Electrically Evoked Auditory Brainstem Responses in Children Fitted with Hearing Aids Prior to Cochlear Implantation.

This study investigates the effect of hearing aid use on the peripheral auditory pathways in children with sensorineural hearing loss prior to cochlear implantation, as revealed by the electrically evoked auditory brainstem response (EABR). Forty children with hearing aids were recruited. Half of them had normal inner ear structures and the other half had inner ear malformations (IEMs). The EABR was evoked by electrically stimulating the round window niche (RWN) and round window membrane (RWM) during the cochlear implantation operation. The onset age of hearing aid use was significantly correl

An integrity test battery for the Nucleus Mini 22 Cochlear Implant System.

The probability of system failures increases as the number of cochlear implants increases throughout the world. Whether a malfunction is a technical or physiological problem remains to be defined, particularly in very young children, while a psychogenic hearing disorder after implantation must not be excluded in adults. The battery of objective measurements used clinically at the Medizinische Hochschule, Hannover has provided useful diagnostic information for distinguishing possible causes of failure. In a normally functioning device, an electrical signal equivalent to the biphasic rectangular

Advances in Management of Pediatric Sensorineural Hearing Loss.

The work-up and management of sensorineural hearing loss in children has been an area of rapid evolution. With the availability of genetic and cytomegalovirus testing, the diagnostic process is continuously refined. Aural rehabilitation should be provided to children in a timely manner. At present, the main surgical options for the treatment of sensorineural hearing loss are bone conduction sound processors and cochlear implants. Investigations into modalities such as auditory brainstem implants are ongoing. With further technological and medical advancements, the evaluation and management of

Auditory brainstem implant outcomes and MAP parameters: report of experiences in adults and children.

UNLABELLED: The auditory brainstem implant (ABI) was first developed to help neurofibromatosis type 2 patients. Recently, its use has been recently extended to adults with non-tumor etiologies and children with profound hearing loss who were not candidates for a cochlear implant (CI). Although the results has been extensively reported, the stimulation parameters involved behind the outcomes have received less attention. OBJECTIVE: The aim of this study is to describe the audiologic outcomes and the MAP parameters in ABI adults and children at our center. METHODS: Retrospective chart review.

Retrosigmoid approach for auditory brainstem implant.

The present paper reports our experience with the surgical retrosigmoid-transmastoid (RS-TM) technique for implanting auditory brainstem implants (ABIs). From April 1997 to August 1998, four patients with neurofibromatosis type 2 (NF2) were operated on for vestibular schwannoma removal with ABI implantation. The subjects (three men and one women) ranged in age from 22 to 31 years. Tumour size ranged from 12 to 30 mm. A classical RS-TM approach was performed. After tumour excision, identification of landmarks (VIIth, VIIIth and IXth cranial nerves, choroid plexus) to the foramen of Luschka was

Multiple brain systems generating the rat auditory evoked potential. II. Dissociation of auditory cortex and non-lemniscal generator systems.

This study addressed the issue of multiple parallel auditory processing systems and their relationship to the skull-recorded auditory evoked potentials (AEPs) in the unanesthetized, unrestrained rat. In the preceding paper (Brain Res., 602 (1993) 240-250) it has been shown that auditory cortex activity does not contribute significantly to the vertex maximal AEPs recorded from the dorsal skull of the rat. In the present study, mapping of the AEP skull distribution revealed two sets of components: one set maximal at the dorsal skull vertex, and another set at the lateral skull), but not the earl

Open set speech perception with auditory brainstem implant?

OBJECTIVE: Only a small percentage of auditory brainstem implant (ABI) recipients treated for neurofibromatosis type 2 (NF2) have proved capable of identifying words using only the sound from the ABI. Recently, the ABI was applied to a series of patients with no cochlear nerve or with cochlear disorders that could not benefit from a cochlear implant (i.e., cochlear nerve aplasia or posttraumatic avulsion) or whose benefit was or would be severely compromised. A significant number of these patients have proven capable of understanding speech, including effortless telephone use. In the present s

Self-reported benefit, sound perception, and quality-of-life in patients with auditory brainstem implants (ABIs).

CONCLUSION: The majority of the patients used their auditory brainstem implants (ABIs) all the time, reporting that he/she would make the decision to receive an implant again if the decision were reconsidered. The findings support that the ABI is a valuable treatment in patients with type 2 neurofibromatosis (NF2) and in children with congenital inner ear and nerve anomalies or cochlear ossification. OBJECTIVE: To evaluate the patients who underwent ABI implantation in Uppsala during 1993-2013. This study analyzed patients' implant use, perception of environmental sounds, perceived benefit fr

Acoustically Evoked Compound Action Potentials Recorded From Cochlear Implant Users With Preserved Acoustic Hearing.

OBJECTIVES: Less traumatic intracochlear electrode design and the introduction of the soft surgery technique allow for the preservation of low-frequency acoustic hearing in many cochlear implant (CI) users. Recently, new electrophysiologic methods have also been developed that allow acoustically evoked peripheral responses to be measured in vivo from an intracochlear electrode. These recordings provide clues to the status of peripheral auditory structures. Unfortunately, responses generated from the auditory nerve (auditory nerve neurophonic [ANN]) are somewhat difficult to record because they

Intra-cochlear Flushing Reduces Tissue Response to Cochlear Implantation.

INTRODUCTION: Intraoperative trauma leading to bleeding during cochlear implantation negatively impacts residual hearing of cochlear implant recipients. There are no clinical protocols for the removal of blood during implantation, to reduce the consequential effects such as inflammation and fibrosis which adversely affect cochlear health and residual hearing. This preclinical study investigated the implementation of an intra-cochlear flushing protocol for the removal of blood. METHODS: Three groups of guinea pigs were studied for 28 days after cochlear implantation; cochlear implant-only (con

Power-efficient ramped stimulation in a fully implantable cochlear implant.

Cochlear implants (CIs) are among the most established neuromodulation devices, providing auditory perception through electrical stimulation of the auditory nerve. While conventional stimulation strategies rely on symmetric biphasic rectangular pulses, alternative pulse shapes may offer improvements in neural activation and energy efficiency-particularly for fully implantable CI systems where power consumption is a key limitation. In this study, we investigate the efficacy of anodic-first ramped biphasic pulse shapes compared to conventional anodic-first rectangular pulses, using a custom-desi

Special Populations in Implantable Auditory Devices: Pediatric.

"Hearing loss in the pediatric population can have significant social and developmental implications. Early auditory rehabilitation by at least 6 months of age is imperative. Although traditional hearing aids are often a first-line treatment option, there is a wide array of implantable auditory devices available. This article describes the indications for such devices as they pertain to the pediatric population, including osseointegrated bone-conduction devices, middle ear implants, cochlear implants, and auditory brainstem implants."
