Assessing barriers to adjuvant therapy among patients with head and neck cancer in Arkansas.

Advanced head and neck cancer (HNC) requires multimodal treatment and in the setting of primary curative intent surgery, adjuvant radiation with pathology-based chemotherapy is necessary for best outcomes. HNC treatment guidelines and best evidence support adjuvant radiation initiated within 6 weeks of surgery [1]. Over half of HNC patients face obstacles delaying adjuvant therapy, especially in rural areas [2]. This study assesses barriers to timely adjuvant therapy, specifically radiation in a rural tertiary setting. The study conducted was a retrospective mixed-methods analysis. The initial

Potential ecological distribution of Cytauxzoon felis in domestic cats in Oklahoma, Missouri, and Arkansas.

The ecological distribution of Cytauxzoon felis, an often-fatal tick-borne apicomplexan that infects domestic cats, has not been evaluated or identified despite its continued emergence. Infection of C. felis is characterized by lethargy, icterus, fever, anorexia, anemia, and death. The natural vertebrate reservoir of C. felis is the bobcat (Lynx rufus). To determine the possible distribution of C. felis in three states where infection is common (Oklahoma, Missouri, and Arkansas), two separate approaches to ecological niche modeling were implemented. First, a model relating several different cl

The Arkansas aging initiative: an innovative approach for addressing the health of older rural Arkansans.

The Donald W. Reynolds Institute on Aging at the University of Arkansas for Medical Sciences in Little Rock is addressing one of the most pressing policy issues facing the United States: how to care for the burgeoning number of older adults. In 2001, the Institute created the Arkansas Aging Initiative, which established seven satellite centers on aging across the state using $1.3 to $2 million dollars annually from the state's portion of the Master Tobacco Settlement. These centers on aging assist the state's population of older adults, many of whom reside in rural areas, live in poverty, and

Societal value of newborn screening for severe combined immune deficiency in Arkansas: An economic analysis.

Newborn screening (NBS) is a public health program that detects genetic conditions in neonates enabling treatment before clinical symptoms manifest. Severe combined immune deficiency (SCID) is a primary immune deficiency found in the absence of functioning T and B lymphocytes. Hematopoietic cell transplantation is a potentially curative treatment if received within the first 42 months of life; without treatment, this condition is fatal in the first 2 years of life due to severe opportunistic infections. SCID was added to the recommended uniform panel of conditions for inclusion in state NBS pr

Issues and biases in matching medicaid pregnancy episodes to vital records data: the Arkansas experience.

OBJECTIVES: This study examines the extent of selection biases identified in the process of linking Medicaid claims with evidence of pregnancy to vital records. METHODS: Two years of Medicaid claims were scanned to identify pregnancy-related diagnoses and procedures. Information on 55,764 Medicaid recipients was provided to the Division of Health Statistics, which linked the information to vital records data on a range of identifying characteristics. Claims were then clustered by date and then into episodes of care surrounding the birth date of the infant. We identified 38,222 pregnancy episo

The epidemiology of Rocky Mountain spotted fever in Arkansas, Oklahoma, and Texas, 1981 through 1985.

From 1981 through 1985, a total of 851 laboratory-confirmed cases of Rocky Mountain spotted fever were reported in Arkansas, Oklahoma, and Texas. Annual incidence rates in each state increased from 1981 through 1983, ranging from a 17% increase in Arkansas to a 137% increase in Texas. In the period 1984 through 1985, the annual incidence rates in each state decreased over 50%. A total of 519 cases were males. A majority of cases (51%) were 19 years of age or younger. April through August were the months of onset of symptoms for 83% of the cases. Differences in distribution of month of onset of

Fatal Case of Splash Pad-Associated Naegleria fowleri Meningoencephalitis - Pulaski County, Arkansas, September 2023.

A fatal case of primary amebic meningoencephalitis (PAM), an infection caused by Naegleria fowleri, was diagnosed in Arkansas in a young child in September 2023. A public health investigation was completed, with epidemiologic, laboratory, and environmental data suggesting that a splash pad (an interactive water play venue that sprays or jets water on users and has little or no standing water) with inadequately disinfected water was the most likely site of the patient's N. fowleri exposure. This case is the third occurrence of splash pad-associated PAM reported in the United States; all three c

Hesitancy towards routine childhood vaccinations before and after the COVID-19 pandemic in Arkansas.

OBJECTIVE: Prior to the COVID-19 pandemic, vaccine-preventable diseases increased nationally, associated with low childhood vaccination coverage. Studies on parental vaccine hesitancy in southern U.S. states is limited, and childhood vaccine uptake has decreased since early 2020. Our study purpose was to examine parental vaccine hesitancy before and after the COVID-19 pandemic among Arkansas residents. METHODS: A repeated cross-sectional design using telephone surveys was conducted among Arkansas parental guardians of children aged ≤6 years in 2019 (n = 407) and 2023 (n = 402). Parental hesit

Arkansas Medical Marijuana Certifications: Higher-Volume Physicians Associated With Less Evidence Of Care Coordination.

Patients' access to medical marijuana has dramatically increased despite the lack of Food and Drug Administration approval. In this study, we profiled individuals and the conditions for which they were certified for medical marijuana, and we examined the evidence of coordination with their physicians providing traditional care. Within two years of initiation, medical marijuana was approved for 3.4 percent of Arkansans ages eighteen and older by 12.5 percent of physicians who had an active license in the state. Posttraumatic stress disorder and four pain diagnoses were the most frequent qualify

Enhancing Naloxone Reach Through Innovative Training Strategies at an Academic Medical Center in Arkansas.

The Arkansas Naloxone Education and Training (ANET) program, based at the University of Arkansas for Medical Sciences, was developed to address the urgent need for opioid-overdose prevention across the state. Funded by the Substance Abuse and Mental Health Administration (SAMHSA), ANET uses a train-the-trainer model to equip health care professionals with the knowledge and tools necessary to deliver naloxone education in their local settings. In its first year, the program launched a three-module curriculum, accredited for continuing education, of health care professionals and delivered using

Generalized knemidokoptic mange (Micnemidocoptes sp.) in a bald eagle (Haliaeetus leucocephalus) from Arkansas, USA.

This report describes a case of generalized knemidokoptic mange caused by Micnemidocoptes sp. in a bald eagle (Haliaeetus leucocephalus). In June 2024, an adult male eagle from Polk County, Arkansas, presented with severe skin disease and in a moribund state. The eagle died soon after capture and postmortem examination revealed extensive skin hyperkeratosis with embedded mites. Molecular analysis of a 632 bp fragment of the mitochondrial cytochrome oxidase subunit I (COI) gene showed the mite was 99.8 % similar to a Micnemidocoptes sp. from a golden eagle (Aquila chrysaetos) in California and

Genomic characterization of mumps viruses from a large-scale mumps outbreak in Arkansas, 2016.

In 2016, a year-long large-scale mumps outbreak occurred in Arkansas among a highly-vaccinated population. A total of 2954 mumps cases were identified during this outbreak. The majority of cases (1676 (57%)) were school-aged children (5-17 years), 1536 (92%) of these children had completed the mumps vaccination schedule. To weigh the possibility that the mumps virus evaded vaccine-induced immunity in the affected Arkansas population, we established a pipeline for genomic characterization of the outbreak strains. Our pipeline produces whole-genome sequences along with phylogenetic analysis of t

Prevalence of Dirofilaria immitis (Nematoda: Filarioidea) in mosquitoes from northeast Arkansas, the United States.

A mosquito survey was conducted to identify which species of mosquitoes carry Dirofilaria immitis (Leidy) (Nematoda: Filarioidea), dog heartworm, in northeast Arkansas. Using polymerase chain reaction, mosquitoes were analyzed for D. immitis, Dirofilaria repens Railliet & Henry, and Acanthocheilonema dracunculoides Cobbold. Mosquitoes were collected from April to October 2009 using black light ultraviolet traps baited with dry ice. Sixteen mosquito species were identified. D. immitis was identified in nine mosquito species, which included Aedes vexans (Meigen), Anopheles quadrimaculatus Say, A

Cortical bone maintenance in an historic Afro-American cemetery sample from Cedar Grove, Arkansas.

The relocation and analysis of 80 skeletons from the Cedar Grove Cemetery, located in southwest Arkansas, provides an opportunity to examine the level of health and nutrition experienced by Afro-Americans in the post-Reconstruction South (1878-1930). The demographic profile lends support to the interpretation that Cedar Grove participated in the nationwide decline in Afro-American health. The high frequencies of skeletal lesions indicative of dietary deficiencies and infectious disease demonstrate that this was a highly stressed population. For this analysis, adult femoral thin sections (15 fe

Mental Health, Drug Use, and Programming: Applying a Needs Assessment Framework in Arkansas' Largest Jail.

Jail programming is rarely informed by site-specific health needs, diagnostic-specific screening tools that are validated, or the input of incarcerated individuals. Using the community needs assessment (CNA) framework, we aimed to fill these gaps among people incarcerated in the Pulaski County Regional Detention Facility (PCRDF), Arkansas' largest jail. Participants were 179 adults at the PCRDF who completed surveys and open-ended questions focused on (a) their mental and behavioral health and (b) programming needs at the facility. Using a concurrent transformative mixed-methods design, we des

Adolescent body mass index as a strong predictor of pre-pregnancy obesity in Arkansas mothers.

Maternal obesity is strongly associated with adverse pregnancy outcomes. However, the extent to which adolescent weight status can predict pre-pregnancy obesity and severe obesity among future first-time mothers has not been well explored. This cohort study utilized adolescent body mass index (BMI) assessments linked to birth records for women 20-30 years old to examine this relationship. The population-based study cohort included 27,370 first-time mothers who had a 10th-grade BMI measurement; among this cohort, 18,881 had BMI assessments in 10th, 8th, and 6th grade. Most women who had severe

Emergency Nurses' Training, Experience, and Confidence With Obstetrical Emergencies: A Cross-Sectional Survey in Arkansas.

INTRODUCTION: The United States has the highest maternal mortality rate among high-income countries, with rural states being disproportionately affected. In counties without birthing hospitals, emergency departments can serve as the first point of care for obstetrical complications. This study assessed emergency nurses' experience and confidence in managing obstetrical complications in the rural state of Arkansas. METHODS: Emergency nurses at Arkansas hospitals were invited to participate in a cross-sectional survey. The web-based survey asked nurses to report their experience in managing 15

Statewide policies for safer teen driving: an evaluation of the impact of graduated driver licensing in Arkansas.

BACKGROUND: Graduated driver licensing (GDL) requirements aim to reduce the incidence of motor vehicle crashes and crash-related fatalities for novice drivers by limiting their exposure to the most risky driving situations. These programs vary across states in their scope, intensity, and impact. The purpose of this study was to evaluate the short-term impact of the 2009 Arkansas GDL on reducing teen crashes and fatal crashes. METHODS: Arkansas motor vehicle crash data for 2008 and 2010 were compared. Changes in rates per 10,000 licensed drivers were calculated by age, during the night restric
