Acute subclavian or axillary vein occlusion during biventricular pacemaker implantation.

During biventricular pacemaker implantation, multiple punctures of the subclavian vein were performed and venous occlusion was apparent during the procedure, which in one case was stopped before lead insertion and in the other patient new access has to be forced through the occlusion by removing one of the already implanted leads. For implanting physicians, it is important to know that acute venous occlusion may occur during lead implantation.

The axillary vein central venous catheter in severely burned patients.

In severely burned patients the approach to the central vein is often difficult due to concomitant edema, but also due to the fact that the skin area, where commonly used approaches are performed, is burned as well, whereas the axillary region is often not involved. In order to perform an axillary approach to the central vein as an alternative to the commonly used approaches in patients, an anatomical dissection in fresh human cadavers was carried out. Considering the anatomical landmarks which were found during dissection of the axillary region, the axillary approach to the central vein was u

Transposed basilic vein versus polytetrafluorethylene for brachial-axillary arteriovenous fistulas.

BACKGROUND: Both transposed basilic vein (BV) and polytetrafluorethylene (PTFE) upper arm arteriovenous fistulas (AVF) are common angioaccess operations. To evaluate the patency and complication rates after AVF, a concurrent series of patients was reviewed. METHODS: Ninety-eight patients underwent brachial artery to axillary vein AVF: 30 BV and 68 PTFE. The PTFE grafts were performed in the standard fashion, whereas the basilic veins were translocated subcutaneously to the brachial artery. RESULTS: Risk factors were similar between the two groups. Basilic vein AVF had better patency at 24 mo

Axillary vein thrombosis in a female backpacker: Paget-Schroetter syndrome.

In 1875 Sir James Paget reported the first case of primary axillary vein thrombosis. Since then numerous reports of axillary vein thrombosis have appeared citing various initiating events and possible causes. We report here a patient with axillary vein thrombosis. The thrombosis appears to have been provoked by carrying a backpack.

A cephalic vein drained into one of the double axillary veins through the pectoralis major muscle.

Although the cephalic vein follows a fairly consistent course, numerous variants have been reported. We found a rare anatomical presentation of the cephalic vein in a 43-year-old Korean male cadaver. The cephalic vein had anastomosed with the basilic vein and brachial vein at the level of the elbow, perforated the pectoralis major muscle between the clavicular and sternal heads, and then entered into one of the double axillary veins. Knowledge of the variations on the cephalic vein is important for clinicians as well as anatomists since the approach through the axillary base is favoured in man

Transpectoral ultrasound-guided catheterization of the axillary vein: an alternative to standard catheterization of the subclavian vein.

Subclavian vein catheterization is associated with failure and complications because of injury to the nearby lung and subclavian artery. Its position, sandwiched between the clavicle and the first rib, makes sonographic imaging difficult. The medially pointed sonography probe makes it difficult to align the needle as well as image the entire needle. The axillary vein lies outside of the thoracic cage and can be easily imaged in its longitudinal view along with the entire needle, guidewire, dilator, and catheter in real-time. All described techniques of venous access using sonography have used

Upper limb deep vein thrombosis due to Langer's axillary arch.

Langer's axillary arch is a recognized muscular anomaly characterized by an accessory muscular band crossing the axilla that rarely causes symptoms. We describe a patient who presented with an upper limb deep vein thrombosis caused by this aberrant muscle, which we believe is the first reported case. Axillary surgery with division of the aberrant muscle relieved upper limb venous obstruction in this patient.

Long-term results of treatment for axillary subclavian vein thrombosis.

Thrombolytic therapy is an effective method for reopening an occluded venous segment, but little is known about the long-term results of thrombolysis via direct intravenous route for axillary subclavian vein thrombosis (ASVT). Our experience with 13 cases of primary ASVT illustrates the possible advantages of a protocol consisting of lytic therapy, surgical decompression and percutaneous balloon angioplasty if a high-grade stenosis persists. If accompanied by adjunctive therapy to correct the underlying cause, thrombolytic therapy for ASVT results in acceptable long-term symptom relief and vei

Central venous and pulmonary artery catheterizations via the axillary vein.

We report the results of over 300 central venous and pulmonary artery catheterizations via the axillary vein. We found that the success rate with this method compares well with those of other catheterization routes, whilst the risk of mechanical injury, even for artificially ventilated patients, was virtually eliminated.

Axillary vein puncture versus cephalic vein cutdown for cardiac implantable electronic device implantation: A meta-analysis.

INTRODUCTION: Cephalic vein cutdown (CVC) and axillary vein puncture (AVP) are both recommended for transvenous implantation of leads for cardiac implantable electronic devices (CIEDs). Nonetheless, it is still debated which of the two techniques has a better safety and efficacy profile. METHODS: We systematically searched Medline, Embase, and Cochrane electronic databases up to September 5, 2022, for studies that evaluated the efficacy and safety of AVP and CVC reporting at least one clinical outcome of interest. The primary endpoints were acute procedural success and overall complications.

Infraclavicular access to the axillary vein - new possibilities for the catheterization of the central veins in the intensive care unit.

Central vein cannulation is one of the most commonly performed procedures in intensive care. Traditionally, the jugular and subclavian vein are recommended as the first choice option. Nevertheless, these attempts are not always obtainable for critically ill patients. For this reason, the axillary vein seems to be a rational alternative approach. In this narrative review, we evaluate the usefulness of the infraclavicular access to the axillary vein. The existing evidence suggests that infraclavicular approach to the axillary vein is a reliable method of central vein catheterization, especially

Internal jugular to axillary vein bypass for subclavian vein thrombosis in the setting of brachial arteriovenous fistula.

Placement of central venous catheter is the most common cause of subclavian vein thrombosis. In the setting of a functioning ipsilateral brachial arteriovenous fistula, venous hypertension symptoms may be exacerbated. We report successful decompression of severe venous hypertension in the right arm of a patient whose only access for hemodialysis was a functioning right brachial arteriovenous fistula and in whom proximal subclavian vein occlusion developed from a previous percutaneous dialysis catheter. Transposition of the right internal jugular vein and end-to-side anastomosis to the right ax

Ultrasonografic changes in the axillary vein of patients with lymphedema after mastectomy.

OBJECTIVE: To determine the prevalence of sonographic abnormalities (SA) in the axillary vein of patients with and without post-mastectomy lymphedema. METHODS: We studied a sample of 80 women, divided into two equal groups, with and without lymphedema, with B mode ultrasound, color and pulsed Doppler. The primary variable, SA, is defined as change in the venous diameter, parietal thickening, intraluminal images, compressibility, parietal collapse at inspiration and feature of the axillary venous flow on the operated side. Secondary variables were: stage of lymphedema, surgical technique, numb

No effect of Valsalva maneuver or Trendelenburg angle on axillary vein size.

BACKGROUND: A new technique for establishing ultrasound-guided central access involves the use of the axillary vein, the distal projection of the subclavian vein, via the lateral chest. OBJECTIVE: To examine the effects of Valsalva maneuver and Trendelenburg positioning on axillary vein cross-sectional area (CSA). METHODS: Using a group-sequential design, we enrolled stable emergency patients and measured their axillary veins sonographically. Patients were measured while supine, then after a Valsalva maneuver, and then at 5°, 10°, 15°, and 17° of Trendelenburg positioning, pausing 2 min afte

Novel findings of the anatomy and variations of the axillary vein and its tributaries.

The anatomy and variations of the axillary vein has significant implications in various invasive procedures such as venous access, axillary block, arteriovenous fistula creation, axillary node dissection, breast augmentation, and other surgical procedures involving the axilla. To clarify the anatomy of the axillary vein and its tributaries, 40 cadaveric upper extremities were examined after dissection and were classified into several types according to the courses and terminations of brachial veins. The brachial veins ended separately (Type A; 72.5%) or made a common brachial vein (Type B; 27.

Pectoralis minor obstruction of the axillary vein: report of six patients.

PURPOSE: Although the usual site of nonthrombotic venous obstruction of the upper extremity is the subclavian vein, other sites may be the cause of such obstruction. This study describes the diagnosis and treatment of six patients with partial axillary vein obstruction by the pectoralis minor muscle, a condition that can mimic subclavian vein obstruction. METHODS: A chart review of patients undergoing pectoralis minor tenotomies (PMT) between 2004 and 2006 revealed six patients (3 men and 3 women), aged 17 to 39, who underwent seven PMT procedures for symptoms of arm swelling, cyanosis, and p

Endovascular treatment of penetrating injury of axillary vein with Viabahn endoprosthesis.

Penetrating injuries of the axillary or subclavian venous system are associated with extensive blood loss and are fatal in more than 50% of cases. Patients are usually unstable and are treated with surgical exploration. We present a case of axillary venous injury that was treated in the operating room with intravenous placement of a self-expanding Viabahn endoprosthesis (W.L. Gore). The device was delivered to the injured site percutaneously via a basilica vein, with immediate control of hemorrhage.

Axillary vein thrombosis during pregnancy in association with a lupus anticoagulant.

A 42-year-old multiparous pregnant woman presented with swelling and pain of the left arm at 34 weeks gestation. She had no discoloration of her arm nor a loss of radial pulse. Duplex scanning demonstrated a thrombosis in the axillary vein. She was found to have a positive circulating lupus anticoagulant. Intravenous heparin was administered and resulted in resolution of discomfort and swelling on day four of therapy. The patient was maintained on therapeutic doses of subcutaneous heparin until vaginal delivery at 39 weeks. Prenatal course was complicated by a resolving infection believed to b
