Inflammatory atlanto-axial subluxation (Grisel's syndrome) in children: clinical diagnosis and management.

BACKGROUND: Grisel's syndrome is a uni- or bilateral subluxation of the atlas on the axis associated with an inflammatory condition of the head or neck. This uncommon entity usually affects children. The condition seems to be secondary to hyperemia and laxity of the atlanto-axial joints. PATIENTS AND METHODS: We report clinical and neuroimaging findings, treatment modalities, and outcomes in 4 children with atlanto-axial rotatory subluxation (AARS) associated with a previous upper respiratory tract infection treated at our Unit during the last 7 years. RESULTS: All children presented with ne

An unusual seat-belt injury in a doctor.

The personal experience of a general practitioner who sustained a fracture-dislocation of the atlanto-axial joint is recorded. The inefficient application of an unyielding (non-inertial) lap and diagonal seat belt permitted this injury, although one does not know what other injuries might have occurred had the belt not been worn.

Occipitoatlantoaxial malformation with duplication of the atlas and axis in a half Arabian foal.

An unusual occipitoatlantoaxial malformation is described in a 2-week-old male part Arabian foal that was unable to stand at birth and showed signs of spastic tetraparesis due to a cervical spinal cord compression. There were 2 atlases present. One was fused to the occipital bones. The other articulated with the first atlas and an axis which had a long dens that projected into the vertebral canal. Examination of the ossification centers of the axis indicated partial duplication of that bone.

Combined occipitoatlantoaxial hypermobility with anterior and posterior arch defects of the atlas in Pierre-Robin syndrome.

Combined occipitoatlantoaxial instability of the cervical spine occurred in a 8-year-3-month old boy with Pierre-Robin syndrome. He also had failure of ossification of both the anterior and posterior arches of the atlas. An in situ fusion from the occiput to C2 restored cervical spine stability. This case is discussed in relation to other hypermobility and instability syndromes of the cervical spine.

Traumatic lesions of the occipitoatlantoaxial complex.

Injuries of the occipitoatlantoaxial complex are relatively rare but form a group of ligamentous and bony lesions that may unnecessarily confuse the clinician. Since fractures of the dens and traumatic spondylolisthesis of the axis have been well described in a number of large series, this article concentrates on the lesions less frequently seen and described. The ligamentous injuries are occipitoatlantal dislocation, rupture of the transverse ligament, and atlantoaxial rotatory fixation. The bony injuries are fractures of the occipital condyles, atlas, and the lateral masses of C2. The incide

Specific entities affecting the craniocervical region: Down's syndrome.

INTRODUCTION: The ligamentous laxities and bony abnormalities associated with Down's syndrome, os odontoideum, achondroplasia, osteogenesis imperfecta, and basilar invagination have been discussed with the imaging of the craniocervical junction. These are significant and require suspicion when dealing with children with the previously mentioned entities. CONCLUSION: Previous adverse surgical results in managing these patients reflect the lack of understanding of the underlying pathology and the appropriate surgical treatment.

Fusions at the craniovertebral junction.

INTRODUCTION: The surgical management of craniovertebral junction instability in pediatric patients has unique challenges. While the indications for internal fixation in children are similar to those of adults, the data concerning techniques, complications, and outcomes of spinal instrumentation comes from experience with adult patients. Diminutive osseous and ligamentous structures and anatomical variations associated with syndromic craniovertebral abnormalities frequently complicates the approaches and limits the use of internal fixation in children. Cervical arthrodesis in the pediatric age

The management and treatment of cervical spine injuries.

The cervical spine is a relatively fragile structure traversed by the spinal cord. Injuries to the cervical spine (c-spine) are therefore associated with potentially catastrophic neurological complications. This article illustrates the importance of routine c-spine immobilization in all trauma patients and highlights how early diagnosis of injuries can optimize outcome.

Atlantoaxial subluxation secondary to retropharyngeal abcess in adults (Grisel's syndrome). Review of literature about a clinical case.

Grisel syndrome (GS) is a non-traumatic atlanto-axial subluxation produced after an inflammatory, infectious or postsurgical process of the upper neck region. It is a characteristic, although currently uncommon complication of acute bacterial pharyngitis in the pediatric age. Presentation in adults is even more infrequent and requires specific management. We present the case of a 68-year-old patient with a history of chronic myeloid leukemia, who presented with infectious polyarthritis, cervicalgia, torticollis and cervical impotence. Cervical radiography, CT, and MRI showed a C1-C2 subluxatio

Bilateral wrist dislocation in trisomy 21: a case report.

Ligamentous laxity is common in Trisomy 21 (Down's syndrome). The literature has numerous reports of atlanto-axial instability, patellar instability, carpal instability and other manifestations of joint instability. To date, no report has been published in the English literature describing non-traumatic unilateral or bilateral wrist dislocation either in trisomy 21 or in any other condition.

[Hot topic of cranial vertebral surgery: the surgical plan of type A basilar invagination].

Regarding the treatment of type A basilar invagination, there is a debate between the combination of anterior and posterior surgery and posterior fixation. The author's personal opinion on the hot topic of cranial vertebral surgery: the surgical plan of type A basilar invagination is expressed in this article. Overall, the combined anterior-posterior approach is applicable in almost all cases of type A basilar invagination, regardless of how severe atlantoaxial dislocation, and simple posterior surgery is only in cases where atlantoaxial dislocation is not very severe. The anterior-posterior c

Retro-Odontoid Pseudotumor without Radiologic Atlantoaxial Instability: A Systematic Review.

BACKGROUND: Retro-odontoid pseudotumor (RP) can be caused by several diseases, especially rheumatoid arthritis, and is usually associated with the presence of atlantoaxial instability. On the other hand, a different group of patients have been identified in whom RP is observed without radiologic findings of atlantoaxial instability. The pathophysiology, clinical characteristics, and prognosis of this latter group of patients are not well described in the literature. METHODS: A PubMed and Scopus search adhering to PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guid

Thalamic syndrome as the heralding manifestation of atlantoaxial dislocation.

In India, Atlantoaxial dislocation (AAD) is the commonest skeletal craniovertebral junction (CVJ) anomaly, followed by occipitalisation of atlas and basilar invagination. The usual presentation is progressive neurological deficit (76-95% cases) involving the high cervical cord, lower brainstem and cranial nerves. The association between vertebro-basilar insufficiency and skeletal CVJ anomalies is well recognised and angiographic abnormalities of the vertebrobasilar arteries and their branches have been reported; however, initial presentation of CVJ anomaly as thalamic syndrome due to posterior

Anterior atlanto-occipital transarticular screw fixation: a biomechanical comparison with posterior fixation techniques.

OBJECTIVE: Atlanto-occipital instability is commonly treated with posterior fixation. However, in patients with congenital or acquired factors, posterior fixation may not be possible. For these situations, a novel anterior atlanto-occipital transarticular screw (AATS) fixation technique has been introduced recently. However, biomechanical study of this technique is lacking. This study was designed to evaluate the biomechanical stability of AATS fixation for the atlanto-occipital joint and compare it with conventional posterior occipitocervical fixation (POCF). METHODS: Six cadaveric specimens

[History, current status and future of the treatment for irreducible atlantoaxial dislocation].

Irreducible atlantoaxial dislocation (IAAD) refers to a pathological condition characterized by instability and difficulty in achieving reduction through traction, resulting from abnormalities in the bony or soft tissue structures around the atlantoaxial joint. Previous treatments primarily focused on palliative decompression, which often led to suboptimal outcomes. As the understanding of its pathology has evolved, reduction and fixation have emerged as core principles: adequate decompression is achieved through sufficient soft tissue release to enable reduction, followed by fixation and fusi
