Electromechanical modeling of human ventricles with ischemic cardiomyopathy: numerical simulations in sinus rhythm and under arrhythmia.

We developed a novel patient-specific computational model for the numerical simulation of ventricular electromechanics in patients with ischemic cardiomyopathy (ICM). This model reproduces the activity both in sinus rhythm (SR) and in ventricular tachycardia (VT). The presence of scars, grey zones and non-remodeled regions of the myocardium is accounted for by the introduction of a spatially heterogeneous coefficient in the 3D electromechanics model. This 3D electromechanics model is firstly coupled with a 2-element Windkessel afterload model to fit the pressure-volume (PV) loop of a patient-s

Impaired respiratory sinus arrhythmia with paradoxically normal Valsalva ratio indicates combined cardiovagal and peripheral adrenergic failure.

Combined reductions of the heart rate responses to deep breathing (HRDB) and the Valsalva ratio (VR) are used as a standard test of cardiovagal function. We observed that some patients had marked reduction of HRDB with a paradoxically normal VR. The mechanism of this paradox was evaluated. We found that these patients had evidence of peripheral adrenergic and postganglionic sudomotor impairment, including an excessive fall in blood pressure on upright tilt. Since VR depends on heart rate responses to blood pressure excursions, we evaluated the phases of the Valsalva manoeuvre and the ensuing h

Echocardiography predictors of sustained sinus rhythm after cardioversion of supraventricular arrhythmia in patients with septic shock.

PURPOSE: The echocardiography parameters may predict the maintenance of sinus rhythm after cardioversion of a supraventricular arrhythmia (SVA). MATERIALS AND METHODS: Patients in septic shock with onset of an SVA, normal to moderately reduced LV systolic function (EF_LV˃̳35%) and on a continuous noradrenaline of <1.0 μg/kg.min were included. Echocardiography was performed at the arrhythmia onset, 1 h and 4 h post cardioversion on an infusion of propafenone or amiodarone. RESULTS: Cardioversion was achieved in 96% of the 209 patients within a median time of 6(1.8-15.6)h, 134(64.1%) patients

Respiratory sinus arrhythmia, negative social interactions, and fluctuations in unmet interpersonal needs: A daily diary study.

INTRODUCTION: This study examined daily fluctuations in the unmet interpersonal needs of thwarted belongingness and perceived burdensomeness in response to daily negative social interactions, as well as the moderating role of respiratory sinus arrhythmia (RSA) across adolescents at lower and higher risk for suicidal ideation. METHODS: Fifty five adolescents with major depressive disorder (MDD, i.e., higher-risk group) and without MDD (i.e., lower-risk group) completed measures of resting RSA, and daily measures of negative social interactions, perceived burdensomeness, and loneliness, as a pr

Resting respiratory sinus arrhythmia moderates the association between social phobia symptoms and self-reported physical symptoms.

The present study sought to investigate the association between social phobia symptoms and self-reported physical symptoms and the moderation effect of resting respiratory sinus arrhythmia (RSA) on this link. Data of 5-min resting RSA, social phobia symptoms assessed by the Social Phobia Scale, and physical symptoms assessed by the Cohen-Hoberman Inventory of Physical Symptoms were collected from 167 undergraduate students. Results indicated that higher levels of social phobia symptoms were associated with higher levels of self-reported physical symptoms. Resting RSA played the moderating role

Respiratory Sinus Arrhythmia Quantified with Linear and Non-Linear Techniques to Classify Dilated and Ischemic Cardiomyopathy.

In congestive heart failure (CHF), dilated cardiomyopathy (DCM) and ischemic cardiomyopathy (ICM) are two highly related pathologies that are not fully characterized. The aim of this study is to assess respiratory sinus arrhythmia (RSA) index of the parasympathetic system, in order to discriminate between both pathologies, DCM and ICM. For this, ECG-signals of 49 subjects (12 DCM patients, 21 ICM patients, 6 ICM patients with diabetes mellitus (DM) type II and 10 control subjects) from the database HERIS II and of 173 subjects (50 DCM, 50 ICM, 15 DCM with DM type II, 15 ICM with DM type II and

Respiratory sinus arrhythmia and auditory processing in autism: modifiable deficits of an integrated social engagement system?

The current study evaluated processes underlying two common symptoms (i.e., state regulation problems and deficits in auditory processing) associated with a diagnosis of autism spectrum disorders. Although these symptoms have been treated in the literature as unrelated, when informed by the Polyvagal Theory, these symptoms may be viewed as the predictable consequences of depressed neural regulation of an integrated social engagement system, in which there is down regulation of neural influences to the heart (i.e., via the vagus) and to the middle ear muscles (i.e., via the facial and trigemina

Daily distribution of atrial arrhythmic episodes in sick sinus syndrome patients: implications for atrial arrhythmia monitoring.

AIMS: Disorders such as paroxysmal atrial fibrillation (AF) and atrial tachyarrhythmias (AT) are difficult to investigate because of their intermittent, and sometimes asymptomatic, nature. The aim of this study was to investigate the daily temporal distribution of AT/AF episodes--onset and occurrence--by analysing data from 250 pacemaker-implanted, brady-tachy syndrome patients who have been enrolled in the Burden II Study. METHODS AND RESULTS: Data were analysed accounting for the mode switch list which includes date, time, and duration of each mode switch episode. Chi-squared tests for good

Ventilator-associated sinus arrhythmia in a preterm neonate--an indicator for a mature autonomic nervous system?

According to control theory, the interactions between respiration and heart rate (i.e. respiratory sinus arrhythmia, RSA, and breath amplitude sinus arrhythmia, BASA) reflect the inner workings of the physiological control systems of respiration and circulation. This paper reports on a preterm neonate (28.5 weeks old, 940 g) who showed the presence of ventilation-associated sinus arrhythmia (VASA) under moderate artificial ventilation. His heart rate variability was entrained to the ventilatory stimulus. VASA and entrainment suggest that the parasympathetic part of the autonomic nervous system

Respiratory sinus arrhythmia as a within-session biomarker in depression psychotherapy: integrating resting RSA and RSA reactivity.

Objective: Enhancing affective regulation is a key focus in psychotherapy for depression, yet within-session markers that capture the dynamics of regulation remain limited. Respiratory Sinus Arrhythmia (RSA), a biomarker of vagal mediation of the parasympathetic nervous system, is well-known for its role in affective regulation. This nonrandomized observational psychotherapy process study examined (1) whether resting RSA can serve as a biomarker for depression severity, and (2) whether RSA reactivity and its interaction with resting RSA can serve as a within-session therapeutic biomarker linke

Reduced respiratory sinus arrhythmia in adults born at extremely low birth weight: evidence of premature parasympathetic decline?

Individuals born at extremely low birth weight (ELBW; <1000 g) are exposed to early adversity in multiple forms. Given that substantial development of the autonomic nervous system (ANS) occurs during the third trimester of gestation, ANS functioning may be altered in adults who were born before reaching 28 weeks of gestational age. The aims of the study were to: (1) determine whether two indices of ANS functioning [resting heart period (HP) and respiratory sinus arrhythmia (RSA)], differed between adult ELBW survivors and normal birth weight (NBW) controls, and (2) ascertain whether ANS functi

Study of pathogenesis of ventricular arrhythmia in experimental rats by separation of sinus and ventricular substitutional rhythms.

Pathogenesis of cardiac arrhythmia was determined by EEC changes after ATP-induced complete atrioventricular block. The re-entry mechanism underlays extrasystoles with equal coupling intervals with complexes of ventricular substitution rhythms, which transformed into paroxysmal tachycardia with equal R-R intervals, ventricular flutter, and ventricular fibrillation. Ectopic automaticity was characterized by extrasystole unrelated to the complexes of substitutional rhythms, which was transformed into accelerated idioventricular rhythm and asystole. During trigger activity, the extrasystoles were

Evidence of a previously undescribed form of respiratory sinus arrhythmia (RSA)--the physiological manifestation of "cardiac aliasing".

Within the fluctuations that occur in heart rate, there is a major component known as respiratory sinus arrhythmia (RSA), which up to now has been defined as heart rate variability which is synchronous with respiratory movements. We have now discovered that RSA synchronous to respiratory movements is only one example of RSA. If a special relationship exists between mean heart rate (fHR) and mean frequency of breathing (fB) such that fB greater than 1/2 fHR, RSA can be observed in a frequency range which is lower than the frequency of breathing. The mathematical fundamentals of this physiologic

Reappraisal and New Observations on Idiopathic Ventricular Arrhythmias Ablated From the Noncoronary Aortic Sinus.

BACKGROUND: The electrophysiological characteristics of idiopathic ventricular arrhythmias (VAs) from the noncoronary sinus (NCS) have not been fully described. OBJECTIVES: This study sought to investigate electrophysiological characteristics and catheter ablation in patients with idiopathic NCS-VA. METHODS: This study comprised 11 patients undergoing radiofrequency (RF) catheter ablation for idiopathic NCS-VA. Angiography was performed to confirm the origin in the aortic sinus before RF ablation. RESULTS: Clinical arrhythmias presented left bundle block/inferior axis morphology in all pati

Longitudinal relation of family dysfunction to young adults' social anxiety: Sex and baseline respiratory sinus arrhythmia matter.

The aim of the study was to examine the longitudinal association between family dysfunction and young adults' social anxiety two years later, and the moderating roles of participant sex and baseline respiratory sinus arrhythmia (Baseline RSA). 109 Chinese college students (M = 18.94 years old, SD = 0.92; 69 males) participated in two waves of data collection two years apart. Participants reported their perceived family dysfunction and social anxiety, and physiological data were collected to capture their baseline RSA. Results indicated family dysfunction, baseline RSA, and participant sex inte

Respiratory sinus arrhythmia magnitude quantification as a potential marker of stress and pain in cows and sheep.

Respiratory sinus arrhythmia (RSA) provides a quantitative evaluation of 'vagal tone' and may be used for pain and stress assessment in livestock. The aim was to quantify the magnitude of RSA in cows and sheep. In 7 cows and 4 sheep standing at rest we measured the 3-lead electrocardiogram (ECG) together with the pneumogram, to identify inspiration and expiration. For each breath, RSA was the difference in instantaneous heart rate (HR) between the inspiratory peak and the expiratory trough, in percent of mean HR. The resting breathing rates (28 ± 2 and 32 ± 5 breaths/min in cows and sheep, res
