Hemispheric alpha asymmetries in fluent and dysfluent aphasics during linguistic and resting conditions.

The hemispheric alpha asymmetries of a group of normal males, normal females, fluent aphasic males and dysfluent aphasic males were investigated during the auditory presentation of sentences, a short story and a resting condition in the absence of linguistic stimuli. No differences were found for the groups' mean alpha ratio scores during the resting condition. Fluent aphasics were found to demonstrate greater right hemispheric alpha suppression than the other groups for the sentence and story conditions. The dysfluent aphasics showed greater right hemispheric alpha suppression for the sentenc

Type and frequency of phonetic errors in aphasics: therapeutic aspects.

Fifty aphasics were studied in three separate groups: nonfluent Broca, fluent Wernicke and fluent anomics in order to form a therapeutic guide of their phonetic errors (omission substitution, addition, repeated phonemes, perseveration of phonemes belonging to the preceding word, reversed order, phonematic targetting and misplacing. Frequency of phonetic error was also assessed.

Plasticity in language cortex and white matter tracts after resection of dominant inferior parietal lobule arteriovenous malformations: a combined fMRI and DTI study.

OBJECTIVE: The dominant inferior parietal lobe (IPL) contains cortical and subcortical structures that serve language processing. A high incidence of postoperative short-term aphasia and good potential for language reorganization have been observed. The authors' goal was to study the plasticity of the language cortex and language-related fibers in patients with brain arteriovenous malformations (BAVMs) located in the IPL. METHODS: A total of 6 patients who underwent microsurgical treatment of an IPL BAVM were prospectively recruited between September 2016 and May 2018. Blood oxygen level-depe

[Improved instrumental activities of daily living in acquired savant syndrome patient after developing interest in drawing].

We performed examinations of a 73-year-old, right-handed man who developed herpes simplex encephalitis, with cognitive dysfunction including severe Wernicke's aphasia. Although he had never previously been interested in arts, use of a coloring book, recommended by his wife, led him to start drawing. A few years after the onset of brain disease, the patient began to copy pictures of landscapes. The lesion was in the left hemisphere and his work showed a strongly realistic tendency, thus we think that this case demonstrated characteristics of acquired savant syndrome. Along with the increase in

Variation in the pattern of omissions and substitutions of grammatical morphemes in the spontaneous speech of so-called agrammatic patients.

We describe the patterns of omissions (and substitutions) of freestanding grammatical morphemes and the patterns of substitutions of bound grammatical morphemes in 20 so-called agrammatic patients. Extreme variation was observed in the patterns of omissions and substitutions of grammatical morphemes, both in terms of the distribution of errors for different grammatical morphemes as well as in terms of the distribution of omissions versus substitutions. Results are discussed in the context of current debates concerning the possibility of a theoretically motivated distinction between the clinica

The disturbance of nonverbal functions in dysphasia.

Various modalities of six neuropsychological functions (graphia, calculia, finger gnosis, right-left orientation, praxia and constructive praxia) referred to as parietal or nonverbal have been investigated in the light of speech disorders. We examined 20 patients with brain lesion of vascular origin, who met the diagnostic criteria of mild and moderate dysphagia, 13 patients with Wernicke's and 7 with Broca's dysphasia. Verbal and nonverbal functions in patients with ischemic focuses of the speech area of the left hemisphere were investigated the Boston Diagnostic Aphasia Examination (BDAE). T

[Group therapy of aphasia patients--a professional report].

Theoretical considerations in, and an exemplary instance of, group speech therapy with aphasic patients are submitted for discussion. Contrary to current practice, individual and group therapy are considered of equal rank, as all aspects of speech and language can be realized in the latter, with both approaches having similar goals. Group work contents is characterized as verbal coping with guided verbal tasks. Group composition is recommended to emphasize speech and language criteria, as opposed to social and psychosocial criteria. Also dealt with is the position of the speech therapist.

Performance on figure ground perception following stroke induced hemiplegia: a compairson of pre-and post-rehabilitation with the neurologically unimpaired.

This article compares performance times on the Figure Ground Perception (FGP) of the Sensory Integration and Praxis Tests (SIPT) in persons with stroke-induced hemiplegia and unimpaired older persons. Two hundred and twelve right dominant persons with hemiplegia on admission and discharge from comprehensive rehabilitation and 321 neurologically unimpaired persons in the community were evaluated on the FGP subtests of the SIPT. There were significant differences in performance between unimpaired persons and those with hemiplegia following a stroke. Among the persons with stroke-induced hemipleg

An approach to analyzing a single subject's scores obtained in a standardized test with application to the Aachen Aphasia Test (AAT).

Methods for the analysis of a single subject's test profile(s) proposed by Huber (1973) are applied to the Aachen Aphasia Test (AAT). The procedures are based on the classical test theory model (Lord & Novick, 1968) and are suited for any (achievement) test with standard norms from a large standardization sample and satisfactory reliability estimates. Two test profiles of a Wernicke's aphasic, obtained before and after a 3-month period of speech therapy, are analyzed using inferential comparisons between (groups of) subtest scores on one test application and between two test administrations fo

Changes in the corpus callosum during the recovery of aphasia: A case report.

RATIONALE: The corpus callosum, which is the most important fiber pathway linking the bilateral hemispheres, plays a key role in information access, as well as the functional coordination and reorganization between the bilateral hemispheres. However, whether the corpus callosum will undergo structural changes during the recovery of aphasia is still unclear. In the current study, a Chinese aphasic patient with stroke was reported to develop changes in the corpus callosum after speech therapy. PATIENT CONCERNS: A 33-year-old right-handed male patient had aphasia only without limb paralysis at 1

[Current status of aphasia therapy].

Aphasia therapy in adults has been established to a larger extent relatively lately in the history of aphasiology, i.e. after its social medical importance had been realized and one of the cardinal problems of neurology solved more satisfactorily--lesion localization by imaging techniques. In order to evaluate the efficiency of aphasia therapy--which is still not quite uncontradicted--it was necessary to acquire sufficient knowledge of the spontaneous recovery process. It takes place--e.g. after stroke--mainly during the first 3 months, coming, as a rule, to a halt during the first year. Longe

[Development of aphasic symptomatology with age. Discussion apropos of an anatomo-clinical case].

Clinical data suggesting that aphasic semeiology undergo some evolution with age are summarized. Three kinds of hypotheses may account for this evolution: (1) a possible evolution with age of the site and extent of ischaemic lesions: (2) an evolution according to age of brain functional organization for language; and (3) evolution with age of psychological and/or social aspects of individuals in whom aphasia occurs. These hypotheses are discussed in the light of a clinico-pathological case of Wernicke's aphasia with logorrheic jargon due to an infarct involving both anterior and posterior part

Patterns of recovery and change in verbal and nonverbal functions in a case of crossed aphasia: implications for models of functional brain lateralization and localization.

We present a 2-year verbal and nonverbal follow-up of a crossed aphasic patient. The patient had suffered from widespread ischemic damage in the area of right middle cerebral artery, with a parieto-temporal lesion. Three months postonset he showed classical Wernicke's aphasia associated with oral, limb and constructional apraxia and left hemineglect. However, follow-up findings showed a complex, dynamic pattern entirely consistent with cognitive models of language and nonlanguage abilities. Current models of functional brain lateralizations could not satisfactorily account for such longitudina

[Cardiac sources of cerebral embolism and infarction: results of the Lausanne register for cerebrovascular accidents].

We studied 305 patients with a first stroke and a potential cardiac source of embolism (PCSE), on the basis of the Lausanne Stroke Registry. We have compared these patients with 1006 patients with stroke but without PCSE, admitted into the Registry during the same period, in order to assess the potential role of PCSE and the coexistence of other causes of stroke. Analysis of the various types of PCSE suggests that some neurologic characteristics are more frequent in patients with than without PCSE (infarcts of the posterior division of middle cerebral artery with Wernicke's aphasia and lateral

A crossed Kana agraphia.

We report a right-handed man who developed selective Kana (phonogram) agraphia following an infarct in the non-dominant right cerebral hemisphere. His ability for comprehension, reading and writing of Kanji (ideogram) was unaffected. Kana errors consisted of substitution with another letter and the number of target words was well preserved. The lesion responsible for his Kana agraphia included the right Wernicke's area (the posterior one-third or one-half of the superior temporal gyrus) on MRI, but he did not have aphasia. Based on these findings, we conclude that the language function in some

Language disintegration in dementia: effects of etiology and severity.

The speech characteristics on a standardized picture description task of 26 subjects with presumed senile dementia of the Alzheimer type (SDAT) and 13 subjects with stroke-related dementia (SRD) were compared to 15 normal subjects over age 59 years. Compared to the normal subjects, the dementia subjects used fewer total words, fewer unique words, fewer prepositional phrases, fewer subordinate clauses, and more incomplete sentence fragments. Lexical deficits tended to be more severe than syntactic ones, confirming prior suggestions that lexicon is more vulnerable to disruption in dementia than

Self-monitoring behavior in a case of severe auditory agnosia with aphasia.

This case report describes an unusual combination of speech and language deficits secondary to bilateral infarctions in a 62-year-old woman. The patient was administered an extensive series of speech, language, and audiologic tests and was found to exhibit a fluent aphasia in which reading and writing were extremely well preserved in comparison to auditory comprehension and oral expression, and a severe auditory agnosia. In spite of her auditory processing deficits, the patient exhibited unexpected self-monitoring ability and the capacity to form acoustic images on visual tasks. The manner in

Apractic agraphia in a patient with normal praxis.

Most right-handed crossed aphasics are not apractic. They usually have agraphia characterized by misspellings but retain the ability to write well-formed graphemes. We describe a right-handed patient with a right parietal lesion who was aphasic and not apractic. He was unable to write any formed graphemes despite a relatively preserved ability to spell aloud. We postulate that praxis and writing are dissociated in this patient because the motor engrams for praxis were located in his left hemisphere and the engrams for writing were in his right hemisphere. In addition, he comprehended commands

Contrasting speech patterns in apraxia of speech and phonemic paraphasia.

This study was designed to determine if perceptual phonological analysis would reveal distinctions between patients with apraxia of speech and patients with phonemic paraphasic speech. Test findings from 10 Broca's aphasics with apraxia of speech were compared to findings from 10 paraphasic speakers (5 conduction and 5 Wernicke's aphasics). Several marked differences were revealed. Predominant locus of errors and relative difficulty of different classes of phonemic segments were significant discriminators. There was a nonsignificant trend for substituted phonemes to be further from target phon
