A Systematic Review on Nerve-Related Adverse Effects following Mandibular Nerve Block Anesthesia.

Inferior alveolar nerve (IAN) block injections are commonly used in clinical practice, but they are not free from complications. The aim of the present systematic review is to assess the nerve-related adverse effects of IAN block anesthesia. A structured and systematic search was performed on the major electronic databases (PubMed, Cochrane Library, Web of Science, Scopus and CINAHL) for studies published in English until 30 September 2021. A total of 131 articles were identified through database searching using combinations of keywords. Fifteen papers were included and assessed for eligibilit

Assessing and conveying risks and benefits of imaging in neonates using ionizing radiation and sedation/anesthesia.

Neonates represent a unique subset of the pediatric population that requires special attention and careful thought when implementing advanced cross-sectional imaging with CT or MRI. The ionizing radiation associated with CT and the sedation/anesthesia occasionally required for MRI present risks that must be balanced against the perceived benefit of the imaging examination in the unique and particularly susceptible neonatal population. We review the perceived risks of ionizing radiation and the more concrete risks of sedation/anesthesia in term and preterm neonates in the context of an imaging

Pressure-support ventilation in a child with merosin-deficient congenital muscular dystrophy under sevoflurane anesthesia.

Merosin-deficient congenital muscular dystrophy (MD-CMD) is the most common and severe form of congenital muscular dystrophy and is characterized by progressive severe hypotonia due to the absence of the merosin chain around muscle fibers. The main anesthetic concerns include a possible association with malignant hyperthermia, the risk of anesthesia-induced rhabdomyolysis, a difficult airway and postoperative respiratory failure. We report the case of an uneventful general anesthesia (GA) in a two-year-old boy with MD-CMD for the placement of an implantable venous access system. The goal of ou

Airway Management Challenges in Three Cases of Vallecular Cysts.

Vallecular cysts, though rare in adults, can pose significant challenges to anesthetic management due to their location near the airway and potential for life-threatening complications such as airway obstruction, aspiration, or cyst rupture during intubation. This case series presents three adult male patients who reported with symptoms ranging from dysphagia and voice changes to globus sensation, each diagnosed with vallecular cysts via imaging and endoscopic evaluation. All patients underwent surgical excision under general anesthesia following awake fiberoptic intubation (AFOI) to avoid air

Cervical epidural anesthesia is associated with increased cancer-free survival in laryngeal and hypopharyngeal cancer surgery: a retrospective propensity-matched analysis.

BACKGROUND AND OBJECTIVES: Regional anesthesia preserves perioperative immune competence and may reduce the risk of recurrence and metastasis after cancer surgery. Cervical epidural anesthesia provides adequate analgesia for head and neck cancer surgery, but its impact on cancer recurrence is unknown. METHODS: This study was a single-center retrospective cohort study of patients undergoing larynx or hypopharynx cancer surgery between January 1984 and December 2008. One hundred eleven patients had general anesthesia combined with intraoperative and postoperative cervical epidural; 160 had gene

Validation of the anesthetic effect of a mixture of remimazolam, medetomidine, and butorphanol in three mouse strains.

Proper administration of anesthesia is indispensable for the ethical treatment of lab animals in biomedical research. Therefore, selecting an effective anesthesia protocol is pivotal for the design and success of experiments. Hence, continuous development and refinement of anesthetic agents are imperative to improve research outcomes and elevate animal welfare. "Balanced anesthesia" involves using multiple drugs to optimize efficacy while minimizing side effects. The medetomidine, midazolam, and butorphanol, called MMB, and medetomidine, alfaxalone, and butorphanol, called MAB, are popular in

Atracurium infusion requirements in children during halothane, isoflurane, and narcotic anesthesia.

We were interested in determining the dose-response relationship of atracurium in children (2-10 yr) during nitrous oxide-isoflurane anesthesia (1%) and the atracurium infusion rate required to maintain about 95% neuromuscular blockade during nitrous oxide-halothane (0.8%), nitrous oxide-isoflurane (1%), or nitrous oxide-narcotic anesthesia. Neuromuscular blockade was monitored by recording the electromyographic activity of the adductor pollicis muscle resulting from supramaximal stimulation at the ulnar nerve at 2 Hz for 2 sec at 10-sec intervals. To estimate dose-response relationships, thre

Possible augmentation of neuromuscular blockade by propofol during recovery from rocuronium.

Propofol is a widely used drug in anesthesia practice, and its pharmacological characteristics are well known. However, propofol is not known for neuromuscular effects. As part of clinical neuromuscular monitoring, the neuromuscular responses to train-of-four (TOF) stimulation were monitored and recorded. We observed, in two cases of balanced anesthesia maintained by desflurane and fentanyl, that administration of a small dose of propofol during almost complete recovery from rocuronium in two patients resulted in marked decreases of both T1 (first twitch response of the TOF) and the TOF ratio.

Association between anesthetic approach and clinical outcomes of ACL reconstruction: a retrospective cohort study.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLr) is among the most common orthopedic procedures, however, large-scale studies analyzing regional anesthesia (RA) as an adjunct to general anesthesia (GA) are lacking. METHODS: TriNetX was used to identify adults undergoing ACLr between January 2014 and January 2024. Patients were categorized as receiving GA (inhalational/intravenous anesthetics with neuromuscular blockers only), RA (local anesthetics for nerve/neuraxial blocks only), or combined anesthesia (general and regional agents). Outcomes were assessed over short-term (1-day t

Ion Channels in Anesthesia.

Ion channels play a pivotal role in anesthesia, including general and regional anesthesia. Two main classes of general anesthetics (GAs) are inhalational anesthetics, such as isoflurane, sevoflurane, and nitrous oxide; injectable anesthetics, such as propofol, etomidate, and ketamine. Besides hypnotic agents, muscle relaxants for immobility and opioids for analgesia are needed to achieve balanced anesthesia. Although our understanding of anesthesia is far from complete, recent studies have revealed the molecular interactions between anesthetic drugs and ion channels, particularly, the ligand-g

Association of Regional Anesthesia With Reduced Post-discharge Opioid Prescriptions Following Immediate Autologous Breast Reconstruction: An ACS-NSQIP Analysis.

BACKGROUND: Postoperative pain may be significant following mastectomy and immediate autologous breast reconstruction, and pain management regimens must be balanced with opioid stewardship. Regional anesthesia has been associated with reduced opioid consumption in smaller studies and may represent a promising method of acute pain management. This study evaluates rates of opioid prescription and renewal following immediate autologous reconstruction depending on receipt of regional anesthesia. METHODS: All adult female patients who underwent immediate autologous breast reconstruction in the 202

[The anesthesiological support for children in extracorporeal lithotripsy on the Urat-P apparatus].

The Urat-P lithotriptor made in this country was employed for remote lithotripsy (RLT) in 94 children with urolithiasis. Altogether 132 sessions were conducted. In 120 cases the concrement disintegration progressed in a sparing regimen (impulse duration 0.3-0.4 ms, shock wave energy 5-8 J). In the rest 12 cases there were additional hard impulses (0.5-0.6 ms, the energy 10-12 J). Anesthesiological and transfusiological problems are considered. It is shown that choice of anesthesiological defense is based on the clinical form of the disease, the concrement size, the child's age. Intubation anes

Differential effects of remimazolam and propofol on heart rate variability during anesthesia induction.

PURPOSE: The effects of remimazolam on autonomic nervous activity have not been elucidated. We investigated the differential effects of remimazolam and propofol on autonomic nervous activity during anesthesia induction. METHODS: Thirty patients were randomly divided into a remimazolam group or a propofol group for anesthesia induction. Hemodynamics and indices of heart rate variability were recorded before and after anesthesia. Low frequency power (LF; 0.04-0.15 Hz, ms2) and high frequency power (HF; 0.15-0.4 Hz, ms2) were calculated from power spectral density of heart rate variability. LF r

Opioid-Free and Opioid-Sparing Pain Control in Surgical Anesthesia: A Scoping Review of Multimodal Analgesia, Regional Techniques, and Recovery Outcomes.

Surgical anesthesia has historically relied on opioids to control intraoperative and postoperative pain, but opioid-related adverse effects such as respiratory depression, sedation, ileus, postoperative nausea and vomiting, delayed mobilization, tolerance, hyperalgesia, and prolonged opioid use have encouraged a shift toward safer perioperative analgesic models. In this scoping review, opioid-free anesthesia (OFA) is defined as the complete avoidance of opioid administration during the intraoperative period, whereas opioid-sparing anesthesia (OSA) is defined as an anesthetic approach that inte
