Surgical options in the treatment of interhemispheric arachnoid cysts.

BACKGROUND: Arachnoid cysts located within the interhemispheric fissure are a matter of discussion as to the surgical indication and the choice of the most appropriate surgical procedure. In fact, in spite of the large dimensions that they can reach, the clinical manifestations may remain subtle and apparently stable over the years. On the other hand, the larger utilization of prenatal ultrasound investigations has increased the number of lesions detected before they can provoke clinical manifestations. As far as surgery is concerned, two main options are available-an extrathecal shunting proc

Arachnoid cysts: does surgery improve epileptic seizures and headaches?

During the period from 1985 to 1992 we treated 43 patients with intracranial arachnoid cysts (ACs). In this retrospective study we assessed the outcome of these patients with regard to non-specific symptoms such as headaches and epileptic seizures. Twelve patients had headaches of obscure origin and a mostly temporal located AC. Six of these 12 underwent surgery. After the operation 4 patients (4/6) had no further headaches, two remained unchanged. The other 6 conservatively treated patients (6/12) had further headaches. Ten of the 43 patients had epileptic seizures. One patient dropped out of

[Intracranial arachnoid cysts in children. Review of 34 cases].

Clinical, radiological, and therapeutical aspects of 34 cases of childhood intracranial arachnoid cysts are reviewed. Two thirds (23) were located supratentorially, the remaining 11 had an infratentorial localization. Of the first group 15 were situated at the sylvian region, 6 were hemispherical and 2 suprasellar. Clinical manifestations were present before 6 months of age in 68% of all the cases. Macrocephaly and focal seizures were the most common clinical findings. Skull X-Ray examination was helpful only when cysts were located at sylvian region. CT Brain Scan was positive in all cases (3

Syringomyelia and arachnoid cysts associated with spinal arachnoiditis following subarachnoid hemorrhage.

A 66-year-old woman with primary Sjogren syndrome developed syringomyelia following two episodes of subarachnoid hemorrhage (SAH) due to the rupture of basilar artery aneurysms. Gait disturbance and abnormal sensation with pain over the foot and abdomen appeared 3 years after the last SAH. Magnetic resonance (MR) imaging revealed a syringomyelia throughout the thoracic cord, from the T2 to T11 levels. In addition, the thoracic cord was compressed by multiple arachnoid cysts in the ventral side of spinal cord. Computed tomography myelography revealed complete block of cerebrospinal fluid (CSF)

Management of pediatric Sylvian arachnoid cysts. A retrospective study.

BACKGROUND AND PURPOSE: Neurosurgical approaches in Sylvian arachnoid cysts include microsurgery, endoscopy, and shunting. Yet, their relative safety and efficacy is still under debate. This retrospective study evaluated 36 pediatric patients with Sylvian arachnoid cysts and treated with different surgical types to contribute to global data. METHODS: The study included 24, 8, and 4 patients receiving endoscopic, microsurgical, and shunt surgeries, respectively. Preoperative and postoperative assessments included the patients' demographics and symptoms, cyst size and type, psychomotor status,

Coexistence of Trigonocephaly and Sylvian Arachnoid Cysts: A Coincidence?

INTRODUCTION: The association between trigonocephaly and Sylvian fissure arachnoid cysts (ACs) has been occasionally reported in the literature. However, the real incidence of this association and its clinical relevance remain unknown. METHODS: The authors collected and retrospectively reviewed all clinical charts and CT scans of patients surgically treated for trigonocephaly at the Pediatric Neurosurgical Department of Fondazione Policlinico Universitario "Agostino Gemelli" IRCCS from January 2014 to June 2023. RESULTS: During the study period, 136 patients with trigonocephaly underwent sur

Spinal arachnoid cysts - our experience and review of literature.

OBJECTIVE: Arachnoid cysts are discrete pockets of CSF or CSF-like fluid found adjacent to normal CSF spaces, either spinal or cranial. Spinal arachnoid cysts (SAC) are most commonly extradural, however intradural or perineural are also described. METHODS: All patients admitted to our center and surgically treated with a diagnosis of SAC, were included in the study. The results were analyzed in terms of the clinical symptoms, location of cyst, surgical procedure performed and outcome following surgery. RESULTS: Eleven patients were operated for SAC during the study period and the mean age at

Shunt dependency syndrome after cystoperitoneal shunting of arachnoid cysts.

PURPOSE: The goal of this study was to investigate the mechanisms, diagnosis, and treatment of shunt dependency syndrome in patients with temporal lobe arachnoid cysts who were initially treated with cystoperitoneal (CP) shunting. METHODS: Thirteen patients with temporal lobe arachnoid cysts who had initially been treated with CP shunt placement and had developed shunt dependency syndrome were treated by the senior author at Tiantan Hospital between April 2010 and January 2012. The clinical manifestations, neuroimaging findings, intracranial pressure (ICP) data, treatment methods, and therape

Cervical spinal intradural arachnoid cysts in related, young pugs.

Seven related young pugs were diagnosed with cervical spinal intradural arachnoid cysts by magnetic resonance imaging (n = 6) and myelography (n = 1). All dogs were presented with skin abrasions on their thoracic limbs and non-painful neurological deficits, indicating a C1-T2 myelopathy. In all six dogs examined by magnetic resonance imaging not only the spinal arachnoid cyst but also a concomitant, most likely secondary, syringohydromyelia was confirmed. Pedigree analysis suggested a genetic predisposition for spinal arachnoid cysts in this family of pugs. Generalised proprioceptive deficits

[Intrasellar arachnoid cysts. Two case reports and literature review].

Intrasellar arachnoid cysts are an uncommon pathology, with only a few cases reported in scientific literature. Their physiopathology is still unknown, with different hypothesis about their development. Differential diagnosis with other cystic sellar lesions is difficult. We report two cases of two intrasellar arachnoid cysts that were treated surgically using transsphenoidal approach and present a review of the literature.

Quadrigeminal arachnoid cysts in a kitten and a dog.

Two quadrigeminal arachnoid cysts with different pathogenesis are described in 2 different species. A 10-week-old male Persian kitten with a progressively decreasing level of consciousness died spontaneously. At necropsy, mild internal hydrocephalus, caudal cerebellar coning, and cerebellar herniation through the foramen magnum were associated with a congenital quadrigeminal arachnoid cyst compressing the rostral cerebellum and shifting the entire cerebellum caudally. In contrast, a possibly acquired quadrigeminal cyst was observed in a 2-year-old male neutered Yorkshire Terrier in association

Magnetic resonance cisternography for preoperative evaluation of arachnoid cysts.

INTRODUCTION: With a high likelihood of clinical improvement and low rates of complications, minimally invasive neuroendoscopic surgery is becoming the treatment of choice for symptomatic or growing arachnoid cysts. In neuroendoscopic surgery, visualization of anatomical landmarks is essential in achieving successful fenestration without complications. Because of the restricted visual field in neuroendoscopic surgery, preoperative anatomical assessment is very helpful. Magnetic resonance cisternography (MRC) with high spatial resolution and contrast, using for example 3-D Fourier transformatio

Arachnoid cysts: case series and review of the literature.

OBJECT: Arachnoid cysts are commonly encountered fluid collections in the central nervous system. Recent advances in neuroimaging have translated into an increased detection of these cysts; that is, diagnoses can be made more frequently at earlier stages. Significant advances have also been made in the surgical management of these lesions. The authors report on a case series that illustrates the diverse forms of presentation and the treatment modalities commonly used for arachnoid cysts. METHODS: Between January 2002 and December 2006, 20 patients with arachnoid cysts underwent surgery perfor

Extradural spinal arachnoid cysts associated with spina bifida occulta.

INTRODUCTION: Spinal extradural arachnoid cysts are an uncommon cause of neural compression in children. Even more uncommon is the association of such cysts with spina bifida occulta. MATERIAL: Two girls, 12 and 8-years-old, presented with left leg pain, deteriorating gait, clinical signs of left L5 and S1 root compression, without bladder or bowel symptoms. The first patient had left foot drop. The second patient had muscle wasting and smaller left foot with pes cavus. Radiographs showed spina bifida occulta of S1 in both. MRI revealed an extradural cyst at the S1 level, indenting the thecal

Arachnoid cysts cause cognitive deficits that improve after surgery.

The authors investigated cognition in patients with arachnoid cysts by assessing 55 patients and 32 control subjects on four different cognitive tests preoperatively and 3 months postoperatively. The patients improved their performance from preoperative to postoperative testing, whereas the control subjects did not show a similar improvement.

Middle cranial fossa arachnoid cysts causing sensorineural hearing loss.

Arachnoid cyst of the middle cranial fossa has never been reported to present with sensorineural hearing impairment. We report the case of a 15-year-old girl who complained of progressive hearing loss on the right side for 4 years. The pure tone audiometry revealed sensorineural hearing loss with an average of 63 dB, and the auditory brain stem response demonstrated absence of all waves on the right side. Magnetic resonance imaging showed giant arachnoid cysts occupying the bilateral middle cranial fossa and extending to the parietal and prepontine areas with the atrophic change of the right f

[Intracranial arachnoid cysts in children. Apropos of 8 cases].

Eight cases of intracranial arachnoid cysts are reported. They are benign congenital lesions who result from an abnormal development of the earliest arachnoid. A subsequent development is possible. The macrocephaly is the first sign and other symptoms give evidence to the localization. Thanks to the C.T. scan one can make the diagnosis, appraise the repercussion and put the therapeutic indications. The surgical treatment is reserved to symptomatic forms. The peritoneal shunt takes the ablation's place. This method is easier and less liable to complications. The results depend on the localizati

Clinical analysis of arachnoid cysts in the middle fossa.

This paper describes the clinical picture and radiological findings of 12 cases of "idiopathic" arachnoid cyst in the middle fossa which the authors have encountered during the past 12 years. Their results are as follows. 1) The age of the patient in 10 out of 12 cases was under 15 years of age, and 8 cases were males. The site of the cyst was on the left side in 9 cases. 2) On plain skull radiography, thinning and bulging of the temporal bone by the lesion, elevation of the lesser wing of the sphenoid, and forward projection of the greater wing of the sphenoid were observed in all cases. 3) O
