeth treated with apexification (APEX) and REPs. Forty-nine teeth from 39 patients met the inclusion criteria and were divided into APEX (n = 25) and REP (n = 24) groups. Clinical and radiographic data were analyzed, with treatment outcomes categorized as "healed," "healing," or "disease." Quantitative radiographic analysis using ImageJ software measured percent changes in root l

The age at which tooth apical closure in dogs occurs is important for planning endodontic surgery. Limited previous studies in this area have demonstrated apical closure between 7 and 10 months of age. This study aimed to determine the apical closure status for canine, maxillary fourth premolar, and mandibular first molar teeth based on retrospective computed tomography (CT) and dental radiography data. Seven hundred and eighty-eight teeth from 149 patients were assessed by 2 examiners as either open, inconclusive, or closed. Fifty percent of studied apices were found to be closed by 10 months

The evolution from apexification to regenerative endodontic procedures reflects a paradigm centred on preservation of the apical stem cell niche as a prerequisite for restoration of the pulp-dentine complex. This case series explores the biological premise in 6 immature mandibular second premolars diagnosed with pulp necrosis and chronic apical pathology secondary to fractured Leong's tubercles. Management employed a tissue-preserving regenerative protocol characterised by minimal mechanical instrumentation and metronidazole monotherapy as the sole intracanal antimicrobial agent. Following can

Unique anatomical deviations in canal structure are rare in anterior teeth, especially central incisors, and thus risk being overlooked. For successful intervention, a meticulous diagnostic procedure and treatment plan, significantly aided by cone-beam computed tomography (CBCT), are crucial. The case at hand explores the management of a maxillary left central incisor in a cleft palate patient, characterized by multiple developmental lobes, a bulbous crown, and an atypical root anatomy. The primary symptom was pain, accompanied by a history of trauma at age 8 years and ensuing tooth discolorat

This report compares and evaluates the treatment outcomes of regenerative endodontic treatment and apical plug as two accepted treatment protocols in a pair of necrotic immature maxillary central incisors of a 12-year old female. The patient was referred complaining of a dull pain and swelling in her upper lip area. She had a history of trauma to the anterior maxilla two years earlier. Both teeth were clinically diagnosed with pulp necrosis and periapical radiographs revealed that separate periapical radiolucent lesions surrounded the immature apices of both teeth. The left and right incisors

The aim of this study was to assess the current status of mineral trioxide aggregate (MTA) as an educational material in dental schools in Turkey. A survey was sent to senior members of the endodontic departments of seventeen dental schools; fourteen responded. All respondents reported that they used MTA in their clinical practice, with apexification, perforations, retrograde fillings, and root resorptions being the most frequently occurring treatment procedures. All reported that information was given to students regarding MTA mainly as part of the curriculum. The third and fourth years were

BACKGROUND: The concept of vital pulp therapy (VPT) of immature permanent teeth has evolved in recent years. There has been a great tendency towards investigating new pulp capping materials for perfect imitation for natural dentin-pulp complex formation process and restoring the normal tissue's characteristics. Therefore, this study aimed to assess the clinical and tomographic outcomes of apexogenesis with human treated dentin matrix (hTDM) compared to mineral trioxide aggregate (MTA).

AIM: This study describes a clinical case of type III dens invaginatus with an extensive periradicular lesion treated successfully.
BACKGROUND: Dens invaginatus is a maldevelopment of the dental germ which occurs as a result of the invagination of the enamel organ. These cases may present difficulties with respect to its diagnosis and treatment because of canal morphology. The success of endodontic therapy requires a knowledge of dental anatomy and its anomalies.
CASE REPORT: A 17-year-old female patient is reported presenting right maxillary lateral incisor (tooth no. 7) classified as type
