Parenteral gold has historically been used to treat several conditions, including rheumatoid arthritis. Treatment records of 1800 patients with rheumatoid arthritis who were included in the clinical trials of auranofin in the United States were examined for data on development of proteinuria. Three percent (41) of 1283 auranofin-treated patients had an abnormal 24-hour urine protein level: 15 had mild (0.15 to 1 g/d), 17 had moderate (1 to 3.5 g/d), and 9 had heavy (greater than 3.5 g/d) proteinuria. Between 1974 and 1981, thirteen patients with pemphigus vulgaris were treated with prednisone supplemented by gold salts. The purpose of our study was to investigate the significance of the presence of anti-Ro antibodies found by us in an earlier study of rheumatoid arthritis (RA) patients with gold-induced side effects. Sera of 29 anti-Ro (SSA) positive RA patients who had gold-induced side effects were studied. Pulmonary damage is one of the rare complications of gold salts therapy. The reaction is that of drug hypersensitivity. The clinical and radiological features as well as the respiratory function tests are that of an interstitial lung disease. This condition is described in four patients and its main features are discussed in the light of the relevant literature. The importance of an early diagnosis is emphasized. Most of the patients recover after the withdrawal of the drug and during corticosteroid treatment.

It has been widely accepted for a number of years that chrysotherapy is a valuable therapeutic agent in the treatment of progressive rheumatoid disease. Forty patients with definite or classical active rheumatoid arthritis were stratified by the minimization procedure to auranofin (6 mg/day) or penicillamine (go slow and low regime).
