[Objective frequency-specific measurement of hearing threshold using narrow-band chirp stimuli with level-adaptive simultaneous masking].

BACKGROUND: In the past, various simulation and measurement paradigms have been introduced and evaluated in order to improve frequency-specific measurement of the hearing threshold using early auditory evoked potentials (EAEP). A promising approach for improvement of detection of stimulus response is the usage of frequency-modulated chirp signals, which optimize the temporal synchrony of neuronal responses along a region of the basilar membrane. AIM OF THE STUDY: This study validated the performance of three generated narrow-band chirp stimuli in combination with a level-adaptive simultaneous

[Study on the bilaterally recorded ABR in the experimental model of acoustic tumor].

The ABRs were bilaterally recorded with monaural stimulation from the animal model of acoustic tumor in 10 rats. It was found that the waveform of ABRs recorded from the right dura was different from the left one. A major feature of right ABRs was that the amplitudes of wave II and wave III were decreased by 3.53 +/- 1.21 microV and 3.94 +/- 1.16 microV respectively, and that the latencies of wave IV and wave V were increased by 0.88 +/- 0.11ms and 1.37 +/- 0.10ms respectively. It suggested that the cochlear nerve near the cochlear nucleus was pressured. A major feature of left ABRs was that t

Auditory brain stem evoked responses to bone-conducted signals.

Auditory brain stem evoked responses to air-conducted and bone-conducted signals were recorded in subjects with normal hearing and in subjects with conductive hearing loss. In normal subjects, the latency to wave V for bone-conducted signals was approximately 0.5 ms longer than the latency for air-conducted signals delivered at the same sensation level. In conductive hearing loss, the separation of the latency-intensity functions for air conduction and bone conduction (corrected for the 0.5-ms delay) provided a valid estimate of the behavioral air-bone gap in the 1,000- to 4,000-Hz region.

Vestibular-evoked myogenic potentials in chronic noise-induced hearing loss.

OBJECTIVE: To investigate the effect of chronic noise exposure on vestibular-evoked myogenic potentials. STUDY DESIGN: Prospective study. SUBJECTS AND METHODS: Twenty patients with chronic noise-induced hearing loss, presenting as bilateral notched audiogram at 4 kHz, underwent audiometry, caloric, and vestibular-evoked myogenic potential tests. RESULTS: Caloric and vestibular-evoked myogenic potential tests revealed abnormal responses in nine (45%) and 10 (50%) patients, respectively. However, when both results were considered together, the abnormal rate reached 70% (14 of 20). The hearing

Evaluation of Speed and Accuracy of Next-Generation Auditory Steady State Response and Auditory Brainstem Response Audiometry in Children With Normal Hearing and Hearing Loss.

OBJECTIVES: The first objective of this study was to compare the predicted audiometric thresholds obtained by auditory steady state response (ASSR) and auditory brainstem response (ABR) in infants and toddlers when both techniques use optimal stimuli and detection algorithms. This information will aid in determining the basis for large discrepancies in ABR and ASSR measures found in past studies. The hypothesis was that advancements in ASSR response detection would improve (lower) thresholds and decrease discrepancies between the thresholds produced by the two techniques. The second objective

Travelling wave velocity test and Ménière's disease revisited.

Transtympanic Electrocochleography (TT-ECoG) and the glycerol test can aid in the diagnostic process of Ménière's disease (MD). Measurement of travelling wave velocity (TWV) has been proposed as an alternative to TT-ECoG to detect endolymphatic hydrops. We assessed the feasibility and obtained normative data of the TWV test in the diagnosis of MD, and compared the test results in MD patients with their symptoms and their TT-ECoG results. The TWV test records two runs of auditory brainstem responses to clicks that are ipsilaterally masked with two different high pass filtered noise bands. The r

Dehydration testing and the diagnosis of Menière's disease: case report.

A 34-year-old female presented to our clinic with complaints of hearing loss, tinnitus, and severe rotational vertigo. A test battery including complete audiometric testing, auditory brainstem response (ABR) audiometry, electronystagmography (ENG), electrocochleography (ECoG), and dehydration testing was performed. Monitoring of hearing sensitivity, speech understanding, and ECoG responses following administration of a dehydrating agent were sensitive measures of cochlear function and confirmed the diagnosis of Meniere's disease. The induced changes also contributed to the therapeutic measures

[Medium-latency acoustically evoked brain potentials used for examination of the auditory pathway (author's transl)].

Potentials of the 10-15 ms latency range evoked by acoustic clicks and Gauss-shaped tone bursts habe been investigated in normal hearing adults, 20 patients with cochlear damages, and 6 cases of temporal lobe processes. Methods and results are compared to those of brain stem audiometry. Mean and standard latency ranges are calculated for the different peaks (Fig. 1). Amplitudes should be used only for side difference evaluation in the same patient, because of their big interindividual variation. In cases of profound high frequency hearing loss (Fig. 2) medium-latency potentials yield true thre

Hearing immaturity found by ABR and its clinical impact on otoneurological evaluation.

Auditory Brainstem Response (ABR) is the most reliable and most frequently used procedure to evaluate audiological conditions in early infancy. However, several reports have demonstrated that developmental change in the central nervous system may affect the results of ABR in audiological evaluations. We examined statistically the reliability of ABR for the diagnosis of profound deafness in early childhood according to our experience over the past 12 years of follow-up in our facility. Subjects included 371 children among 1,041 children who were admitted to Kanariya-Gakuen (institute for pre-sc

Diagnostic testing for endolymphatic hydrops.

The development of a sensitive and specific diagnostic test for endolymphatic hydrops has eluded investigators for over 30 years. The absence of such a test has hampered basic and clinical research into Meniere's disease and related entities. Presently used tests are limited in their applicability because of their low sensitivity and specificity and a poor understanding of the underlying physiologic principles. Despite this, it is generally agreed that some of these studies have merit in selected situations. This article reviews the present state of diagnostic testing for endolymphatic hydrops

Electrocochleographic findings: Menière's disease versus sudden sensorineural hearing loss.

Electrocochleography was conducted in 185 patients with Menière's disease and 117 with sudden deafness. Electrophysiological findings from the two groups were compared in order to identify pathogenic similarities between the two conditions. Parameters investigated were the summating potential (SP)/action potential (AP) ratio, audiometric profiles and audiometric changes after medical therapy. A high SP/AP ratio was found in the patients with sudden deafness, thus supporting a possible hydropic aetiology in this group. In conclusion, electrocochleography can be considered a useful technique as

Comparison between electrocochleography and glycerol test in the diagnosis of Meniere's disease.

Electrocochleography (ECoG) and glycerol test were performed in 51 Meniere ears. The positive rate of both tests was compared. The positive rate of ECoG and glycerol test was 63% and 51%, respectively. The ears with positive result of both tests and of either test were 15 of 51 ears (29%) and 43 of 51 ears (84%), respectively. The positive rate of ECoG was higher in ears with a moderate to severe hearing loss at high frequencies, while the positive rate of glycerol test was higher in ears with a moderate to severe hearing loss at low frequencies. The present study has demonstrated that ECoG is
