Novel Approaches for the Diagnosis of Concealed Nodo-Ventricular and His-Ventricular Pathways.

BACKGROUND: Confirming the presence and participation of concealed nodo-ventricular (cNV) or concealed His-ventricular (cHV) pathways in tachyarrhythmias is challenging. We describe novel observations to aid in diagnosing cNV or cHV pathways. METHODS: We present 7 cases of cNV and cHV pathway-mediated arrhythmias and focus on several laboratory observations: (1) differential ventricular overdrive pacing (VOD) from the base versus apex, (2) response to His refractory premature ventricular complexes, (3) paradoxical atriohisian response (shorter atriohisian interval during tachycardia than that

Hypoxia and cardiac arrhythmias in breath-hold divers during voluntary immersed breath-holds.

The incidence and nature of cardiac arrhythmias during static apnea were studied by monitoring the electrocardiogram (ECG) and oxygen saturation (SaO(2)) of 16 recreational breath-hold divers. All subjects completed a maximal apnea with a mean (+/-SD) breath-hold duration of 281 (+/-73) s without clinical complications. Both heart rate (HR) and SaO(2) decreased significantly with breath-hold duration. The decline in SaO(2) was inversely related to the decline in HR (r = -0.55, P < 0.05). Cardiac arrhythmias (supraventricular and ventricular premature complexes, right bundle branch block) occur

Outpatient cardiac telemetry detects a high rate of atrial fibrillation in cryptogenic stroke.

BACKGROUND: The etiology of cerebral ischemia is undetermined in one-third of patients upon discharge. Occult paroxysmal atrial fibrillation (PAF) is considered a potential etiology. A high rate of PAF detection with 21-day mobile cardiac outpatient telemetry (MCOT) has been reported in two small studies. Optimal monitoring duration and factors predicting PAF have not been adequately defined. METHODS: We performed a retrospective analysis on patients evaluated by MCOT monitoring within 6 months of a cryptogenic stroke or TIA. Multivariate analysis with survival regression methods was performe

QRS subtraction and the ECG analysis of atrial ectopics.

The recognition of the ubiquitous role of short coupled ectopics in the initiation of paroxysmal atrial fibrillation has renewed interest in their localization with the ultimate aim of facilitating their neutralization by catheter ablation. The P-waves resulting from such ectopics are partly or completely concealed by the QRST of the preceding beat and therefore their morphology has been underutilized for localization purposes. Subtraction has been the most commonly used technique for QRST suppression and though an averaged template offers a higher signal-to-noise ratio, the immediately preced

Bundle branch reentry: a possible mechanism of ventricular tachycardia.

Electrophysiologic studies were performed in three patients suffering from attacks of paroxysmal tachycardia with wide QRS complexes. Two patients had atrioventricular dissociation. The arrhythmia could be initiated and terminated by premature ventricular stimulation in all three patients. One patient developed the arrhythmia after rapid atrial stimulation. In each subject, the QRS complexes during tachycardia were identical to recorded supraventricular beats (left bundle branch block pattern in two cases and right bundle branch block pattern in one). A His bundle potential was noted before th

Association of Atrial Electrophysiological Abnormalities With Cognitive Decline and Cerebrovascular Disease.

BACKGROUND: Atrial electrophysiological abnormalities (AEA) are associated with cognitive dysfunction. We evaluated the associations of AEA with longitudinal cognitive decline and incident dementia and investigated underlying mechanisms. METHODS: In subjects without atrial fibrillation followed prospectively for 5 years, 12-lead ECGs were evaluated for AEA, defined as the presence of sinus node dysfunction (SND), frequent premature atrial complexes, advanced interatrial block (a-IAB), or P-terminal force in V1 (>40 mm*ms). Rate of decline in global cognition (Z-score averaged from 6 cognitive

Automated Detection of Normal, Atrial, and Ventricular Premature Beats from Single-Lead ECG Using Convolutional Neural Networks.

Accurate detection of premature atrial contractions (PACs) and premature ventricular contractions (PVCs) in single-lead electrocardiograms (ECGs) is crucial for early identification of patients at risk for atrial fibrillation, cardiomyopathy, and other adverse outcomes. In this work, we present a fully convolutional one-dimensional U-Net that reframes beat classification as a segmentation task and directly detects normal beats, PACs, and PVCs from raw ECG signals. The architecture employs a ConvNeXt V2 encoder with simple decoder blocks and does not rely on explicit R-peak detection, handcraft

Fetal atrial tachycardia diagnosed by magnetocardiography and direct fetal electrocardiography. A case report of treatment with propranolol hydrochloride.

At 26 weeks of gestation, fetal tachyarrhythmias (about 250 bpm) and ascites were detected by ultrasonography, and oral treatment with propranolol (30 mg/day) was commenced. Within 10 h, the fetal heart rate changed to approximately 85 bpm. The averaged fetal magnetocardiogram triggered by R peaks showed P wave and QRS complexes and an extra P wave. In addition, many extra nonconducted P-waves were detected in a fetal direct electrocardiogram. At 27 weeks of gestation, fetal tachycardia occurred again, and arrhythmia was diagnosed as the result of a blocked premature atrial contraction (PAC) w

Challenging the Limitations of Atrial Fibrillation Detection in the Presence of Other Cardiac Arrythmias.

Atrial fibrillation (AF) affects millions of people worldwide and needs to be diagnosed in its early stage to provide proper treatment. However, the numerous wearable devices available today are not yet able to discriminate AF episodes from other cardiac arrhythmias and merely detect normal vs abnormal rhythms. In this study we investigated the performance of a traditional classifier - designed to distinguish AF and sinus rhythm (SR) using inter-beat intervals (IBI) - when confronted with other - non-AF - arrhythmias. This classifier was challenged with data of 37 patients wearing an optical he

Predictors of non-pulmonary vein ectopic beats initiating paroxysmal atrial fibrillation: implication for catheter ablation.

OBJECTIVES: The purpose of this study was to investigate the predictor of non-pulmonary vein (PV) ectopic beats initiating paroxysmal atrial fibrillation (PAF). BACKGROUND: Non-PV ectopic beats can initiate PAF in some patients and play an important role in the recurrence of PAF after PV isolation. Information on the predictors of non-PV ectopic beats initiating PAF is unknown. METHODS: This study included 293 patients (215 men and 78 women, age 60 +/- 14 years) with clinically documented drug-refractory PAF. Of the 94 patients with non-PV ectopic beats initiating PAF, 38 (40%) patients had

Supraventricular arrhythmias following breath-hold submersions in cold water.

Twelve subjects undertook one submersion into water at 5 degrees C and two at 10 degrees C wearing either a wet or dry suit. During the submersions the subjects held their breath for as long as they could and then breathed through respiratory tubing for a further 10 s before being removed from the water. Bradycardia (heart rate < 60 beats/min) was observed during breath holding in 10 subjects in 28 of the 36 submersions. Ectopic arrhythmias were observed in 11 subjects in 29 of the 36 submersions, a much higher frequency than previously reported. These ectopic arrhythmias included premature at

Frequent premature atrial contractions as a signalling marker of atrial cardiomyopathy, incident atrial fibrillation, and stroke.

Premature atrial contractions are a common cardiac phenomenon. Although previously considered a benign electrocardiographic finding, they have now been associated with a higher risk of incident atrial fibrillation (AF) and other adverse outcomes such as stroke and all-cause mortality. Since premature atrial contractions can be associated with these adverse clinical outcomes independently of AF occurrence, different explanations have being proposed. The concept of atrial cardiomyopathy, where AF would be an epiphenomenon outside the causal pathway between premature atrial contractions and strok
