Knee Arthroscopy: A Simulation Demonstrating the Imperial Knee Arthroscopy Cognitive Task Analysis (IKACTA) Tool.

BACKGROUND: Virtual reality and cadaveric simulations are expensive and not readily accessible1. Innovative and accessible training adjuncts are required to help meet training needs. Cognitive task analysis (CTA) has been used extensively to train pilots and surgeons in other surgical specialties2-6. However, the use of CTA tools within orthopaedics is in its infancy. Arthroscopic procedures are minimally invasive and require a different skill set compared with open surgery. Residents often feel poorly prepared to perform this in the operating room because of the steep learning curve associate

Modifications to the Hip Arthroscopy Technique When Performing Combined Hip Arthroscopy and Periacetabular Osteotomy.

In the realm of hip preservation, hip arthroscopy is often used to address intra-articular impingement pathology, whereas periacetabular osteotomy (PAO) is used to address dysplasia and instability. Indications to combine these 2 procedures include hip dysplasia and symptomatic instability with a concomitant symptomatic labral tear or the other symptomatic intra-articular pathology (i.e., loose body, chondral flap). The arthroscopic portion of the procedure allows repair of the injured labrum and close inspection of the hip joint, and the PAO addresses undercoverage and/or inappropriate versio

Return to sport after hip arthroscopy: aggregate recommendations from high-volume hip arthroscopy centers.

Hip arthroscopy is a minimally invasive surgical technique often performed in athletes who want an expeditious return to sport. To the authors' knowledge, no studies in the literature provide a time frame or criteria for return to sport after hip arthroscopy. The purpose of this study was to develop an aggregate recommendation for return to sport after hip arthroscopy based on data assimilated from high-volume hip arthroscopy centers. Twenty-seven orthopedic surgeons from high-volume hip arthroscopy centers completed a survey regarding return to sport after hip arthroscopy. The questionnaire a

[Therapeutic possibilities of arthroscopy in chronic painful wrists. Apropos of 27 cases with 55 arthroscopies].

55 arthroscopies of radio- and midcarpal joints of the wrist were performed for chronic wrist pain with a difficult diagnosis and/or treatment. None of the patients suffered a deterioration and no complications were observed. The results were analysed according to Jackson's and Abe's criteria. In the overall series, arthroscopy was considered to be beneficial for the diagnosis in 80% of cases and for treatment in 49% of cases. Wrist arthroscopy was beneficial in 100% of cases for exact evaluation of the lesions of osteoarthritic wrists or wrists with intracarpal disorganisation and guided the

Accessory Portals in Hip Arthroscopy.

Over 18 portals have been described in the literature for arthroscopic access to the central and peripheral compartments of the hip. While basic hip arthroscopy procedures may only warrant 2 or 3 portals, accessory portals may facilitate improved access, facile instrumentation, safe anchor placement, and suture management for a variety of procedures ranging from basic cam resection and labral repair to more advanced complex procedures such as arthroscopic labral and capsular reconstruction. This article defines common accessory portals and deconvolutes historical nomenclature and further descr

[Hip arthroscopy for painful endoprosthesis].

OBJECTIVE: Hip arthroscopy for a painful endoprosthesis is performed to confirm and evaluate or detect pathologies (sampling for microbiology/histology, function), which are treated during the procedure or can indicate treatment options for the further course of the procedure. INDICATIONS: Hip arthroscopy is indicated for the diagnostics of unclear persistent pain after implantation of a hip endoprosthesis (low-grade infection, metal reaction/metallosis, loosening/misplacement of prosthesis components) and for treatment (iliopsoas impingement, removal of biomechanically disruptive osteophytes

Regional Anesthesia for Hip Arthroscopy.

Pain after hip arthroscopy can be severe, yet we lack a consensus method for non-narcotic analgesia. Here we describe anatomic elements of hip arthroscopy and our current understanding of the relevant sensory innervation as a prelude to the evaluation of locoregional analgesic techniques. Many regional nerve blocks and local anesthetic infiltration techniques are reviewed, including 2 newer ultrasound fascial plane blocks. Further study of targeted, motor-sparing approaches, either ultrasound-guided or under direct surgical visualization is needed.

Heterotopic Ossification in Hip Arthroscopy.

Introduction  Hip arthroscopy is a rising surgical technique due to the increase in hip diseases, especially femoroacetabular impingement. One of the several complications related to such procedures is heterotopic ossifications (HO). The aim of this study is to describe the prevalence of HO after hip arthroscopy in a series of patients with femoroacetabular impingement and to compare its preoperative and intraoperative variables with a matched control group of patients without HO. Methods  All patients who underwent hip arthroscopy for femoroacetabular impingement between 2010 and 2017 with a

Elbow arthroscopy - Indications and technique.

Throughout the years, elbow arthroscopy has advanced tremendously due to improvements in technology and surgical techniques. It is now considered a safe and effective treatment for a variety of elbow disorders. Due to the small working space and nearby neurovascular structures, it is a technically challenging procedure. It can be used to successfully treat complaints caused by loose bodies, osteoarthritis, arthrofibrosis, OCD, lateral epicondylitis, VEOS and fractures. The most devastating complication of elbow arthroscopy is (permanent) nerve injury. Therefore, distortion of the anatomy of th

Hip Arthroscopy: extra-articular Procedures.

Hip preservation is one of the fastest growing fields in orthopaedics and indications of intra-articular procedures are well established. In the last decade, extra-articular procedures have gained momentum and arthroscopic solutions to peri-articular hip pathologies have been offered. It should be noted that many of these pathologies are well-treated conservatively and only those who fail conservative management should be treated operatively. These indications can be divided into 5 categories: greater trochanteric pain syndrome; internal hip snapping; anterior inferior iliac spine/sub-spine im

Hip arthroscopy for femoroacetabular impingement.

The purpose of this article is to give a general overview of femoroacetabular impingement (FAI) and how it could be treated arthroscopically, with some details about indications, the procedure itself and some of the complications associated with the surgery.FAI is a dynamic condition of the hip that can be a source of pain and disability and could potentially lead to arthritis. When symptomatic, and if conservative treatment fails, FAI can be addressed surgically. The goal of surgical treatment for FAI is to recreate the spherical contour of the femoral head, improve femoral offset, normalize co

Hip Arthroscopy for Femoroacetabular Impingement.

BACKGROUND: The first recorded attempt at arthroscopic visualization of the hip can be attributed to Dr. Michael Burman in 1931. Since then, hip arthroscopy has become widely used for the management of femoroacetabular impingement (FAI) because of its clear benefits, including a low complication rate and limited patient morbidity as compared with traditional open approaches. DESCRIPTION: Arthroscopic management of FAI begins with arthroscopy of the central compartment, where the intra-articular damage is identified. Standard portal placement provides optimal access for surveying and accessing

Wrist Arthroscopy for Athletic Injuries.

Management of hand and wrist injuries for athletes often places emphasis on an expeditious return to sport. Arthroscopic techniques have the advantage of directly visualizing joint derangements and correcting them via a minimally invasive approach. This article discusses the evaluation and management of common wrist injuries treated with arthroscopy in athletes, including scapholunate and lunotriquetral injury, triangular fibrocartilage complex tears, hamatolunate impingement, and arthroscopic-assisted reduction of wrist fractures.

Hip arthroscopy: less invasive technique.

The popularity of hip arthroscopy has increased significantly over the past decade. It is technically demanding and may be associated with complications such as iatrogenic damage to the articular cartilage or acetabular labrum, compression injuries to the structures in the perineum, and distraction injuries to the leg. Some of these complications can be avoided by paying meticulous attention to the technique and reducing the traction/surgical time. We describe a technique by which the risk of injury to the articular cartilage may be minimized by introduction of the surgical instruments under d

Pulmonary embolism after shoulder arthroscopy.

Deep venous thrombosis and pulmonary embolism events are common complications following lower-extremity orthopedic surgery. Conversely, deep venous thrombosis and pulmonary embolism events are rare following upper-extremity surgery, specifically shoulder arthroscopy. The purpose of this article is to emphasize the necessity of performing a thorough preoperative workup to uncover possible risk factors for deep venous thrombosis/pulmonary embolism despite the rare occurrence of developing a pulmonary embolism following shoulder arthroscopy. This article describes 3 patients who developed a nonfa

Ankle fracture following hip arthroscopy.

This article describes a case of an ankle fracture following hip arthroscopy. A 58-year-old woman underwent hip arthroscopy for a labral tear. She was placed in a lateral decubitus position with her foot in a padded boot. Traction was maintained for approximately 30 minutes. She was instructed to bear weight as tolerated with crutches postoperatively. At 2-week follow-up, she reported ipsilateral ankle pain. Radiographs revealed a minimally displaced medial malleolus fracture. She was treated with a cast followed by a cam walker boot and successfully went on to complete union and resolution of

MRI-arthroscopy correlation: the knee.

MRI findings can often be helpful in diagnosing and managing common knee injuries. The association between the normal and pathologic changes in the surgical anatomy of the knee commonly found on MRI and those found at arthroscopy can be demonstrated by examining a series of cases that correlate MRI and arthroscopy findings. Regular interactions are recommended between surgeons and musculoskeletal radiologists to improve MRI interpretation and achieve better surgical outcomes for patients.

Dry arthroscopy and its applications.

Wrist arthroscopy can be performed without infusing fluid, as simple traction suffices to maintain the working space. The lack of tissue infiltration by fluid keeps soft tissues in pristine condition if open surgery is needed after the arthroscopic exploration. The dry technique makes fluid distension irrelevant, thus opening new possibilities by combining arthroscopy with moderate-sized incisions. Although any modification of a technique with which one is familiar can be regarded with major reticence, the advantages of the dry technique merit its use. Accomplished wrist arthroscopists will ha
