Rotational atherectomy to an angulated calcified lesion is always challenging. The risk of catastrophic complications such as a burr becoming stuck or vessel perforation is greater when the calcified lesion is angulated. We describe the case of an 83-year-old female suffering from unstable angina. Diagnostic coronary angiography revealed an angulated calcified lesion in the proximal segment of the right coronary artery. We performed rotational atherectomy to the lesion, but intentionally did not advance the rotational atherectomy burr beyond the top of the angulation. We controlled the rotatio

Stent underexpansion is a well known risk factor for stent thrombosis. We report a case of acute stent thrombosis which occurred after primary percutaneous coronary intervention to the right coronary artery for an inferior ST-elevation myocardial infarction. Minutes after completion of the procedure, the patient had acute stent thrombosis, manifested by new chest pain and new ST-elevation. IVUS showed the stent to be underexpanded. Despite high pressure balloon inflation, the stent remained underexpanded. This was then managed with rotational atherectomy within the underexpanded stent, followe

OBJECTIVE: We analyzed our results with percutaneous rotational atherectomy catheters and specifically examined whether they were more likely to be associated with a successful outcome when used to treat smaller diameter vessels such as infrapopliteal (IP) arteries compared to larger diameter femoropopliteal (FP) arteries and infrainguinal arterial autogenous vein grafts (GRAFTS).
MATERIAL AND METHODS: Between January 1, 2005, and December 31, 2006, athrectomies were performed on 32 patients for claudication (14), gangrene (9), rest pain (4), and failing GRAFTS (5). Treated vessels included 1

BACKGROUND: Over 300,000 coronary artery bypass operations are performed annually in the U.S., in which saphenous vein grafts remain the most often utilized conduit for myocardial revascularization. Still, the relatively short life-span of reverse saphenous vein grafts (SVG) demands that revascularization techniques be developed for SVG occlusions. Extraction atherectomy (EA) as a pre-treatment to percutaneous transluminal coronary angioplasty (PTCA) may offer advantages to PTCA alone in the treatment of these lesions. While a randomized study would best define the potential advantages of this

Orbital atherectomy (OA) is a highly effective atherectomy device used to treat heavily calcified coronary arteries. The technique for using OA is critical and depends on appropriate use of the dedicated guidewire. OA employs a centrifugal, differential sanding mechanism with bidirectional movement. When used with proper technique, the device appears to be associated with a low rate of complications, such as bradycardia and slow flow, compared with rotational atherectomy (RA), and results in high procedural success rates. We describe our experience with the OA device and procedural techniques

BACKGROUND AND AIMS: Endovascular recanalizaiton (ER) has been proven to be a feasible method for Thromboangiitis Obliterans (TAO). The aims of this study were to evaluate the effectiveness and safety of atherectomy for TAO compared to nonatherectomy ER in our center.
METHODS: Patients diagnosed as TAO were reviewed from January 2016 to June 2021 in our center. Basic characteristics of patients before ER and perioperative data were collected and compared between the atherectomy and nonatherectomy groups. The vascular event-free survival and limb salvage were calculated to evaluate the prognos

BACKGROUND: The use of atherectomy for lower extremity revascularization is increasing despite concerning reports about its long-term safety and effectiveness. This study compares the outcomes of atherectomy to percutaneous transluminal angioplasty (PTA) and stenting for treatment of isolated femoropopliteal disease.
METHODS: All patients undergoing endovascular treatment of isolated femoropopliteal lesions in the Vascular Quality Initiative (2009-2018) were identified. Patients with concomitant open surgery, acute limb ischemia, or iliac or tibial intervention were excluded. Patients were di

BACKGROUND: There is paucity of data regarding the temporal trends and outcomes of coronary atherectomy in the United States.
METHODS: We queried the National Inpatient Sample database (2011-2016) for hospitalizations of patients undergoing coronary atherectomy procedures. We also compared outcomes of non-orbital vs orbital coronary atherectomy in a more contemporary cohort.
RESULTS: Our analysis included 2,990,223 hospitalizations with PCI, of which 114,462 (3.8%) included an atherectomy procedure. A significant increase in coronary atherectomy procedures was observed over time (0.66% in 20

BACKGROUND: Infrapopliteal (IP) lesions are common in patients with critical limb ischemia (CLI). Optimal revascularization strategies including the use of adjunctive atherectomy have the potential to improve the outcomes for these patients.
OBJECTIVE: To compare laser atherectomy (LA) vs. balloon angioplasty alone for the treatment of IP lesions in patients with CLI.
METHODS: This was a two-center retrospective study of patients with CLI who underwent endovascular interventions for IP lesions. One and 2-year target lesion revascularization (TLR) was the primary outcome. One and 2-year limb

BACKGROUND: The incidence of lower extremity peripheral artery disease (LE-PAD) continues to increase and associated morbidity remains high. Despite the significant development of percutaneous revascularization strategies, over the past decade, LE-PAD still represents a unique challenge for interventional cardiologists and vascular surgeons.
METHOD: Typical features of atherosclerosis that affects peripheral vascular bed (diffuse nature, poor distal runoff, critical limb ischemia, chronic total occlusion) contribute to the disappointing results of traditional percutaneous transluminal angiopl

BACKGROUND: Interleukin-8 (IL-8), a CXC chemokine that induces the migration and proliferation of endothelial cells and smooth muscle cells, is a potent angiogenic factor that may play a role in atherosclerosis. Previously, IL-8 has been reported in atherosclerotic lesions and circulating macrophages from patients with atherosclerosis. Therefore, we sought to determine whether IL-8 plays a role in mediating angiogenic activity in atherosclerosis.
METHODS AND RESULTS: Homogenates from 16 patients undergoing directional coronary atherectomy (DCA) and control samples from the internal mammary ar

Although percutaneous transluminal coronary angioplasty (PTCA) has been an effective treatment for primary reperfusion in acute myocardial infarction, patients with thrombolytic ineligibility, thrombolytic failure, cardiogenic shock, and vein graft occlusion remain at high risk for complications with PTCA treatment. The transluminal extraction catheter may be useful for treatment for such patients owing to its ability to aspirate thrombus. At 2 clinical centers, extraction atherectomy was prospectively evaluated in 100 patients (age 62 +/- 10 years). High-risk features included thrombolytic fa

BACKGROUND: Post-percutaneous coronary intervention (PCI) abnormalities in the index of microcirculatory resistance (IMR) and coronary flow reserve (CFR) reflect microvascular dysfunction and are associated with adverse clinical outcomes. Atherectomy devices (ADs) are commonly employed to optimize stent delivery and expansion in heavily calcified lesions; however, their impact on post-PCI microcirculatory indices remains uncertain. This study aimed to evaluate post-procedural IMR and CFR values in patients undergoing PCI with and without the use of ADs using the bolus thermodilution method.

PURPOSE: Orbital atherectomy (OA) is used to treat heavily calcified peripheral arterial lesions, aiming to improve vessel compliance and reduce the need for high-pressure angioplasty and stenting. However, clinical data raise concerns regarding distal embolization, vessel injury, and uncertain long-term benefit. This study aimed to evaluate biomechanical and embolic effects of OA in calcified human femoropopliteal arteries (FPAs).
METHODS: OA was studied in 10 human FPAs with advanced atherosclerosis and calcification. A pulsatile flow loop approximated FPA hemodynamics. Sequential OA and ba
